On sept 9th, 2022, I was involved in a collision, traveling approximately 30 - 40 mph and struck a vehicle that pulled into my path. I struck the other vehicle perpendicularly (t-boned). My vehicle impacted on the front, initially, and then on the driver's side at full force. The result of the accident caused the front bumper to be damaged and the entire left side of the car was crushed. The force of the accident caused the driver's window and port-hole to shatter. The air bags did not deploy. None of the air bags deployed. The vehicle is still in the auto repair lot and has yet to be inspected by anyone. There we no prior indicators of any problems with the airbag system.

Air bags failed to deploy in car crash: my 2010 Prius was stuck by another car who ran a red light, in the front right (passenger side). The car that struck me was traveling approx 35mph and pushed my car (also traveling approx. 35mph) into the path of a wooden telephone pole that suspended the traffic lights over the intersection. My car traveled about 20-30 feet further and stopped when my car hit the pole head on. The telephone pole broke in two in the impact. None of my air-bags deployed. The police report is incorrect regarding the airbags. I have contacted the police department to amend the report. As seen in the accompanying photographs, the air bags did not deploy, contrary to the police report.
My wife was driving our 2014 Toyota Prius eastbound on broadway rd. , approaching sossaman rd. , in mesa, az, on Friday December 13, 2019 at about 10:00 am. Weather conditions were clear and dry. She was alone in the vehicle, wearing her seat belt, traveling at 20 mph in a construction zone, wanting to turn southbound on sossaman rd. , some 200 yards further east. Construction crews had removed a portion of the roadway, creating a ditch, about 8 feet long in the direction of travel, about 10 to 12 inches deep and about two lanes wide. After passing a construction vehicle, she began her turn to the right and ran into the ditch. The front wheels of the Prius dropped into the ditch and proceeded across the opening some 8 feet, at a slight angle, and impacted the concrete roadway on the other side, causing a sudden and violent deceleration. Due to the relatively shallow ditch, and the low ground clearance of the Prius, the car rode up on the concrete and the brunt of the impact was absorbed by the undercarriage. There was some fascia and bumper damage. The wheels did not impact the concrete barrier. The driver's side airbag did not deploy. I filed a claim with Toyota, and they agreed to have the vehicle's event data recorder downloaded. Analysis of the event data recorder information, provided by Toyota, shows a total time of 123 milliseconds for the vehicle to reach a complete stop from the point of initial impact, with a maximum longitudinal deceleration of -14g's (my calculation. ) my wife suffered severe neck injuries. Toyota sent me a letter saying the airbag system operated as designed and was not at fault (my summary. ) I don't see how that can be, due to the extent and nature of my wife's injuries. Or, Toyota's design criteria are at fault and did not anticipate this type of collision.
Tl-the contact owns a 2013 Toyota Prius v. The contact stated that while driving at approximately 25 mph, the contact crashed into the rear of a pickup truck. The contact stated that the air bag failed to deploy. A police report was filed. There was no injuries sustained. The vehicle was towed to an independent mechanic. The vehicle was not taken to be diagnosed nor repaired. A local dealer was not contacted however, the manufacturer was contacted and informed of the failure. The approximate failure mileage was 27000. Ah.
I was turning left and was hit by an oncoming car. Most damage done to the front right. The car was totaled. I was going about 5-10mph though the oncoming car was going approximately 40mph. The firemen, police and insurance adjuster were all very concerned that the airbags did not deploy. My babies were safe in the backseat thanks to their car seats. I had to go to the hospital and suffered a concussion. We just had our car tuned up at our local Toyota dealership not even 6 mos ago. They did not say anything about a concern with the airbags.

Air bags failed to deploy after being rear ended straight on at 55 mph, not a single airbag deployed.
My vehicle was rear-ended and shoved into a third vehicle. We were on an interstate highway with wet pavement, and traffic was slowing. We were traveling straight. The airbags did not deploy, and my wife, belted in the front passenger seat, was twisted sideways by the impact. She experienced a pelvic fracture and aggravation of her arthritic left hip joint. We believe that airbag deployment would have prevented these injuries. I have attached three event data files and vehicle control history files. (I can't see if all three are attached. ).

Takata recall possibly. Wet pavement, traffic slowed on interstate to about 45 mph, tailgating small pickup hit my rear crumple zone, shoved me into a small SUV that crushed my front crumple zone. Airbags did not deploy. Neither my wife (front seat passenger) nor I felt any discomfort, but a few days later she complained about pain in her waist area. She was later determined to have a fractured pelvis and a jostled left hip joint.
Tl the contact owned a 2011 Toyota Prius. While driving 40 mph, the vehicle lost power without warning. As a result, the contact crashed into a tree and a cement barrier. The air bag failed to deploy. The vehicle was restarted and was able to be driven to the contact's residence. The contact sustained back, neck, and chest injuries that did not require medical attention. A police report was not filed. The vehicle was not taken to a dealer or independent mechanic for diagnostic testing. The vehicle was later deemed destroyed. The contact stated that the VIN was included in NHTSA campaign number: 18v684000 (hybrid propulsion system). The manufacturer was notified of the failure and provided a case number. The approximate failure mileage was 72,000.
I was driving down the road going roughly 40-35 miles per hour when the vehicle in front of me slammed on their breaks. I collided with the vehicle in front of me, we came to an abrupt stop. The airbags did not deploy, causing my face to slam against the steering wheel. Leading to facial and teeth injuries. My car frame was bent, it is totaled.
Tl the contact owns a 2017 Toyota Prius. While the brake pedal was depressed, the rear wheels seized. As a result, the contact lost control of the vehicle and crashed into a ditch. The air bags failed to deploy. A police report was filed. There were no injuries. The vehicle was towed to a body shop and the body damages were repaired so that the vehicle could be driven. The vehicle was taken to kalispell Toyota (2845 u. S. 93 s, kalispell, mt 59901) for diagnostic testing, but the cause of the failure was undetermined. The manufacturer was notified. The approximate failure mileage was 7,000.

My 2017 Prius v was involved in 2 recent crashes in which the air bags did not deploy: front end/bumper impact on 6/1/18 and front & rear end impact which totaled car on 7/3/18. Both times air bags did not deploy. Impact on 6/1/18 caused other car's (2008 Honda accord) air bags to deploy, but not mine. Toyota sent inspector after 6/1 crash; report enclosed. 7/3 crash was even more severe traveling at 61 mph when impacted in the rear by another vehicle traveling at excessive speed. My car sustained front and rear damage after impact by other car in the rear and impact in the front into highway center cement divider. My car was totaled, but still air bags did not deploy. I am concerned there may be a mfr defect in the Prius v air bag system similar to the other Prius air bag recall required by you in January 2018. Please investigate further. I have bought another car, and am recovering from minor bruising injuries. I do not plan any legal action. I am mainly concerned about the safety for other Prius v drivers. Thank you.
Struck a car at approx 50 mph spun then crashed into a tree dead center of car. Air bags did not deploy.

2011 Toyota Prius. Consumer writes in regard airbag not deploying after vehicle crashed and safety recall no 18v-684.
2013 Prius v had a collision with a mustang converible. Both vehicles are totaled. The mustang's airbags deployed. The Prius's airbags did not deploy and the driver has a severe concussion. Ems apparently told the hospital this was the sending one they had seen where airbags didn't deploy. The hospital told me I needed to look into airbags (3xs) based on what ems said. When I went to Toyota they asked if I was the one that had called a couple of days prior due to airbags failing. (no I had not called. ) so, was ems referring to a second Toyota where airbags had not deployed in 2 days? please look at pictures. This was a serious accident. Police report states both vehicles doing 35mph. Again, Prius airbags didn't deploy.
Tl the contact owns a 2015 Toyota Prius. While driving approximately 40 mph, the contact crashed into the front passenger side of another vehicle. The air bag did not deploy. A police report was filed. The contact sustained injuries that required medical attention. The vehicle was damaged and towed to a body shop where it was repaired. The dealer was not called. The manufacturer was made aware of the failure. The cause of the failure was not determined. The failure mileage was approximately 37,000.
Airbag warning light illuminates. Toyota service techie's diagnostic showed a disconnected spiral cable. Techie explained that the spiral cable has electrical connections to the driver's airbag module and the driver's air bag would not deploy in an accident. He said I should install a new cable spiral assembly kit (aka "clock spring"), part # 8430747020 at a cost of nearly $1000, including over $700 for the part. I did some research. Part # 8430747020 was installed in many Toyota vehicles, and has been recalled for the same problem in the corolla, matrix, highlander, tacoma, rav4, and yaris. See safety recall e0m (e1m) April 2015, update 5/12/14. The Prius is not mentioned in the recall, although my car has the same exact problem with the identical part, and the Toyota service supervisor agrees it is the same problem and part. I explained this to the service technician and supervisor, who called Toyota, and Toyota offered to cover 50% of the cost of the part, and none of the labor cost. I asked whether they could do the same fix in my car as they do on the recalled vehicles, namely installing part # 04004-07122, and they declined. See technical instructions for safety recall e0m spiral cable replacement. Toyota should be recalling my car and fixing this problem at their expense, not just reducing the cost of the part. Second, they should not be replacing this defective part with the same defective part. They should do a fix that will last. I assume the fix they've done on all other vehicles, with part # 04004-07122, would work for my Prius. They refuse. Fyi- the airbag light has been on and off for years, and seen by Toyota during that period. This particular problem was diagnosed may 1, 2017. They believe the previous airbag warning lights were unrelated to this particular issue.
The contact owned a 2010 Toyota Prius. While driving approximately 30-40 mph, the contact crashed into the rear of a police vehicle. The air bags did not deploy. The contact's chest and face struck the steering wheel. The contact received injuries to the clavicle area, shoulders, face, eyes, head/brain, and neck area. Extensive medical attention was needed. The vehicle was not towed; however, a police report was filed. The vehicle was taken to the dealer where it was diagnosed that the air bag sensor was missed by a couple of millimeters, which prevented them from deploying. The vehicle was deemed totaled. The manufacturer was not made aware of the failure. The VIN and failure mileage were unknown.
I was involved in an accident in which I rear-ended the car in front of me on a almost empty city street. The car in front of me stopped abruptly and was stationary at the time of the collision. I was going approximately 25-30 mph at the time of impact, as I did not have the opportunity to brake effectively. My car was totaled in the collision. I suffered bruising to my sternum and knees as well as severe bruising on my knees and hip. My airbags did not deploy, and this was remarked upon by the firefighters, paramedics and police that responded to the accident. I am concerned because this is the second Toyota I have been in an accident with in which the airbags did not deploy. The last accident was in a 1996 Toyota corolla in a high speed accident on the freeway in which my car had locked onto the car that had hit me and we spun out on the freeway. Again my car was totaled and no airbag deployed. After doing research on the internet, it would seem this is a continuing problem with Toyotas, which should be remedied as a matter of public safety.
The car was involved in a moving collision that totaled the drivers front end and caused the driver to be injured because the air bags failed to deploy. I suspect that the cars steering failed while going around a sharp curve. The car has lost steering one time before and was taken to the dealer who could not duplicate the problem.
The contact owns a 2006 Toyota Prius. While driving 55 mph, the contact rear ended another vehicle. The air bags failed to deploy. A police report was filed and there were no injuries. The vehicle was towed. The manufacturer was not notified. The failure mileage was 135,000.
