I was driving my 2024 Toyota Prius and was involved in a front-end collision. The impact was strong enough to cause significant front-end damage, but the front airbag did not deploy. I was wearing my seatbelt at the time. This is a serious safety concern because the airbag should have deployed in this type of frontal impact. If the crash had been worse, I could have been seriously injured. I believe this may be a safety defect in the airbag system.

2 Air Bag problem of the 2006 Toyota Prius

Failure Date: 09/07/2025

The contact owned a 2006 Toyota Prius. The contact stated that while slowly driving into an intersection and turning left at less than 3 mph, the owner of a Ford f-150 coming from the driver's side of the vehicle at 35 mph crashed into their driver's side door. The crash caused the vehicle to slide to a complete stop. None of the air bags deployed. The driver of the Ford f-150 stated that the sun created a glare, preventing them from noticing the red light they had driven through. The contact's vehicle was towed and deemed totaled by their insurance. A police report was filed. The contact received a broken rib, rotator cuff, whiplash, and back injuries. Vehicle: 2005 Toyota Prius VIN: [xxx] date of incident: April 2024 location: citrus heights, California incident summary: in April 2024, I was involved in a front-end collision while driving my 2005 Toyota Prius. I rear-ended another vehicle at moderate to high speed. Despite the severity of the impact, the driver-side airbag failed to deploy. As a result, I was thrown forward and hit my head on the windshield, suffering lacerations and head trauma. After being evaluated by emergency responders and undergoing a hospital visit (mri included), I was told I had not sustained any life-threatening injuries. However, since the crash, I have experienced severe cognitive symptoms, including confusion, chronic fatigue, sudden drowsiness, memory loss, and extreme mood swings. These symptoms are ongoing and have made daily life extremely difficult. I had brought the vehicle to roseville Toyota in roseville, CA, for a full pre-purchase inspection prior to the crash. They provided a carfax vehicle history report, but they did not inform me that the airbag had failed to deploy in a previous incident listed in the report. I was unaware the airbag system might be defective, and if I had known, I would not have driven or purchased the vehicle. To this day, there has been no recall issued for airbags on this vehicle, and I believe this is a dangerous and potentially fatal defect that must be investigated by NHTSA. I am asking for an investigation into why the airbag system failed, and whether other Toyota Prius models from these years have similar undisclosed issues. Supporting info: •the vehicle had dealership service history. •Toyota has documentation showing the vehicle was inspected and that I received the carfax, but they failed to alert me to the airbag defect. •I was paid out for the vehicle’s value and partial medical expenses through insurance, but no accountability has been taken for the **failed airbag information redacted pursuant to the freedom of information act (foia), 5 u. I was hit in the back by an SUV (my speed=40 mph, the SUV's speed=70 mph). As a result of the impact, my Prius flew off the road, hit the roadside railings, and then rolled over 4 times landing upside down. However, none of the airbags deployed. This includes the front, side, and curtain airbags. The vehicle was serviced at a Toyota dealership a few years ago where they performed the airbag recall. However, the airbags did not deploy at all. Only sheer luck saved me from death due to the airbag malfunction.

I was driving car on [xxx] from santa cruz towards sanjose. Suddenly airbags deployed. There was minimal to no damage to the car. My auto insurance giving me hard time. They are not responding anything that how I can fix my car plus they haven’t gave me any rental car. My claim number is [xxx]. I am too scared to drive that car again because it just got rubbed with the curve with the minimal impact but the air bags deployed. My other car usually got rubbed with curve so many times but the airbags never deploy. My car is currently at quick lube and smog shop address is 691 w tennyson rd, hayward, CA 94544. The contact stated that while parallel parking and straightening the vehicle, he tapped the accelerator pedal, and the vehicle suddenly accelerated and crashed into a stoop. Traveling approximately 15mph I collided with a full size truck the front end of my car totaled. I was wearing my seat belt and air bags did not deploy causing me to hit the steering wheel with enough force to fracture my sternum. This is the 2nd time a frontal impact occurred causing damage to the front bumper and air bags did not deploy.

The contact was driving a company-owned 2017 Toyota Prius c. The contact stated that while driving at 65 mph, another driver merging onto the highway attempted to cross multiple lanes of traffic, misjudged, and collided with the front passenger-side fender of the contact’s vehicle. As a result, the contact’s vehicle struck the left shoulder barrier, became airborne, and overturned three times before coming to rest in the middle of the roadway. Despite the severity of the crash, the air bags did not deploy. No warning lights were illuminated. The vehicle was towed to a storage facility and deemed a total loss by the employer’s insurance provider. A police report was filed, and injuries were reported. The contact was transported by emts to a local hospital and treated for multiple nondescript injuries, including a damaged right rotator cuff and body pain. The contact also underwent chest and torso x-rays due to concerns of possible internal bleeding. The manufacturer was not notified of the failure. The failure mileage was unknown.

13 Air Bag problem of the 2018 Toyota Prius

Failure Date: 02/13/2024

The airbag lights were lit, the person sitting with seatbelt on (flashing) and the person sitting with an inflated airbag in front of them. I ran an obd2 scan on srs and got code b1656. Driver seat belt buckle left hand. I investigated the latch and found one wire was snapped off and a couple of inches shorter that the wire that was still connected. I have included 2 pictures of the back end of the seat belt latch. You can see the broken wire and where it use to be connected.

There was a driver side collision and the airbags did not deploy. Brakes do not slowly apply they slam down causing vehicle to slide/skid in emergency braking, abs does not function properly. The dealership recently did an appraisal (4/2/25) on the vehicle that states “lost communication with side airbag sensor lh, side airbag sensor lh initialization incomplete, front speed sensor rh & lh circuit, and d- door motor ecu communication stop”. Insurance company inspected vehicle after crash, and there was a police report filed. There were no warning indicators prior to incident or after.

Both headlights failed, one earlier, then the second, on a cloudy night on an unlit, deserted rural stretch of interstate highway. Driver started slowing down from 60 mph, looking for a place to pull over. An accident resulted--only the defective car was involved. No airbag was deployed. The driver was injured. The lights spontaneously turned off while driving on a cloudy night on an unlit section of interstate. While slowing the car down, it went onto gravel, semi-circled around and flipped over onto the lower median. No air bag deployed. No protection from shoulder seat belt. Head hit steering wheel, and I briefly lost consciousness. Tests showed skull and cervical fractures. The car was "totaled. " I was planning a move. Now I consider myself to be lucky to be alive. I am rewriting because of previous a NHTSA computer malfunction changing a serious injury to "no" injury on the questionnaire.

I had lost functionality for some buttons on the steering wheel that didn't directly impact safety. On replacing the clockspring mechanism inside the whweel I decided to inspect the old unit. Four of the lines in the flex cable are impacted by corrosion, three have already failed. The fourth line is one of the airbag's electrical lines. I'm filing an issue report because the airbag would be next impacted by this failure and I believe there's a fair chance the corroded line could fail during the vehicle's attempt to fire the airbag on the wheel.

The airbag sensors for my passenger seat do not work. Regardless of if there is someone sitting in the passenger seat, buckled it, the system indicates that there is no one sitting in the seat and that they are not buckled in. This is of course a safety issue since the airbags would likely not go off even if someone was sitting in the seat. My vehicle is available for inspection upon request. They first appeared shortly after I purchased the vehicle from the last owner. The vehicle was lightly used and is very clean, and I have no reason to believe the passneger seat was abused in any way. My concern is this: this airbag sensor issue is a recall I have seen for other 2010 Toyota Priuses. I understand that in the systems, this recall does not show up for my car. However, I think it's interesting and worth looking into that I have the same make, model, and year, and my vehicle is having the exact same issues. This is an issue that was not triggered by any crash/collision, and I did not have anyone even sitting in my passenger seat prior to this issue, so it seems to be purely a system issue that I could not have prevented myself.

My husband and I rented a car from avis on June 19, 2023. The car they we selected was not available so they picked a Prius out of the lot and gave it to us. On the morning of June 20, I was slowly turning into the driveway at 8812 via tuscany, my foot on the brake the whole time to slow to a stop so I could park parallel to my realtor's car. My realtor, jamie garber, witnessed the slow speed, as did many of the neighbors, as it was a quiet residential street. I continued to depress the brake but instead of stopping, our rental car, a Toyota Prius, suddenly lurched forward through the garage door! at no time did I lift my foot from the brake from the time I slowed the vehicle to enter the driveway. My husband can attest to this. In fact, he told me he was about to remove his seat belt, as he expected we would come to a stop. The car ended up crashing through the back wall of the garage causing extensive damage. The air bags did not deploy either. I cannot explain why the vehicle would suddenly accelerate while my foot remained on the brake. I am now being sued by the homeowners insurance company. Fortunately no one was hurt nor killed. I filed a claim initially with avis two days later and asked for a report or dcm download of the brake system. Their ins. Co. Is named sedgwick and I have followed up with their reps and no one can tell me if the brakes were checked and what was found. I have never been in a crash in my life and this is extremely upsetting. 25 Air Bag problem of the 2011 Toyota Prius

On Friday June 9, 2023 we drove the 2011 Prius on our daily travels. This was uneventful. Parked the vehicle around 10pm that same night. We drove it on the following Saturday around noon on a very short errand, maybe 30 minutes. Parked it again a little after noon. On Sunday morning around 730am, we discovered the side curtain airbag on the passenger side was deployed. There in no external damage and the vehicle was still locked. Upon starting it, the airbag systems check light illuminated for about three seconds then went out. Although the airbag is currently deployed, the airbag dash light does not stay lit indicating a problem. In order to determine if this was a soft code, we started it, drove it around the block, and shut it off. We did this a few times. The airbag light always lit for about three seconds then went out. The airbag has "pa66, vmq" stamped on it.

The second vehicle's rear driver's side passenger was later treated for an undisclosed injury. A police report was taken at the scene and the vehicle was towed away. Abs system displayed airbag symbol displayed brake symbol displayed seat belts light flashing airbag system showing off. Toyota dealership is aware of the problem with the abs system along with the items mentioned. Accident could have been avoided contact if the abs system was working. Airbags did not deploy. Advise Toyota corporation on 27 February 2023 file number 230-227-00 2210 in regards to the abs they were also advised of the airbag symbol displayed in the seat belt fasten flashing on driver side when an individual is seated in the car and buckled in. Vehicle has not been inspected yet however on Monday 6th of March it will be inspected by copart, by the direction of the usaa insurance company.

The driver airbag deployed without an accident while driving in the highway. The airbag deployment caused near control lost of the car. I was lucky to not have any other cars near me so I managed to get the car safely out of the highway. The airbag deployment was scary and caused me not able to see through the windshield due to the gas from the airbag.
