My 2023 Tesla Model X began experiencing safety-related failures immediately after a Tesla software update. Right after the update, the vehicle’s cameras stopped functioning correctly and gps/navigation also stopped working properly. Since that point, subsequent Tesla software updates have failed to install successfully. Tesla diagnosed the issue as requiring replacement of the hw4 computer at my expense for approximately $4,500 because the vehicle is just outside warranty. I believe this may be a safety defect or a software-induced hardware failure because the malfunction began immediately after Tesla’s software update and affects safety-relevant vehicle functions, including camera-related visibility systems and vehicle localization. My VIN does not show an open recall, but this vehicle previously received completed Tesla software recall remedies related to camera/hw4 issues. I am filing this complaint because Tesla should investigate whether this failure is related to a broader hw4 defect pattern, software update defect, or inadequate recall scope. I am requesting investigation of failures involving camera function, gps malfunction, and repeated software update failure after an over-the-air Tesla update.

2 Warnings problem of the 2016 Tesla Model X

Failure Date: 11/12/2025

Description of problem: instrument cluster complete failure my 2016 Tesla Model X instrument cluster has completely failed and permanently displays "car off" regardless of the actual vehicle status, including while actively driving at highway speeds. Critical information I cannot see while driving: gear position (p, d, r, n) - I have no visual confirmation of what gear the vehicle is in vehicle speed/speedometer warning lights (brake, abs, tire pressure, etc. ) battery status any critical safety alerts this is an extremely dangerous condition that prevents me from knowing essential driving information required to operate the vehicle safely. Federal safety violations: this defect clearly violates federal motor vehicle safety standards (fmvss) no. 101 and no. 102. Gear position indicators must be "permanently displayed in view of the driver" critical safety information must be accessible to a properly seated and belted driver instrument panels must provide essential vehicle status information Tesla's pattern of similar recalls: Tesla has previously recalled vehicles for similar instrument cluster issues: 2023 recall (NHTSA campaign 23v-404): Tesla recalled 2012-2023 model s and 2016-2024 Model X vehicles for incorrect instrument panel warning light font size, specifically stating that warning lights with smaller fonts "can make critical safety information on the instrument panel difficult to read, increasing the risk of a crash. " 2022 recall (multiple campaigns): Tesla recalled model s and Model X vehicles for touchscreen failures affecting backup camera, window defroster/defogger, and instrument cluster displays.

4 Warnings problem of the 2018 Tesla Model X

Failure Date: 04/22/2025

Our Tesla Model X is failing recently with several issues as listed below: 1. 'autiopilot disabled' - autopilot was not available to use since 09/2024 after upgraded to 12. X fsd upgrade. Tesla service diagnosed in 11/2024 with some error connected to evaporator fan, advised me to replace the kit at an estimated $580 2. 'unable to start' - hv battery failed on 03/06/2025 with series of failures that was reported to NHTSA (voq#11651899). Tesla replaced the battery, and service delivered vehicle on 03/24/2025 3. 'hood unlatch' failure - while driving home from service center on the same day at 70 mph on I-95 freeway, the 'hood' popped up blocking the front view, and had to pull vehicle to shoulder to safety. There was no warning message on the screen at all. Took the vehicle to service immediately, and service team pointed that they forgot to turn on 'auto-latch' resulting hood failure. They offered the 'free' repair of hood. 4. 'ecu autopilot failure' - while taking the delivery of 'hood' fix on 04/22/2025, I noticed the 'autopilot' was still not working. Serviced diagnosed with 'failed autopilot ecu' at an estimated cost of $1791. 55. Currenlty the vehicle is with Tesla service team. My vehicle is with service team since 03/06/2025, and has been identified with series of issues/failures. It has caused severe safety failure putting our lives at risk with 'unlatched' hood while driving on a freeway. Also with hv battery failure its completely unreliable on when it might fail during driving. Now with failed autopilot ecu, we are unsure of the vehicles ability to navigate, drive function and safety concerns. Service team is unable to provide precise root causes or fixes, or restore it to a functioning vehicle. Every visit with service team there is a new issue with vehicle. Our family is greatly concern with the quality of the vehicle, persisting issues, safety concerns and overall reliability of the vehicle. Please find enclosed all service records.

5 Warnings problem of the 2019 Tesla Model X

I own a 2019 Tesla Model X. After Tesla upgraded my vehicle from ap2. 5 to the fsd hardware module, the front camera view became blurry. I disassembled the camera housing and found that a heated line in the windshield, intended to prevent fog or ice buildup in front of the camera, runs directly through the camera’s field of view. This line now causes noticeable image distortion, which interferes with safety-critical features such as fsd, forward collision warning, and lane keeping assistance. The Tesla service supervisor confirmed this is not my fault. He explained the issue may have been caused by heat from the line gradually affecting the glass, or that the upgraded fsd hardware, which captures clearer images, is now revealing a flaw that was previously less visible. He also mentioned that Tesla redesigned this component in newer Model X vehicles to prevent this issue. However, because my car is out of warranty, they are refusing to replace the windshield. Since the fsd upgrade, I have experienced multiple warning messages while using fsd. On one occasion, while driving on the freeway, the system suddenly disengaged and displayed a message saying the camera had weak visualization and the driver must take over. I believe this is a serious safety issue related to the blurry front camera caused by the heated line in the windshield. Although Tesla acknowledges the problem and that it is not due to misuse, they have declined to offer any resolution. I am now unable to safely use the fsd system I paid over one thousand dollars to install, and Tesla has taken no responsibility. I believe this issue poses a risk to driver and public safety and should be investigated regardless of warranty status.

6 Warnings problem of the 2016 Tesla Model X

Failure Date: 05/30/2024

On may 30, on a 55 mph highway, my Tesla Model X was traveling at highway speed in thick traffic. The vehicle was using cruise control, but not auto pilot. In other words, the auto steering function was not engaged. A vehicle that the Tesla Model X was following came to a sudden, unexpected stop. I applied the brake pedal with full force. The anti-lock brakes engaged and the brakes began pulsing (normal anti-lock brake behavior). However, while the brake pedal was depressed with full force, the anti-lock brakes stopped pulsing and the brake pads disengaged from the rotors. The car coasted as if the brake pedal was not depressed. The brake failure resulted in a frontal collision with the stopped vehicle. A police report was filed at the scene of the crash. There were no physical injuries and the Tesla Model X was driveable after the incident. However, the car that was struck had significant damage (but no bodily injuries). Because the Model X was driveable, I attempted to recreate the incident 12 hours later. However, as I tested the brakes, they behaved normally. With the fully depressed brake pedal, the antilock brakes stayed active until a full stop of the vehicle. Is it possible that the frontal collision system malfunctioned? as the frontal sensors detected an impending crash, did the car's computer disengage the brakes mistakenly? the car had no warning lights prior to may 30. However, at the exact time of the crash, two codes were registered. (I used the Tesla service mode to access them). One code states the anti-lock brakes were active. The second code registered exactly 2 seconds later and states "pedestrian safety system fault". There is a police report, but we have not received it yet. I had to check no in the form because the form would not allow me to move forward unless I uploaded the report. I can upload the police report when I receive it.

Failure Date: 07/09/2023

As I was trying to make a turn into the parking lot of a convenient store, my Tesla Model X started accelerating suddenly and unintendedly without my foot being on the accelerator. I kept pressing on the brake to stop the car, but it would not stop. I had no control and while it accelerated for 15-20 yards eventually it hit two other cars that were parked there. This happened so quickly. We all were terrified and shocked as I could not stop it from accelerating. I kept yelling "it is not stopping" to the person seating on the passenger side. I am confident that I did not press the accelerator pedal. The car was not on autopilot. My initial speed was probably 5 miles/hour, but it accelerated so suddenly that four airbags deployed, and the car is not drivable. Insurance company is evaluating whether the car is repairable, but I am afraid of driving with this car again until the problem is found and fixed. This could have turned to a major tragedy.

The windshield wipers, which are automatic only do absolutely nothing when it is raining. The only thing I can do is hit a button on the steering wheel each time I need the wipers to clear the windshield. Tesla claims is it working as intended. . . Which apparently is not at all. When it began raining the last time I drove the car, it was nighttime and I had to stop in the middle of the road because the car gets overwhelmed when you hit the wiper button too many times in a row and it stops wiping entirely. This is very unsafe. And here is the much worse issue. The automatic emergency braking triggers randomly all the time. If there is a shadow across the road, it might slam on the brakes. If there is an overpass, it might slam on the brakes. If there is nothing in front of you on a perfectly straight freeway with no other vehicles around, it might slam on the brakes out of nowhere. I just drove this car 60 miles today and had 6 braking events where it went from 60-70 mph to 30 mph or less. A few of these incidents happened with cars directly behind me. This car is going to cause me to be injured when it suddenly stops for no reason and a driver behind me cannot react in time. It needs to be recalled or taken off the road. One of the braking events was so sudden and harsh that the tires squealed for a moment. Also, this is my 4th Tesla. The first three had none of these issues. The latest one is dangerous to the point that I am not comfortable driving it anymore.

This contains new information, a smoking gun, and is associated with NHTSA complaint numbers 11505487, 11505488, and 11505489. Nhtsa recall: 21v-035; Tesla manufacturer recall: sb-21-21-001 I was threatened a $35/day charge and tow/impound though no maintenance was performed on my vehicle. I went to pick it up and the vehicle was still suffering from the recalled emmc issue which causes the screens to blank, freeze, etc. All driving information, safety features, and some exterior safety features are lost - it's a major hazard. This recall has two parts, the first part was a Tesla over the air update, which it pushed to the affected vehicles. I believe in doing this, Tesla has now stated to the NHTSA that the recall was complete. However, the recall has a secondary part "to be specific, Tesla will replace the vcm daughterboard that has the hynix 8gb emmc with a vcm daughterboard that has an enhanced 64gb micron emmc, free of charge. " which Tesla is completely denying accomplishment. I have requested the car be repaired 7 times over the past 6 months and they have denied and avoided the recall repair, stating that there are no active recalls and forcing a paid upgrade to the mcu2. Tesla is abusing the over the air update capability and falsely stating that they're correcting issues to avoid correcting safety hazards. My vehicle has experienced 13 failures over our 6k miles of ownership in 6 months. In those 13 failures, 6 nearly caused catastrophic accidents. Additionally, the service center in fort worth (275 university drive) has been altering/cancelling service records to avoid completing required recall repairs. The gm, jennifer was recently fired/resigned and the senior service rep dennis was moved. Tesla must fix its unsafe vehicles and I recommend double checking all recalls Tesla has "completed" via over the air updates. They're artificially avoiding accomplishment of the recall and leveraging the safety hazard on the consumer.

Tesla has killed two motorcyclists in the past 4 months. At night the ai can't tell the difference between a motorcyclist up close and a car far away. So, it assumes car and runs right into it. Another interesting issue with Teslas autopilot. They claim its 10x safer than a person. That not true. The claim is based on sheer accidents per mile. Autopilot is only really used on the highway, eating up miles quickly and in an environment where, although consequences of crashes can be more severe, they happen less often. When adjusted to compare highway driving autopilot to highway driving people, it's really only about as safe as a person, if that. In their 2022 cars Tesla has done away with radar. So all new Teslas have no idea the true distance something is from them. They are 100% cameras, while every other autonomous driving company is utilizing lidar. Lidar is expensive and ugly. That's why Tesla isn't using it. And taking radar out of their cars was just another cost savings measure. They were having a problem with the video and radar being at odds as to what's in front of the car. So instead of spending the money to fix it and fuze the sensors into a more robust solution. They just ditched radar and make the assumption that the video analysis is correct. ".

On 8/16/2022 Tuesday, I went to work at about 10:24 and parked by the road next to mcdonalds to call my husband. I wanted him to take the car to park elsewhere as that spot had no parking starting at 10:30. My husband was not available so I was about to move the car and had my feet slightly touching the brake. At that point the car suddenly accelerated and jumped forward without any control and hit a car that was coming out of mcdonalds driveway. That car was about 15/20 feet away from my car and I hit on the driver’s side. My car was in such speed that it pushed the other car 6 feet on the other side which in turn hit a pedestrian crossing the road. I am 100% sure that I did not press the accelerator and the car moved by itself in such force that I had no chance of controlling it. I’ve asked Tesla to investigate as well.

13 Warnings problem of the 2018 Tesla Model X

My wife was driving the car in was turning left on to a major road, she apparently hit another car trying to turn right on to the major road from opposite side. Both cars must have started from a stop sign and neither of them knew the speed. My wife does not remember the details of the incident, she was shaken up, initially declined to seek medical care but soon after could not recognize people, so had to be taken to er. The car with all the technology failed to alert the impending crash, did not apply emergency brakes, nor did not record the event which it was supposed to have.

This issue most likely plagued our Model X during it's original factory warranty based on the class action, nthsa inquiry, and nthsa and Tesla recall for the same issue which affects Model X buil between 2016 and 201. These are the things we have personally experienced 12 times over our short ownership, 6 times these nearly caused accidents - this catastrophic malfunction has occurred (on average) once every 500 miles during our ownership: primary driving instrument and safety info screen (small screen/driver dash screen) and larger infotainment screen both: freeze, “lock up” or “go blank” turn black (an issue known by some drivers as “the black screen of death”) fail to turn on repeatedly reboot display a “touchscreen unresponsive” message stop working completely otherwise malfunction these issues generally occurred at the worst possible times, most likely when the deficiently designed computer was over taxed (night, weather, traffic, cold - though it also occurred twice under basic conditions). While idling in drive having applied my foot to the break, the dash displayed h for holding the break. My car accelerated by itself forcefully into the vehicle in front. My seat belt did not lock up, air bags did not deploy, emergency braking didn’t work, the Tesla box report does not match the dash cam timeline. This was sudden unintended acceleration and I pray it doesn’t happen to anyone else.

While driving on an interstate at night traveling at 70mph the computer/console froze navigation and wouldn’t respond to touch commands. Then everything on the car went dark. No screen, no signal lights, nothing. Luck is the only reason I did not have an accident. This was terrifying and dangerous and I and others could easily have gotten hurt. My car had no lights and other cars couldn’t see me as I changed lanes (without being able to signal) to pull in the emergency lane. The car has had connectivity issues starting at 1 year after purchase (about one month ago). I am sometimes unable to play music and the computer screen reads “no connectivity”. It may be a coincidence that the free 1 year trial of premium connectivity just ended at the same time these issues began. I now pay $10/month for the service. Right before the screen went dark I had been on the phone, which was docked in the charger. This all just happened and I have an appointment in 10 days at the Tesla service center.

While driving on an interstate freeway on 8-14-2021, using the vehicle autopilot driver assistance mode (which I had been using for the previous 18 months without incident), the vehicle failed to avoid hitting the vehicle in front of me. This collision resulted in no passenger injury, but resulted in both driver and passenger airbags to engage and resulted in total vehicle loss of the Tesla Model X. No other vehicles involved had airbag engagement. Tesla may have actual information on the functioning of the vehicle and the autopilot software that may explain why the collision avoidance system failed, but refused to supply me with this information.

19 Warnings problem of the 2017 Tesla Model X

Failure Date: 11/21/2020

Touch screen extremely sluggish, sometimes unresponsive during driving trip, losing many active safety features such as forward collision warning, side collision warning, rear view camera, active navigation leading to distractions from trying to use alternative hand phone navigation. Lost speedometer function while driving as shown on image below making driving unsafe. Image below showed driving on a city street while both control screens center and driver side became frozen leading to panicking distractions. Car often slow to boot up or wake up, taking several minutes, in order to be drivable. This situation may become unsafe in case of real emergency requiring to drive the vehicle immediately.
