I am writing to report and formally document a very serious and distressing safety incident involving my Tesla Model X that occurred for a second time, on [xxx]. While driving routinely into the parking garage at my workplace, my vehicle suddenly and violently stopped without warning. There were no obstacles, vehicles, or pedestrians in front of me. The abrupt stop caused my body and head to jolt forward with force, resulting in whiplash. I am still feeling dizzy, dazed, and disoriented, and I believe I may need to consult a neurologist as I have a headache from the whiplash. This is not the first time this has occurred. A similar but less severe incident happened previously while I was driving on [xxx] . At that time, I dismissed it as a possible glitch, but after this most recent and violent episode, I am deeply alarmed. I am extremely concerned about the safety of this vehicle. The autopilot ecu failed on my car per Tesla. There was no warning on the car that my safety features were disabled. This is also reported online as a common issue and Tesla is charging $2k to repair a safety computer. The computer was advertised as two chips as a failsafe but that was not the case as both failed on my car. I suspect this software updates broke the computer as well and not at fault from the customer.

Advanced driver assistance systems (adas) – safety incident report: while using the vehicle’s ‘summon’ feature, the car failed to stop when I released the control button. It continued forward, drove up and over a large curb, struck a signpost, and came to rest with one tire elevated on the curb/grass and the other three tires on the pavement. This “jumping curbs” behavior and collision with fixed objects represents a serious safety hazard. I am extremely grateful that no person was nearby or inside the vehicle at the time, as this failure could have resulted in injury or death. The incident is documented on video. The vehicle is currently pending inspection. Despite my request, Tesla has not responded or addressed the issue, leaving the defect unmitigated. At no point did the vehicle display any warning lights or messages indicating a malfunction before or after the incident. Given the nature of this failure, its potential to cause harm, and the lack of response from the manufacturer, I am submitting this report to alert federal safety authorities and initiate appropriate investigation into the safety of the ‘summon’ feature and the vehicle’s adas systems.

Was stopped at a red light behind the car in front of me. I was stopped at a appropriate distance from the car in front of me. I had my right foot on the brake pedal, when the car accelerated on its own. It launched its self into the car ahead of me. I looked down and my foot was still on the brake pedal. None of the air bags inside the car deployed.
Based on thousands of posts, replies, etc, this seems to be a well known issue with this model/year Tesla as well as other years/models. It seems that the wheel sensor cable is not fastened properly, and over time, the cable runs against the tire, causing the internal wires to be exposed, this in turn causes numerous faults to occur, which includes causing abs, lane departure avoidance, traction control, stability control, abs, and regenerative braking to stop working. Hi, my steering wheel turning very hard even though NHTSA issued the the recall last month but your website says your vehicle is not on recall. Also my rear camara is working still Tesla said needs to be replace again even though Tesla replace in 2023 which is I paid. My autonomy is not working even though I paid $8000 plus $500 tax when I purchase my Model X in 2016 for full self driving/ Tesla doesn't want to honor this to us. Just got [xxx] yesterday morning. I have ~350 miles on this version now in ~24 hrs. First time I've ever completely turned off fsd and went back to autopilot only. This release is absolutely 100% dangerous and needs to be pulled and rolled back ASAP observations: 1: does not honor turn only lanes and drives straight through them. 2: constant lane switching no matter what setting it's on. 3: lane changes without using turn signal. Example, in right lane, it fully changes lane to left lane using turn signal, then decides to just go right back to the right lane again without using turn signal cutting off other vehicles. 4: speed issues, it can't decide on speeds. 45mph road. I do like 5mph over. With no other cars around, it will speed up to 55-60 (above max setting) then drop down to 40-43mph. It's all over the [xxx] place! even worse on freeway. So it actively exceeds speed limits, as much as enough to get my license automatically revoked under wisconsin law! 5: redesigned where set speed limit is displayed. No longer shows unless you actively change it. Then it disappears again. Need to constantly fiddle with set speed just to see how crazy it will decide to break the law, or how slow it will go and [xxx] other drivers. 6: rolling stops, had a number of rolling stops today and one where it blew straight through a stop sign. 7: route choices are absolutely alinidine. Seems to prioritize taking the least efficient routes. 8: drives like a bucking bronco with accel and decell. 9: ping pongs within the lane. False emergency braking events. Last release the double stop at stop signs had gotten better. This release they are full back again. It is once again stopping at both sides of the intersection. Vehicle departing lane warnings when driving manually, even when my turn signal is on so I can pull into a parking lot or my own driveway! with the above, there have been no other changes other than firmware update. On oct 2, 2024 while going to school to pick the kids, when my wife was turning right into a parking lot after slowing, my Tesla Model X plaid car acceralated and went into high speed (plaid mode) hitting a tree at top speed even though brakes were applied. Upon research, this looks like sudden unintended accerelation. The car luckily hit the tree on the passenger side and the front right wheel flew to the back seat and all air bags were deployed. The car was totaled. Miracleously, she had minor external injuries but due to the speed of impact with the tree, she was flung forward and after the CT scan, we found that she had vad with severe damage to both her neck arteries. She was taken to hospital in an ambulance, had a surgery and still has trauma and medical appointments to fully recover. In Mar 2024, there was always a software problem with my car failing to detect the car was open and hit the garage roof with the falcon wing. On another occasion, the car failed to detect my other car when it was too close to it and I had to apply the brakes although the car was to stop automatically. Tesla refused to treat this as warranty even though the car should not move when doors are open. After 6 months, I ended up fixing the car at a Tesla collision centre. After multiple follow ups via email, chats on Tesla app and phone calls post the accident on oct 2, 2024 with Tesla pleading with them to provide the edr report and address the issue as warranty and not hearing back, I am logging this so that customers are aware of this dangerous flaw that can be fatal. It looks like Tesla wants to delay the case for long so that they can force the customer to accept a low settlement. I was also planning to buy a cyberbeast but not going to anymore.

When driving my car last Friday evening at ~5:45pm pst, I was about 1 mile away from home when alerts popped up on my car including- "regenerative breaking unavailable", "abs disabled", "stability control disabled", "traction control disabled", "steering assist reduced", "automatic emergency braking is unavailable" front passenger safety restraint system fault" "park assist unavailable". I drove the last mile to my house and when I pulled into my garage my gear shift would not work to put my car into park. I tried to then do a "soft reset" by holding both buttons on the steering wheel down, and this would not work. I frantically called the nearest Tesla service center and after 20 min the person there told me there wasn't anything they could help me with and gave me the number for tessa roadside assistance. This whole time I had to have my foot on the break pedal to keep my car from moving forward and going through my garage wall into my house. I was on hold with Tesla roadside assistance awaiting an answer for another 30min, and after talking with an agent he told me to "find a large rock to put under my tires to prevent the car from moving forward", this wasn't a feasible option as I was alone with a 12 year old girl, there are also not any large rocks near my garage. Tesla service then connected me with a towing agency, but let me know that since my car warranty only covers my battery and drive train, I would have to pay for the tow out of pocket. The tow truck driver came about 20 minutes later, so at this point I was sitting in my garage with my foot on the break pedal for well over an hour. There was no way that anyone could power off the car or get it into another gear other than drive. There was no way to access the 12v batter and disconnect the car since I couldn't open my frunk as the car was not in park. Without the assistance of a tow truck I could have been stuck in my car for hours holding the break pedal.
On July 29, 2024, when my husband backed the vehicle out of the garage slowly, the vehicle suddenly accelerated and it hit the stone column causing a big damage to the car. I believe the pedal speed control and emergency brake have problems. When I reported to Tesla, they said the vehicle was not at fault. They said the issue had been resolved remotely by the engineer and cancelled my service appointment. When I asked for the engineer report, Tesla rep told me to download the data report. After I downloaded the data report, it shows no data 2 hours before and after the time of the incident. When I tried to retrieve the video on the replay function in the vehicle, the video was not there anymore. Fortunately, my husband recorded the video on his phone and I have a copy of the video. I feel very disappointed and frustrated with how Tesla handled the issue. If they are not at fault, how come the data report and video are not available? I filed a claim under my auto insurance policy and got the vehicle fixed. This sudden unintended acceleration happened to me back in June when the vehicle accelerated by itself to 41mph. My daughter, who was sitting next to me in the front passenger seat, saw my foot was in the air but the vehicle kept moving at 41mph. We were so shocked and I had to use both feet to step on the brake to stop the vehicle. Tesla eventually replaced the steering column control in the beginning of July. Then, on July 29, the sudden acceleration happened again to my husband. This safety issue needs to be addressed by Tesla more seriously. I should at least get a written explanation of what happened and not just a phone call telling me the vehicle was not at fault. Since the damage to vehicle is fixed now, we would drive the vehicle more often. I don't want this sudden unintended acceleration to happen again, hitting something or someone on the road!.

The car was being summoned out of a parking spot using the Smart summon feature, as the car began pulling out of parking it did not see the car to the right of it and turned into it causing segnificant damage to my car as well as the other.
8:40am Saturday Apr 20 I was driving southbound on hwy 121 in dallas, TX (near spring creek exit) in Tesla modelx, with 2 kids in the back seat. It is raining. My speed is around 60-65mph and I am in the rightmost lane. Suddenly the vehicle skids a little, and my steering wheel and brakes are not working anymore. It seemed that the car took complete control and does a full 360 rotation, crossing 4 lanes on the highway. I recall the 10 seconds when I am facing the incoming cars and the only thought I had "this is the end". For whatever reason, the incoming cars slowdown and my Tesla makes a full circle, crosses 4 lanes again and gets positioned correctly in the flow of the traffic and in the rightmost lane. My speed now is around 20-30 mph and I suddenly regain control of the brakes and the steering wheel. My kids are crying in the backseat. I still am trying to figure out how and what happened, I cant believe I am alive and I did cry thinking that anything could have happened to my kids. We could have all died on the spot given the rain and how fast the highway traffic goes. I am 47 and have been driving for about 30 years. I know how to control minor skidding in rain. I go skiing frequently and can drive on ice (but this was no ice, just water). The only explanation is the malfunction of the Tesla mirrors/cameras (perhaps due to rain water), and taking the wrong decision to swerve the car completely across 4 lanes back and forth. I wish nobody has to go through such life-altering experiences due to a car malfunction, I will never forgive Tesla for the nightmares I still get and the trauma I and my kids had to go through due to the error of a machine. There were no issues before or after with the car, no damage, our tires were relatively new, no police was called. Tesla of course must record incidences like this and they did call me and escalated this to their highest level.

Purchased this vehicle new from Tesla on March 29, 2024. As I got home I hear a pop sound come from underneath the car and the abs warning light came on. The vehicle lost some of the functions like regenerative breaking and cruise control. The suspension was also inoperable. I brought the car in on the first availible service date to get the repair done which took a few days. After repair was completed I went back to pick the car up only to have the same warnings appear on the odometer and lost of functionality again 20 minutes into driving home. I contacted Tesla and they gave me another appointment at a different service center. I then brought the car to the other service center and they corrected the abs and other warning light issue. However, I been experiencing vibration on highways when breaking and regenerative breaking. Since the beginning of ownership. I mentioned it to them but the told me vibrations can be normal with these cars so I endured it ever since until it felt worst after a while. I brought the car in again and they diagnosed it. They recommended changing the rear rotors and brake pads. After picking the car up its still doing the same thing. This costed me $1200 out of pocket. I brought the car in for front end noises and they changed parts under warranty. The noises was gone after the service but a week later I heard the noises again and it worsened. Now they are refusing further repairs and recommended that I take the car to a auto body repair shop.
I was driving on i395 from carson city to reno in washoe valley while on autopilot, the car suddenly alerted with very loud alarms, showed 1. Steering assist reduced, 2. Automatic emergency braking is unavailable 3. Traction control disabled 4. Stability control disabled 5. Park assist unavailable. I pulled over to a safe parking area. Then the car locked out for 75 mins, in neutral gear, can't shut off, can't park, can't reset the car at all even after talking to Tesla support. While waiting for the tow truck, I turned it into tow mode, was able to change into park mode finally, and hard reset the car. I took it in and Tesla told me the steering system was the problem and I was out of warranty so I had to pay a lot of money to repair/replace it.
The Tesla recall under NHTSA recall 23v-838 has not been completed by Tesla. The upgraded computer I purchased from Tesla and had installed in December 2021 cannot accept the ota update. Tesla is requesting that I replace the computer again for 2 thousand dollars for the recall ota. Telsa said my current computer has an internal bug and needs to be replaced. I should not have to spend 2k for another computer that barely lasted 2 years.
Ever since a recent software update, when turn signal is activated, right fender camera feed appears on screen but is completely black instead of showing a visual of the right blind spot. Due to this other features such as autopilot, automatic emergency braking, and likely most of the adas functions are also disabled. Due to the vehicle's poor natural visibility, there is a high reliance on the camera systems functioning properly. Not having a blind spot camera endangers turns and lane changes, especially if one has become accustomed to their usage. With most if not all the adas features disabled, we are returning to a 1990's level of auto safety. The problem remains unresolved despite a recent 2 week stint in Tesla service. Since this is most likely a software issue (possibly a regression introduced with the recent rear camera recall fix) there is no recourse except for Tesla to issue a bug fix. At night, vehicle incorrectly identified a dog running alongside the vehicle as a human and engaged the automatic breaks causing 5 occupants to suffer minor injuries including whip lash. One occupant was a baby in a car seat. Two weeks later also at night, the same event occurred. Tesla has refused to provide a means to permanently disable the automatic breaking feature when a human is in control. The software incorrectly identifies a dog as a human, this is a serious software bug that has caused harm to my family. If a human is in control, I am responsible, if Tesla is in control then they should be responsible.
I had an interaction with this vehicle on snow/ice packed i70 in colorado, almost to the kansas border. This car passed me, then immediately cut in front of my truck in the right lane and quickly slowed to about 10 mph with no warning. The car then pulled over onto the snow packed shoulder.
The vehicle was in cruise control and had a phantom braking event. The vehicle was going at 70 mph and suddenly braked even though there was significant distance between the car in front and nothing on the road. The sun was glaring on the road and possibly the car detected the shadow on the road as an object and braked because of it.
I was driving the car in stop-and-go traffic on a city street. After another stop, the car in front of me started moving. I pressed on the accelerator with the usual force, but my car took off like crazy. I pressed on the breaks, but the car didn't seem to respond to the brake pedal and ran into a car in front of me. Automatic emergency breaking system didn't engage as well. Luckily, nobody was injured. Had there been any pedestrians crossing the street, they would have been badly injured or killed. This uncontrolled response from the car is very dangerous and I cannot trust it anymore. As a result of this accident, the car in front suffered damage in the amount of a few thousand usd, and my Tesla Model X - $40k.
The steering wheel was dysfunctional; the wheel was locked in the middle of the highway, and the car started to change lanes, causing a massive accident. There was a significant accident between my car and a truck; both cars were totaled. My airbags didn't open as well. I got a massive concussion, and I do not know about the condition of the other driver. There was a recall about the steering wheel, and we brought our car to the dealership, and supposedly they fixed the issue. There was no warning or message before the accident. The message appeared on my Tesla screen for the first time less than a few minutes before the accident.

1. Tesla failed to adequately warn us that the 6 sensors in the front and back of the 2023 model have been removed. Comment from Tesla collision center team in carlsbad: "we both understand the calibration issue won’t be fixed overnight. A program should be in place to educate customers on the change from sensors to cameras. " 2. Objects and images in the rear camera are much closer than they appear and the camera sensors do not work properly. Where was this warning when we bought this car? from jared's Tesla service report: "there are certain feature limitations which can trigger warning messages and cause some inaccuracies when using this feature. Service engineering is aware of these issues and is working towards implementing improvements via future firmware updates. " 3. As a result of Tesla's failure to warn us, I relied on the rear camera and camera "sensors" and backed into the corner of my neighbor's open garage causing $6,322 in damage.
Brakes will lock up a pre collision will activate in forward and reverse when no objects are present. It is a hazard to operate the vehicle since it will randomly slam in the brakes will driving down the road or reversing out of a parking space. Also electrical issues vehicle will no longer lock or unlock.
The autopilot was engaged and the system failed, it james the breaks hard and it started scraping the body above the tire.
Suddenly getting harder to turn the steering wheel to either direction. The problem happens randomly and also looses the power steering, needing to forcibly bring the steering back and making the driving difficult. When this happens many alerts pop on the dash including traction alert, break alert, and line alert.
I drive a 2020 Tesla Model X long range plus. I bought it new and now have about 50,000 miles on it. Over the years, I have experienced quite a few phantom breaking incidents when the car was in auto pilot (self steering mode). Most of these were fairly minor in nature and not much of a concern. This July I drove the car to colorado and back and had innumerable severe phantom breaking events. In the past the breaking would be fairly gentle and a tap on the accelerator would return the car to the preset speed. In July, the car was essentially “slamming on” the brakes and required an aggressive press of the accelerator to disengage the breaking. It felt like the car was going to come to a stop in the middle of the highway. I was cruising about 70 mph and would say that by the time I could get to the accelerator the speed would be down to about 60 mph. None of these events created any problem with other vehicles, but they were disturbing, nonetheless.
On multiple occasions (between 6 and 10) while driving east and west on interstate highway 80 between reno and salt lake city the vehicle executed a phantom braking maneuver while autopilot was engaged. I was east bound on 2/2/23 and 2/24/23 and west bound on 2/7/23 and 3/5/23, I. E. , these events occurred on multiple days in multiple locations. On no occasion was there any danger of collision with another vehicle or other obstacle. In fact, the braking was most likely to occur as my vehicle topped a rise in the highway followed by a gental downgrade with no other vehicle or turn in the road for at least a mile ahead. The braking was very hard and caused my vehicle to slow by at least 10 mph before I could react. On one occasion, I immediately accelerated to return to the posted highway speed of 80 mph and the vehicle twice more braked hard after I accelerated back to highway speed. Had there been a vehicle behind me, it's likely I would have been rear-ended on multiple occasions. The problem has not been independently verified (though I understand the NHTSA has received many similar reports). The vehicle has not been inspected. There was no warning that the vehicle was going to brake hard though I recognized the circumstances in which it was likely to occur and was prepared to accelerate to counteract the effect. The section of I-80 in which these events occurred is lightly traveled and if Tesla autopilot should be flawless, it should be flawless there! another source of autopilot phantom braking is flashing lights next to the road warning of animal crossing or prison zones. However, in this case, the braking is gentle and not a hazard. In contrast, 2022 when driving the same section of I-80, phantom braking, as described above, was much less of a problem but flashing lights prompted the vehicle to brake hard. It appears Tesla has fixed flashing light-related problem.
When exiting an on-ramp on a freeway, the car suddenly veered to the left on its own, almost colliding into another vehicle. There was no dangerous activity nor any stopped cars anywhere around the car.
While driving in the carpool lane with my mother in law to work with my wife, the car abruptly activated the brakes while in auto steer mode and necessitated my immediate slamming on the gas to increase my speed to avoid a rear collision.
Continuation of complaint 3 of 3 though this occurred 12 times over 6 months and nearly causing 6 accidents - tell me if you need me to make 6 separate complaints - 12/8/2022 - vehicle key fobs stopped working (still able to start vehicle from the Tesla mobile app) and the screens completely locked up while driving multiple times nearly causing multiple accidents. It's also likely that the key fob failure was a result of the malfunctioning computer. Cleared computer cache. 12/23/2022 - battery fault (vehicle may not start warning) bms_w035, front passenger safety restraint fault, buggy and unsafe screens; still no information from Tesla regarding the recall for the malfunctioning screens and computers. Tesla stated that they could not "see" any of the faults in the vehicle or in their server, though my wife took photos. 1/16/2023 - service request scheduled for warranty/recall repair for unsafe screen hazard that was resetting while driving, vehicle towed to Tesla; 14 Jan 2023 Tesla cancelled the service request and input their own request that describes a customer inquiry for an infotainment (computer) upgrade and that we've declined the repair. Based on this, I am unable to download the invoices because they've jammed up the system to further impede proving their negligence. Currently, Tesla has locked up the app so that I cannot create service requests for the warranty/recall issue, they're failing to repair the vehicle, they're failing to communicate beyond threats of daily charges and towing, and it is impossible to reach someone at the service center though I've been able to describe the issue on multiple occasions to the Tesla phone answering service folks. Tesla is out of control and has placed my family and others in harms way due to their negligence to inform and accomplish their warranty and recall requirements. I respectfully request your help in holding Tesla accountable so others are not longer put at risk, and so their vehicles get repaired.
This is a follow on complaint due to this occurring 12 times over the past 6 months, 6 of those times nearly resulted in catastrophic accidents. I wrote Tesla via certified mail, per their owner's rights requirements in Dec 22 and Jan 23, stated the problem and requested repair, replace, or repurchase. I gave Tesla ample opportunity to repair the vehicle. Tesla now refuses to repair and is failing to adhere to their own warranty, recall, stated owners rights, TX code, and nthsa guidance (recall and likely codes due to breach), and they're now threatening me and behaving negligently by asking that I take their malfunctioning and hazardous vehicle back out on the road. Our vehicle has been out of service 2 months total and 1 month currently over multiple corrective action attempts for a recalled item. Service visit invoices versus reality, plus dates in and out of service do not include additional vehicle down time while waiting for service visit time slots - we purchased the vehicle on 7/23/2022 - I do not have access to records prior to this date, though I am certain this vehicle was plagued by the recalled issue under factory warranty. I firmly believe, based on Tesla's actions under my ownership, that they will do anything in their power, including blocking or changing vehicle history so they can avoid their responsibilities. 7/23/2022 - key fob battery replaced; invoice failed to include service center employee hard resetting the computer system and faulty ac system, plus trim piece identified as needing repair 8/10/2022 - hvac and trim corrected; invoice failed to include issues with computer and technician described that some of these require resets sometimes, but failed to provide info about the recall and the nature of the failures. I described that the screens would unsafely reset on their own and the technicians failed to provide any further information. 9/16/2022 - vehicle failed to charge at high speed charging and buggy/malfunctioning screens included.
When using autopilot (handsfree) feature, it brakes automatically. In some cases, there is no car in front of it and applies short brake automatically. During bottleneck traffic it gets worst, applies throbbing hard brake and due to excessive hard braking rotors/pads are going bad very often. Whatever is on your seats, will be on the floor and feels like you are having a stroke. Due to the defect in autopilot feature, rotors/pads are going bad and need to spend lot of money replacing them. As Tesla service centers are pretty expensive. Attached is the invoice. I feel that brake pads/rotors/labor should be all covered under Tesla warranty. Autopilot feature is very dangerous to use and it also creates lot of stress when the feature is engaged. One of the location/time stamp sharing it below, pretty sure Tesla can pull it off from their history as well. There was no car in front and it just applied hard brake: Dec 7th 2022 at 10:10 am.
