The contact owns a 2021 Ford F-150. The contact stated that while driving approximately 45 mph, the contact was cut off by another vehicle that had pulled out in front of the vehicle. The contact depressed the brake pedal with force to keep from crashing into the vehicle. The contact stated that the seat belt pretensioner held firmly in place; however, the passenger's seat belt failed to restrain the passenger in the seat. The contact stated that the passenger almost crashed into the front passenger’s side dashboard. The vehicle was taken to a local dealer, where it was diagnosed, and determined that the passenger’s side seat belt retractor and pretensioner needed to be replaced. The vehicle was not repaired. The contact researched online and related the failure to NHTSA campaign number: 21v653000 (seat belts); however, the VIN was not included in the recall. The manufacturer was not informed of the failure. The failure mileage was approximately 46,000.

The contact owns a 2021 Ford F-150. The contact received notifications of NHTSA campaign numbers: 21v653000 (seat belts), 23v509000 (parking brake), 22v142000 (visibility) and 23v163000 (visibility). The contact stated that while driving 60 mph, the parking brake independently activated. The contact pulled over to the side of the road. The contact turned off the vehicle and, after waiting for a short while, the vehicle was restarted. The message "service parking brake" was displayed. The contact stated that on a separate occasion, while turning off the vehicle, the parking brake fail to function as intended. The contact stated that recently the seat belt failed to retract and remained loose. The vehicle was taken to the dealer; however, the dealer refused to repair the vehicle under the seat belts and parking brake recalls. The contact stated that the recall repairs for visibility were performed. The manufacturer was not made aware of the failure. The failure mileage was approximately 119,000. Front seatbelts on 2002 Ford F-150 become unlatched unexpectedly while driving. This has been noticed over the last two or three years occasionally, but has been getting worse. Seat belt has become unlatched about 5-6 times during a 600 mile road trip. Seat belt was confirmed by two passengers that it was indeed latched correctly and nothing pushing the release button. Seatbelt was confirmed to be latched by firmly tugging on seat belt to witness that it was latched. Both passengers confirmed that the seatbelt become unlatched randomly while driving. Passenger and driver safety is at risk. If driver seatbelt comes unlatched while driving or during accident, the driver could loose control of truck and put other drivers at risk. The unlatching does not show any warning signs as to when it will become unlatched. Seatbelt just becomes unlatched and starts to recoil without warning. Ford service just says that the truck is out of warranty and that they dont cover seat belt replacements as there is no open recalls for such. Seatbelt is still currently installed on vehicle but can be available for inspection on vehicle or removed for inspection.

6 Seat Belt problem of the 2004 Ford F-150

The vehicle rolled and killed the occupant because the roof caved in. The shoulder portion of the seat belt did not hold the occupant (only the lap portion) and the occupant was partially ejected out of the rear of the cab. The vehicle was inspected and purchased by state farm insurance since the vehicle was totaled. The 2000's Ford F-150s have roofs that are too weak to support the weight of the vehicle, thereby putting the occupants in danger of injury or death. This is well documented. There were no warnings.

11 Seat Belt problem of the 2013 Ford F-150

Failure Date: 08/26/2024

The contact owns a 2013 Ford F-150. The contact stated while driving 75 mph, the speedometer dropped to zero and the instrument cluster became inoperable. The power train warning light and the seat belt warning light were illuminated. The contact turned off the vehicle, opened and closed the driver's side door. The contact stated that upon restarting the vehicle, the instrument cluster became operable. Additionally, the contact stated while driving 65 mph, the vehicle jerked, and the transmission downshifted from 6th to 2nd gear unexpectedly. The contact stated while coasting to the side of the road, the transmission shifted to the proper gear and the contact was able to continue driving. The dealer was notified of the failure and informed the contact that the vehicle was previously repaired under Ford campaign number: 16s19; however, the failure persisted. The vehicle was not repaired. The contact related the failure to NHTSA campaign numbers: 19v433000 (power train) and 19v075000 (power train) however, the VIN was not included. The manufacturer was not notified of the failure. The failure mileage was approximately 103,290.

12 Seat Belt problem of the 2014 Ford F-150

The instrument panel air bag light stayed on continuously, so I was concerned that the air bags might be defective. When I took the vehicle to a Ford dealership for servicing, I was told that the front passenger side seat belt pretensioner/retractor was shortened internally. I was told that unless the part was replaced, the air bags would not deploy upon impact. If I chose not to repair the part, I was told that the vehicle would pass our state's inspection. In order to protect my family's safety, I believed that I had not option but to replace the part. I requested that the dealership give me the defective part, which they did. I still have the defective part. That repair cost me a total of $1097. 98.

The contact owns a 2015 Ford F-150. The contact stated that while his son was driving at an undisclosed speed, white smoke was coming from the base of the driver's seat belt clip. The contact stated that the air bags warning light illuminated with a message that the driver's side door was opened; however, the door was not open. The contact's son was able to safely pull the vehicle to the side of the road and the vehicle was towed back to the residence. The vehicle was not diagnosed or repaired by an independent mechanic or the local dealer. The contact related the failure with an unknown recall; however, the manufacturer confirmed that the vehicle was not included in the recall. 14 Seat Belt problem of the 2018 Ford F-150

Failure Date: 06/01/2024

The contact owns a 2018 Ford F-150. The contact stated that while driving at 60 mph, the vehicle hydroplaned as the contact lost control of the vehicle. As the contact struggled to regain control of the vehicle, the back end of the vehicle hit the left side of the highway retainer wall. As a result, the vehicle spun out across three lanes as the front end of the vehicle hit the right retainer wall. The air bags deployed upon impact; however, both the front driver and front passenger seat belts independently unclipped during the accident. The contact stated that once the vehicle came to a stop after hitting the second wall, both seat belts were unbuckled. Both vehicle occupants were taken to the hospital via an ambulance. The contact was treated for a concussion, pain to the left shoulder and a torn lcl on the left knee. The passenger suffered a broken right hand, concussion, pain in the neck and bruising on the torso. A police report was filed. The vehicle was towed from the scene to an independent mechanic where it remained in their possession. Both the dealer and the manufacturer were notified of the failure; the contact was given a case number by the manufacturer. The manufacturer then referred the contact to the NHTSA hotline for assistance. The vehicle had yet to be repaired. The failure mileage was approximately 118,000.

Front driver's side seatbelt latch failed. Upon disassembling the latch assembly, it was found that a part of the mechanism broke off causing the latch to fail. The part appears to be cast metal, and it failed along a narrow portion of the mechanism. It seems that this part would be more robust.

17 Seat Belt problem of the 2014 Ford F-150

I have owned this F-150 since new and most of its life it has been stored indoors. Recently I noted that any time the vehicle is driven in the rain or left outside while it rains, the rear corners of the cab leaks water. The source of that water is a failed seal surrounding the rear window, which I diagnosed in my shop after removing the headliner and other trim in order to observe the leaking with some water and compressed air. The issue that I noted during the repair was that the airbags that are mounted above the rear passenger windows in the headliner on both sides of the vehicles wicked up moisture that had been caught by the headliner. They were both wet to the touch, which was concerning. Further, the airbag inflator which is mounted in the rear corner post of the vehicle and the seatbelt recoil mechanism both also showed signs that they also recieve moisture from this leak. The vehicle is out of warranty, so there is no manufacturer remedy for the issue unless I were to pay for a complete rear window reseal. I did not have a dealer take a look at this issue at this time. Multiple other owners of f150's of this age have had similar issues with water leaking into the cabin and leaching into the airbags, as described in numberous online forums.

My 2015 Ford F-150 truck has had multiple issues related to performance issues, with only 80,000 miles on it. I have had issues with it losing power while driving, both with and without towing a trailer where it just powers down on the highway from 70-75 mph down to 40-50 mph in traffic. I usually just have to pull over, turn the engine off and then turn it back on and everything runs normal. It is currently at phil long Ford in colorado springs, CO, having the short block and driver side cylinder head replaced at 80,000 miles because it was smoking out the exhaust and they determined that it had coolant leaking into the #4 and #5 cylinders. This is after it had a completely new engine installed at 75,000 miles due to coolant leaking into the 3# cylinder and causing severe damage to the engine. It was in the repair shop from August 2023 until late December 2023 because the engines were are backorder. The passenger side cylinder heads are currently on a 13 month backorder according to the dealership, which is why they are only replacing the short block and driver side cylinder head this time, after only 5,000 miles on the replacement engine. I have also received multiple recall notifications regarding this vehicle, the most recent of which was on 4/22/2024 for #24e03 powertrain control module software update, none of which have been repaired during the multiple times and extended durations that my vehicle was at the Ford dealership for repairs. Two recalls existed and should have been repaired when my vehicle was at the dealership between August and December of 2023, and the most recent recall shows "no remedy available" for a pcm module software update?.

On February 16, 2024, the vehicle (f150 XLT super crew) that was being driven was going approximately 40 miles ph when an other vehicle came out of a side street who had a traffic stop sign. There were two school buses blocking their view and my view of anyone coming out of the development. My vehicle hit head-on the other vehicle and I blacked out. No airbag driver side deployed, but the seat-belt tension-er fully pulled on me. I awoke with alarms going off and smoke in the cab and in the engine. Fire department cut all wiring. I was taken out of the vehicle by paramedics and taken to the hospital with fractured sternum and bruises. Police were on the seen and did a report with the other driver at fault. Insurance company proceeded to have the vehicle towed and inspected and totaled the vehicle. Did not have vehicle inspected by independent company that I know of. Police report stated no air bag deployment. Hospital report showed no air bag deployment. Issues a year earlier with engine replacement by dealership under manufacturer warranty due to engine failure. No other known issues appeared for the failure.

I have inflatable seat belts on the rear seats. The buckles fall down into the seat making it difficult to fasten the seat belts. They were replaced under warranty for this concern. After a week they started to fall into the seat again.

While sitting in my driveway getting ready to go to town, I pulled the seat belt to lock it into place when I hear a pop then the seat belt would not retract at all. It was as if the retractable spring broke. I left the truck sitting until I could get a replacement belt from Ford (450. 00) to repair this safety issue. Had this belt failed in a collision it would have caused severe damage to the driver. This issue needs to be addressed before someone dies in an accident. There was no warning signs before this happened and I was very lucky it happened in my driveway and not on the highway.

The dash cluster randomly shuts-off/restarts while driving the vehicle and when it did last time Ford app sent 3 notifications of faults with antilock brakes, tpm, restraint indicator warnings. If there is an issue with these items and I have road emergency and my brakes and restraints do not work properly it could be a safety issue for all involved in accident (I would not know how fast I was going since dash cluster is off). I took the truck to dealership and they are passing it off on ssm 51757 (which my truck also has an issue with) and a loose pin connection. But this happens with no checking of trailer tire information. The speakers made a loud crackling noise and my phone disconnected from infotainment center just prior to the incident.

I need to address a safety concern! on 3/13, I was driving back home. My 3 year old daughter was sitting in her car seat (behind the driver seat). She pulled the seatbelt from the middle seat and wrapped it around her neck choking herself. I had just got home and thank god I did because this would have been a totally different story. I pulled up to my mailbox to get my mail. I happen to look back and saw she was purple on the verge of blue. I had no scissors in my car to cut the belt. I drove up my long driveway ran into the house came out with scissors and cut the belt as it was locked and there is no emergency unlock button. Her neck was very red and has broken blood vessels. I have pictures to prove this! no where on the belts does it say “choking hazard” I thought seatbelts were supposed to save lives! my seatbelt can be replaced however, my daughter life cannot! thankfully, is ok!! I called a Ford dealer to get the belt fixed. Was told it was $1,000!!!!!.

Safety belts have become locked & unusable after a collision. The seat belts on the driver and front passenger side will not pull out from the retracted position. Vehicle was inspected by state farm insurance & caliber collision, in connection with boggus Ford dealer, & all have determined that this is not collision related. This decision was made because the front passenger seat was unoccupied at the time of collision. Therefore I am reaching out to whoever will listen, this vehicle is no longer safe to drive as it apparently has a potential deadly defect.

While driving I noticed my wife's seat belt was not fastened, and there was no alarm chime or icon on dash alerting of incident. I then undid the drivers side and had the same results. I had been having issues with the ac leaking on inside at passenger floor. I took the truck to five star Ford dealership for repairs. The ac evaporator and carpet was replaced. They said they issue with the seatbelts was not repairable unless a code was thrown. I was told it wouldn't cause any issues with air bags, but I'm not comfortable with that. The dealership said they would contact Ford and see if there was anything that could be done. After ac issues that has been ongoing several times now I believe moisture got somewhere it shouldn't have. I kept telling them what if a child was in seat and undid seatbelt and then an accident occurred. They said the airbags should still work. I dont trust that. This could be a very serious issue. I was told I could buy a new truck. I can't afford to buy a new truck every couple of years on retirement. The dealership said they believe the issue is in one of four modules but Ford will not authorize them to just start replacing modules without error codes being thrown.

I had a front end collision with my truck. The damage was a direct hit on the airbag sensor, and my airbags nor my seatbelts deployed. I was going about 45 to 50. The pre-collision warning appeared right before I hit the vehicle but no automatic braking. The body shop that has my vehicle had safety concerns because my vehicle took the brunt of the crash right on the airbag sensor. No airbags or anything deployed, but the sensor was broken in the accident. The concern is if the sensor was hit directly, why didn't it send a signal to through the system to deploy safety restraint systems. I called Ford to inform them of my concerns. After going through several people, I was finally given a case number cas37213430 after days of trying to get them to take me seriously. They told me to take it to a Ford dealer to have it checked, but it is wrecked so how will they check it. My concern is that it didn't deploy the restraints because obviously wasn't working properly. I couldn't get all my pics to load so have more.

I was driving 60 mph and a car pulled out in front of me causing me to tbone the vehicle. Drivers seat belt and airbag did not deploy causing me to hit the steering wheel then cracking my head on the wind shield. I bent the steering wheel gettin the force of the impacted and smashed the wind shield with my head causing my neck to bend. The accident has caused pain in my head, neck, left (hand, wrist, elbow, shoulder) right (hand, wrist) and chest that happen on 5/4/2022, in result of the safety mechanisms failing to properly work.

The contact's husband owned a 2018 Ford F-150. The contact stated that while her husband was driving at 62 mph, he hit the back of the a semi truck on the front driver which sent the vehicle to tailspin and caused her husband to hit the semi truck for a second time. The second collision sent the vehicle into a another tail spin before coming to a complete halt. A police report was filed. According to the police department investigation, the airbag inflator had deployed which struck her husband's forehead. The investigation also determined that the her husband's injuries were not consistent with a proper airbag deployment. The report also noticed that both the air bag and air bag inflator were found on the floor near the rear passenger side seat. In addition, the report showed bruising constant with the lower lap belt locking but the driver did not have any bruising/burns from the upper seatbelt. The contact was pronounced dead on the scene and was taken to a moratorium. The vehicle was taken to an independent tow yard and deemed a total loss by insurance. The dealer nor the manufacturer had yet to be notified of the failure. The failure mileage was 36,667.

After driving the vehicle for 14,000 miles I was traveling out of state when my wife and I discovered that the passenger seat belt was not anchored to the floor. When we arrived home there was a recall notification in our mail. I immediately went to serramonte Ford in colma, CA and scheduled the required inspection for November 1. They assured me that any necessary repair would be done at that time. Later that day they called and told me that I needed a new part but they were required to order it from a special source because it was a recall item. (what)! they called two weeks later and advised that the part arrived and I made an appointment for November 16 and dropped the vehicle off. Later that day they called and told me that they needed a gasket so the repair couldn’t be done. I picked up the vehicle and they told me they would call me to reschedule the repair. It is now January 2022 and I have been driving this vehicle for eleven months without a seatbelt. They can’t tell me when to expect the repair.
