While turning right at approximately 5–10 mph, the vehicle was struck on the front passenger side. The impact caused moderate intrusion of the passenger door, severe damage to the front-right corner of the vehicle, and damage to the dashboard. Despite the structural damage and intrusion into the occupant compartment, none of the airbags deployed, including driver frontal, passenger frontal, side torso, or curtain airbags. Both occupants were wearing seatbelts, which activated normally. The driver sustained a right-side rib fracture caused by seatbelt force, required ambulance transport to the emergency room, and continues to experience ongoing symptoms. A child passenger seated in the front passenger seat struck her head against the window, resulting in a brain concussion with symptoms including headache and dizziness, requiring emergency medical evaluation. No airbag warning lights were present before or after the crash. The vehicle is preserved in post-crash condition and is available for inspection. This appears to be an airbag system failure / crashworthiness defect that resulted in enhanced injuries due to airbag non-deployment.

Keeps bottoming out suspension is specification is stated at 5. 6 inches and I measured 3. 3/4 unloaded never get a response on my complaints from my last repair and resolution on what they admitted that suspension was too low and wouldn’t put it in writing only saying that vantage mobility saying that it’s height requirements are as stated by vantage but no mention of ground clearance that I wanted them to verify that it’s specifications are stated at 6. 5 inches and I measured 3 3/4 empty then they tell me not to go by specs and I’m a retired from wright patterson air force base in the engineering department with logistics command and I’m like really guys you have to to have a specifications for a engineered system and they can’t even call back after they admit you have to go by specs! and then the authorized repair specialist tells me I had a bad brace on the front tire that was welded wrong and didn’t even mention it on the invoice or reported it to vantage mobility to call me about anything I had concerns about ?.

On may 18, 2025, 4:00pm, my wife was applying brakes to stop for a stop sign at a t-intersection. Suddenly, the car lurched forward and accelerated uncontrollably. Despite her attempts to stop, her vehicle crossed four traffic lanes, vaulted over curb, nosedived into turf, and then crashed through brick wall into a fire station. My wife was unable to brake and stop the forward progression of the vehicle. My wife steered to avoid hitting trees but could not avoid the brick of the fire station wall. The airbags did not deploy. My wife sustained back injuries and bruising of the front torso. Her right foot is bruised from pressuring the brake pedal. The vehicle sustained heavy damage as did the fire station.

The contact stated that while driving alone at 70 mph and attempting not to hit tire tread in the road, she crashed into the median, her vehicle drove into a ditch, the vehicle spun twice, and then crashed head-on into a tree where it came to a stop. On [xxx], I was involved in a car accident in saint john, indiana, while driving my 2017 Toyota Sienna. The vehicle was hit on the passenger side, but none of the airbags deployed. As a result, I sustained serious injuries including cervical disc herniations, nerve compression, and ongoing pain. Since then, I’ve been unable to perform daily tasks, drive, care for my children, or work as I did before. I confirmed with Toyota that my vehicle has no active recalls, including for airbags, yet this failure occurred. There were no warning lights or messages prior to the incident. The airbag system has not yet been inspected or confirmed defective by a service center, but I am willing to make the vehicle available for inspection if needed. I am requesting that NHTSA investigate this failure, as it may represent a serious safety defect that could affect others driving the same model. Toyota has been aware of an occupant classification sensor defect since at least December 2023. Their automated systems state the remedy should be available by 2024 q3. And yet we were in Feb 2025 and no remedy is available. I don’t think they intend to fix this at all, leaving me with a car that is not safe to use the passenger seat.
Recall 23tb15 was issued on Dec. 20, 2023. The remedy was supposed to be out by the 3q24. I just called dealer service, and there is still not a remedy out. I am driving a car without knowing if the front passenger seat airbag would deploy in an accident. This is unacceptable to go this long with no corrective action plan yet issued. Can you take any action to get the manufacturer to correct this defect?.
Our srs airbag warning notification went on suddenly for no reason we could ascertain, and we're having to get it repaired outside warranty. The van has around 80,000 miles on it, but is only 3 years old.
Air bags did not deploy during a major crash took place on 9-25-2024 the vehicle was a total loss.
This recall has been open for 7 months and a remedy has still not been issued - this doesn’t seem reasonable for a safety concern. I have also several times experienced a failure in the “extra” 8th passenger seat (middle row middle seat) sensor - it alarms that the seat belt isn’t buckled even when that seat is not in use and not even installed at the time.

The vehicle was towed to a local towing company and the vehicle was released to the insurance to be sent to CO-part. The dealer was contacted and the vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 117,000.

At the begining of 2024, I received a recall letter from Toyota regarding the airbag malfunction. I was told to wait for a remedy by q3 of 2024. I patiently waited and was very anxious driving my vehicle. I recently did talk to my local dealer during my maintenance and they stated that they do not have any information regarding that and that I should call Toyota. I tried calling Toyota and was instructed to wait as they still have not got a remedy and that if I was getting in any accidents Toyota will be fully responsible. I bought the car to drive for my safety an this is the level of safety that I am getting. Thus I am here looking for help on how I can get this matter resolved as driving knowing that airbag has failed on the passenger side is very unsafe.

The dashboard has so many cracks literally everywhere. All the way from the driver side to the passenger side. When the temperature changes hot to cold or cold to hot. There’s nothing but cracking sounds.

It has been almost 2 years and a remedy is not yet available?? I don’t feel safe in this car without this being fixed!.
There is a recall on the passenger side airbag sensor of this vehicle. The time it is taking to get it fixed is unacceptable. Toyota makes new vehicles with working components. They care more about money than lives. Since the recall includes the air bag, people will die before this gets fixed. Toyota's solution is to tell us to not use the front passenger seat. That is ridiculous; this is a family vehicle and all seats are needed. We have contacted Toyota twice in which we have been advised to rent a car - for 2 years - and we will "probably" be reimbursed - after the recall is all fixed. We do not have the space for another car or the desire to put that much money up front into a rental car. We could sell the van and buy a newer one but would lose a lot of money in doing so, nor could we in good conscience sell this vehicle to anyone else. In our latest conversation with Toyota, we were told that the first customers to have access to the recall fix would be this September, but since we live in utah, it would not be available to us until 2026! what? that is discrimination. If the fix is available, it should be given to all who need it. I can't believe how Toyota is treating their customers, especially when the recall can be life-threatening.
After starting the van, an error pops "srs airbag system malfunction visit your dealer. " this occurs a day after turning the car on and off and driving for several hours. The next day, it may come back on, or it may not. I have taken it to a dealership, and they stated it was a firmware update they completed. After they updated the warning light, it came back on less than a week later.

My van was struck directly in the driver side rear sliding door while sitting still at a stop sign. The other driver collided straight into the passenger compartment door at an approximate speed of 30 mph crushing the door, exploding the window and neither the driver side airbags nor the side curtain airbags deployed. Resulting in my wife severely jarring around in the driver's seat, as well as my son who was securely fastened in a child booster seat in the driver side rear seat. The glass from the rear door window shattered and covered my son and the interior of the vehicle. There are no active recalls, but I read about a recall concerning the passenger side door welds that could affect operation of the side airbag and side curtain airbag sensors, but this only applied to a small number of 2019 Sienna's. Could this have been a manufacturing defect that bled over to the other door structures affecting the sensors in the same way.

A deer was struck on the driver to middle of the front of the van while traveling 75 mph on the highway. Caused significant enough damage that van was inoperable. What was concerning was the airbags did not deploy despite the significant force and high speed associated with this accident.

Sienna had recall on airbags that were fixed. I have a warning dashboard light that says there is an issue with my airbags and the seat belt light is on. According to the dealer there is an issue with the computer board. They purposed to fix the issue at 800. 00 dollars. This is a safety issue due to the expensive cost to fix this. The dealer already had to replace the airbags and now there is something wrong with the computer board and currently I do not have airbags.

There was a burning odor detected inside the vehicle. The manufacturer was contacted and offered to pay a nominal portion of the repair for the mirrors and referred her to the NHTSA hotline for assistance. The approximate failure mileage was 72,000.
I had sever car accident while I am 7 months pregnant including my 5 years old sons . During the accident my airbag was not deployed which lead me hematoma in my uterus. That car is totally losses and the accident was severe . Thank god we put the sit belt and even if the frontdash board hit me bad . I will attach all the details of the accident and the police report. We tried to submit our complaint to Toyota and they refused to help . As if the air bug should not supposed to deploy with that kind of severe accident they say they don’t use this information .
Srs airbag warning light is in the dashboard. It is confirmed by the Toyota dealer a faulty radio head unit can not register the srs airbag. In the event of collision, the srs airbag will not be deployed, causing passengers at risk.
