I noticed at 3 thousand miles my abs light would come on and the abs would kick in and hear a grinding noise when braking over a slight dip in the road and entering a parking lot (breaking on incline slope or decline slope. I brought it into the Toyota dealership and they said they looked at everything (only 5 minutes) and nothing was wrong with brakes. The abs kicking in while braking on dry pavement has never gone away. At about 28,000 miles, I started to hear a brake squeaking noise coming from the front drivers side tire while braking at slow speeds, and when backing up. It came and went and now it is consistent and does it all the time and also squeaking when not breaking and turning to the right. I had a mechanic remove my tire and he said the bolt to the brake caliper is loose and the caliper itself is loose and that the brake pads are fine. I am now seeing there is a safety recall for this exact complaint for my exact car, Toyota Rav4 hybrid 2024 , however, when checking if my vechile is part of the recall, by entering my VIN #, it says my vehicle is not part of this recall. This needs to be addressed and fixed by Toyota as this is a manufacturing defect. A caliper bolt should not come loose on its own if properly installed.

I was pulling into a parking space at work and went to break to park and my car accelerated up a hill. After I released my foot from the break,and tried pumping the break the car eventually decelerated and I was able to come down off the hill to park. I had damage to the passenger corner panel and had to file and insurance claim for repairs. I called the dealership and they said they had not heard of any type of issues with breaks on this model and to file an insurance claim and they would inspect the vehicle for default.
The incredibly frightening incident occurred 2 times within 3 days. My 2023 rav unexpectedly and very forcibly accelerated on its own when I was pulling into a parking space. Luckily I am in the habit of parking my rav far away from other vehicles in parking lots when the option exists. The first ‘acceleration and/or braking’ malfunction occurred 5/31/24 in a walmart parking lot. When pulling into a parking spot my right foot was fully off the accelerator (I use my right foot only for the accelerator and brake pedals) and was gradually depressing the parking brake pedal to come to a complete stop. Instead my rav suddenly and forcibly accelerated forward through the empty parking spot in front of me and beyond!! I would have absolutely smashed into the car and or person if one were there!! as the rav shot forward I began desperately pushing full force on the brake pedal; the rav did not stop! so, I began full force repeatedly pushing and pumping the brake pedal. My rav eventually came to a halt as the tires may have locked up which it skidded about 12-18 inches. By god’s grace no one was hurt! on 6/2/24 ‘it’ happened again!! pulling into a parking spot at an outdoor cafe, as I was gradually depressing the brake pedal to come to a complete stop my rav instead quickly and forcibly accelerated forward!! I thank god that I did not pull into the other spot (my sister took that one) which was just a few feet away from around 12 people and families enjoying their outside lunch dining!!! this 2nd horrifying incident my rav took longer to come to a complete stop. I fully and forcefully pushed on the brake pedal but my rav kept going!! I continued repeatedly releasing and fully depressing the brake pedal but it kept going! the next thing my rav was approaching was someone’s backyard and house!! I forcibly pushed the brake pedal one more time and also threw the rav shifter into “park” finally the rav stopped! I am so shaken up by these traumatic malfunction incidents.

The contact owns a 2024 Toyota Rav4. The contact stated that while driving at an undisclosed speed, the brake pedal was depressed, however the vehicle did not stop as intended. The contact stated that the failure occurred while the contact was trying to avoid a crash. The contact stated that while trying to prevent the oncoming vehicle from crashing into the contact's vehicle, the contact crashed into a curb. The contact stated that while driving off the curb, the brake pedal was depressed and the steering wheel was steered to the left, the vehicle did not stop. The contact stated that the vehicle crashed into a pole and then the vehicle stopped. The vehicle was damaged on the passenger's side from the front bumper to the passenger's front door. The driver's front door rear view mirror was damaged. In addition, the transmission fluid leaked and the passenger's front wheel was detached. The fire department and police department arrived on the scene. The contact sustained injuries to the right lower leg and left wrist. The contact’s left wrist was burned due to the air bag deployment. The contact received medical treatment for the injuries at the scene. A police report was filed. The vehicle was towed to an independent mechanic. The manufacturer was made aware of the failure and a case was filed. The failure mileage was approximately 5,500.
The car self accerated while I was pulling into a parking spot and hit a cement wall. The brake sign came on and I braked. Damage to the front of vehicle and the collision sensor was demolished.
I tried breaking and was on a hill with a small incline stopping behind a car as a pedestrian was crossing. When I pushed on the breaks lightly the car didn't break. So I pushed harder as the car in front of me was a few feet. When I pressed harder on the breaks the car actually accelerated and hit the car in front of me. The car made a strange noise and the warning that I was close to another car flashed on but the breaks failed to work at all. I had my breaks checked about 2 months ago when I had the tires changes and they said they were fine for now and just to have them looked at next tire rotation to evaluate. I've nver had issues with my breaks and my car took some damage. It was pretty terrifying.

I'm driving a rental Toyota Rav4. I stopped the car at a curb, pushed the button to turn off the engine, and got out. There was a beeping sound (accessory on beep). I ignored it, as I wanted to quickly check something outside. I started to walk away and someone yelled 'car rolling'. I ran back, couldn't block the heavy car, opened the door and jammed on the foot brakes. Then I noticed the car was still in drive. With no engine-transmission torque against the wheels. The car is proximity-keyed, but does not require the transmission to be in the park position, to turn the engine off. This has happened twice in my driveway as well. In retrospect I'm lucky not to have been struck or run over by the car or clipped by the door. -------- yes I 'should' have put it in park. But every regularly keyed vehicle I have driven won't let me remove the key until it is in park, and as I said this has happened when I was doing other things in the car in my driveway. Going proximity keyless has dropped the transmission interlock requirement. This is a recipe for disaster.
The brakes failed, without warning, while parking; causing me to hit a pole/wall in my employer’s parking garage on April 12, 2024. I was injured, and obtained care from uchicago medical center. Toyota assigned an independent investigator to assess my vehicle; which was completed on April 29, 2024. Per Toyota, no brake issues were noted. My vehicle remained at oak lawn Toyota from April 12, 2024- may 31, 2024.
I was turning left pulling into my carport with my foot lightly on brake and the car suddenly accelerated forward !!! I slammed the break hard and was able to stop the car from hitting the concrete wall because my front tire hit the wall to the left and the car hit the cement parking bumper in my carport!! it scared me so bad I am afraid to drive the car in fear I could hurt someone. The dealer could not replicate the problem and sent me home with the car. I am not happy with Toyota !! the car is 8 months old with 7,000 miles on it!!.
In at least 5 or more instances the vehicle has had unintended acceleration and thrust the car forward while I was braking and trying to come to a stop. Once it happened at a 4-way intersection and propelled my car into the middle of the intersection and I had to swerve to avoid a head-on collision with another car. I , or the other driver, could have been killed. In all cases the braking failed to stop the car and in fact sped it up even though the brakes were applied.

When driving home two weeks ago, I got a message on my screen saying, "a brake system malfunction was detected. Carefully find a safe place to park, and call dealer". I took it to an ase certified mechanic, and he says it's a brake booster going bad. This car is my personal vehicle, and at this date has 33,152 miles on it. The mechanic and I cannot find any recalls for my 2020 Rav4 . The mechanic said it's still drivable, and what I should look for when it goes completely out, however I'm afraid to go far as to not knowing when it will go. This is definitely a safety risk, for myself and others on the road.

The contact stated that while having routine maintenance at the local dealer, the contact was informed that the driver's side and passenger's front tire threads were at 3 on the inside and 4 on the outside. I bought this car last year on October 4th from carmax (mileage was 62k at the time of purchase). I started to have problems the next month on November 6th with the brakes which was rusted and had to replace the pads. That was very dangerous and could have hurt myself driving around the heavy rain at the time. I was scared for my safety and others wellbeing so I took the car to the dealership. Their mechanic checked the car and told me that this wasn't fixed or checked since April 2023 before I bought the car in October 2023. I had problems with the infotainment system. The map navigation for my android and iphone doesnt work anymore. The mechanic at the dealership fixed the rear break pads and discs but couldnt fix the infotainment system even after updating the system. He issued me a 250 dollar refund and was charged 680 dollars for the rest of the job. Another issue happened on March 2024 for the front barring needed to be replaced from a carmax mechanic shop. Paid 380 to fix. This month I got a message saying " engine maintenance required, go to dealership". I took the car to the mechanic and explained that the computer code error message means the coolant valve is defective. I'm thinking of going to the dealer tomorrow up until I found out the car never got the recall repaired from this website. Could this be the reason why I'm having this may problems? the car hasn't been inspected by police or insurance, only the Toyota manufacturer on November 6th who told me about the car not getting fixed since April 2023. I have the invoice of all these repairs done. The latest mechanic that told me about the coolant valve being defective thought I needed to change my oil. He did change the oil and charged me 189 dollars. I'm planning on taking the mechanic to fix that problem since the Toyota dealership was gonna charge me to fix the transmission fluid which isn't the problem. I will upload the receipt for the oil change job.

On Nov 05, at approximately 11:10pm, I was driving my Rav4 on alafaya trail, orange county, orlando. While approaching the traffic signal which had turned red, despite multiple attempts to engage the brakes, the vehicle continued moving, ultimately leading to the collision with the traffic turning left at the signal (coming from her right). The impact of the collision resulted in significant damage to the Rav4, and unfortunately, the airbags did not deploy as they should have. As a result, I hit my face on the steering wheel, and then hit the head on the left window when a second vehicle hit my Rav4 on the passenger side. The complete failure of airbag deployment has raised serious safety concerns for me. As of 28th November, I have been informed by our insurance company that the vehicle cannot be repaired and has been totaled. Our insurance company geico has been requested to hold on to the vehicle for necessary investigations.
The contact owns a 2018 Toyota Rav4. The contact stated that while driving at an undisclosed speed, upon attempting to slow the vehicle, she heard an abnormal sound emanating from the front of the vehicle, and the brake pedal required an excessive amount of force to slow the vehicle. The contact also stated that later, while driving at an undisclosed speed, the vehicle inadvertently lost all motive power. She was able to coast to the shoulder of the roadway, where the vehicle was able to be jumpstarted. No warning lights were illuminated. The vehicle was taken to an independent mechanic, who diagnosed a failure with the braking system but did not provide a diagnosis in regards to the vehicle's loss of power. The vehicle was repaired. The manufacturer was not notified of the failure. The failure mileage was 40,000.
To begin with, the 2011 Toyota Rav4 in question was a loaner provided to me by fiore vw Audi (1000 s. Logan blvd in hollidaysburg, PA 16648) at @ 10:45 am on 9/16/23. My 2018 Toyota sienna was being serviced at this location on 9/18/23 and they provided me the 2011 Rav4 in the meantime. The brakes failing, the steering wheel locking up, and the sudden acceleration of this 2011 Rav4 occurred later that afternoon @ 4 pm in a sheetz parking lot at 3261 w. College Ave. State college, PA 16801. As I was turning left into an open parking space at the rear entrance of this sheetz, I began to turn the steering wheel to the right, straighten out, and park properly in the parking space; I also began to press down on brake to stop the car. As I pressed down on the brake pedal, the brakes quickly locked up and became unresponsive. Simultaneously, the steering wheel locked up becoming unresponsive with the front wheels still slightly turned to the left. I was still coming out of my turn when this vehicle failure began. I quickly took my foot off the brake, then the car suddenly burst forward with an intense acceleration. I immediately put my foot back down on the brake, but the car still proceeded forward towards the building; moving slightly to the left due to the steering wheel still locked in place and unresponsive. I slowly rolled into a metal obstruction at the end of the parking space, hitting it at an angle. I had my seat belt on and no air bags were deployed. No injuries to me, but some cosmetic damage to the front of the Rav4. There were no warning lights or audible warnings prior to this vehicle failure. I didn't notice any warning lights or alarms during the failure. Prior to this incident everything seemed fine. After the incident, I drove the car home, drove it to my insurance agent on 9/18/23, and I drove it back to the dealer on 9/20/23. That was it. I would believe sheetz has this complete incident recorded w/security cameras at that back entrance way.

This is 2 times now my brakes give me trouble since I bought the vehicle, the 1st time the brakes died n I had to keep pumping for it to work, the 2nd time it stuck to the floor n I had to use my hand brakes on the freeway.
As I was entering a covered parking structure, I came to a complete stop at the entrance of the gate to obtain my parking ticket. I was driving very slow less than 5 mph, and engaged the brakes as to stop to get my parking ticket before entering the parking garage. I have my foot on the brake pedal, I am at a complete stop of 0mph for several seconds now, and suddenly the engine revved and my car lunged forward a bit. I had not lifted my foot from the brake pedal, and yet the engine revved and my car lunged forwards. My passenger at the time yelled, whoaa why are you pressing on the gas!!? so my passenger in the car at the time was also startled by this incident and felt the car surge forward. He thought that I had pressed on the gas pedal. If someone was to be in front of my vehicle they would have potentially been hit with my car, or if I had been closer to the parking entrance gate my car could have hit the gate. I am highly concerned this just occurred as this could cause a great accident or injury. I have 1,100 miles on the car. It was purchased brand new. Another issue I have been experiencing is when I engage the gas pedal, it does not accelerate and takes a few seconds to accelerate causing my forward momentum to stall out, this is an issue when I have been trying to merge into traffic and my car does not drive as I have anticipated. I am not sure if this is an issue with it being in "eco" mode or not, but there is a stalled acceleration issue, that happens sometimes, but not always and I cannot pin point why it is happening at certain moments.
Multiple times when coming to a stop the vehicle is stopped and then the brake pedal depresses further on its own while at the same time the engine roars and the vehicle lunges forward about 4 feet. It almost put me in front of oncoming traffic. Also I felt like I had no control over the brakes. It happened 4 times and also did this while I was parallel parking and almost slammed into the parked car ahead of me. It was in the shop last month. The brake calipers were cleaned and the brake lines were bled. This vehicle only has 40000 miles on it. I took it back to Toyota again today to see if they can once again try to figure out what is causing this. When it surges forward it sounds like if your foot was on gas in park, only my foot was on the brake im afraid to drive it.

For the past three months, in increasing frequency, whenever parking, firm pressure on brake pedal engages sudden burst of acceleration, with engine loudly revving at 1000 revs or higher (audibly beyond the cabin to passers by on the street), that lurches vehicle forward. Within close spaces of parking garage pillars or walls or while parallel parking this is a hazard and threatens sudden accident of 1 mph or less up to 5-10 mph and a 1-2 yard-long jerking lurch to try to recover from. This occurred, twice this week, and 4 times within past 3 months’ time, to where this is no longer a fluke or due to my immediate reactions. This is an increasing pattern that threatens myself and others. I am headed to local dealership, within the hour, to request diagnostics and to attempt to have technician repeat this issue.
I was on highway 41 going around 40mph when my car out of nowhere slammed on the breaks ( the pre collision censor ) I believe is what caused it. About 10 mins prior to the incident when I was backing out of the gas station it braked (abruptly with noise) even though nothing was in the way or approaching my vehicle. I would say that I was stuck in the car for about 5 mins before I could get it to start driving again. The electrical flashed and shut off right when the car braked. I did have my two dogs in the backseat and thankfully no one was harmed.
When I drive this vehicle I have to be very careful. The moment you take break pedal the vehicle doesn't stop, it started shuffling and showing a warning sign of slippery road. It takes extra couple space to fully complete stop. It is summer time there is not slippery road road around. I hope they will fix this issue before it gets bad or kill someone. This is a dangerous act when you can not control your break pedal. All break pads are new this is 2023 vehicle built in June.
I was parking the car in a gravel parking lot. When I pressed the brakes and shifted into park it instead accelerated and went over the side of an enbankment. I could not stop it even though my foot was pressing hard on the brake. Luckily there was no other person or property affected by the error and my car did not suffer external damage. It damaged a sensor on the bottom side of the car which had to be replaced. There was no indication of any problems beforehand. My mechanic did an extensive review of the car and could not find anything that would have explained the unintended acceleration.
2001 Toyota Rav4 was sold at auction, to a used car dealership. I purchased this car from the used car dealer on August 5, 2023. Unfortunately, the dealer tampered with the brakes. I witnessed their mechanic working on the rear brake system in person on August 4, 2023. The owner told me that because there was a noise coming from the rear, they were "fixing it". Upon buying this car and driving it to a gas station, filled it up, much to my horror, gas spilled out. The gas tank has a leak in it. Also, upon driving it home the engine warning light suddenly illuminated. I promptly brought it to a Toyota dealership in wooster, ohio and requested a complete examination. I have their full report, and they have determined this car to be unsafe to drive. The brakes had been adjusted when in fact, the brakes and brake lines need to be replaced, this was the used car dealer at fault. I feel the used car dealership bought car at auction and was unaware of the gas tank leak and the bearing plates being fragile, the engine cradle could break at any given moment, causing the engine to fall out. The car was purchased at and maintained by an akron Toyota dealership since it was brand new in 2001. After 23 years why did this dealership allow this car to be released into the general public? frame damage so bad, the bearing plates that hold the engine up are beyond fragile, the cradle that holds the engine integrity is grossly compromised, and the gas tank has a hole in it. Bad brakes + leaking gas tank. One spark and = ka boom. I would be dead. So many things wrong in this situation. Personally, my physical and emotional health suffered. I had chronic diarrhea, headaches, and went to the dr several times. Please let me know how this matter should be resolved. I am upset and financially compromised all from this car. The abs and brake booster systems failed, and my traction control was not working as intended. Every light related to the braking system came on all at once and the ability to break became slowed, could not stop if needed to at anything other than a slow decline in speed (wouldn’t be able to go to a full stop without ample planning or control - being cut off, too close to a red light or car needing to turn, or on the highway would’ve ended in an accident). Thankfully was on surface streets both times. Problem was recurrent. Toyota dealership confirmed issue and noted loss of braking power but noted the code was intermittent and failure not definitive, hard to reproduce. Attempted to repair issue, which temporarily worked but re-occurred 1 month later. End result - needed to replace battery, stroke position sensor, brake booster with master cylinder/abs actuator, which was almost $4,000 in repairs, with no promise it would fix the issue. No warning lamps, signs, or notable symptoms before braking failure. Battery died twice before brakes situation, but attributed to a faulty update system sending data continuously.
While on eastbound I-80, my tire pressure sensor light came on. Approximately 5-8 minutes later, my right rear tire came off my car. I was in the slow lane; however, when I went to slow down even more, my brakes failed. I had no brakes on I-80 with only 3 tires. I placed my car in neutral and coasted 1/2 in the slow lane & 1/2 in the bike lane until my car would stop.

Sudden acceleration when braking caused crash of 2011 Toyota Rav4 and driver injury. Making a right turn into a parking space that faces a concrete wall, I had my foot on the brake to slow the car down to park, going about five miles an hour. Still riding the brake, never having moved my foot off it, when I was about two feet from the wall I pressed on the brake to ease into a stop. Instead, the car suddenly accelerated with a loud rev and crashed into the wall. The car was going so fast the airbag deployed and thrust me against my seatbelt. Airbag caused fracture of 5th metacarpal in my right hand. Was unable to drive for 12 weeks. When I got the car back from the Toyota body shop ($9000+ in repairs), I took it over to the dealer and began filing a case with the Toyota customer experience center to have Toyota inspect the car. I am especially concerned about the brake sensor system. Case is pending.
I was at stop position. I pressed the hold button and then took my foot off the break. When traffic started moving I pressed the hold button again with my foot still on floor the car started jumping and pressing the the two other car on my front. The two other cars got totaled.
I was pulling into a parking space and when I applied the brake to stop, the vehicle did not slow down and hit a building. The driver side airbags deployed (steering wheel and knee airbags). The was no one else in the vehicle. The vehicle was declared a total loss by the insurance company. I have reported the issue to Toyota and signed an authorization for them to obtain the vehicles electronic data.

While traveling on highway with adaptive cruise control engaged and maximum following distance selected, adaptive cruise control applies excessive braking force than necessary when vehicle in front exits highway. Safety concern because following vehicles must abruptly apply brakes. Internet search reveals numerous, similar complaints. We were almost rear-ended on the highway. This feature is great, but needs attention for safety reasons.
When applying the brakes to stop at a stoplight, I noticed the brake pedal was very hard and was not stopping the vehicle as it normally did. The stop at the light was not a sudden stop. At the time this incident occurred, the brakes were being gradually applied as the vehicle was coasting to a slow stop. As the vehicle came to rest, the engine started to race with tachometer showing 4500 rpm. I placed the transmission into neutral fearing the car would accelerate through the stoplight. As I placed the car into neutral, the engine raced up to 6500 rpm. When the light turned green, I made a right turn into a parking lot, and the engine stopped racing and the brakes and engine began performing properly. Today was the first time this happened. The stop was being made on level ground at time of incident. The car's speed before the incident had been 35 mph and was at less than 5 mph when issue began happening. Prior to traveling in the 35 mph zone, the car had recently been on the highway traveling between 55-70 mph for 20 minutes. Mileage on vehicle is currently 141,295. The vehicle is available for inspection. Slow response braking and unintended acceleration place vehicle occupants as well as other vehicles at risk for collision and possible injury. Problem has not been reproduced nor inspected by a dealer or independent service station yet. No parties have inspected the vehicle yet. No warning lights or messages were generated at the time of the incident. There have been no prior symptoms or similar problems which would have indicated this brake/acceleration issue may occur.
