The dim headlight only, not the bright, only on the drivers side front goes out 4 or 5 times a year. The front brights are not affected and have never gone out nor has the dim headlight on the passenger side front ever gone out. The car is driven daily usually and on city streets only.

Headlight bulbs burn out frequently. We have replaced probably ten (10) individual bulbs over the past several years. Toyota dealer achknowledged there is a problem with Prius v headlights but would do nothing for us. . . We would have to pay to have any work done on wiring harness, etc. , to hopefully correct the problem.

Headlights go off without warning while driving at night. Very dangerous!!.
Headlights turn off or flicker while driving. It happens intermittently. It is a major safety issue when driving at night as it nearly caused an accident. Using the high beams helps for a short period but the headlights still turn off on their own. Turning them off and on brings them back on most of the time but only for a short period of time.
One headlight turns off randomly. Every now and again turning the lights off and on again will fix the problem, but when you're driving 70mph down a dark highway you don't want to have to keep your fingers crossed that the lights will come back on.
My vehicle shut down on me while driving. All lights came up on the dashboard. Toyota dealership says the wire harness needs to be replaced for 4600 dollars. Couple days prior to vehicle shutting down, headlights began shorting.

While driving at night, left headlight goes out intermittently (about every 3 minutes). I have to turn lights completely off then on again to reset. This is ongoing and appears to be happening more frequently with time.
Every day the headlights go out randomly at night. Very dangerous.
Headlights shut off while driving at night. I received two warning tickets from highway patrol as I was driving back from ft. Worth to nacogdoches, but when I stopped, both headlights were working. Then as I was driving, both headlights (with bright lights on) turned off. Had to pull over and then they started working again.
My headlight consistently flickers on and off. I have changed my bulb, but it continues to turn off at times. From my research online, it looks like the 2007-2009 Prius models were recalled due to faulty headlights.
One of the front headlight sometimes work sometimes not. I just get this car today and after purchasing I saw this problem.

Low beam headlights turn off/fail. I have replaced 7 low beam headlights in the past year, four in the past 2 months. In the two most recent cases both low beam headlights failed within minutes of each other while driving at night at highway speeds. I had a Toyota dealer check the wiring harness per Toyota warranty notice after the last outage and they stated nothing was wrong. When I had the lights checked by a different Toyota dealer in Jan 2019 they indicated the socket for the bulb was melted. Bulbs have gone out under, wet, dry, hot, and cold conditions. The most recent outage occurred on March 8, 2019 while driving on the interstate. .

The passenger side headlight turns off unpredictably, usually within 10 minutes of driving time. Turning the lights off and back on sometimes gets the headlight to turn back on, but not always. Then within 10 minutes it will turn off again. Not only is this a safety issue and driving violation, I also cannot sell the car with non-functioning headlights.
The lighting system has electrical markings on the relay and fuse blocks for daytime running lights, with no daytime running lights installed. Headlight and license plate light systems works intermittently. Causing bulbs to fail.
After the original light bulbs for the headlights burned out we bought high-quality replacements only to have them burn out rather quickly. It's now been a year-and-a-half and the light bulbs are burning out an average of every two months at most. We've since discovered that there has been a wiring harness recall for all sorts of other Prius makes and years but Toyota refuses to acknowledge this problem with our car. A very cursory and 15-minute google search shows that this is an issue with my model year and several others that has gone unaddressed. The last time this happened we discovered that are headlights had burned out because we were driving at night, on a lit but darkish road, when suddenly people started pulling out in front of us at the intersections we were crossing. This problem needs to be addressed before someone dies through no fault of their own. The problem started around 60,000 miles about a year-and-a-half ago.
While driving driver side headlight will go out, followed by passenger headlight. Both will return on when the headlights are turned off then on again, only to occur again within 10 minutes. Both headlights have been replaced within the last 10 days.

We have had multiple low beam bulb failures. Usually it starts with 1 the burns out the other within a week or two. In the last year we have replaced both bulbs 3 times. The last time was February 24th last night on March 11th we received a warning ticket for a headlight out. I went to replace the bulb again and it was good, after removing it I reinstalled and it worked. Shut the car off and came back out and it wouldn't come on again without disconnecting the harness and plugging it back in. There is noticeable discoloration on both headlight plugs.
Headlight would burn out a year after replacement. Manufacturer issued a variant of a recall intending to provide a fix for high rate of burnout. I took my car in to a dealership to receive the fix and the dealership caused a problem with the front windshield wiper apparatus. Wipers would now work if the headlights were off, but would no longer work once the headlights were turned on. This would occur whether stationary or moving.
Front head lights went out within a few weeks of each other. Replaced both bulbs. Then they went out again with no warning in about a month. Had to use brights to drive home in the dark. Dangerous! replaced them again and now one of the head lights is out again after only 2 weeks even though it is a different brand. Headlights have gone out while driving and when it is parked.

Headlights keep burning out. Both sides. About every four months. This is dangerous!!! and unnecessarily expensive!!!.
My 2012 Toyota Prius; the vehicle repeatedly blows the front head lights. My vehicle is less than 6 years old and I have had 10 front lights replaced. The lights just go out while in motion. The low beam head lights both fail while driving on the freeway at night. The road went to total darkness. I feel lucky the high beams were still working. I have taken the Prius c to the shop and have been told the headlight assembly on drivers and passengers have melted. The will have to be replaced.
In the past year we have replaced multiple low beam halogen headlights. One week ago, both burned out causing a nighttime safety issue. I researched the lighting issue and learned there is TSB t-sb-0204-12 for headlamp durability enhancement and a customer support program "zhb" for this identical problem. In the zhb program, Toyota will make the necessary repairs free for only 2010-2011 Prius'. Since the zhb program only supports 2010-2011 Prius' I called Toyota to request this repair under that program. Headlights flicker and go out even after replacing the right headlight that seems to be the problem. Not safe to be going d down road and headlights go. Out completely. . It has been happening for. Nearly 6. Months. Had headlight replaced 3 months ago.
There's a voluntary recall that Toyota is doing on the Prius right now that seems dangerous and poorly executed. The low beam headlight wiring harness has a defect that causes it to burn out headlights or short out in a way that the headlights won't work. Sometimes you can get the headlights working again by wiggling the wiring harness, but I've had this occur several times while on back country roads and you just have 1 headlight, which seems super dangerous. Toyota won't fix it unless you come in with the headlight busted which means you have to make an appointment and either drive the car around illegally with 1 working headlight or just not drive it for a week while you wait for your appointment. This is a safety defect and they need to replace the wiring harness without hoops to jump through.
Headlights constantly going out while driving. I do not operate the lights in daytime. It's a major flaw and problem with this 2006-2009 model and a safety hazard. Not to mention I am almost always subject to police issues tickets and fines. I have replaced my bulbs countless times. The lights go out while stationary and in motion. Always one at a time.

The right headlight intermittently goes out. Sometimes it takes repeated attempts turning off and on to get it back on. Both ballasts and hid lamps have been replaced, yet this intermittent problem persists. I know of many complaints but do not know if Toyota has an open recall on this important safety issue.
Passenger side low beam headlight will not work. I replaced the bulb, and checked the fuses, and it still will not light. This has been an ongoing problem with this car.
The headlights come on and off. Bulbs have been replaced, fuses and still they periodically come on and off. When I take the to dealer they say no issues.
Front headlight (both) sometimes all lights will go off. Most of the time it is one or the other but I have had it happen where both completely turn off while driving. On the freeway!!! this happens daily and ive even been pulled over more than 3 times due to me not knowing about a light out! turning on and off the lights is only a temporary fix!.
Every day one of my headlights are not working. One day it could be the left one, the other day could be the right one. I'm afraid both might go out during night time driving.
While driving (in motion), one headlight (either right or left) will suddenly go off. If I turn both lights off and back on, they will both come back on. But one or the other might go off again soon afterwards. It sometimes happens on short trips (a few blocks) and sometimes only after driving several miles. It happens on neighborhood streets and on the highway, while going straight. Sometimes it happens after a noticeable bump in the road, but other times it happens when the road is smooth. It happens at slow speed (<30 mph), medium, or highway speed.
Both front headlights frequently turn off at the same time while driving. Turning the lights off then on fixes the issue temporarily, until it happens again a short time later. This happens multiple times per hour.
