While the car was parked in the garage, person sat in the passenger seat and buckled, leaned back against the headrest and the headrest impact function triggered, pushing fully forward. Following Chrysler’s instructions on resetting it, I found the plastic locking mechanism was completely broken off.

I had to take my car to the dealership about 2 years ago for my headrest popping out and the said there was a recall then. I want to know if there still is a recall because my passenger headrest has done the same thing. Also my blindspot warning is not working. Also my video system does not work.
I was driving and all of a sudden I heard a loud pop and the driver side head rest deployed on its own. There was no accident, I was just was driving. The headrest struck the back of my head resulting in me having a sore neck.

Headrest deployed with no occupancy or impact. Vehicle was parked in the garage when this occurred.
Today my wife was driving our 2013 Chrysler town and country. I took some pictures of the headrest popped open. I watched some youtube videos about it to find out that it is a safety feature if the vehicle is in a rear end collision. This van however was not in an accident. Since it is Saturday, my dealer's service department is not open. I will have to call them on Monday.
I was driving and all of a sudden we heard a loud pop and the passenger head rest deployed on its own. There was no accident, just was driving. Looked online to see if I could push back together, but learning about all the pieces, the plastic parts that hold the pin in broke as it was deployed. Will need to call the dealership now to get it fixed/probably entirely replaced.
Just driving down the road and passenger head rest just exploded hitting my 17 year old son in the back of his head. No impact or collision just normal driving.
While driving on the highway, the passenger side headrest deployed without any warning or accident causing it to. A plastic part that holds a metal bar in place seems to have broken. No one was injured at the time. This seems to be a common problem. Chrysler needs to fix this! who knows when the driver side one will deploy on its own with no accident.

I was sitting in my 2014 town and country and my active headrest restraint randomly activated hitting me in the back of the head. I was parked with one passenger in the vehicle. It hurt my neck as it shot my head forward.
Passenger side headrest literally exploded open due to a broken plastic tab. I was only mildly injured but definitely felt the after effects in my neck and back. Definite safety concern. I'm worried the driver side headrest will do the same thing as we are driving. Absolutely should be a safety recall.

The headrest on the front passenger seat deployed when there was no accident. The car has been stopped and was still running. My son was driving and it hit me in the back of the head. If this had happened to the driver's side headrest while driving, it could have easily caused an accident by distracting the driver.

I am the original owner of a 2014 Chrysler Town & Country minivan. In the spring of 2023 I went out to go run and errand and when I got into my van I saw that the headrest on the passenger side front seat had opened up. I tried to close it but it wouldn't move. When I got home I started checking to see what the deal was with the headrest and found out that the headrest had deployed without there being any sort of accident and fortunately there was no one in the seat at the time. Chrysler is not taking any responsibility for the problem. Now I have a failed headrest that is supposed to prevent injuries to passengers that is inoperable. And I have a questionable headrest on my driver's side seat that I don't know when it might decide to deploy one day when I am driving my vehicle. This is totally unresponsible of Chrysler and I think they need to be held responsible for this and replace their headrests at their own expense.
The headrest on the passenger side burst open. From what I’m reading online due to a faulty plastic part from Chrysler. I was not in any accident. It burst on its own.
Both headrests in the vehicle have malfunctioned and popped without the occurrence of an accident. The headrests are broken and cannot be reset in their proper functioning position. Therefore, it poses a safety hazard for occupants. The components have not been inspected by the manufacturer or anyone else other than the owner of the vehicle. There are no indicators denoted the issue.
The active headrest on the driver seat, sprang open with a loud pop, hitting me in the back of the head while I was driving. I have heard of many other drivers having the same problem. There was no accident. There was no collision. Or was no warning. I have not taken this in to get fixed yet. People online are saying it costs up to $750 to get fixed. It is uncomfortable to drive the car with the headrest in this position.
The passenger seat headrest has internal damage and fallen forward. There have been no accidents and nothing has disrupted the headrest. It is now unsafe for a passenger to be in the seat with a faulty headrest.
The passenger seat headrest has fallen forward. We have tried to put it back however the part inside is broken it appears. I have not been in any accident. Nothing has hit the seat either. I am not sure what would happen if one of us is sitting there an an accident happen; out head is not protected. Especially if my teenager is there. I had not warning lights. Unknown. Inexplicably, the driver's headrest had deployed (see photos). I noticed it while opening the vehicle's driver door in a parking lot. The vehicle was not parked near any other vehicle. I brought the minivan to the closest Chrysler dealer. For more history please see text that immediately follows from my original 3/2024 communication to you. Original 3/24 request #1488564 case/reference 103 8151 the case concerns a driver’s headrest that deployed in 2022. On September 21, 2022 I brought my 2013 Chrysler Town & Country Mini-van to schumacher Chrysler of delray beach, FL. After 2 days (on 9/23/22) the service advisor informed me that their parts department didn’t have a matching headrest in stock and they would have to order one. At that time the vehicle had 68,530 miles. I followed up monthly, then bi-monthly, then quarterly until October 2023. At that point (10/30/23) I called your office where it was assigned case 103 8151. Your office told me to call stellantis, which I did the next day. It became their case 878 2888 3. They told me that they’d contact the dealer, schumacher, and I’d hear from the dealer “soon. " I never did and I finally called stellantis on 3/12/24 as followup; 4 1/2 months is a long time for “soon. " stellantis claimed they tried calling me in December 3 times. I never received any such calls or phone messages (and in checking caller id on my phone — which I basically never erase — had no record of any phone calls from either stellantis or schumacher. You’d think someone would have the courtesy to leave a message. ) stellantis told me yesterday that I needed to start-over; make the 25 mile round-trip drive to schumacher (the closest Chrysler dealership) to “diagnose” the problem. What is there to “diagnose” on a deployed headrest that they, schumacher, already saw and diagnosed? I am requesting that you, NHTSA, take this matter “under your wing. ".
The front passenger side headrest exploded forward while parked and it cannot be reset. It appears that plastic clips inside of the headrest snapped. The fact that the same type of headrest is installed on the drive side is extremely concerning because should it malfunction the same way while driving myself of my children could become involved in an accident when it hits the drive in the back of the head while on the road. There was no jolt or warning whatsoever we head a popping sound while the vehicle was parked and the headrest shot forward with extreme force.

Headrest came apart. Metal rod held by plastic gave way and now the rod cannot stay in the plastic housing correctly. Plastic is broke on both ends of the metal rod.
Driver side active head restraint popped open. The air bag did not deploy but the plastic tabs holding it together broke inside.
I was standing beside the parked vehicle with the side door open, driver's side, inspecting the 2nd row swivel seat to see if it is removable. I was bent over the forward facing 2nd row seat when I lifted the release lever that tilts the seat back forward. The seat back flung forward with violent force, the headrest hitting on the right side of my face. It broke my nose, which bled profusely and had to be manually straightened by a nurse.

Passenger side front headrest deployed with no occupation or contact and will not reset. Had someone been sitting in the seat it could have caused serious trauma to their head and/or neck. It absolutely startled and scared me. Had a vehicle been in the neighboring lanes I very well could have caused an accident after the deployment. No prior warnings of any kind preceded the incident and I have not been to a dealer for inspection.

I was just driving down the road and noticed my headset was pushing out toward my head. Upon inspection, it had been deployed but there was no accident that caused it.
