Tesla has auto-park in the supervised fsd. My wife was using it to park the car in a public garage and it hit a post. The car was damaged. It was too quick for my wife to intervene. I know it is supervised, but there is no time to intervene when it comes to parking where the spot to park is close to other cars or posts anyway.

My 2023 Tesla Model X began experiencing safety-related failures immediately after a Tesla software update. Right after the update, the vehicle’s cameras stopped functioning correctly and gps/navigation also stopped working properly. Since that point, subsequent Tesla software updates have failed to install successfully. Tesla diagnosed the issue as requiring replacement of the hw4 computer at my expense for approximately $4,500 because the vehicle is just outside warranty. I believe this may be a safety defect or a software-induced hardware failure because the malfunction began immediately after Tesla’s software update and affects safety-relevant vehicle functions, including camera-related visibility systems and vehicle localization. My VIN does not show an open recall, but this vehicle previously received completed Tesla software recall remedies related to camera/hw4 issues. I am filing this complaint because Tesla should investigate whether this failure is related to a broader hw4 defect pattern, software update defect, or inadequate recall scope. I am requesting investigation of failures involving camera function, gps malfunction, and repeated software update failure after an over-the-air Tesla update.

My 2023 Tesla Model X suffers from severe, intermittent safety system failures. Specifically, the automatic emergency braking and steering assist randomly become unavailable while driving. Tesla service refuses to repair this, claiming they are "unable to replicate" the intermittent alerts. However, the vehicle's history proves severe safety flaws: Tesla previously had to replace the entire steering wheel assembly and the driver's seat belt lap pretensioner due to failures, and had to recalibrate the forward adas camera because it went out of spec causing autopilot failures. These unresolved, randomly occurring defects pose a severe crash hazard that Tesla is ignoring.

My Tesla Model X was in autopilot mode (v13. 2. 9) at the time of the incident. The vehicle’s advanced driver-assistance system was actively controlling steering and speed, while I remained attentive with my hands on the wheel I was closely monitoring the car’s operation but was not manually steering, braking, or accelerating during autopilot engagement the vehicle collided with stationary flexible delineator posts. The impact occurred at 55mph. The flexible delineator posts were the only object struck. No other vehicles were involved in the accident, and no persons were injured. No passengers or bystanders affected. The collision with the flexible delineator posts caused damage to my vehicle’s front bumper area and cracked right headlight. There was no damage to any property besides my own vehicle. The incident was captured on the Tesla’s built-in camera system (dashcam). I have saved the video footage, which clearly shows the car’s approach and the impact with the flexible delineator posts. This video is available and can be provided for review.

I am writing to express my deep concern regarding ongoing safety and service issues with my 2024 Tesla Model X. This is the third Tesla vehicle we’ve owned, and while our previous two vehicles performed excellently, this one has presented serious and recurring problems with the full self-driving (fsd) system. Since the second service visit, we have experienced four separate incidents in which the vehicle has either driven off the road or abruptly slammed on the brakes without warning. Despite two prior service appointments at the Tesla service center in sarasota, FL, these issues remain unresolved. We also scheduled a third appointment, which Tesla later cancelled.

Issue description gps freeze: the navigation system remains stuck at one location even when the vehicle is moving. Park assist not available. Fsd not available. Rear camera shows black screen, completely disabling reverse visibility. These issues occur intermittently, often lasting for days, and then resolve themselves unexpectedly. Unfortunately, when scheduling service, the problem often disappears by the appointment date, making diagnosis inconsistent. However, the failures have already occurred more than three times, and each time they severely impact both safety and usability. Safety concerns lack of rear camera visibility creates a direct safety hazard during reversing. Disabled park assist and fsd compromise Tesla’s advertised safety features. Gps malfunction affects navigation and system reliability.

While driving our screen shut down, autopilot and gps stopped working and the doors would not open. My kids were stuck on multiple occasions and had to climb through the front seats. Tesla has already confirmed that the driver assistance system ecu (turbo a and turbo b processors) is a known issue. Over 1600 cases reported. This defect disables safety-critical driver-assist functions without warning, which meets the federal definition of a safety defect under 49 u. S. C. § 30102. Provide a free remedy to owners — including replacement at no cost — as required by 49 u. S. C. § 30120. Telsa denied coverage because we are out of warranty. I asked whether the known issue has been reported and why it is not being covered as a known safety defect. Because the problem involves loss of a safety system, it should not be treated as routine maintenance or wear and tear. It is a known safety defect and therefore must be reported and remedied at Tesla’s expense. Tesla refuse to address my concern and would only let me talk to store manager. They told me I had to email business resolutions to get more information. I have not heard back. I was forced to spend $2k to get my car fixed.

The odd thing is that the alerts come and go as they please. I can't figure out what it is. Could be software related. Possibly a software update has messed it up?.

10 Adaptive Cruise Control problem of the 2023 Tesla Model X

The primary issues are: the vehicle consistently chooses the wrong lane and drifts across yellow lines. The fsd visualization fails to correctly distinguish road markings, lanes, and turning arrows, causing them to move erratically on the display. The car attempts to merge into lanes that appear in the visualization but do not exist on the road. The vehicle attempts to merge into lanes that are ending, without recognizing the lane-ending markers. My service history for this single, unresolved issue is as follows: June 12, 2025 (invoice #3000s0013397552): I brought the vehicle in because autopilot was incorrectly detecting lanes. The service team diagnosed "severe hazing over forward facing cameras," cleaned them, and returned the car to me. The problem was not fixed. July 1, 2025 (invoice #3000s0013588350): I returned for the same issue, stating that the fsd was driving over yellow lines and not recognizing traffic patterns. The diagnosis was changed to a bad alignment; service notes indicate "multiple angles out of specification". A four-wheel alignment was performed, and I was assured the issue was resolved. July 10, 2025 (invoice #3000s0013675264): on my third visit for this issue, the service team verified that the autopilot visualization showed "random turn lane markings". However, I was told this behavior was "considered normal at this time" and would improve with future firmware updates. The "correction" was listed as "remote - education". July 28, 2025 (invoice #3000s0013834479): I returned for a fourth time, explaining that the problems persisted and were a safety concern. I noted that the car had recently tried to merge past a solid yellow line into the shoulder. The service team acknowledged the issue but concluded it is an "issue with v13 due its design" and that "no repairs are recommended for this concern". The assertion that this is a "known characteristic" is unacceptable. The fsd and autopilot features are now unusable and unpredictable.

I own a 2019 Tesla Model X. After Tesla upgraded my vehicle from ap2. 5 to the fsd hardware module, the front camera view became blurry. I disassembled the camera housing and found that a heated line in the windshield, intended to prevent fog or ice buildup in front of the camera, runs directly through the camera’s field of view. This line now causes noticeable image distortion, which interferes with safety-critical features such as fsd, forward collision warning, and lane keeping assistance. The Tesla service supervisor confirmed this is not my fault. He explained the issue may have been caused by heat from the line gradually affecting the glass, or that the upgraded fsd hardware, which captures clearer images, is now revealing a flaw that was previously less visible. He also mentioned that Tesla redesigned this component in newer Model X vehicles to prevent this issue. However, because my car is out of warranty, they are refusing to replace the windshield. Since the fsd upgrade, I have experienced multiple warning messages while using fsd. On one occasion, while driving on the freeway, the system suddenly disengaged and displayed a message saying the camera had weak visualization and the driver must take over. I believe this is a serious safety issue related to the blurry front camera caused by the heated line in the windshield. Although Tesla acknowledges the problem and that it is not due to misuse, they have declined to offer any resolution. I am now unable to safely use the fsd system I paid over one thousand dollars to install, and Tesla has taken no responsibility. I believe this issue poses a risk to driver and public safety and should be investigated regardless of warranty status.

12 Adaptive Cruise Control problem of the 2024 Tesla Model X

I am reporting a potential safety issue with my Tesla Model X. When the vehicle is placed in reverse with the steering wheel turned to full lock, it unexpectedly initiates self-movement without any driver input. I raised this issue with Tesla service, and they responded with the following statement: "we are aware of this and [it is] currently a known software characteristic at this time. It is under investigation and to be resolved with a future software update. " Tesla has acknowledged this issue but has not provided a timeline for a fix. Safety concerns: this unexpected movement can pose a serious safety risk in environments where precise control is needed, such as driveways, parking lots, and tight spaces. It could indicate a malfunction in the vehicle’s drive-by-wire system, creating potential hazards for drivers and pedestrians. There is no way to predict or prevent this movement without significant driver intervention, increasing the risk of collisions or unintended acceleration incidents. Request for investigation: since Tesla has already acknowledged this as a known issue, I urge the NHTSA to investigate its potential safety risks. If this is affecting multiple vehicles, Tesla should be required to release a timely fix rather than leaving owners waiting for an undefined future software update.

Since February 20th, I have begun keeping a log of these occurrences. Below is a summary: [xxx]: 2-3 occurrences on [xxx] (from estes park to fort collins, CO) and 2 occurrences on [xxx] northbound. [xxx]: 2-3 occurrences on [xxx] southbound. [xxx]: 1 occurrence on [xxx] southbound and 1 near the [xxx] exit in colorado. [xxx]: 2-3 occurrences on [xxx] southbound. [xxx]: 2 occurrences on [xxx] southbound, on the morning of my service appointment to drop the vehicle off at the Tesla service center in loveland, CO. [xxx]: 2 occurrences on [xxx] northbound, on the way home from the service appointment. Each time the car abruptly slows down by 10-15 miles per hours, unless I intervene and stop using this feature. Phantom breaking and vehicle slow down events occur near daily when using the highway when adaptive cruise control is engaged. The vehicle will abruptly try to stop, or slow down aby 5-10 mph. This has caused several close calls with other drivers on the highways. Date of the incident is the latest. In the last two weeks prior to the incident, it has occurred at least 6-10 times.

Episode #2 'near-death-experience" Tuesday[xxx] Tesla Model X "auto steer" abruptly stopped on [xxx] between 1. 10pm - 1. 15pm sent a message to Tesla around 1. 30pm, exactly the same scenario happened on [xxx]. Coming from [xxx] going to [xxx] , after 10-15 seconds, car stopped abruptly. I do not know what happened and still shaken from episode #1, this incident happened with in 2 weeks apart. After Tesla's review, it was noted that autopilot was not active at 1. 30pm. Tesla can't do anything about this incident, as it is under beta version and it is the responsibility of the driver to be aware of such situations and should take full control of the vehicle. At that time in this situation, what control I have to take immediate control of when car stopped abruptly. Luckily no vehicles were coming behind and survived second time. Laptop screen blank, pressed gas pedal, breaks and after 30-45 seconds, laptop screen started showing up on the left side panel and quickly drove from the ramp. Basically malfunction of the software and/or a glitch caused car to stop abruptly. Now. We are shaken and worried what to do and go to work and do other chores. We have two Tesla and we are totally lost, now what to do and what options to choose for driving. Based on details posted on internet and number of complaints filed, it looks like that no action has been taken. What are the next steps from Tesla's perspective and NHTSA? f in the last couple of years, more than 1500 hundred complaints have been filed, fatal accidents and people died. While driving on highway with autopilot on . I received the message "steering assist reduced . Vehicle will require additional force to steer". I believe I hit a rough patch on the highway and my steering lost power and became very hard to steer the vehicle. Somehow I managed to drive for 20 miles to safety. When I recached home I did power reset few times and the issue went away. I noticed similar issues with Model X but not for year 2018 Model X. I have opened a service request with Tesla . Tesla send me an invoice of $5500 for repair.

Episode # 1: Sunday, [xxx] near-death-experience 2 times in 2 weeks while driving Tesla Model X 2023 with fsd. Both myself and wife were travelling and coming from toronto to lake george, NJ all of a sudden car stopped in the middle of the highway. Laptop screen was blank. In the mean time, turned on the emergency light. After a minute or so, white ute came (white) color came behind us with in less than a minute came from behind but he was able to steer on the left, honked couple of times and drove. I started panicking and do not know what to do and kept on saying what happened, what happened,,. . . . And at the same time, my wife asked me to pull car aside quickly otherwise someone is coming from behind is going to hit and that is going to be the end of our life (die) in a crash. Somehow started pedalling gas, break and then noticed laptop screen lightning up and quickly put the car in drive and pulled the car on the right hand side (extreme right) and no service lane. Pulled the car partially on road and partially on the road. Reached lake george late evening and stayed in sleep in & suites, queensbury, stayed over night, Monday and left sleep in on on Tuesday, [xxx]. I just drove couple of miles and was not feeling and was looking unstable and wife did not let me drive all the way from lake george to new jersey. As we parked our car at quick chek for a short break for few minutes. She noticed that instead reversing car from the car park and put car in drive and immediately applied breaks and at that time she did not let me drive. As soon as we reached back and went straight to our son's house and as our son is a dr. , and when told him about the episode he took me straight to jefferson's er worrying that I may have small strokes. Emergency admitted me in the hospital and started diagnostics e. G. , ctc, mri and other blood tests. Next day they released from the hospital and referred for a followup to neuro dr. This complain is regarding the recent recall regarding fsd Tesla Smart summon failures. Incident date June 02, 2024 car was parked in a parking lot outside a movie theater. I was less than 20 feet away from car with clear visibility, and tried Smart summon. Car pulled out of the parking making an acute turn damaging driver side door, and front bumper of car parked next to it. Tesla was contacted immediately regarding this malfunction of Smart summon and requested them to stop offering this service to owners as it is dangerous. They took 3 months to conduct an internal investigation but refused to share written results. Instead read a generic statement from their website. Still not received a copy of the investigation. Tesla also refused to take take financial responsibility for the damage and their false claim. Smart summon is dangerous and Tesla should not be allowed to advertise.

While driving CA-17, as is frequently the case, I had to disengage automatic steering several times to correct its poor lane keeping. Much to my surprise, the vehicle stopped maintaining speed, creating a dangerous situation with vehicles behind me as the uphill road caused the vehicle to rapidly decelerate the second I turned the wheel. After experimentation, I discovered that Tesla has completely removed cruise control capabilities from these vehicles unless you completely disable the fsd beta functionality, which makes this vehicle highly unsafe to drive in any environment where autosteer is inadequate, including CA-17. Further, this isn't just unsafe on weird highways. This poor decision on Tesla's part could very easily cause rear-end collisions on freeways. Nobody in their right minds would expect a vehicle to suddenly massively decelerate just because you correct its poor steering. This is just plain dangerous. This so-called "full self driving (supervised)" version needs to be recalled immediately, and Tesla must be forced to make cruise control fallback available regardless of the fsd state. Until they do, it is fundamentally unsafe to allow Tesla vehicles to remain on public highways and freeways with fsd (supervised) enabled.

22 Adaptive Cruise Control problem of the 2023 Tesla Model X

Issue is with Tesla full self driving subscription for $199/month and is available for inspection. No warnings or symptoms were indicated. The vehicle recommended I re-enable the service every time the vehicle restarted after it was shut off. Tesla's phone support stated the following is normal expected behavior, so an inspection isn't needed: 1. Automatically changed lanes on interstate into the space occupied by another vehicle multiple times. Thankfully, crashes were avoided by attentive drivers of involved vehicles. 2. Confused "40 mph minimum speed" signs for speed limit, resulting in over 30 instances were speed was drastically and suddenly reduced, without warning, every time the sign appeared. 3. Automatically changed lanes on interstate from rightmost lane to center lane despite no traffic present. This seems to be a default behavior that cannot be adjusted. 4. Disabled cruise control and lane keeping assist when driver overrode automated systems (accelerated, braked, manually adjusted steering) to avoid a crash. Kept these systems disabled as a punishment until driver parked car and restarted.

When operating under adaptive cruise control, the vehicle frequently slams on the brakes for no reason. This has become so bad that we are simply terrified to operate the vehicle under adaptive cruise control. The highway is completely empty in front of us, there are no hazards and no issues with visibility on the road. The cruise control simply reacts to a phantom in a highly dangerous manner. > what component or system failed or malfunctioned, and is it available for inspection upon request? the adaptive cruise control - you are welcome to inspect it. > how was your safety or the safety of others put at risk? we have suffered two near-miss rear-end collisions at high speed on the interstate. The behavior throws us against the seat belt restraints, causing near loss of control. > has the problem been reproduced or confirmed by a dealer or independent service center? we have raised the problem repeatedly with Tesla, and there are numerous confirming reports by other Tesla owners > has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? the system has been checked by Tesla, nothing found wrong. Tesla maintains that they are continuing to "train" the system, but after 5. 5 years, there has been no improvement. > were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? nothing - the car simply slams on the brakes.

This is regarding two Tesla recalls: - NHTSA recall number: 23v-838; tc2023-657 - NHTSA recall number: 24v-051 as noted in both recalls, they are to be delivered via software updates. My car is currently running version 2022. 8. 10. 16 7477b4ff8e78 which is clearly lower than the version required for both recalls. I have taken my car to service as my software version is not updating. Tesla has refused to comply with the recall and update the software version on my car. It should be noted, although irrelevant for the recalls, that my car is still under manufacturer's extended warranty, which we purchased along with the car when buying it new from Tesla.

I received a notice from Tesla for recall no. 23v-838 for my 2017 Tesla Model X - p100d. The notice states that I should check if my software version is "2023. 44. 30" or a later release. Unfortunately, the 2017 Model X is on a different update schedule, as it has different computer and sensor systems than newer Model X's. My software version is from 2022. Accordingly, the recall was not remedied by Tesla for my vehicle and all other older Teslas with the prior sensor suite. I checked for any software updates but none was available. At the very least, the notice is deficient, as if they pushed a separate update for older model Teslas, it was not communicated therein. Thanks!.

It have come to my attention by Tesla that NHTSA instructed Tesla to disabled auto steering for a week time, unlike in the past that it would be disabled for the rest of the drive, we have a 2023 mercedes eqs with same system and had other cars in the past with this system, this does not exist in any other car manufacturer and you did not instruct any other manufacturers to suspend the system for a whole week if unattended, this is a gross discrimination against Tesla and grounds for a lawsuit for discrimination.

On 1/12/24 at 12:55pm my 2020 Tesla Model X long range plus suddenly started braking while on the highway. I was driving 75mph to san diego from arizona. There was no vehicles around or in front while using the cruise control. This also has been an issue with our 2023 Tesla model y and previous model 3 while driving to las vegas at 70mph. I really think this is safety concern. Luckily there was vehicles behind us with all incidents that took place. I recently took the 2020 Tesla Model X on 1/19/24 for the issue and the technician found no issues while driving it.

Tesla issues encountered with Model X 2016: 1). Bought a demo car with a new car warranty, the car is supposed to be checked and verified that it meets safety guidelines, and certain standards, a). The car had a yellowish tinge had to get it buffed and detailed to look true pearl white cost $400 b). My tires were so bad the tire shop told me they are unsafe to drive so had to get them replaced the morning after I bought the car from Tesla-ohio branch and got back to chicago I informed Tesla about all nothing was done cost approximately $2000 c). After I purchased the car before I could drive to ohio to pick it up Tesla employee backed up the car into a pole I was told it will be fixed it was not cost $4k d). Wheels were all scratched up had to get them refinished cost approximately $900 2). Warrant is not honored always. Over the years camera and sensor very unsafe and every time Tesla says can’t reproduce even after providing proof of videos, one point finally got tired paid out of pocket to replace front 2 sensors they never bothered to take it apart and verify it’s working safely and properly, after paying out of pocket at least the lane change worked 98% of time cost around $500-600 3). Auto pilot is misleadingly unsafe, cameras and sensors have either reached end of life or simply not working but definitely unsafe because sometimes the car drives like a drunk person on streets, confused if lanes merging I had incidents where it would have caused an accident had I not taken over and there was no warning 4). Advertised range is deceptive and misleading, claim is my car to get 255 miles in reality I get around 120-130 which is more like half the range makes it hard to go out of town, an 8 hr trip becomes a 13-15 hr trip.

Display mcu1 is unsafe or at end of life and unsafe locks up goes dark and unresponsive for long periods of time, unable to drive sometimes my my car is under warranty either they should fix it or upgrade it, I was told I need to upgrade it but my car is still under new car bumper to bumper warranty either fix it or replace it, if mcu1 doesn’t work then replace it with mcu2 and honor the new car bumper to bumper warranty.

The auto-speed (cruise control) associated with the latest software release, 2023. 27. 12, is experiencing a problem that had occurred in Tesla cars a couple of years ago, namely "phantom braking". In the previous situation, the "phantom braking" was random. In this software release it is reproducible and, in my case, occurs at the same location every time I drive in that location. The car slows in a few seconds from 50 mph to 23 mph or less until I take intervention. This is dangerous in traffic. It occurs regardless of the level of autonomy selected or whether it is in auto-speed only or auto-pilot.

The contact stated that while driving 70 mph with the cruise control feature activated, the braking system erroneously activated causing the vehicle to abruptly stop in the middle of the highway. During the failure, there were no other vehicles or objects nearby. After shifting into park, the contact was able to restart the vehicle and continued driving. The cause of the failure was not yet determined. The local dealer was contacted. The manufacturer was not yet notified of the failure. The failure mileage was 4,200.
