The contact's father owned a 2010 Toyota Sienna. The contact stated while his father was driving to the hospital at 30-40 mph for medical emergency, he lost consciousness. The vehicle crashed into a steel fence. The air bags failed to deploy. The contact's father was transported to the hospital by emergency units where it was diagnosed that he had sustained a broken neck, which made impossible for the medical personal to treat him. The contact's father deceased while in the hospital. A police report was filed. The vehicle was towed to a tow yard where it was totaled. The dealer was not notified of the failure. The manufacturer was not notified of the failure. The failure mileage was unknown.

Srs airbag check issue. Last few days, my car has been showing following symptoms 1) day 1: srs airbag check warning is flashing on the display. The passenger airbag is always off regardless of the passenger being seated or not. 2) day 2: srs airbag check warning goes away, but passenger seat airbag is always on regardless of anyone sitting or not. Also the seat belt chime is sounded for the passenger seat. 3) day 5: srs airbag check warning comes back and passenger airbag is off. (same as day 1). The auto mechanic says there is a code about occupant classification system that is malfunctioning. Dealer wants to charge me a lot of money just to get it inspected and diagnosed. The car has not been in any collision. This seems like totally Toyota's design and part reliability issue. In no condition the driver or passenger safety should be compromised in such manner. Yet Toyota dealership is not standing behind their mfg commitment to safe driving experience.

Keep in mind: brand new Sienna!!! 1. What component or system failed or malfunctioned, and is it available for inspection upon request? -the main and most important component was the entire airbag system which displayed "malfunctioned" only after the collision. The tpcs (Toyota pre-collision system) also was defaulted to "off" and said that the sonar malfunctioned. 2. How was your safety or the safety of others put at risk? -very compromised! my whole family and my children would've been seriously injured or departed from this world had we not had the seat belt on and the way it all played out! all of the airbags, including the driver's knee airbag, passenger seat cushion airbag, front seat side airbags, rear outboard seat side airbags, and curtain side airbags for all three rows , malfunctioned and car was in a standstill on the fastest left lane #1 on I-95 with no cushioning or failsafe mechanism active 3. Has the problem been reproduced or confirmed by a dealer or independent service center? -no, and Toyota was very quick and desperate to get their hands on my vehicle and to get me to sign their authorization instead of wait. As a computer scientist, I know when information systems can be exploited/manipulated and all my driving data and accident/metadata is collected constantly by Toyota so in short: "no transparency" 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? -yes, two very biased partisans involve: the liable party's insurance (allstate) and Toyota motor CO. So I need some nonpartisan assessment/evaluation ASAP 5. Any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? -nope! no warning lamps, no vehicle health messages, no Toyota app notification prior to the accident. Everything mentioned in the last sentence went haywire exactly after the collision and displayed lamps/played warning alarms non stop. Refer to uploaded files.

The contact owned a 2018 Toyota Sienna. The contact stated while driving at 74 mph, a deer ran across the freeway and as a result, the contact crashed into the deer. The air bags failed to deploy. A police report was filed. The contact sustained a sore shoulder but did not require medical treatment. The was not drivable and was towed back to their home address. The vehicle was not diagnosed or repaired. A dealer was contacted and informed the contact that they would run tests on the air bag once the vehicle was fixed. The manufacturer had not been informed of the failure. The failure mileage was 60,000. The VIN was unavailable.

I own a 2019 Toyota Sienna with 52,200 kms. I bought it January 29, 2021. On Saturday, June 26, 2021, I was exiting the quebec to lévis ferry on the lévis side. As I went up the metal ramp at 10-12 km/hr, all the airbags suddenly deployed with a great boom. We hit nothing. All the front and side airbags deployed. The front windshield was broken and one of the side windows was totally shattered. In fact it was not there any more. Pieces of broken glass flew all over the car and hit us. The vehicle filled up with smoke. On board was my wife and I and our two special needs sons. We had to hurriedly get them unstrapped and hand them to ferry workers on the outside. I was the only one injured. I received the airbag straight in my face and it hurt my neck. I have had a headache and neck ache ever since (today is June 29). I ignore the reason for which the airbags deployed. But I do know that Toyota has had to recall other 2019 Siennas for spontaneous deployment of their airbags. Maybe it is an airbag defect. My vehicle was still under warranty. The fire department and police were on the scene. The levis ferry director wrote a report. The levis police report is lvs-210626-008. Thank you.

When turning left in a busy intersection, our van was struck by another vehicle on the front passenger corner of the car. The strike occurred with my van going about 10 mph, the other car going about 45 mph. All of the airbags deployed. When the airbags deployed, the coverings on the lights came off and the small lightbulb and silver casing projected out, scratching my son's arm. Nothing in the vehicle should break or become a projectile when the airbags deploy. I am not certain on the last few questions.

Water intrusion from front sunroof when vehicle is parked at a 5% grade with the nose downward. Multiple vehicles exhibit this issue. Toyota is not creating any TSB and will mot even remedy it even if we have an extended warranty. Toyota dealerships want me to first pay $1000 in labor to pull down the headliner and test. Then ask the warranty to cover the assembly, if they don't cover it. . . Then I am on the hook for the work. The missing seam sealer is a common issue that will not manifest inside the 3yr / 36k mile warranty. My issue started around 2020 that I noticed and every dealership would not help me other than place a large burden on me. This can impact the electrical and the airbag curtains. The issue needs to have a seal sealer added to the sunroof assembly as the fix. As a family vehicle, this is a dangerous issue for multiple occupants in a single 7 or 8 person hauler minivan. Please ask Toyota remedy this issue that has been going on for over a decade. Please ask them to stop forcing customers to have sunroofs and panoroofs. . . When they can't build it correctly. Last Wednesday morning another car rear ended my Sienna at high speed at stop sign while I was waiting for my turn to proceed. I was travelling with my family. Sienna got totaled due to this. Side doors, back door, back bumper, cargo and third row seat area all got impacted due to this and everything caved inward by few inches. My entire family has been shaken to the core and me and my wife are sufferings from severe pain in back and neck since the accident. The cabin was filled will white smoke (powdery stuff), but no air bags got deployed (opened) due to the impact. We are lucky that we all are still alive since we had been hit from the back at high speed. The car hitting us from the back must be travelling above 40 miles an hour since my fully loaded Sienna with 3 adults and a child skid forward by almost 8-10 feet. This is a serious concern that even after such a high impact collision, none of the air bag opened. The vehicle was stationary, on a highway at a stop sign (ridge rd and route 52 junction in kendall county, IL). I have attached the police report and also the rear of my car. It rendered my car undrivable and I had to get it towed to the garage.

Brought car in for airbag safety recall. Was told it was completed. After this received another recall mailing for the same problem. Called the Toyota recall resolution team number to verify. Was told that a temporary airbag was installed and that I needed to have the good one reinstalled when they got them in. She called the dealer to set up an appointment then told me that it was taken care of and a new airbag was not needed. And stated that she was confused. And that I needed to go back to the dealer to get a written statement that the work was done. I noticed and explained that it looks like a substitute was made and the receipt says : �bag 0. 8. 2007-2009 yaris hatchback 2007-2009 yaris sedan�. So it looks like a temporary substitute was made and that I was not informed. I said that if they give written documentation that the repair was made it makes it legally their responsibility but this would be after the fact if a problem or fatality would occur and not solve the issue of assuring that a proper airbag had been installed. Can you give clarification and suggested follow up (possibly third party) to guarantee safety? ## #vpic# 1 - check digit (9th position) does not calculate properly; 4 - VIN corrected, error in one position only (indicated by ! in suggested VIN), multiple matches found; 14 - unable to provide information for all the characters in the VIN. ; 400 - invalid characters present #vpic#.

The vehicle was taken to the voss Toyota (2110 heller dr, beavercreek, OH 45434, (937) 431-2100), where a technician test drove the vehicle however, was unable to duplicate the failure. The contact stated that the vehicle was repaired at the same dealer under NHTSA campaign numbers: 16v858000 (structure) and 16v340000 (air bags) and prior to the repairs the vehicle had not experienced a failure. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was 161,000.

Nothing happened to the vehicle, but north hollywood Toyota refused to replace the airbags ( after waiting in their service department for 5 hours) because they said we 'altered' the vehicle. It is a mobility van purchased thru mobilityworks- who added a ramp for a wheel chair. The front bumper and dash were not altered.

I have a 2014 Toyota Sienna (as indicated by VIN number) and I currently reside in the united kingdom on us military orders. After getting notice from the NHTSA about the fact that my vehicle had an open recall I contacted my local Toyota dealership here in the uk. I was able to have the affected parts replaced after they were shipped from the us. However, the uk Toyota dealership has been unable to update the NHTSA database due to being outside the us. We have been trying to correct this issue through Toyota and the NHTSA for well over a year but no progress has been made. I have paperwork that shows the recall work was carried out. Please help me figure out how to get this updated. My family has received orders to return to the us but we are unable to ship our vehicle with an open NHTSA recall.

Took in for recall. Got van back and driver side sliding door no longer opens with door handle. Also large wire ran in 2nd row behind driver seats and just left there. Also jumper fuse ran under steering wheel. Electrical nightmare. Horrible maintenance. Unacceptable. Created more safety hazards.

Takata recall (?) we purchased this van on 4/18/2019, previously the van was a rental. The next evening, going to dinner on a Friday night about 9 p. M. I was on a 2 lane road in a rural area, traveling at 60 mph and a dog walked in front of me. I had no time to react. The front end of the van was laying in the road, not drivable. Again, a front end collision and no air bag deployment! 2nd Toyota Sienna, first a 2017 and now a 2018, both rental sales, both had front end collisions and both had no air bag deployment. At 60 mph there's no reasonable excuse for no air bag deployment.

The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. Hayward allen Toyota 2910 atlanta hwy, athens, GA 30606 (706) 549-7002 was contacted and confirmed that the parts were not available for the recall remedy. The manufacturer was made aware of the issue and were not able to confirm when the parts were to become available. The contact had not experienced a failure. (parts distribution disconnect). Jw.

Takata recall westbury Toyota has a 2011 Sienna le in silver on sale on their lot. I addressed them via email about my concerns on recalls specifically the takata airbag recall. When I entered the VIN on the nhysa website it showed one recall 1/9/2019 for airbag but no official recall number assigned to it yet. As soon as I emailed them back at night around 9 pm I noticed the recall notice had disappeared. The carfax also shows 1/24/2019 that manufacture maintenance was conducted. So all of a sudden at 9:30 pm they magically fixed the airbags!? I'm worried about the legitimacy of the fix they conducted and afraid someone might get injured should they buy this vehicle. Please investigate or reach out to them.

This problem is identical to a previous recall (NHTSA campaign # 13v014000) Toyota says they do not have to fix it under the recall. Perhaps the recall does not cover all of the vins impacted.

Takata recall. Recall g0p serviced by servco Toyota (this is not a complaint against servco; they did great work) 30 Aug 2018 (receipt in hand). I am getting ready to ship my vehicle and the shipper needs the safercar printout (showing 0 recalls) which still reflects an open recall. Just spoke with the Toyota service desk and was told that it can take a year for the data to appear in the system. It will be a year in three days (30 Aug 2019). If they just started the upload process today, I cannot ship my van in the next two weeks. How can I get this update expedited? how can I get my VIN cleared?.

"takada recall" I am writing because my takada airbags have been under recall for an extended period of time. Every single attempt (multiple attempts) have gone unreturned, despite the fact that I have called 888-546-1750 on multiple occasions! because this recall as the potential to cause either injury or death, I would think it would be in takada, Toyota and my best interest to get back to me. I am very dissatisfied with the recall and very dissatisfied with how the Toyota recall team has not contacted me. There is a second recall on the doors for the Toyota Sienna. Because I cannot get a hold of the recall team, I can also not schedule an appointment to get this recall addressed in my vehicle. This is unexcusable!.

While waiting in traffic on a county road to turn left, our Sienna was hit from behind by a car traveling at approximately 40 mph. Front row seatbelts did not engage, nor did front airbags deploy. Our Sienna hit the stopped vehicle in front of us upon impact as well. Only right side curtain airbags deployed and middle row seatbelts restrain. This resulted in multiple injuries to the front row passengers, including a severe concussion.

Takata recall we took the car to the Toyota dealer in katy in mid February 2018 to solve the air bag recall and power doors. They said the do not have the parts; we have go back twice but they say they still do not have them. Now almost 3 months later they have not done anything about it. We had not have any accident but we do not feel safe when driving the car.
