Failure Date: 07/24/2025

The contact owned a 2021 Chrysler Pacifica. The contact stated that occasionally, while driving at various speeds, upon depressing the brake pedal, the vehicle would surge forward 3 to 4 feet before proceeding to slow down or come to a complete stop. No warning lights were illuminated. The vehicle was taken to a local dealer, where it was diagnosed, and the contact was advised that the failure was not duplicatable and nothing was found to be wrong with the vehicle. The local dealer proceeded to replace the spark plugs, the brake fluid, field spark plug weld, and gasket cylinder head cover, however the contact stated that after the repair the failure increasingly persisted leading them to be involved in a crash where the contact stated that while driving on a highway at 55 mph, there were two vehicles stopped ahead, the contact depressed the brake pedal and the vehicle proceeded to surge forward and crash into the vehicle infront of them with the entire front end of their vehicle. The impact caused the vehicle in front of the contact to crash into the rear end of the vehicle in front. That vehicle sustained damage to the entire rear and front end of the vehicle as the impact caused that vehicle to crash into the rear end of the vehicle in front of it. No property damage was reported. The vehicles came to a stop upon rebound after the impact. The air bags were deployed. The vehicle was towed to an auto body center, and no warning lights were illuminated. A police report was filed. The vehicle was destroyed. The contact was injured while seated in the driver's seat; the passenger in their vehicle was not injured. An individual in the vehicle in front of the contact was injured. The contact sustained injuries to their ribs and sternum. Medical treatment was needed for the injuries, and the other individual also required medical treatment for unknown injuries. The individual at the front of the line in this collision was not injured. The local dealer was contacted however, the vehicle was not diagnosed as it was decla.

My son was driving the vehicle down the interstate and attempted to exit. He hit the brakes and the car would not slow down. He had the pedal all the way to the floor and the vehicle still would not slow down. The vehicle went off the exit ramp, through a grassy hill area, across the entrance ramp, and finally stopped when the tire broke off. Even with the tire broke off and the vehicle not moving the engine was showing it was going 30-40 mph. No airbags were deployed. I received a luckily the only injury was my daughter's sprained wrist. The entire front end of the vehicle has been replaced, including the frame, and yet no airbags were deployed. I did receive and abs failure alert on my app but was told that was because of the tire breaking off and not what caused the accident. I have requested the vehicle be inspected since the accident occurred. Insurance stated it would need to be repaired before an inspection could occur. I have filed a case with stellantis who now states they will not have anyone look at it because it has to be done before repairs are made and since insurance has paid for the repairs, they have to request an inspection. We had multiple issues with the tpm system not working properly and had it to a dealership for repairs, that was the only warnings we received. The vehicle is still sitting at the dealership/collision repair shop as they feel it warrants further investigation as well.

The brake pedal felt spongy and did not engage without me pumping the brakes at a traffic light. I had to quickly change lanes to get the brakes to engage. Otherwise I would have crashed into the vehicle in front of me. Also sometimes when sitting at a traffic light the brake pedal pulls away at random as if it is disengaging which cause the car to roll forward. I have to again pump the brake pedal. This issue has occurred 2-3 times so far. I took the van to two different auto repair shops. One shop said it’s the brake booster. I got a second opinion at another mechanic shop and he did not find a problem with the car and could not recreate the problem. The second mechanic indicated it is most likely a Chrysler specific problem and the car should be taken back to the dealer. As the car needs to stop when the brakes are depressed as it could cause an auto accident. I was advised to check to see if there is already a recall relating to the abs brakes system for the Chrysler Pacifica. I also mentioned the electronic stability control service light is coming on the dashboard display at times.

When driving on hybrid mode with battery charged up suddenly warning message showing “transmission failure” and message saying “stop safely vehicle is going to shut off soon”. Breaks don’t work as normal when this happens and vehicle wouldn’t stop immediately. Feels unsafe to drive. Engine warning lights turn on and stay for a day after this happens. Checked with dealership and they mentioned recall is still being worked upon and not yet available. Tried reaching mopar directly but no response yet.

I am writing to report another incident related to my 2017 Chrysler Pacifica. Today, while driving on a major highway, my vehicle unexpectedly shut off and braked violently multiple times without warning, creating a serious safety hazard. Fortunately, there was no accident, but due to the sudden and forceful braking, I am now experiencing pain in my head, neck, upper back, and shoulders. I previously reported electrical issues with this vehicle on January 22, 2024. Given the recurring nature of these problems, I am extremely concerned about the ongoing safety risks. Please advise on the next steps to formally document this incident and any updates regarding similar reports or investigations into this issue. The contact owns a 2017 Chrysler Pacifica. The contact stated that while driving at an undisclosed speed and attempting to slow the vehicle, with the brake pedal depressed all the way to the floorboard the vehicle failed to stop as intended. As a result, the contact had to go through a stop sign eventually coasting to a complete stop. No warning lights were illuminated. The vehicle was taken to an independent mechanic, who determined that the brakes had seized. The contact was also informed that the front end struts needed to be replaced. The manufacturer was not notified of the failure. The VIN was not available. The failure mileage was 140,000.

Hello. Writing to you about a serious safety concern on my 2023 Chrysler Pacifica hybrid. Overnight, I had it parked in my driveway, and this morning, I found it had rolled down the driveway. In addition, because it was plugged in charging overnight, the movement of my van rolling, caused it to pull on the cord and ultimately snapping my charging port. Fortunately,no one was walking at that time as you could understand the concern this would be if this had been the case and my van would’ve harmed someone. I checked my cameras, and it shows the van beginning to slowly roll on its own around 9:40pm until it reaches the end of my driveway. I believe the only reason it did not go any further is because our sidewalk is a bit raised where it meets the end of the driveway, which stopped it from sliding even further than it did. I would like to see if this is a known issue and if not make it aware as this is a serious concern to the public. Also what steps can I take to Chrysler to fix this issue, I don’t feel comfortable now with the car sitting idle in the driveway with kids playing. Also corrected steps to repair the damage to my car. I contacted the dealer and have an appointment scheduled for Friday 12/27/2024. My car will start, I will start to drive and when I am driving a “electrical stability” label will pop up on the dash and then the battery icon and check engine icon will show up on dash. My radio screen will go blank and then the car will shut off completely. It will then pop up on the dash and say stop safely vehicle with shut pff soon.

On November the 17th of 2024 this problem occurred for the third time in the past 90 days. I purchased this vehicle approximately 90 days ago so that means that has happened three times within the past 90 days. As I step on the brake pedal to stop the car accelerates and lunges forward. The first time this happened I was involved in an incident. The third time it happened took me through an intersection but I was wanting to turn left but because of the excessive acceleration lunging forward scenario I could not slow down the vehicle enough to make the left turn but continued through the intersection. After pumping on the brakes it finally stopped accelerating and I felt that I was under control then of the vehicle.

Adaptive cruise control slammed on brakes at least five times when nothing there in front or on the sides. First time I ended up with whiplash…now I hear it start to stop and I override system by speeding up. It also sometimes gets so close to car in front without braking so I have to manually brake. I’m concerned about being rear-ended if it slams on brakes for no reason. Five times there were no deer, people, etc.

In December 2018 a recall on the shifter was issued for Pacificas 2017 and 2018. The emergency brake would engage when ignition was turned on and van would not go forward or reverse. We purchased our 2017 Pacifica van on November 24, 2020. We started having this issue again last year in September 2024. The van’s emergency brake engaged when husband stopped at a red light and couldn’t start the van until after four light changes. Drivers behind us were not happy. He finally got the car to the Chrysler dealership. They had it for 6 weeks without much communication with us and what the problem was. They put a diagnostic device on it and it drove fine from December 2024 until yesterday, March 9, 2025, when the same brake issue came up again and we couldn’t get van started. We left it in the parking lot and our son came and gave us a ride home. Around 10pm last night our son and my husband went back to see if they could get it started to at least get it home. Many tries were unsuccessful until the last try when our son had the driver side door open and turned on the ignition. On the screen it said “shifter needs service”. My husband had been unable to reach a service personnel to read the device on the van. In the meantime, I called another Chrysler dealer, told them what was going on with van, and asked about the recalls. The lady there was very nice and asked for the VIN on van. She looked it up and said the recall for shifter went out in December 2018, and the issue on our van was fixed in 2020. Is there a way to find the month and day as well as the year in the history? we purchased the van on November 24, in 2020. Is it possible the issue has returned? the recall said it had something to do with dashboard design and something was replaced with the dashboard that was a real safety issue. We were planning to leave tomorrow for an over 900 mile trip to see family and we are concerned about traveling safely. No one has yet to return our calls from the dealership.

Brake booster has failed. There was previously an extended warranty published for this same issue on November 11, 2023 (no d-22-15) for xa7 brake booster, however that expires after 5 years after the vehicle was placed in service or 150,000 miles (whichever is sooner). The brake booster failure on our vehicle happened just a few short months after this "expiration date" of the extended warranty. Also, the date of the extended warranty was issued was November 2023. This left only 5 months from the issuance of the letter to the expiration of the warranty, which does not seem to be a reasonable amount of time. Our vehicle is not safe to drive as the brakes are not functioning properly which could result in serious and fatal injury's to the occupants or other vehicles on the roadway. Additionally repair parts are not available to purchase and are on a "national back order". Attention needs to be given to this topic as there are many vehicles on the road that are not safe because of this lack of quality.

The vehicle's brake booster is going out. The vehicle's warranty was already extended once but is now out of that 5-year timeframe. This issue began about 2 weeks ago. Now, the brake pushes back and doesn't want to engage which poses the risk of rear-ending someone. Chrysler is aware of this situation and refuses to extend or fix the issue.

The contact owns a 2023 Chrysler Pacifica. The contact stated that the vehicle would begin to roll away, when attempting to park on an incline. The contact also stated the gear shifter was placed in reverse, the vehicle continued to drive forward. In addition, when applying the brake pedal, the vehicle would take an extensive amount of time before coming to a stop. The contact stated it felt as if the hydraulic fluid was low despite there being no leak. The backover prevention camera failed to display the image from the rear of the vehicle due to there being an unexplained fracture. No warning lights illuminated. The vehicle was taken to the dealer who was unable to diagnose the cause of the failure. The vehicle had not been repaired for the brakes and gear shifter. The back-over prevention camera had been replaced but the failure persisted. The vehicle was returned to the dealer however had not been diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was 30,600.

When at a stopped position, the brake pedal gets stiff and starts pushing back … and then the car starts creeping forward. I’ve almost hit countless cars in drive-through, and my car tries to creep through red lights. It’s also very hard to brake. I went to one shop and they said it was the brake booster and vacuum hose , and that a lot of people were complaining about this issue in the Pacifica’s. I took the car to a Chrysler dealership to get it repaired and they said the brake booster is on back order because it’s a common problem. The only way to get the car to stop from rolling when you’ve got the brake on is to slam down on the brake (then it’s a crazy crunching sound) or to throw it into park before it gets you into traffic. It’s scary and it’s not safe.

My problem started about a year ago with the auto start/stop light coming on- said it needed serviced. They replaced my auxiliary battery. Mind you, my van was only about 2. 5 years old at this point (bought it brand new). So then about a month ago, same warnings. Stop/start isn’t working. I get it inspected at the dealership, they say everything is fine. Well yesterday, my van stopped emergently in the middle of an intersection. The brake engaged, couldn’t use any pedals, steering or switch gears. I had to shut my vehicle off and restart. In the middle of a busy intersection at 4pm. The auto start/stop light keeps coming on. Take it to the dealership today and they said it’s a sensor issue with my hood switch. My hood thinks it’s open so my van freaks out and stops driving. It’s a $540 fix. This is a huge safety concern. From what I read, this van has had numerous start/stop issues and sensor problems. It’s a 2020 with 62k miles. I’m glad I wasn’t on the interstate or somewhere more dangerous.

The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving at 70 mph, several warning lights appeared on the instrument panel. The contact also stated that the brake pedal had locked in place and could not be depressed. Due to the failure, the contact was forced to coast the vehicle to a complete stop. The contact had the vehicle towed to a dealer where it was diagnosed with an abs module failure. The contact stated that the abs module was repaired under warranty. The contact stated that the failure recently returned while approaching a stop sign. The same symptoms occurred during this failure as the contact managed to make a right turn and again coast the vehicle to a complete stop. The vehicle was towed back to the dealer where it was then diagnosed with a brake booster failure. The contact was informed that the parts were on backorder and that the repair would be covered under warranty. The vehicle had yet to be repaired. The manufacturer was notified of the failure but offered no assistance. The failure mileage was approximately 55,000.

On a brand new vehicle with less than a few hundred miles, the electronic parking brake engaged and would not disengage until the battery was disconnected. This left an error code and the dealership replaced brake actuator. Then, the safehold feature started to randomly activate when vehicle started to move from a stop light or stop sign. An error message would display on the screen, the vehicle shifted itself into park and activated the parking brake. The message indicated the vehicle had to be turned off and restarted. After restarting, the parking brake would disengage and the transmission could be shifted back into drive. This happened in the middle of traffic. This did not leave any kind of computer error code and therefore the dealership was not able to address it. Separately, the parking brake would refuse to disengage after repeated attempts, again without an error code, and the dealership was not able to address it. The vehicle has currently been under inspection for over forty days in total, and over twenty days since the last incident. The "safehold" safety feature is an accident waiting to happen.

While @ highway speed this vehicle lost ipower steering, power braking and the engine stalled out…this latest event prompted my complaint and was the third time I have experienced this situation. In all three incidents I was simply able to coast to the side of the road and stop. Thankfully after a few attempts to restart the vehicle started and ran. It was very frightening given it lost total power on all three occasions without notice or warning it like it just turns itself off. I contacted rollx vans the folks from whom we purchased this handicap transport vehicle and they informed me of a safety recall v53 for 2017-2019 Pacificas. The recall details the exact symptoms of what I experienced. I took the recall information and the vehicle to a local dealership larry h miller Chrysler, tucson az and they mostly dismissed my account and said there is no recall on my VIN despite the v53 recall. They said the tech couldn’t get it to duplicate and situation. They said they checked the grounds and it’s all ok. I am not convinced considering the timeline of their alleged repair and the answers I rec’d from the service writer when I asked specifics about the repair. I suspect they simply test drove it when it did not duplicate the situation they said it’s ok.

We have tried to figure out what is going on with our brakes for months now. We have changed our brakes to new brakes and took into two car repair shops. Both even after explaining to them couldn’t figure it out because it does not do it all the time. We thought it could be the brake booster or something but they don’t want to changed it if it’s not our brakes become hard will not brake even to the point we can’t stop and slide through lights almost like we are on ice. This has happened many many times. We just recently have been reading forums to see if this is happening to anyone else and there are so many complaints! this is so scary to drive espically with teens and two little ones and winter drives. We expect a recall soon or someone will be sued ASAP.

The brake booster created unsafe vehicle handling/driving . Safety was at risk as the car would not remain stopped when the brake pedal was pushed. The pedal would resist pressure and the car would move forward into instersections, cars in front of this car, and it required that I place the car in park to stop the car. The part has been replaced and the brakes work as they should. There was a recall of the 2018-20 Chrysler Pacificas for the same reason. I read that others have experienced this brake booster problem with other years, excluding 2018-20. Lives of the people in the car were at risk until this was fixed. Also, it was not noted on the Chrysler computer check as a problem. I had to insist that it was happening in order to get this repaired. There has been precedence for this malfunctioning part and I request that I am reimbursed what I paid for repair, along with a recall issued. Lives are at risk!.

The power brake booster was failing and causing the vehicle to not stop as expected. When it was brought to an auto mechanic a few weeks after noticing the issue, they described it as appearing defective beyond normal wear-and-tear and reminiscent of a recall for earlier year models of Chrysler Pacificas. The issue caused the vehicle to not respond to the pushing of the brakes and required a pumping of the brakes until the vehicle stopped. Occasionally, when the brakes were depressed and the car was at rest, the vehicle would start moving and the brake pedal would push strongly against being depressed. There were no warning lamps or messages associated with this issue; the issue was discovered simply by noticing that the brakes were not working as they should.

The forward collision alarm was triggered when there was no collision. Additionally the abs system and cruise control went offline. This occurred numerous times during winter while at a full stop as well as when driving between 45-55mph. Once triggered the collision alarm remained on for hours to days at a time. During that time cruise control was not able to be used and abs was offline. Additionally the assisted parking was not available. Abs and stability control is important for safe driving and braking in icy/slippery situations putting both my own, my passengers, and other drivers at risk. The vehicle was inspected 4 times by the bath NY Chrysler dealership. A sensor was replaced the first time but this did not correct the issue. The 2nd time another component was replaced, but the technician stated the issue was still present and I would need to bring it back in for further testing. The 3rd and 4th time the dealership was not able to recreate the issue and did not perform any repairs. The issue has recurred, but the dealership was not able to get the car in at that time. This vehicle has not been inspected by manufacturer, police, insurance etc. There was no prior warning to the issue. When it occurs the warning lamps that come on are: brake, front collision, front collision off, abs, seatbelt, and the traction control. I have a photo of the dash from one of these incidents.

Unknown while driving, the electrical system seemed to malfunction causing the lights to flash on/off, the rear windshield wiper to activate, the horn was blaring (quick repeated beeps, with occasional blaring), the dash indicated that the parking brake could not be used, indicated that the airbag system was not working, indicated that the driver’s side seatbelt was not connected, amongst other warnings that I cannot recall. The abs brakes were not working, nor was the power steering. The car indicated that it needed to be put into park, and allowed for the turning of the dial; however, the dash indicated that the vehicle was moving too quickly to shift into park. We were able to make a turn onto a street with an uphill incline from the road we were driving on; and the brakes stopped the car - using a lot of force as the abs braking system was still malfunctioning. The vehicle was put into park and the parking brake indicated that it was active; but of course could not be trusted as the indicators on the dash had been so haywire just moments before. While the vehicle was “off” the horn was still blaring (both short beeps and intermittently long blares), the rear wiper was still active (while in the off position), and the interior lights were flickering. This continued until the red connector was disconnected from the battery. The black connector seemed to make no change, connected or not. When the red connector was reconnected, the electrical issues seemed to be replicated with the horn and rear wiper. The vehicle was towed and we have no current answers. This was extremely dangerous as we lost all power and control of the car. We were driving, on what is oftentimes, a very busy street.
