The outer rear-view mirrors on my 2021 Toyota Rav4 vibrate all times, and excessively at highway speeds (approximately 50–70 mph), creating a shaky and distorted image of surrounding vehicles. This failure of the mirror mounting system makes it difficult to accurately judge the position and speed of other cars when changing lanes, merging, or reacting to traffic. The mirrors remain installed on my vehicle and are available for inspection. This issue poses a serious safety hazard. Because the mirrors do not provide a stable, undistorted view, my ability to monitor blind spots and adjacent lanes is compromised, putting myself, my passengers, and others on the road at increased risk of collision. The problem has been reproduced and confirmed by Toyota through technical service bulletin t-sb-0079-20, which instructs dealers to install longer mounting studs, reinforcement brackets, and bonding adhesive to correct the defect. Toyota currently limits the repair to vehicles under the 3-year/36,000-mile warranty, leaving many unsafe vehicles unrepaired. I recently purchased the vehicle outside the warranty coverage. There were no dashboard warning lights or system messages to indicate the defect. The only symptom is the visible vibration of the mirrors, which first appeared shortly after purchase and persists across normal driving conditions. Because the defect directly impacts driver visibility and compliance with fmvss 111, I believe this should be escalated from a technical service bulletin to a mandatory safety recall.

Vehicle has both rear view mirror distortion (assembly mirror face on both sides) vibrate causing distortion deeming outside rear view mirrors useless and hazardous. Toyota issued a technical service bulletin for 2019-2021 Toyota Rav4 models but refuses to pay for this safety defect.
The windshield has a huge blurriness that started forming few weeks ago and has expanded across driver's side of view. The blurriness on the windshield severely affects visibility as it makes the driving difficult, especially at night. We are certain and consulted with another person working at another dealership who said that the blurriness can only be caused by defective glue between the two windshield panels. However, the inspection at the autonation buena park dealership concluded the windshield blurriness to be caused by external factors and not a factory defect. This makes no sense as we cannot touch or influence what's inside the windshield.

The passenger side mirror and the collum that holds the windshield on the right side makes a complete blind spot that you can't see anything on that area. If you are turning you can't see even a whole car. The left side mirror and collum that holds the windshield is ok, but not the right one. I got into an accident with the fire hydrant at a parking lot in town because I could not see it. I was turning right into the street and hit the fire hydrant.

Small stone hit front windshield exactly in middle where window wiper sensor attaches and windshield split down the middle in two. Something is defective with these windshields. This is not normal to completely split like this.
Wipers turn on by them self, the radio shuts down on its own and then comes back on.
Uknown. The panoramic sunroof of my vehicle exploded unexpectedly while driving on the freeway and the glass shattered everywhere. There were no debris in sight that would cause the incident and the glass of the sunroof was facing outward when inspected seems to indicate that the breakage occurred from inside the vehicle. The shattered glass and shards caused minor injuries on myself and one other passenger. The 'explosion' caused the debris glass to have potentially hit surrounding cars on the freeway. The problem has been reported to the dealer but they denying any reoccurrences of this issue . The vehicle has not yet been inspected but has been reported to insurance. There were no other symptoms prior to the failure.
The side mirrors shake heavily especially on highways. It puts my safety at risk and others as I am unable to properly focus on the road ahead of me when I am trying to clearly see if there are cars behind me while I am changing lanes. It causes me to take extra time looking at the mirror that I shouldn't have to in order to change lanes safely. This is a known issue and a technical service bulletin has been created for it, but it requires a payment of over $1,000 to fix when this is an issue caused by the maker of the vehicle. It has been inspected by the maker at a dealership and was agreed that the issue I am facing aligns with the TSB. There were no warnings, as it has been noticed to have gotten worse in the last couple of months.
Appears to be a manufacturing defect. Side doors back and front fill with water. Windows rolled up and down have streaks of water(wet all the time). Water pours out of door frames. Sloshing of water in doors. Car has no more than 15,000 miles on it. Always keep covered in garage. Car does not accumulate debris or leaves. Have taken into Toyota palo alto twice now. No real help. Cannot see well from windows - safety issue as side windows stay streaked obscuring view and mold concerns with wet interior doors.

My Toyota rav 4 the power steering light comes on and says low & it always happens when it’s heavy raining and my wipers are on. The car just stops & wont start again for about 20 mins. This is super dangerous and has happened 4 times now.
When it rains, water leaks in the interior of the car where the windshield meets the roof of the car. It leaks at the both visor attachments, the light in the middle, and along the sides of where the windshield is attached. I understand there is a recall for older Rav4's for a similar issue. I think the same thing is happening here. It's is causing visibility issues when it is raining and distracts the driver.

Without any warning and without anyone touching the car my back windshield exploded out. This flung pieces of glass inches from my toddler as we were about to get in the car. I have a remote car starter (for 4 years on this car) and had started it a few minutes before walking to my car to put my stuff down, I opened the passenger door, put my bags on the seat and closed the door. I then turned around to help my toddler into the car and it suddenly exploded inches from us.
Purchased a 2021 Toyota Rav4 limited and the side mirrors vibrate while driving to the point it is unsafe to use them as their intended purpose. Nationally, Toyota seems to have acknowledged the problem via t-sb-0079-20 and only covers them on vehicles with 36k miles or 36,000 miles. Our vehicle was on the dealer lot for a year and the coverage for this TSB expired in February 2024 and has over 36k miles so my local dealer has told me it does not qualify to be repaired but recognized that the issues meets the issues described in the above TSB. My complaint is that it should be made a recall and should not be something only quietly covered. No engine or warning lights to indicate this issue and no public notice from Toyota. I only discovered this TSB via online forums, facebook groups, and reddit groups dedicated to this 2019+ model years of the Rav4 and this seems to be a rampant issue that Toyota does not want to tell people about!.
Hello, I recently purchased a Rav4 2021 limited and immidiateky noticed the side mirrors are shaking while the car is on the move to an extent where it is hard to keep track of the other vehicles from the mirrors. When I looked up this is a Toyota design defect for which they have a service bulletin. I believe this should be a safety concern as the mirrors are integral part of driving and excessive shaking limits the usuability as well as strains the driver’s eyes.
Toyota oem windshield glass has severe distortions/waviness in glass. I have had my windshield replaced 3 times now with Toyota oem glass and the issue is still not resolved all 3 have had this distortion/wavyness in the glass. I don’t want aftermarket glass in a brand new car especially with all the safety features that relies on glass clarity, but Toyota is making it hard not to choose that option. Toyota needs to do better on quality control of their glass. The distortion is happening in the line of sight for the driver causing dizziness and issues while driving, as well as bad wavyness around the edges. Ive done some research into this as well and I am not the only Toyota driver having this issue with their glass. I’ve never owned a vehicle that did this and it’s very disappointing and frustrating that I am not able to have a clear windshield in my brand new car. I called Toyota of America but they basically pushed me off onto the dealership. Someone needs to contact the mfg and make them aware of this problem so I can get glass that is normal.
Driving at highway speeds with no other vehicles around, sunroof closed but shade open, sunroof exploded. Edges of sunroof that didn't fall in were pushed up as if an over pressure condition had occurred. There was no evidence of anything striking the glass found upon inspection. Quick internet search confirms others with this same make/model have experienced the same issue.

Hood shakes in the middle presenting the appearance that the hood will open on its own, side mirrors shake causing distorted view.
While driving down the highway at the speed limit in our town on a clear evening with no weather, the sunroof of our 2023 Toyota rav 4 exploded. There was a loud bang and it shattered as if the glass had been under significant stress or pressure. The glass bowed up in the middle and a section the size of a small saucer dropped out. There was a rigged break along the front edge of the sun roof. There were no bumps or holes in the road - it was smooth. There were no other vehicles nearby - no one threw a rock at our car. The noise was so loud that it was disruptive and shocking and thankfully I was not in heavy traffic or an accident might have occurred. I researched the web and found numerous instances of this sunroof explosion occurring. Please get Toyota to fix this before someone is serious injured.
1. Most recent. 03/01/2024 sunroof spontaneously exploded during my drive to work. Broken glass fell on me as I drove. No bad weather. Nothing hit it. Just spontaneous explosion. 2. Older claim. About a year of having purchased the car the front windshield cracked while parked at work. Like the other incident no accident. I had that fixed. But the wipers do not work properly. Toyota charged me to have them check but did not fix it. To this day they only work at medium or maximum speed. Wipers do not work on low or intermittent. After doing some research on the internet and youtube it seems that this is becoming common with this model car.
I was driving on a highway posted speed limit 75mph on a clear day with no precipitation and without cause, the panoramic sunroof above my head imploded with glass shattering down upon my head. I did not hit anything and nothing hit me. No rocks, no debris, no outside force caused this. The panoramic sunroof showed a crack around the seal where it meets the body of the car and it imploded and cracked (shattered) inward from this cracking.
2014 Toyota Rav4 perfectly good headlight bulb stops working on low beam selection, severely limiting visibility for night-time driving. Looks like everyone on the internet is having the same problem.

As I was driving, I heard a loud pop. I looked up and my sunroof was completely shattered.
I was driving on the highway there was no cars in front or next to me, it was warm outside about noon the sun was out and I heard a loud bang and a minute later another loud bang, I thought it was a balloon that popped that was in my car and I pulled over and when I saw none of the balloons had popped I thought it was a soda can that exploded in the trunk but I saw they were all intact. When I got home, that is when I noticed the sunroof shattered and it was tented up. Toyota wouldn't honor the warranty, it took 4 days for the dealership to try to find blame on me.
The moonroof on this vehicle shattered without warning as I was driving on the freeway. It sounded like a loud boom and when I looked up the tempered glass was falling on me and inside my 2023 Rav4. This is a huge safety issue and im not only concerned about my safety but how much glass is in my car - I take my kids to school in this car. Im going to take it to the dealership today to have them repair it - this should br a warrantied item but I have read there are multiple issues in other Rav4's.
While driving on highway 394 in minneapolis on August 19, 2023, my sunroof exploded. It was approximately 4:55 p. M. , and I was driving between 60 and 70 miles per hour. It was around 90 degrees outside at the time. The highway was busy. As I was driving in the far left lane with my nearly 3 year old daughter in the backseat, I was startled by a loud bang like a gunshot or an explosion right above my head. I had no idea what had happened because the shade for my sunroof was closed. As I merged over to the nearest exit, I saw people looking at my car, including looking up toward my roof. After exiting the highway, I found a nearby street, mt. View avenue, and parked my car. I got out and looked at the roof, seeing that the sunroof had shattered nearly completely. I noticed immediately that most of the glass shards seem to have shot upward. There was not a large amount of broken glass laying on the sunroof shade. This indicates that the sunroof erupted from internal pressure, not an external object coming into contact with the glass. Additionally, I did not see any foreign objects in the window shade area on a visual inspection. Luckily the shade was closed, so no glass got into the interior of the cabin or hit me or my daughter. After inspecting the damage, I called a dealership that I regularly receive service from but it was closed, and I have not yet heard back. I explained the issue to a representative, who said they would notify the glass repair people and that they'd call me back ASAP. I drove the rest of the way home off of the highway. When I got home, I researched the issue and saw that this is not an isolated event. Specifically, many cars with sunroofs have had them explode, including Toyota Rav4 models dating back to 2014. There is no recall for the issue, and I have never been warned that my sunroof could randomly explode. Neither the manufacturer or insurance have inspected the damage yet. No warning light came on prior to the explosion.
Have had the vehicle three weeks and only drive on paved roadways. Visible chip cracked instantly during drive. Windshield seems weaker than our 2 4runners and vw. I’ve never seen a crack instantly occur without a large rock or hail.
Toyota service bulletin: t-sb-0079-20 (August 14, 2020) outer rear view mirror image vibration while driving (causes distraction and could lead to and accident because image in side mirrors are unusable due to vibration) dealer refuses to do this repair because it was only covered under a 36mo or 36,000 mile warranty. My car currently has 23,500 and this repair should be covered. . . Or be a recall! some 2019 – 2020 model year Rav4 and Rav4 hybrid vehicles may exhibit a vibration/shaking of the image shown in the outer rear view mirror(s) while driving. Mirror vibration can be observed at highway speeds of approximately 50 – 70 mph on typical concrete/asphalt road surfaces.

It is a common occurence on the 2020 models that the driver and passenger side mirrors begin to extrmely shake after being exposed to water. There are rentention clips that just fall out which results in the mirror itself beiginning to shake extremely making the driver disoriented along with making it very difficult to use see traffic coming from behind. Especially in high speed situations such as freeways.
We have water becoming stuck in our back passenger door due to a weather strip design flaw in Toyota Rav4 hybrids. This will begin to happen to all weather strips and eventually destroy the interior and electrical if not repaired. This is a common design flaw in newer Toyota Rav4 hybrids and we are lucky it's just our one door so far. I've been informed (by our Toyota tech) that eventually all of the doors and the roof will leak due to this Toyota design flaw. when we took the car to get fixed at the dealership, they let us know it was an expensive repair, advised it was a design flaw, other customers are affected, and allowed us time to research before paying for the repair. We may attempt it ourselves since Toyota corporate has refused a reimbursement for the repair. This is a Toyota design flaw and requires a recall. They are charging hundreds to fix this. This will literally destroy the cars electrical.
While driving on the freeway with outside temperatures of about 89/90f, at about 67-68 mph, our passenger window randomly exploded and shattered. There was no prior damage to the window or any indication that a rock, projectile or other debris had hit the window. Prior to this incident, we had observed that the passenger window would rattle inside the door pocket/window frame when the car was in motion. When the window shattered, it did so instantly without any warning, as if it had exploded, startling us. As we continued driving, the outside air started pulling apart the shattered window, causing the broken window glass to fall apart into thousands of little pieces that ended up all over the passenger, driver, and back seats, which were also occupied with children [xxx] and [xxx] old). This was a very scary and traumatic experience for us, as things could have taken a turn for the worse and we could have been seriously injured. Luckily, we had nothing more serious than minor cuts in the aftermath. However, our family and especially our children were very affected by this incident and our car suffered damage to the outside body and some more minor damage on the inside from the glass flying off from the shattered window. We believe the rattling of the window caused it to eventually explode and shatter. We believe the rattling is due to some kind of manufacturing error: either improper installation or possibly low quality glass material or incorrect size to match the door frame or a missing part that should hold the window securely. We would like our car inspected for any further damage not visible to the eye (such as glass inside the door pocket and the safety of the door window retainer of the passenger door as well as a check of the integrity of the rest of the car’s windows to check that they are property installed and secured in place to prevent rattling. The driver side window in the car continues to rattle and we fear that this incident may occur again. Information redacted pursuant to the freedom.
The contact owns a 2012 Toyota Rav4. The contact stated that upon engaging the windshield wipers, they responded erratically, moved significantly slowly, changed direction, and paused in the center of the windshield. No warning lights were illuminated. The contact stated that the vehicle had previously been repaired under NHTSA campaign number 15v577000 (visibility/wiper), however, the failure reoccurred. The vehicle was not diagnosed nor repaired by the dealer or an independent mechanic. The manufacturer was notified of the failure, and a case was opened. The failure mileage was 112,000.

I have now had two spontaneous, large cracks form in my windshield without any impact of debris.
I just want it in writing that my windshield just randomly cracked. It was parked and nothing hit it and it just cracked.
