After starting to accelerate from a stop light ,the dashboard had a warning message that the accelerator and brake pedals were pressed simultaneously. The acceleration stopped and the car slowed automatically. After a few seconds the warning indicator went away and the car could accelerate. The cars behind my auto had to stop suddenly. At the time. I though that I might have hit both petals at the same time. When it happened again I opened an incident report with Toyota. Incident opened on 07-19-25 case # 250719000440. The Toyota rep told me it should be take to a Toyota dealer to diagmose any error codes. The next time the same problem occured I took it to my Toyota dealer and was told there were no error codes to indicate a problem. I have also taken the car to a local repair shop after the problem happened and was shown there were no stored errror codes. The dealer wants to have my auto for an extende time to reproduce the problem. If they need error codes to solve the issue but no error codes are recorded when the problem occurs what can they determine? I am hoping that there are other Toyota Avalon owners which might have had the same issue.

The contact owns a 2015 Toyota Avalon. The contact stated that while driving at an undisclosed speed, there was an abnormal grinding sound coming from the brakes. The contact stated that the brake warning light illuminated, and the brake independently activated. The vehicle was taken to a specialty brake shop, where it was diagnosed that the brake actuator had failed, causing the abs system to activate independently. The vehicle was repaired. On the drive back to the residence, the failure recurred. The vehicle was taken to an independent mechanic where it was diagnosed with brake master cylinder failure. The brake booster, gasket, and brake actuator were replaced. The contact stated that two weeks later, the failure reoccurred. The vehicle was taken to the dealer where it was diagnosed and dtc: c1441 and c1431 were retrieved, indicating an issue with the abs system lateral acceleration sensor circuit or the brake booster. The dealer became aware that the front bumper bracket had paint on the part that did not allow for the vehicle to brake correctly. The brake pedal support assembly with sensor was replaced, and the paint was removed. The vehicle was repaired. The contact related the failure to NHTSA campaign number: 15v728000 (forward collision avoidance). The manufacturer was made aware of the failure. The failure mileage was 83,829.

I was pulling into a parking spot when my car suddenly accelerated to a very high speed. I traveled over a curb, on some pavement, through some bushes, knocked down a light pole and miraculously came to a stop. My foot was pumping the brakes to no avail. My entire car was damaged but miraculously no one was hurt. Car was towed to a mechanic who confirmed that the car accelerated with him as well. Front, back, underside, roof, tires and frames were all badly damaged.

I have been experiencing sponginess in the brakes. The air conditioner stopped working, the temperature gauges stopped working, and the abs and brake light are all on in the dash. Reviewing possible fixes according to Toyota nation forum online all these symptoms are a result of an abs control module. This part alone costs almost $1500. 00 and supposedly is a very common problem with this type of Avalon, thank you for researching this further.

While driving the pre-collision system (pcs) turns on unexpectedly, without any warning lights or sounds, and applies the brakes. This happens when I am driving in traffic with plenty of space between the car in front of me. I typically drive between 3-6 seconds behind the car in front of me to allow for at least 300 feet in space when driving at highway speeds. Despite leaving plenty of room for cars in front of me, on multiple occasions the car has started braking abruptly while going anywhere between 50, 40, 30, or 20 miles per hour. This can be verified for inspection upon request. Despite there being a recall for my specific year, make, and model car (2015 Toyota Avalon) for this exact issue my VIN number is not covered. Toyota is requiring me to pay out of pocket for them to 1) determine if its malfunctioning and 2) replace the system, which is a huge conflict of interest. Even though the driver's manual says that I can disable the pcs by clicking on the pcs on/off button within the driver's glovebox, there is no such button, only a blank button cover. Not having the ability to disable the pcs system, or worse, allowing it to continue braking while I'm driving is a serious hazard. The car can start braking while I am accelerating to merge onto expressway on-ramps, or while changing lanes at the same speed of traffic. The car can also unexpectedly brake while cars are traveling too close behind me. I want to know why my VIN was not included in the initial recall and why I now have to pay to have it looked at or repaired when its clearly malfunctioning and can't be turned off as the driver's manual says it can. I should not have to pay $175 an hour for several hours for a Toyota dealership to diagnosis it and then another $175 an hour for 2. 5 hours for them to install the fix and whatever the new component costs. I recently bought this car at 35,500 miles and the issue immediately started occurring just as the warranty expired at 36,000 miles.

Bosch abs anti skid, anti lock brake moduled failed. No brakes. Toyota us has no replacements parts available. Bosch no longer produces. Called Toyota customer service three times. Got to the management end and senior advisor told me, warranty expired, norhing we can do. I said, there are a bunch of 2000 to 2004 Toyota Avalons/Lexus 300 vehicles out there (same car except nameplates) with the same problem! same answer: Toyota, llc has no further interest in those models and warranty has expired. So, I have limited options: 1. Junk the car (funs and drives like new), 2. Start a legal proceeding (costs prohibitive), 3. Notify NHTSA. So, I (and x# of others that own this model) have a seroius safety problem. What can the NHTSA do?.

Multiple warning lights suddenly appeared on the dash while stopped at a traffic signal. (photo attached) note: the photo does not show the red "pcs" light which was flashing. I was able to get off the road safely although the brakes did not feel normal. Had the car towed to capitol Toyota in san jose the following morning for repair. They diagnosed the problem as a failure in the master cylinder booster unit and replaced it. Note: the attached invoice misstates the issue as "check engine light" which is incorrect.

My Avalon is trying to kill me. I promise I have done nothing wrong! when going down the highway at 85mph I have had people cut in front of me. When pushing on the brakes to avoid collision, the brakes lock up and do not release for about 5 seconds. Naturally, this is quite unnerving for myself, passengers as cars that scatter around me when the Avalon takes control --- slamming on the brakes --- and wont hand control back until it has caused quite a mess. This has happened three times now and no one can tell me what is wrong. I am told to get a brake job at the Toyota dealership even though the pads still have plenty of life.
I represent the family of a fatally injured rollaway victim. It is on their behalf I am reporting the circumstances of this incident and to strongly urge that NHTSA update the rollaway requirements specified in federal motor vehicle safety standard 114. On August 22, 2020, it is believed that while attempting to retrieve the key fob, the driver inadvertently bumped the gear selector forward into neutral, releasing the torque to the wheels that had kept it stationary on their sloped driveway. (unlike shifting out of the park position, which requires application of the brake pedal and depressing the lock button on the shift selector, the shift selector can be moved from drive forward into the neutral position, without a secondary interlocking action. ) the drivers wife, who was behind the vehicle. Was pinned by the car and dragged under to her death. The position of the transmission gear selector was not documented by any witnesses or first responders when the vehicle eventually came to rest on the opposite side of the road, not is it documented in the extensive data stored within the vehicle. However, the data retrieved from the subject vehicle during a subsequent inspection revealed that the vehicle control history (vch) included a field for “driver exited vehicle when shift position wasn’t p” which showed that at the time of the incident that is exactly what happened. Finally, there is ample evidence to support the need for an upgrade of the rollaway provisions within fmvss 114 – as the facts above show. It’s not enough for a safety agency to simply allow manufacturers to do so voluntarily. . Instead, the Toyota was content to collect the data and blame the driver for an event that it clearly knows occurs and has extensive information about, but fails to prevent. The public needs to rely on NHTSA to promulgate updated regulations that meet the safety needs presented to drivers of modern vehicles.

This incident has happened twice in three years. Hence it is a very rare occurrence. Both times involved the same events. Cruise control was on. I was traveling in the left lane of the interstate highway, approaching an 18-wheel truck which was in the right lane. When I was approximately 50 feet from the rear of the truck, the truck began to enter my lane. I suspect I was in the trucker's blind spot. My car did not sense the truck. The cruise control did not disengage. The brakes did not engage. But the most troublesome was that the car did not respond to my pressing on the brake pedal. I had to press 4 or 5 times before the cruise control would disengage and the brakes engage. Since I was traveling approximately 70 mph, the distance between my car and the truck (each time) closed rapidly. In the first instance, the truck tire hit my right front fender. I moved to the shoulder of the road and eventually the car braked. In the second case, I avoided collision by about 10 feet. I use the cruise control frequently and in every other circumstance, the sensors on my car have worked correctly and disengaged the cruise control and I have never had a problem with breaking. I suspect a blind spot in the sensor system but why, only in this situation, does the brake system fail to disengage the cruise control when I press on the brake is a very troublesome mystery to me. Answering question on web page: my safety - life threatening, no dealer consultation, regular maintenance schedule maintained, no warnings.
The contact owns a 2014 Toyota Avalon . The contact stated that while driving at various low speeds, light pressure was applied to the brake pedal and the vehicle inadvertently accelerated. The contact stated that there were four incidents of unexplained acceleration when the brake pedal was applied. The fourth incident resulted in a collision when the contact's wife was pulling into a parking space and applied light pressure onto the brake pedal, when the vehicle suddenly accelerated and hit another parked vehicle in the parking space ahead. No medical treatment was required. No police report was filed. The vehicle was towed to lia Toyota of rockland (618 NY-303, blauvelt, NY 10913) where a diagnostic test and a test drive was performed and informed the contact that there was nothing wrong with the vehicle. The manufacturer was notified of the failure and a case was opened. The manufacturer had a third-party test drive and diagnosis the vehicle however, the failure could not be replicated. The failure mileage was 75,000.
On every Avalon 2021 I have driven or the dealership has driven there is a thunk when braking. Every car every time. A tech case was opened at wesley chapel Toyota and Toyota of tampa bay 01- 21-2021 new car was purchased 01-13-2021.
Power sunshade is stuck in the up position blocking the back windshield and check engine light turns on which disables traction and stability control. These make the car dangerous to drive.

I was in an accident 10/30 someone hit me while driving from behind . The car I believe was in an accident prior to ownership. I have to apply the parking brake at all time no matter what position the car is in at that time. If not, the car would make a movement to where it acts like it want to jump out of the gear that the car is position in. Even if the car is in position park position if the brake is not applied the car would make a hard movement like it want to keep moving. When the parking brake is applied and the automatic transmission is still in the drive or reverse position and the stationary pto switch is engaged by the operator, the engine may rev and overcome the parking brake's ability to keep the vehicle from moving. Unexpected vehicle movement can increase the risk of a crash or injury.
On may 25th, 2020 I was pulling into a taco bell drive through. It was located up a little hill. As I drove up the hill I hit a little curbing. I backed up and adjusted the car to miss the curb. When I put the car into drive it lurched forward at a high speed. Seeing I was going to hit the car in front, I hit my brakes. The car did not stop until I had hit the car in front with a lot of force. That car was thrown into the truck in front of it. That was the beginning of a huge scare! I put the car in reverse so I could observe the damage to my car the the car in front of me. When I did so the car accelerate at full speed in reverse going back down the hill and headed straight for a drugstore. The car was making a whiring sound, like when you rev up the engine. I pushed the brakes to the floor but the car would not stop but keep going faster! I pumped the brakes some more but it would not stop. When I saw I was headed into the store, I turned the wheels to avoid it and tried braking again without stopping. I finally hit the stop engine button but not before the car hit a parked truck in the store lot which stopped it. It was the most frightening experience I have ever had. I am now afraid to get into a car. I was too upset to take photos. I will list the crash report in the file below.
On March 7, 2020 the driver entered a parking lot of a retail establishment. The driver, using the brakes and traveling at less than 5 miles per hour, turned into a parking space. As the driver negotiated the turn into the parking space, the car suddenly accelerated and crashed headfirst into a retail signage pole. Despite depressing the brakes as hard as possible, the driver could not stop the car once the car suddenly and unexpectedly accelerated. The car would have landed in a busy four-lane road, if it had not crashed into the signage pole. Fortunately, there was no fatality. The car was totaled and there was some property damage to the retail establishment. It was truly a horrific experience for the driver.

The vehicle was inspected and totled by the contact's insurance company. The dealer Toyota walnut creek 2100 n broadway, walnut creek, CA 94596 925-357-3700 stated there was nothing they could since the insurance company took over the claim and totaled the vehicle. The manufacturer contacted the contact by mail and stated that a rep would call . The contact had not been able to speak to the rep in the past three months. The failure mileage was 55,000. Tf.
On Feb. 5, 2020 at about 6:30 am, my wife was pulling up behind cars stopped at a red light, as she applied the brakes to stop, the car suddenly accelerated, she pressed the brakes as hard as she could, the car continued to drive forward hitting the car ahead of her in line. She could hear the car engine racing and tried to put the car in park, but it went into reverse and rapidly moved backwards, even with her foot on the brake, hit the car behind her. She than once again tried to up the car in park, but put it back in drive and the our car accelerated forward again fill speed hitting the car ahead for a second time, all the time holding her foot down as hard as she could on the brake. After hitting the car ahead for the second time, she pushed the engine stop button and the engine stopped running at full rpm. She noted that there the front tires were smoking alot since the tires were rotaing even though the car was at a stop, due to the engine racing full rpm. Police were called and they dispatched a tow truck for our car. The police never arrived, the tow truck loaded our car and towed it to the aaa storage yard .
The contact owns a 2018 Toyota Avalon. The contact received notification of NHTSA campaign number: 20v024000 (air bags) however, the part for the recall repair was unavailable. The contact stated the manufacturer exceeded a reasonable amount of time for the recall repair. The contact stated that while driving approximately 40 mph when another cut in front of them and in order to avoid hitting the other vehicle the contact veered to the left, the contact ended up crashing into a side barrier on the side of the road. The air bags did not deploy. A police report was filed. The contact received injuries to his left arm, leg and chest/torso area medical attention was needed. The vehicle was towed. The contact contacted the unknown dealership who diagnosed that the left side of the vehicle, steering, and braking needed to be repaired. The vehicle had received repairs to the body, steering, and brakes of the vehicle. The contact stated that the vehicle was still difficult to steer and there was a rumbling coming from underneath the vehicle whenever driving or braking the vehicle. The manufacturer was made aware of the issues, however, there was no resolve provided. The approximate failure mileage was 10,000. VIN tool confirms remedy not yet available.

Tl the contact owns a 2013 Toyota Avalon. While reversing out of the driveway, the emergency brake automatically engaged and the brake warning indicator illuminated. The failure occurred intermittently at various times and speeds. The vehicle was taken to Toyota of des moines (1650 se 37th st, grimes, IA 50111, (515) 276-4911) where it was diagnosed that the brake injunction with the left turn single failed and needed to be replaced. The vehicle was not repaired and the failure recurred. The manufacturer was not contacted. The failure mileage was approximately 82,500.
My husband, an 84 year old, was driving the car and got into a double accident. First, he was parked in a parking lot and he got in the car and backed up. As he backed up he hit a vehicle parked behind him causing that vehicle to hit two other vehicles. (please note: we are still waiting for full police report for information as to exactly how far he would have backed, but I believe not much more than 1 to 2 car lengths. ) this car had the Toyota care package installed, which included the safe backing features to slow a vehicle when backing and another object is blocking the path. My question - what happened to these safety features. My husband left that accident and drove about 1 block down the street, running the end of the yellow light and into red light, and hit the right rear of another vehicle. His vehicle then drove out-of-control into the corner planter and hit a sign, which then fell onto two vehicles. It is my understanding that after he hit the tail of the vehicle, his vehicle sped up into that planter area. Again, the vehicle is equipped with the safety features that are supposed to slow or stop the vehicle - but didn't. My husband was in the wrong - and should not have been driving. However, he was the one hurt the worst with 7 broken ribs, a broken femur, a broken nose, an injury to the spleen and bleeding on the brain. My concern is the functioning of the safety features that Toyota dealers are touting about the vehicles.
On June 7, 2019, I was driving the Avalon, the engine suddenly stopped while I was driving the vehicle on city street. With the loss of power steering and brakes, I was traumatized and did my best to guide the vehicle to side of the road. I had the vehicle towed home and hooked up to a code reader, which yielded codes p0345, p0300, p0302, p0304, p0306. We did further diagnostics and discovered the 3 bolts that hold the variable-valve timing (vvt) gear to the intake camshaft came loose, these bolts are supposed to be temper-resistant and never be removed which led me to believe the result of engine dying is caused by serious manufacturing and design defects. Please refer to attached three pictures showing 3 bolts laying next to vvt gear, vvt gear loosely held on and the vibration of bolt left cutting mark on the gear, and all intake cam rockers falling out. I did more research and concluded the root cause of engine stopped while the vehicle is being driven in our Avalon is identical to Toyota safety recall 13v-395 in Lexus vehicles 2006 to 2011. Please find attached two documents, rcrit-13v395-3180. Pdf on recent recall for Toyota safety recall 13v-395, rcdnn-13v395-1839. Pdf on defect information report from Toyota to NHTSA on Lexus intake-side variable-valve timing (vvt) system part 573. On June 27, 2019, I called stevens creek Toyota and Toyota hq and logged case #1906242897. I was informed by Toyota there's no recall for Avalon with the VIN, and the vehicle had no factory warranty (currently 153k miles), and no extended warranty with engine stall for Avalon. As a result, we need to repair the vehicle in our own expense which requires engine rebuilt as engine stall has caused severe damage to the vehicle. I was appalled by Toyota for lack of initiation to address the safety risk in our Avalon and we appreciate your thorough investigation in this matter.
I have had to replace my front brake rotors 4 times on my 2011 Avalon. . I currently have 173,800 miles with 75-80% of this mileage on the open highway. Several mechanics have indicated that this premature wear on the front rotors is highly unusual. My attempts to contact Toyota about this premature rotor wear issue have either no response or a form letter stating they have no issues with their brakes. My vehicle is serviced regularly and to me this is a very serious and potentially dangerous issue. Perhaps you can get the attention of Toyota to look into this matter.

I was driving on a highway when the car accelerated by itself, brakes did not work. The car put itself in the middle of two lanes, then accelerated turn half ,I think, crashed (the front of the car)with a small hill some how did not stop and ended up somehow turning back and crashed again this time the back of the car crashed against the hill and stopped. The car suffered several breaks in and a transmision repair. This is my third Toyota. It was perfect before the incident. It was very scary. Just a few min before the crash I was passing a toll at about 28 mph. After passing the tool I was faster than that but I do not like to speed.
On 10/25/18 I was involved in a head on collision when my gas pedal hung and lurched forward while I was pulling into a parking space at a doctor's office. I had slowed down to park when my car jumped the curb and hit the building head on. It was 3:50 in the afternoon and a clear visibility day. The police report has my speed at about 20 mph. My airbag didn't deploy, I did have my seatbelt on. It was straight front end damage to my car. As a result of the impact I am suffering from whiplash and possible (traumatic brain injury), also neck and back pain. I believe that the cause of this accident was the gas pedal �sticking� and as a result of no airbag deployment causing more motion on my head, neck, and back.
On October 1, 2018, at 8:25 a. M. I was pulling into a parking space at work in my 2016 Avalon (12,141 miles). I was �riding the brake� to pull all the way into the parking space. The car took off into landscape, crossed a road and went down a hill. I hit the parking lot and was knocked to my side. The car went back up the hill and crossed the road again, went through more landscape, bushes, etc. Back into the road again and stopped. There was over 11,000 damage to my car and I hurt my back and neck. Toyota examined the black box and said the gas pedal was being strongly depressed with no braking. That is impossible because after I hit the parking lot after coming down the embankment, my foot flew off the brake and was on nothing as I was all the way on my right side. 2nd incident yesterday, on 03/16/19, I was pulling into a gas station and was almost stopped after tapping the brake to get pulled up to where I was to stop and the exact same thing happened as my foot was on the brake, the car took off towards the gas pump. I was pressing the brake as hard as I could and was trying to grab the gear shift to put into park. I only went about 20-30 feet before the car stopped missing the gas pump by only about 1 and 1/2 feet. I can't say for sure if the brakes stopped the car or if I was able to get it into park. I had the car towed to my home and will never drive this car again. I am not sure what I am going to do because the car is far from paid for and I know Toyota will be of no help. I know for sure that eventually someone is going to be killed as I truly believe this is not just happening to me. After the first incident, I sent a certified letter to the president of Toyota and their legal department and their reply was Toyota was not at fault.
I was attempting to park my car by backing into the parking space. As I was going forward to position the car before backing up into the space, my car suddenly accelerated and I had to quickly step on the brakes before it slammed into a tree. Even normal everyday driving I have noticed that there is a surge forward while applying the accelerator. I am really concerned because I do a lot of highway driving.
