During an accident, both side airbags deployed (front airbags did not deploy). Front right passenger injured by side airbag -- bleeding puncture wounds (approx 3) on upper right wrist, severe raised gouges (below the skin) from that point up the entire length of the arm (more severe on forearm). Passenger recalls something shooting out from "a" post. Passenger went via ambulance to er. Later, found broken jagged piece of white hard plastic in the right front passenger seat. Piece is in our possession as well as photos and medical records.

The 4runner struck a vehicle that ran a red light. The 4runner was at approximately 35 miles per hour and struck the other vehicle, a Nissan altima side on with enough force to propel that vehicle two lanes into another vehicle. The airbags failed to deploy on the 4runner but did deploy on the altima. The roads were drive and visibility was clear. Injuries to the 4runner driver included: head from contact with grab bar above drivers door, neck, back, wrists, fingers and arms. Altima driver had to be removed by paramedics and rushed to the hospital. Investigation of the vehicle computer showed the vehicle was unknowingly sold to me with errors in the multi-terrain, esc and pre-collision system. Vehicle was purchased in November of 2019. Not sure. Wife was driving 2015 4runner into parking lot at low speed and it jumped a tree. Not at all saying it is a Toyota issue - just to be on safe side reporting. Air bags did not deploy. Understanding is it just ended up there when she was parking. Epic photo - all the same - worth documenting. She advised she was stopped or just stopped prior to the tree-climb. Pax side was daughter and taking her in for medical today due to complaints of pain and weakness. Police came but did not issue report.

On 6/15/2017, the driver's side airbag in my 2008 Toyota 4runner did not activate during a frontal, 35 to 40 mph collision. I was traveling at about 35 to 40 mph on a four-lane state road. Another driver pulled out in front of me from a two-lane state road. I couldn't stop and ran into the driver's side of that vehicle. The result was a "t-bone" traffic accident. I was wearing my seatbelt and my airbag did not deploy. I suffered somewhat minor injuries to my right hand, the right side of my chest, and my right foot and ankle. The left rear side of my neck was sore. I could not walk due to the foot/ankle injury so I was transported by ambulance to the emergency room. The other driver was not injured but I think his passenger might have been. The other driver was cited for "failure to yield to oncoming traffic". Many police officers and firefighters on scene asked me why my airbag did not deploy. They all thought it should have worked due to the damage they observed on my vehicle. My 4runner has not been subject to recall. I have photos of my 4runner, injuries, and the police accident report available but I could not attach them to this letter. The insurance company has said that the vehicle is repairable even though it has frame damage, major front end damage, and airbag deployment with an estimate of $12,000 in damage.

The inspector send me an edr report and it showing airbag not commanded. ( unable to read / understand all other numbers on that report). The inspector informed me that, Toyota will contact me about the report. As of today no contact or any informations from Toyota about the edr report.

As per the inspection reports, Toyota says I was driving 50 mile per hour when the traffic stopped suddenly in front of me. And I stepped on the brakes but the vehicle did not have enough time to stop and I hit the vehicle in front of me in the rear. The vehicle was inspected on may 17 2013, because I filed a complaint through safecar. Gov. When I had the accident I contacted Toyota few times to inspect the vehicle while the vehicle was in the repair shop. However Toyota refuses to do that. Now the edr report from Toyota says, vehicle appeared to have been already repaired, so making a complete determination as to why the airbags did not deploy cannot be done. That letter also asked me to send any pictures of my damaged vehicle. I send the pictures to Toyota and now another letter saying, after review of the photographs of the damage, this was not a direct frontal impact but an impact directed to the left front of the vehicle. There was insufficient abrupt forward deceleration of the vehicle to deploy the airbags. My insurance company estimated damage to the whole front part of the vehicle and paid $9300. 00 to repair the vehicle. I believe Toyota don¿t want to say that it¿s the airbag fault, because then they have to recall all the 4runners to repair the airbags. Anyway I am still alive, and I don¿t want any other owners to go through this kind of tragic accident. Hope safecar. Gov will watch Toyota for its failure in the airbag safety of this vehicles.
