The front seat passenger side headrest spontaneously busted open abs cannot be put back because the rings that hold it in place broke. We were not in motion. Someone sat in the seat snd it happened without cause.
The headrest opened without warning or collision while driving down the highway. Driving at about 45 mph while slowing down going over rumble strips. The plastic housing that holds the bar which hold the headrest down had broken. The bar is still heald by two teeth.
The active head restraint on the right front seat inadvertently deployed. The vehicle was not involved in any kind of impact and no airbag deployment took place. The head restraint will not reset due to the sled bracket being broken.
Passenger headrest assembly randomly engaged some sort of "airbag" mode. Two springs deployed the headrest forward rapidly.
Driver seat active head restraint deployed when not in a collision. The car was off and we were loading kids in the car. Can't get it to go back together.
While parking my vehicle in my driveway and after coming to a stop, my driver side headrest (active head restraint) deployed, striking me in the back of my head. I was not injured. There was no associated vehicle nor did the vehicle strike an object. Upon further examination, a plastic seating that holds the metal pin had broken. Attempts to reset was unsuccessful because of the broken plastic pin seating. The attached photo shows a strap around the headrest so that the vehicle can be driven safely.

My vehicle was parked in the driveway stationary, and as I walked to the vehicle to enter the car with my 2 young children and as we approached I heard a load pop noise. I found the ahr headrest had deployed, with no one touching or moving the vehicle. The vehicle has never been in an accident and the headrest has never been reset for any reason. The plastic joint mechanism and spring that holds it in and together cracked and broke. I called Chrysler dealership and they will not fix or replace unless I pay out of pocket, approx $1000 for 1 headrest. The gentleman on the phone acknowledged that he knew of this happening often in Chrysler t&c vans, however could not help me as my warranty is now up and there is not a recall. I am terrified to drive my van now, knowing that the drivers side headrest ahr may deploy at anytime no warning and cause injuries to me, which could cause me to crash my vehicle while driving endangering my children and family. I have ziptied both the passenger headrest and drivers head rest shut. The drivers headrest is ziptied in case it randomly deploys and the passengers is zip tied as the head rest internal plastic mechanisms are broken.
Passenger front headrest exploded while stationary. We were at a stop light, there was no accident or impact. It cannot be reset and needs to be replaced. I do not feel like we should have to pay out of pocket when this is a common issue and a safety issue.
Was driving from PA to new york on the highway and about 150 miles into the trip the headrest exploded out word and stuck me in the back of the head. Causing a little confusion and a instant headache. There was nothing that could have caused this. The road was not bumpy or was there any kind of impact to the van. Thank god my wife was not driving because the impact of the headrest probably would have knocked her out and caused an accident. The headrest system is supposed to be a safety device but is defective and a risk to anyone that is driving a Chrysler product.

Early on/about August 27, 2020 I went out to my Chrysler t&c , parked overnight in my home's driveway, and discovered that the front passenger seat headrest had deployed. The vehicle had not been involved in any incidents of sudden contact with any other vehicle or object. Examination of the vehicle exterior shows zero evidence of any contact with another object on any of the outside surfaces. The headrest was not deployed when I was last in the vehicle, but was deployed when I looked at it the next morning. I have not yet taken the vehicle to any repair facility and no one except me and my wife have seen the problem. I am the sole person to drive this vehicle. No other person, including my wife, drives it. .

Drivers seat problem. Active headrest restraint faulty the plastic retention tabs holding the metal bar in place spontaneously broke ,the metal teeth were still locked down on the bar showing it didn't deploy, just the plastic tabs broke. This caused the headrest to 'activate' and move forward. This is a safety issue. Vehicle was parked in driveway. No injuries as no one in the car.
