The contact owns a 2017 Tesla Model S. The contact stated that he received notification of NHTSA campaign number: 21v035000 (back over prevention, visibility, exterior lighting) and the vehicle was taken to the local Tesla service center for recall repair. However, upon picking up the vehicle, the center display screen turned black and was no longer operative. The vehicle was taken back to the Tesla service center where it was diagnosed that the computer chip that allowed the vehicle audio and radio to operate needed to be updated. The dealer informed the contact that he would need to pay a repair fee. The vehicle was in the process of being repaired. The manufacturer was notified of the failure. The failure mileage was approximately 13,715.

38 Exterior Lighting problem of the 2014 Tesla Model S

Failure Date: 06/15/2022

My 2014 Tesla Model S is an unsafe vehicle as my dash goes black constantly and I lose control of the car safety items included in the dash panel such as speed, back up camera, visibility and power train. I have complained many times but Tesla does not respond with results that will make the car safe again.

39 Exterior Lighting problem of the 2021 Tesla Model S

Failure Date: 06/06/2022

While driving with a fully charged battery, the car dash warned “pull over to safe location. Battery cannot support all functions”. We were unable to fully pull over in the time before the car experienced a critical loss of functionality, as the car shut off in less than 1 minute from flashing the warning. The car lost all functionality, including loss of its flashers, putting us at significant risk since we were unable to alert other motorists to our presence on the side of the road as it was getting dark. The car is currently with the dealer and thus far they have been unable to identify the root cause of the issue but have acknowledged the existence of the problem. They’ve claimed the 12v battery died and the car does not have any safety redundancy built in to be able to access power from the main driving battery to power critical features such as the hazard lights. The biggest risk of safety is loss of indicators/flashers while on the side of the road. This feature in vehicles is meant to alert other drivers to the presence of a disabled vehicle.

40 Exterior Lighting problem of the 2022 Tesla Model S

Failure Date: 05/20/2022

Steering yoke safety issue: Tesla Model S plaid cars come with a steering yoke instead of steering wheel. The steering yoke’s horn, turn signal, high beam, voice assistant, windshield wiper, and cruise control led indicators that identify these buttons become invisible due to sun glare when the sun is behind or to the left of the driver. The glossy surface of the yoke combined with the weak led lights of the indicators make the yoke unusable for honking the horn, make it impossible to find the horn, turn signal, windshield wiper, and other buttons, effectively making these features unusable. At a minimum, these critical safety buttons being invisible and having no tactile way to finding them during sunny days causes a significant distraction away from looking at the road. In the attached photo, you will see the effect of sun glare on the yoke while driving. This design flaw in the steering yoke/wheel puts my safety and the safety of others at risk because of inability to use the horn, windshield wipers, voice assistant, high beams, and turn signals when the sun or any other bright light causes yoke glare. Furthermore, the lack of tactile indication of where the buttons are, because they are only indicated/separated by lighting (leds) causes the driver to have to divert attention away from the road and onto the steering yoke to find the controls, or push parts of the yoke randomly until the correct function button is found. This is a serious safety issue which can be resolved by using a different material for the steering yoke indicator buttons, by having tactile separators/notches where the yoke buttons are so the user can find where to press during sunny days.

45 Exterior Lighting problem of the 2017 Tesla Model S

Failure Date: 02/02/2022

The led daytime running lights are failing for the second time on my vehicle. By failing, I mean they are burning out and no longer effective. This is a well-known issue with Tesla vehicles, however Tesla is doing nothing to resolve the problem. The only solution offered is to pay for an entire set of new headlights which costs nearly $2,600. The replacement headlights eventually fail within 24 months. Not having daytime running lights could be a safety issue. Tesla is aware of the component failure however no recalls for this defective part has been issued. I believe most Tesla owners of the "refreshed" Model S experience a degradation of the led portion of the headlights, which are the daytime running lights. Today I was quoted $2,582 to replace both headlights. My lights were previously replaced in 2019 under the original vehicle warranty. Is there anything that can be done to have the lights replaced under a recall or have Tesla cover the cost of the repair?.

46 Exterior Lighting problem of the 2016 Tesla Model S

The contact stated that while driving at an undisclosed speed, the center display screen went blank, causing the gear shifter, cameras, and defroster to become inoperable. The center display screen inadvertently rebooted shortly afterwards. The contact stated that the Tesla app confirmed that the emmc was still operating and functioning as designed. The vehicle was not diagnosed or repaired by the dealer or an independent mechanic. The manufacturer was not made aware of the failure. The failure mileage was unknown. Parts distribution disconnect.

48 Exterior Lighting problem of the 2016 Tesla Model S

Failure Date: 12/30/2021

The tail light flooded during rain and the tail light would not turn on. I was notified of this issue after an officer pulled me over and gave me a notice. There were no warning signs. In search of the issue online, it appears that it is a very common issue with the Model S tail lights.

49 Exterior Lighting problem of the 2015 Tesla Model S

Failure Date: 11/27/2021

System failure: adaptive speed control safety risk: my 2015 Tesla Model S, at highway speeds, smashed into the rear of a jacked up Ford f250 pickup. A head on collision at highway speeds put my wife (and dog) and I at great risk. Has the problem been reproduced or confirmed by a dealer or independent service center? I am unsure if or how Tesla is dealing with the problem of adaptable speed control failure. Has the vehicle been inspected? yes. Dan's paint & autobody and usaa insurance company adjuster my accident happened in the southbound passing lane at mile marker 178 on I-10, south of chandler on the gila river community in maricopa/pinal county it was dark outside with heavy traffic in both southbound lanes. My software version was 2021. 40. 6 I had the distance between cars at the closest setting , and had the adaptive speed control set at ~75. I was attentive with both hands on the wheel, eyes forward watching traffic with my wife whom was also watching the traffic. Without warning a Ford f250 stopped hard in front of me and my adaptive speed control did not react. The few beats I gave the adaptive speed control to respond first cost me precious braking time. I did hit my abs brakes and could feel the vibration as they attempted to slow down, but it was too late. I submarined under the bumper of the truck at a very high speed. There was no warning lamps, messages at the time of the accident. There was one early symptom that I recall. There was a Tesla software update a few days before the collision. On 11/27/22 before leaving for my trip, the dash screen to the far left did not display the map which it usually does when you set the gps to map the path. I rebooted with the steering wheel toggles, and the display returned to normal. There were no other software malfunctions that I recall prior to the adaptive speed control failure.

50 Exterior Lighting problem of the 2015 Tesla Model S

The vehicle was taken to the dealer where the vehicle was serviced under NHTSA campaign number: 21v035000 (back over prevention, visibility, exterior lighting) however, the failure persisted. The service center offered the contact a special offer of $2,000 for an entertainment system upgrade (nvidia tegra 3 to intel atom processor). The contact declined and Tesla acknowledged that there was a known issue with nvidia tegra 3 processor in the mcu and that an upgrade to the nvidia tegra 3 would resolve the known failure. The contact was concerned that the backup camera failure might result in a crash with another vehicle or pedestrian. The vehicle had been taken to the dealer on three separate occasions and on each occasion, the vehicle remained in their possession for approximately 2 weeks. The manufacturer was not notified of the failure. The failure mileage was 80,000.

55 Exterior Lighting problem of the 2012 Tesla Model S

Failure Date: 10/05/2021

As the car drains the battery pretty quickly, I have turned on energy saving mode. In this mode the car enters a deeper sleep, consuming less power. Sometimes in this mode when starting the vehicle, the turn signals will not work (this appears to happen when the car sits for a while and the miles left on the battery is under 120 or so). After a quick stop (2 minutes out of the car), the turn signals will start working, and the driver display will show the blinking turn signal when they work, but it will not display when they do not work. Another issue is that after entering the car and "turning it on", by pressing the break, sometimes the driver display (which shows speed and current drive mode, prnd indicator) will be blank, not showing any information, despite the ability to operate the car, etc.

59 Exterior Lighting problem of the 2021 Tesla Model S

Failure Date: 07/30/2021

My car has a « yoke » steering wheel that feels unsafe and should be illegal along with the touch turn signals and horn. I almost crashed today when I had to swerve to avoid a mother crossing the road with her child. I tapped on the honk, but unfortunately, it’s not in the middle of the steering wheel as one one would expect, but it’s a small touch button right next to the cruise control touch button. Each time I need to engage it, I have to take my arms off the road and find this button. It’s very distracting, especially in emergency situations. The turn signals are also « touch » only and there’s no way to really know if you have them or not. When turning in a roundabout, the yoke steering wheel is almost upside down and trying to signal is nearly impossible. I think the yoke steering wheel and touch buttons are marketing gimmicks used to get people to talk about Tesla at the expense of the safety of the owners and other innocent users of the road. This is extremely dangerous in my opinion and I’m now officially scared to be driving my car. Please make Tesla recall these cars and retrofit a normal steering wheel with a regular horn and stalks for the turn signals. Thank you for reading you are our only hope. 60 Exterior Lighting problem of the 2012 Tesla Model S

Failure Date: 07/22/2021

I recently had my Tesla Model S serviced due to the touchscreen going blank (losing functionality of defrosting, temperature, sunroof, and a list of other issues). Driving with a fogged up window is dangerous. Thank you to the NHTSA for forcing Tesla to replace it free of charge. Unfortunately, it appears that Tesla is not replacing it with a new unit. Instead it is a refurbished unit. Just days after servicing I got in the car to drive and the following error comes up on the touchscreen: "center display memory storage device is degraded. Without repair, center display may enter limited functionality mode and only display rear view camera. When in limited functionality mode, it may be required to press brake for 30 seconds or press key fob unlock to authorize drive. Please use the Tesla mobile app to schedule service. " I find this quite troublesome and felt it worthy to apprise the NHTSA that Tesla is clearly not abiding by its requests.

62 Exterior Lighting problem of the 2017 Tesla Model S

Failure Date: 07/01/2021

I don't have this problem anymore because I had the circuit board replaced. The issue I have observed is the auto high beam feature doesn't work properly. They get stuck in the high setting and don't adapt to on-coming traffic. It's primarily in model y cars but all models have been observed. This might be as simple as needing the adaptive setting to be default or in my case a new control board replaced. The remedy before I got it fixed was to not have high engaged with the stock. I don't think there is a manual solution with the y or any newer Teslas.

64 Exterior Lighting problem of the 2016 Tesla Model S

Failure Date: 06/02/2021

This automobile has had the Tesla recall completed, replacing the 8gb with the 64gb. However, this fix is temporary and after a few months the control and information screens are malfunctioning and with ever-increasing frequency. I understand Tesla’s recent update ensures basic driving functions will continue to be operational during a screen blackout but it’s at best distracting and at worst plain dangerous depending on how lights/hvac are set prior to the screen freeze, not to mention distractions associated with a sudden loss of navigation and an inability to control other components. As a frequent highway driver this is dangerous, even after experiencing various versions of this event. Per service dept. , the only existing actual (non-temporary) repair for this is replacement with the newer version of the mcu, yet Tesla insists on using language as a loophole, calling the mcu2 an upgrade- which it was, but now as the sole remedy to restore previously existing functionality and safe operation of the effected vehicles, this should not be categorized as an option or an upgrade. Owners should have a better course of action than having to pay for an unwanted “upgrade” to regain original functionality.

65 Exterior Lighting problem of the 2018 Tesla Model S

Failure Date: 06/01/2021

The daytime running lights on both front headlights have stopped working with 45k on the odometer. Tesla service center has refused to cover the safety issue because the car warranty is 5k over warranty mileage. The vehicle no longer has working day time running lights.
