The contact stated that he added exhaust fluid to the vehicle per owner manual’s instructions; however, the message remained displayed. The vehicle was taken to an unknown dealer, where the failure could not be duplicated. The dealer advised the contact that the vehicle would return to normal operation if he continued driving the vehicle. The contact stated that as he continued driving, the vehicle failed to exceed 55 mph. A message indicating that the speed would decrease to 45 mph for 40 miles, and then to 5 mph, was displayed. The contact pulled into a gas station. The vehicle was towed to the residence. The vehicle was then driven to the dealer, where the failure could not be duplicated. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 74,000. The VIN was unavailable.

The contact stated that while operating the vehicle the temperature gauge would not operate and the vehicle would not properly accelerate while pressing the accelerator pedal. Also while operating the vehicle the contact had become ill with headaches and dizziness which required medical attention. Upon inspection it was discovered that a hair like fiber was present coming from the exhaust pipes. The vehicle was taken to the local dealer who informed that the temperature gauge and exhaust system was faulty and needed to be replaced. The hair like fibers were not identified. The contact was concerned that the hair like fibers had clogged the exhaust system causing carbon monoxide to enter the cabin of the vehicle. The manufacturer was notified of the failures. Tl the contact called on behalf of the owner of a 2007 GMC Canyon. While driving at an unknown speed, the contact became nauseous. The contact pulled the vehicle off the roadway, exited the vehicle, and passed out for several minutes according to a witness. The driver experienced symptoms due to carbon monoxide poisoning. The contact attempted to move the vehicle once she regained consciousness, but became overwhelmed. The contact stopped the vehicle and, while attempting to retrieve documents from the glove box, she noticed flames inside the glove box. The contact exited the vehicle. The fire department was notified and was able to extinguish the fire; however, the vehicle was destroyed. A fire report was not filed. The driver was transported by ambulance to a nearby hospital. The driver received oxygen for six hours to remove the carbon monoxide from her lungs. The vehicle was towed to a private lot. The dealer and manufacturer were not notified of the failure. The approximate failure mileage was 85,003.
