The high pressure fuel pump “grenaded” while driving on the interstate. Caused an immediate shutdown and barely got off the main lane to the shoulder in extremely heavy traffic putting me, my wife and others in imminent danger. The components were inspected by the dealer mechanic and insurance adjuster and they said that is what happened and that there was no reason for it to have happened. A warning of reduced power appeared about 3 seconds before the truck completely shutdown.

Engine started pouring fuel out in large quantities coating the entire chassis. I was lucky, I had planned to go fishing early but had boat issue. Had I gone early in the dark it would have been catastrophic. Large amounts of gas. Pouring out. I'm lucky, when moving the truck my wife noticed the fuel was leaving a trail down the road and started screaming. I immediately parked the vehicle and I am waiting on support to get to dealer. The fuel system is a known defect on 2025/2026 silverado but not yet identified for this hd 2500 with same 6. 6l engine. Entire cab is full of gas fumes. This was extremely scary.

My pickup truck has a duramax diesel engine with a cp4 diesel injection pump. That pump has not failed yet in my truck. However it has been known to fail in numerous other gm vehicles with duramax engines and when it does fail the engine comes to a complete stop and cannot be restarted. This is a potentially hazardous even fatal situation. Repair if the pump fails is very expensive - $12,000 to $14,000 in some cases. I would like gm to replace the injection pump at my dealership to ensure this known failure does not occur. My pickup truck is otherwise in excellent condition.

My 2024 Chevrolet 2500hd duramax experienced a forced emissions derate event with a speed-limited countdown. The vehicle suddenly lost normal operating capability and became unsafe and undrivable. The truck was disabled in enid, oklahoma and had to be recovered and transported to a dealership. Prior to failure, the vehicle showed abnormal def/emissions behavior. The failure involves the scr/def emissions system. I later confirmed this vehicle has a completed product emissions recall for scr fault detection. Despite that recall, the emissions system failed and triggered a derate and loss-of-mobility event. The truck is currently at classic Chevrolet in owasso, ok for diagnosis.

On February 3, 2026, my vehicle was taken to rick hendrick Chevrolet of buford for diagnosis of a fuel system failure. The dealership charged a $235 diagnostic fee and concluded that the failure was caused by fuel contamination from an outside source. Based on this determination, my warranty claim with general motors was denied and my case (#89096714) was closed. After this determination, the vehicle was transported to an independent diesel repair shop for further inspection. During the initial inspection, the technician removed the intake cover and discovered a large crack in the fuel injector pump. The pump had catastrophically failed and internally disintegrated, sending metal fragments throughout the fuel system. This clearly indicates the contamination originated from an internal mechanical failure of the fuel injector pump, not from outside fuel contamination. The independent technician confirmed that the injector pump failure caused metal debris to spread throughout the fuel system. Additionally, there was a visible leak at the front of the engine, which should have prompted further inspection in that area during the dealership’s diagnosis. It appears the dealership’s conclusion was based primarily on fuel sampling, without performing a full mechanical inspection of the injection pump and related components.
First issue was around April-may 2025 when my engine started knocking and burning fuel. Warning light came on and it went right in to the dealer where I have all my maintenance done. They determined that it due to (2) faulty fuel injectors and I had them replaced at that time. In December of 2025 my engine started doing the same thing as before with the knocking and burning fuel and reduced speeds when on the highway so again it went into the dealer. It has since been in there and first was told it was the fuel injector again. They priced out the one was bad and I asked them to replace the remaining 6 fuel injectors so I didn't have this issue again in the near future. They did and the vehicle had the same issues going on. During the next inspection it was determined that the engine needed new rocker arms. These were replaced but this also did not solve the problem and the vehicle would not pass the regeneration on the engine. During the next inspection it was determined that we needed to replace a cylinder and one of the valves. After this was complete they were still having issues and then I was told that the piston was bad and there are (2) bad valves so I need an entire engine replacement. My truck only has 130,000 miles and has been in the shop for almost 2 months last year from engine issues and it has had all maintenance completed at he same dealership with records.

I own a 2021 Chevrolet Silverado 2500hd duramax diesel with approximately 100,300 miles. The truck began displaying a “service emissions system” warning and a countdown message that in a certain number of miles the vehicle will be limited in speed (for example to 65 mph). Gm’s own duramax diesel owner’s supplement explains that when a “service emission system” message appears it is accompanied by a distance-until-speed-limit countdown and that the truck will progressively limit speed first to about 65 mph and then to about 55 mph if the condition is not corrected, telling the owner to see the dealer. This creates a safety risk because the derate can occur while driving on highways or rural roads, where an unexpected limit on maximum speed can interfere with merging, passing, and keeping up with traffic. This is our only vehicle, and we must use it for important medical appointments, including cardiology and surgery-related visits. The condition started only about 300 miles beyond gm’s 5-year / 100,000-mile powertrain warranty. I opened a case with Chevrolet / gm corporate (service request no. Xxx) and took the truck to sons Chevrolet in columbus, georgia on November 17, 2025 for this issue and a rear wheel lockup recall. The dealer kept the truck from about 9:00 a. M. To 3:00 p. M. And indicated they might need to keep it for at least two more days, but I was not given a clear written explanation of the diagnostic trouble codes or a firm repair plan. Because gm’s own manuals describe this “service emission system” message together with speed-limit countdowns, and because the vehicle can actively limit speed on the road if the fault is not resolved, I am concerned this represents a broader emissions/derate defect on late-model silverado hd duramax trucks. The truck randomly shuts off whether just running or driving, the fuel pump fuse blows, as long as we switch the fuses it starts right up and runs again, sometimes it happens several times a day, other times it doesn't happen for a week or two. When driving it shuts down without warning and lose of everything.

While driving on the freeway in a construction zone, I smelled a burning oil smell. I considered the possibility it was in relation to road repairs being done (roads being repaved, ie tar resurfacing). I was cut off by another vehicle and depressed my brakes and I saw a poof of smoke over the front passenger area near tire. I noticed all of my gauges were in the normal ranges (temperature, battery voltage, etc. ) in less than a minute my truck went into limp mode and advised it had a def system issue and it would be limited to a set mph and a set number of miles. Check engine light came on and another driver alerted me that the underneath of my vehicle was on fire. I pulled over to the side of the highway and my truck became engulfed in flames in. Truck is under factory warranty, unmodified, never been in an accident and has had all recommended services performed at local dealership. There was a significant amount of traffic and luckily I did not collide with anyone or set neighboring dry grass field on fire. Fire department responded and extinguished the fire, but it is a total loss. Insurance company has been notified, and vehicle will be available for inspection by appropriate agencies. While local law enforcement did respond, they did not take a report because I didn't hit anything, and the only property damage was my own. (there is documentation of the event, but just dispatch notes. ) prior to the situation above the truck had been at operating temperature for at least 20 minutes and I had traveled for about 20 miles on the highway. The total distance traveled from when I smelled the burning smell was maybe 1. 5 miles.

The fuel pump inside the tank shorted out and left us stranded. This is the same pump that was used on the the 2022 duramax systems however they upped the fuel pressure on the 2024 refresh and it has become a known problem of shorting out.
With 19,000 miles on my Silverado 2500 hd I was traveling down a hill towards a stop sign at a t intersection. About 100 yeads from the stop sign I had many flashing errors on the dash, very little brake and very little steering. The engine just shot off. Fortunately I was able to stop the vehicle right at the stop sign while traffic from right and left had no stop sign. The truck would not start after. It did have electrical power. I tow was needed. The next day the chevy dealer did find a bulletin for a wire in the engine compartment chaffed. Symptoms were the same as what I experienced. Repair was made. Bulletin #24-na-124. There is no doubt a bulletin should be issued but I believe this is a real safety issue with the engine just randomly shutting off leaving no power to brakes and steering. Had this happened while I was towing our camper I would not have stopped and t boned another vehicle causing damage and no doubt injury to others and myself. If traveling on an interstate at 70 mph this could have been even worst. This is a known problem and should be a recall and not just a bulletin. Thanks.
Driving down the road in heavy traffic and had complete engine failure due to loss of fuel. Diagnosed as fuel control module. I have seen where gm recalls have been made for similar issues. This put me in danger of collision. Truck is at the dealer and will cost $957. 00.

2 low pressure fuel pumps have failed. Truck stalls in traffic. Currently in show with extremely large bill for repair.
I was driving my truck and lost full power to the truck. Which happened as I was pulling out of a parking. The truck went into a limp mode. With the truck losing full power, it put my child and I into a safety risk. The truck got repaired at the dealership. The dealer was able to reproduce the issue. The truck was inspected at the dealership. Warning system did come on saying speed would be limited if (roughly 150 miles to a max 65mph). When I called another dealership, they said this was a normal thing for these trucks and chevy hasn't fixed it. I feel like if its a known issue with the wiring becoming free, why would they not proactively fix this issue before someone gets hurts from thinking they have the power to drive. The dealership that I took this too knew where to look right away as this is common.

Def system failure causing truck to enter limp mode being reduced to 65 mph, then 55 mph, and then restricted to 5 mph. Error in def system led to def heater to go bad causing the incident to occur.
The def system and turbo system. It will not let me tow nothing. And has started limited my speed to 65mph and says in 55 miles it will restrict to 55mph. I need my truck for work.
The def heater short circuit and derates the truck. If the government says we have to have this they should cover the cost to fix it.
Vehicle loses power when entering roadway. Vehicle fires at idle. Vehicle dies on roadway. Vehicle lurches aggressively without control. Vehicle shifts erratically and with aggression. Dealer stated gm known issue where wires were run to the oxygen sensor wrong. Run across exhaust pipe. Exhaust melted wires to O2 and caused a short which computer poorly responds to.
Dash warning: check engine light, "engine power is reduced!" and traction control. Code reader results: p2138 (app) accelerator pedal position sensors - not plausible. Replaced with new oem part(s) - app, electrical throttle body and m15 fuse - which did not fix issues and now due to safety concerns (further damage to vehicle and personal preservation) will be towing to dealership for diagnosis. Believe to be a known wiring issues most likely due to water exposure/ short in connectors. - see ip, ecm/pcm and possible false activation to the app circuit. This should be a recall due to the severity of mechanical issues of the malfunctioning system.
Engine suddenly stopped running while driving and could not be restarted. It was towed to a local diesel repair shop. Diagnostics showed a catastrophic failure of the high pressure fuel pump which polluted the complete fuel system with metal.

On [xxx] my 2015 chevy 2500 crew cab ltz duramax with 50,959 miles stopped running going down a large hill, barely made it off the road coasting into a fire department parking lot. 2nd failure occurred while driving on a main route in wv. At 66,600 miles [xxx], was almost rear ended because truck again just stopped running going 50 mph around a turn. Diesel fuel was pouring onto the hot engine second occurrence. I don't know how the truck didn't catch on fire. Thank [xxx] I wasn't towing my 14,500 lb fifth wheel either time. Both incidents were because of a cp4 injection pump failure completely taking out and polluting the entire fuel system. Per gm service department in wintersville, OH. I no longer feel safe driving the truck and wouldn't even consider towing with it again. Too dangerous for me and my family !! Vehicle stalled then would not start.
The fuel tank vent got plugged, which collapsed the fuel tank, damaged the fuel pump causing the truck to stall at highway speeds on the highway in traffic. Gm certified mechanic at the dealership confirmed and mentioned that he had already repaired 4 trucks with this issue in rochester, mn. Prior to the failure, the fuel gauge was inaccurate and a few days later the check engine came in at which time we scheduled an appointment with the mechanic.
The fuel economy ump module is not working. The dealership said this is happening to all GMC models and it is a nation wide back order. Some dealerships have had vehicles sitting for 3+months and GMC will not do a recall. The expected fix date is “hopefully” February 2025.
Engine shut off while driving on the freeway (70mph), lost steering and braking capacity. Lucky to make it to shoulder, the failure is the fuel pump control module, second time(first failure at 48k miles, second at 98k miles) this has failed (part # 13558813). Once this fails part is impossible to find. Repair technicians at dealer says this is a very common failure. To my knowledge part has not been inspected. Dealer is not an option for repair due to the length of time they will keep vehicle (in key west was going to ve 3 months before they could look at it,) this failure the local dealer in athens GA, did not have a diesel mechanic to repair (not sure why this needed a diesel mechanic), warning lights/message is : reduced engine power. . . . But did not say why reduced, engine just slowed and shut down in very heavy traffic.

Cp4 fuel pump caused the vehicle to shut down and stopped running.
While hauling a trailer I received a warning tone and looked down to read the message, "reduced engine power", and the service engine light appeared all at the same time. The engine shutoff at the same time I received the warnings and didn't allow me to safely pull off the road. . I lost power steering, power braking, and controlling the vehicle and trailer while coasting to the sholder was difficult. Fortunately it was early in the morning and I was only traveling 35 mph on a straight road, and no other vehicles were on the road. If I was on an interstate traveling 55+ mph or on a curvy road it would have caused an accident or worse. I was very fortunate. This was the first time I experienced this problem and hadn't received prior warning messages or lights. The truck would not restart and the truck and trailer were towed to the dealership. The diagnosis was a failed "fuel pump control module. " after waiting several weeks the replacement fuel pump control module did solved the problem. I kept the failed fuel pump control module and is available for inspection. Please investigate thoroughly! I was extremely fortunate the part failed when it did and I was able to walk away unharmed.
Complete engine and emission fail while driving. Complete loss of power. Created a serious safety issue while driving due to other traffic and inability to move vehicle from road side to safe area. Problem was confirmed by mechanic. Contact to GMC resulted in no response. Mechanic has reported numerous other GMC trucks with the same issue. Check engine light came on shortly before and then went off again. Contact was made with dealer as well as GMC directly regarding the issue.
The def system does not provide a level indication. Whenever the def is filled it always results in a def spill to the environment since the operator does not know whether to add 5 gallons or one gallon.
The def fill port is not vented and provides a tight fit with the def dispensing hose and nozzle at a truck filling station. The def dispensing nozzle cannot be placed entirely in the port since it pressurizes the def tank and causes block back spray on the operator. Filling the def tank at a truck filling station always results in def spillage regardless of the filling technique employed.
The def fill tube is under the hood on the opposite side of the fuel filling tube. To fill the def at a truck stop, one has to park really close to the pumps and stretch the hose across a hot engine. One has to be careful that hot engine does not burn a hole in the def hose and cause a spill.
Lack of proper venting of the def holding tank causes tank to buckle and expand resulting in very loud metal banging sounds as vehicle travels between high and low elevations. For example driving from flagstaff az (el. 7000) to phoenix az (el 1600) results in loud metal noises from the tank as it partially implodes due to change in pressure. Should the def tank develop cracks the result is a leak in def to the environment.
Fuel fitting break and leak fuel everywhere I have had two of these trucks that have done this and it has made my animals sick.
