There is a Tesla software defect. 1. I stepped into my Tesla Model Y. There was no vehicle in front of my driveway. A delivery van came to park in front of my driveway temporarily. It is a traffic violation according to the California vehicle code section 22500(e). 2. I put my foot on the brake and put my car in the reverse gear. 3. Then I made an adjustment on the control screen. 4. I put my foot on the accelerator to continue to back up my car. The screen doesn't show the rear view anymore. This is a Tesla software defect that the screen should display the rear view as soon as I push the accelerator in this case.

The Tesla “self driving” features are inconsistent, at best, and frequently fail. I can not believe that the government approves these capabilities or the Tesla is accepting liability for road use. If these features are not reliable under supervised conditions, why would they be considered safe for robo taxi release? I have repeatedly tried to report this to Tesla and they say the systems are operating as designed.

Shortly after purchasing the vehicle in late 2024, I began experiencing multiple safety-related malfunctions. These issues include: full self-driving (fsd) malfunction: on the third use of fsd, the vehicle attempted to enter a left-turn lane but failed to fully move into the lane, resulting in the right front panel of the vehicle colliding with the bumper of a truck in the adjacent lane. There were no warnings from the system prior to the collision. The incident occurred at low speed. The truck sustained no visible damage, and the rubber from the truck's bumper transferred to the car, which was removed by hand later. This event raised serious concerns about the safety and reliability of the fsd system. No police report was filed, and the manufacturer was not present at the scene. Backup alert system failure: on several occasions while reversing, the car failed to issue audible or visual alerts despite vehicles being within 10–15 feet. In one instance, the screen finally showed a red zone when a car was within 10 feet, but still provided no audible warning. This issue has been intermittently reproducible but not consistent. It poses a significant risk to pedestrians and other drivers, particularly in low-visibility or driveway situations. Manufacturer's response: the vehicle was inspected by Tesla. After one in-person service visit and extended discussions, Tesla service representatives stated there was “nothing more they could do” and that a buyback was the only next step. A buyback request was submitted by a Tesla representative but has not been acknowledged or resolved more than 45 days later.
This started at about 65000 miles in my 2023 Model Y. The rear camera first lost picture and I received a warning that it may be intermittent. It has progressed now to the point that autopilot, navigation, all cameras, cruise control, auto headlight, and auto wipers are all nonfunctional. The car does not know its location. All safety features such as lanekeeping, cross traffic alert, auto braking are disabled. The rearview camera no longer functions. Software updates no longer complete downloading. While there are numerous reports of computer short circuits in hardware 4 cars, the only solution I am offered is to have the computer replaced at my own cost of just under 3000 dollars. And that would be warranted only for one year or 12500 miles. Reviewing the submissions on the NHTSA website as well as searching online, this problem is not infrequent. I do not feel safe letting my wife or daughter drive the car. Certain 2024 and 2025 models with this problem have been recalled for new computers. This must be expanded to include 2023 models as well.
I bought a Tesla Model Y 2025 and got my car delivered on [xxx] I barely drove it and within a week, I noticed all sensors stopped working (all cameras and camera-related functions stopped working, including the legally mandatory backup camera for new cars, gps has stopped working, making the premium connectivity package I paid not usable). Not only was I not able to use the self driving capability, which I paid $8000 for, but also the car lacked the most basic safety features. I contacted Tesla service center, they refused to admit that the sensor malfunction is a security concern, and still put me on an appointment three weeks later (Dec 13). After the first appointment, they did not fix the problem at all and noted it was a major computer issue, as reported by several media outlets [xxx] ). So my brand new car has a defective core component upon delivery. I was scheduled to meet them after they get the computer part on Dec 27. The appointment was rescheduled to Jan 14 again for missing parts. During the process, I found some weird tricks online and semi-fixed my car’s navigation but the sensor issues still exist. And on Jan 14 again, they claimed the computer part was missing again and I had to wait until that upcoming weekend to fix it. To summarize the situation: I was given a car that is defective upon delivery on its most important core component for 2. 5 months, when the car lacks even the basic legally required safety features, as well as premium features that I already paid off! during the process, I also spent a lot of time dealing with their customer service, and rescheduled appointments (I even had to drive to their service center several times to get a loaner and had to return the loaner early before they did not have parts to fix my car). On 2/15/2025, while backing out of a driveway at about 7:30pm, the vehicle backed into a parked car directly behind it parked in street. There was no warning signal emitted that an object was behind the car. There was no safety alert on the display. Essentially the vehicle's sensors did not see the vehicle directly behind it.
This report addresses concerns regarding recent safety updates in the Tesla Model Y, specifically focusing on the "rear cross traffic alert" and the blind spot warning system. These updates are intended to enhance driver awareness and prevent collisions. However, there are significant shortcomings in their current implementation that may compromise driver safety. 1. Rear cross traffic alert: intended functionality: the "rear cross traffic alert" is designed to warn drivers of approaching vehicles from the side when reversing, thereby preventing potential collisions. Current implementation: the system currently highlights a portion of the infotainment screen in red when a potential hazard is detected. There is no accompanying audible alert to notify the driver of the danger. Safety concerns: drivers are likely to be looking over their shoulder or at their mirrors while reversing, rather than at the infotainment screen. The lack of an audible alert means that drivers may not notice the visual warning, especially if their attention is directed away from the screen. This could lead to an increased risk of collision, as the visual cue alone may not be sufficient to capture the driver's attention. 2. Blind spot warning system: intended functionality: the blind spot warning system is designed to alert drivers of vehicles in their blind spot when the turn signal is activated, helping to prevent lane-change collisions. Current implementation: similar to the rear cross traffic alert, the system highlights the screen in red when a vehicle is detected in the blind spot. No audible warning is provided to alert the driver. Safety concerns: in adverse weather conditions, such as rain, drivers may rely more heavily on blind spot warnings due to reduced visibility. Drivers typically turn their heads to check for vehicles, which means they may not see the screen's visual alert. Without an audible warning, drivers may inadvertently change lanes into an occupied space, increas.
I took delivery of the Tesla Model Y yesterday (12/24/2024). The car has 73 miles on it as of now. First, the rear view camera stops working this morning (12/25/2024) and parking assistant is not available. Then the autopilot won't turn on and the car shows an alert that "power limited". After I arrived home from a supercharger (the car is charged to 80% as of now) around 1pm, the screen turns brick, and it will not turn on again after rebooting. It can drive, but the power feels weak. I called Tesla roadside service. The car has been towed to the fresno service center.
Widespread issues acknowledged privately by Tesla service managers, advisors, and engineers. Affects all models built from July 2024 to present with most recent “ap4” computer and hardware, primarily out of fremont factory. Suddenly, computer is short circuiting. This results in multiple systems failing given effectively all systems are dependent upon this “ap4” computer and hardware, and initially causes gps and camera sensors to fail also. This system failure includes: -federally required rear view vision system -adaptive cruise control -automatic emergency braking -blind spot warning -forward collision warning -lane departure warning -lane keeping assistance -parking collision warning -rear cross traffic warning -visibility and wiper systems -all external lighting systems -speed control internally, Tesla service advisors and managers are being told to advise customers that vehicles can be safely driven until fixed - which is currently diagnosed to be a removal and replacement of this “ap4”, which is heavily supply chain restricted. Customers are being told to wait “weeks” - with estimations in January and February - for this to be fixed. Service advisors and managers are being told to not acknowledge clear safety concerns given vehicles are no longer compliant with all applicable federal laws and regulations, such as those requiring rear view vision field of view expansion, etc. Further, internally sales advisors and managers are being told to continue heavily pushing sales to meet record sales targets to impact stock price. Several private conversations regarding ethical concerns of pushing noncompliant and known defective vehicles for stock value and potential criminality. Service advisors and managers also privately concerned regarding legal liability of impacted and defective vehicles in accident, given Tesla is communicating to continue driving. My own personal vehicle has been impacted, and information above has been gathered privately.
After latest Tesla software upgrade, all cameras and gps stops working. All automatic functionalities of the car stops working without cameras, and it is really unsafe to drive, for example back up the car since the physical rear view mirror is very small, and car is naturally designed with those cameras to function properly.
Starting on 12/4/2024, my car's camera stopped working, no blind spot warning, no navigation, no rear view camera and no cruise control. Reported to Tesla on 12/8 and they said it is a known problem nationwide and some autopilot computers are getting shut or something. They have given me an appointment for 1/4/2025. The vehicle is really unsafe to drive without these components working and the vehicle draws on heavy amount of battery usage without these controls and for my commute on highway, I am always nervous about if there is going to be enough for me to reach the next charging station. I would expect this falls into safety recall and Tesla should at least send out a service bulletin as well as provide alternate vehicles for the owners to use until this can be fixed.

Without a functioning navigation system, I cannot properly plan my travel or locate charging stations, which severely affects the usability of the vehicle. Safety concerns: the failure of these systems, especially safety features like forward collision warning, lane keeping assistance, and blind spot monitoring, puts both the driver and others on the road at risk. Losing access to safety-critical functions after an update is unacceptable and poses a serious danger to vehicle occupants and surrounding traffic. Incident details: • the issue occurred immediately following a software update. • Tesla pushed the update remotely without providing any warning or indication of potential risks. • the failure appears to be related to a hardware short in the ap4 computer, directly caused or exacerbated by the update. Desired outcome: • a thorough investigation into Tesla’s software update process and its effect on vehicle hardware and safety systems. • a resolution that prevents similar failures in other vehicles and ensures system updates do not compromise vehicle safety or critical functionality.
The back camera of the Tesla Model Y does not provide necessary image quality to identify any objects/persons of cross traffic when reversing the vehicle. In one incident I was about to hit a person behind the vehicle as there was no proper cross traffic alerting and no proper camera view. When I approached Tesla for support they identified the problem and determined it is by design and would not be able to help much. As per the images uploaded, there is clear distinction of camera quality between side camera and back camera. Also the images taken when the vehicle is in stop mode making the back light illuminate otherwise the images would be much darker. Also I want to point out that the back camera functions good during day time and the issue happens only during night.
Vehicle has frequent occurrences of phantom breaking while in cruise control. Tesla reviewed 3 occurrences and determined the system was working as Tesla intends it to and refuses to remedy. The system is prematurely identifying potential hazards which are not yet hazards and applying the brakes thereby creating a hazard due to vehicles being behind me and could reared my vehicle. One instance is driving on a 4 lane highway with a median, cross section traffic vehicle coming from left hand side pulls into the median and is not a hazard. The Tesla system decides the vehicle is a hazard and applies the brakes which could have resulted in a rear end collision. Another similar instance was a cross section vehicle at a stop sign which slightly moved forward while at the stop sign. The Tesla system decides the vehicle is a hazard and applies the brakes which could have resulted in a rear end collision. Tesla camera systems also falsely identify emergency vehicle lighting which results in immediate braking on an interstate. This system flaw almost resulted in an 18 wheeler rear ending me.
When their is a car in my blind spot the car does not beep or try to pull the car back unless you are basically hitting the car next to you. It also does not beep or show me space out side my range of view to prevent backing into another car. My car (Tesla Model Y) has had non-function blind spot warnings and backing up warnings since we purchased it in Feb 2023. There have been multiple times when I have not seen other cars and almost ran into them (they were able to swerve). On this Monday night the other car was not able to swerve because there was someone in the lane next to her (3 lane road) so we crashed. My car is available for inspection upon request but someone would have to come to my home because the car is not drivable and is waiting to get into the Tesla body shop.
I purchased my Tesla Model Y in January of this year. Tesla removed all ultrasonic sensors from the car, and is instead relying only on "Tesla vision". However, 9 months later and I still don't have several safety features Tesla promises. Rear cross traffic warning and blind spot warning don't work at all. The lane departure warning only works over the solid outside line, and doesn't warn at all when crossing between lanes (ie a dotted line). In addition, the adaptive cruise control randomly brakes dramatically for no apparent reason. This is extremely dangerous, as drivers behind don't expect you to quickly decelerate in the middle of the highway for no reason.
Drove to the carwash today a couple miles away to see if its just emergency vehicles the car doesn't see but its not seeing any vehicles in the right lane or passenger side of the car until they are about 150ft ahead of me. To clarify the cars vision system is not seeing any emergency vehicles (driver side, passenger side, ahead, behind, moving away, moving towards) like they don't even exist (invisible) and also not seeing any vehicles approaching from the passenger side blind spot. Switched my ig to private, was tired of all the hecklers and people reporting me and none of the government agencies doing any of their responsibilities except trying to screw me over on made up charges lol. Hello it's me for the 500th time. I was sitting at a light when a new song came on that's actually a movie clip from pulp fiction directed by quentin tarantino about hitmen that kill people that speak out, like whistleblowers; for their boss. So I'm sitting at the light and the cars behind me are all glitching between 3 lanes but nobody is moving and my volume is up and it starts 'yelling' "storing dead niggers aint my fucking business!". This was soon after almost being ran off the road by a white Cadillac escalade that I called in and reported to the local pd. I was rear ended by a white Cadillac escalade in 2019, I almost died and received death threats while leaving a competitors business. And I'm permanently partially handicapped at 31 years old. About 1 mile down the road my car started encountering 'ghost cars' that show up on the gui of the screen but aren't actually there in real life lol. Just so I'm clear the car is basically threatening me after Tesla says there's nothing wrong with it so I disabled my $12,000 fsd and will be keeping this car in the garage from now on. The screen multiple times has glitches and froze. We weren’t able to use backup camera, turning lanes cameras or clearly see our screen. We couldn’t access any of the icons that allowed us to control any features of the car. It was like driving blind.
While backing out of my parking lot space I almost hit a pedestrian emerging from behind a nearby parked car. My user manual, provided at the time of delivery, referred to warning sounds in situations such as these. I notified the Tesla service about this issue and was informed that my vehicle did not include these warnings because Tesla stopped installing the ultra sonic sensors required to activate the warning. Tesla service indicated this would be corrected in a future software release relying on Tesla vision. That initial service call was early January and Tesla has still not provided the updated software nor a target date for the change. The lack of sensors has also resulted in near impact in the front of the car due to the lack of sensors. This is a particular problem for senior drivers.
