My Chrysler Pacifica was parked in my driveway, and I went out and heard a large boom and my panoramic sunroof shattered. This is a known problem with panoramic sunroofs especially Chrysler. The weather was warm and sunny. This is a no impact incident and a safety related glass failure. I have several photos and am waiting for the dealer and Chrysler customer care to open today.

I am a full-time power wheelchair user with multiple sclerosis and rely on this 2024 Chrysler Pacifica wheelchair-accessible van as my primary means of transportation. Since purchase, the vehicle has repeatedly experienced dangerous loss-of-power events while being driven. Immediately before the failure occurs, warning beeps sound and multiple warning lights illuminate, including check engine, etc, park brake, fcw, esc, and mil. After these warnings appear, the vehicle loses acceleration and enters a reduced-power/limp-mode condition. Pressing the accelerator does not result in normal acceleration, creating a serious safety hazard in traffic. Most recently, on June 4, 2026, this occurred approximately six times during a single trip. During one incident, the warning lights illuminated, the vehicle lost acceleration, and we were forced to pull over with emergency flashers activated in a dangerous location. A police officer came upon us, stopped to assist, and advised that the location was extremely dangerous. Because I use a power wheelchair, I cannot safely transfer into a standard vehicle, taxi, rideshare, or other transportation. We explained to the officer that if the van were to be towed, I would effectively be stranded because there was no practical way to transport me in my power wheelchair. The condition later cleared and we were able to continue. The vehicle has been inspected multiple times by Chrysler service departments and drivemaster, the local mobility provider working with freedom motors. The problem has been reported repeatedly but is intermittent and has not been reproduced during inspections. Despite multiple repair attempts, the problem persists. The vehicle is available for inspection. These recurring loss-of-power events create a serious safety risk for myself, my family, and other motorists.

2019 Chrysler Pacifica hybrid experienced a serious charging system failure during routine overnight charging using a standard level 2 home charger that had previously worked without issue. In the morning, the charging connector was physically trapped in the vehicle charge port and could not be removed despite repeated normal troubleshooting attempts, including repeated unlock cycles, charger power cycling, breaker reset, and reseating attempts. Because the Pacifica hybrid disables operation while a charging connector is detected, the vehicle became completely undrivable and required towing. Multiple aaa tow operators were unable to move the vehicle normally. We ultimately had to personally research and guide the tow operators through the manual neutral override procedure in order to get the vehicle into neutral and onto the tow truck. The dealership has suggested that the connector may have fused or arced together during charging and is attempting to characterize the situation as “not the vehicle’s fault. ” regardless of the precise electrical mechanism, this event occurred during ordinary intended use of the vehicle’s charging system. The end result was that the vehicle trapped the connector, immobilized itself, and stranded the owner without any meaningful owner-accessible recovery method. This is not ordinary wear and tear or a cosmetic inconvenience. This was a charging-system immobilization event involving a vehicle platform that already has a documented history of charging-system recalls and software-related charging concerns. The vehicle was being used exactly as intended. No misuse, modification, or improper operation occurred. A vehicle shouldn't become completely inoperable during routine charging. I am requesting review of this matter as a significant charging-system failure and requesting goodwill assistance regarding the substantial diagnostic and repair costs now being imposed on the customer for an event that occurred during normal operation.

I am writing to formally express my concern and frustration regarding the unresolved recall on my vehicle. It has now been over a year since the recall was issued, and my point of purchase dealer still has not received the necessary parts to complete the repair. This extended delay has moved beyond inconvenience and has become a genuine safety issue. I rely on my vehicle daily, and continuing to drive it under an open recall, especially one involving safety components is unacceptable. I have been patient for more than a year, but the lack of progress and communication has left me with serious concerns. I expect prompt attention to this matter and a concrete plan to resolve the recall as soon as possible.
We were stopped for fuel. Prior to fueling we turned off the engine and opened the fuel door, inserting the gas nozzle and then began to pump fuel. While the nozzle was running, a passenger opened the sliding door by pressing the auto open button - and the door slid into the hand of the individual holding the fuel nozzle, crushing his hand between the door and the nozzle - causing the nozzle to dislodge from the filling neck , releasing gasoline all over the driver and j hiring his hand. There is no interlock preventing the door from auto opening while the fuel tank is open - which is a critical design flaw, that can result in injury to the individual fueling and hazardous material spills/ hazardous conditions where the pump attendant is directly drenched in fuel. A simple sensor on the door with a software interlock would resolve this issue and prevent future instances. In this case the injury was minor/bruise and discomfort, but it could have been more severe had the driver not reacted quickly, or had the driver had underlying medical conditions such as brittle bones or was on blood thinners.
There is a safety recall on my Chrysler Pacifica van mopar , Chrysler and rairdons auto dealership will not repair this for me.
I keep getting check engine lights and they have repaired serval of the items and I did get a letter in the mail saying the battery would now have a lifetime warranty if it threw a certain code and I had two codes come up and some people told me to replace my 12v battery and it would probably clear up. So reset the codes and put new battery in and ran fine for a week or so (I don't drive much) then when I got in the other day and had it fully charged and it showed on screen only 69% and I thought that was strange then the check engine light came on again showing code p0a0a high voltage system interlock circuit so I am really worried the battery is having a problem and needs replaced. Will it be covered under the lifetime warranty? I see on safety recalls that these vehicles were recalled??.

Entered vehicle through driver’s side door. Put car in reverse and backed out of parking space. As the driver put the car into drive, we advanced approximately 25 yards, the rear tailgate opened and lifted without alerting the driver. No warnings, alarms or dash board notifications. Since we were in a parking garage, as the door lifted, it hit a concrete ceiling support, smashing the tailgate window and broke the tailgate lamps.
The van has just approx 55000 miles. While driving on highway, it suddenly lost power and from 60mph it came down to 20 miles in a matter of few seconds after few violent jerks. I was lucky that no vehicle was behind me. After that I pulled a side and then again started. It kept jerking and loosing power. Very carefully I got it to a mechanic. They are checking but they say looks like transmission issues. Engine light also came on. This happened few months ago however it was not this bad and my mechanic looked and he could not find anything. That problem went away after that incident. Kindly advise what to do in this situation. Chrysler Pacifica 2018 this van has lots of problems in other crucial area also like air fender switch, oil hose and cover that went bad as they are made of plastic. Then the trans mounts broke. It costed me dearly. Please advice what to do? best regards [xxx] information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).
The power liftgate safety detection has failed numerous times, hiting both myself and my husband on our heads with the full force of the liftgate closing. It is not detecting an obstruction in the way and going back up again, this is incredibly dangerous - I was hurt quite badly today.

My van has had to sit in the extreme cold in my driveway with a battery pack because of no resolution. Dealer still confirms "no parts available". This poses a risk to my kids outside playing in the driveway. I'd like to push this forward or find a solution since I don't have the option of selling the vehicle.
Four separate incidents but related incidents. Primary concern: vehicle turns off and puts itself in park without notice and cannot be restarted. Two incidents were in traffic at a stop light, and the vehicle could not be started for 12-15 minutes. Two times it occurred while the vehicle had been parked for a short period of time but could not be restarted. March 4, 2026 driver was stopped at a light, after about 20 seconds, driver received a dash message “press brake and push button to start” indicating the vehicle was off, and the vehicle was in park. Driver depressed the brake and the vehicle started. March 7, 2026 driver pulled into a store and when they excited the vehicle, they notice their carplay was still attached to their phone/music. Driver disconnected their phone but notice a dash message “auxiliary connected. ” driver attempted to start the vehicle, but it would not start, however all of the lights, carplay, messages were cycling as if the vehicle was on. When driver attempted to start the vehicle there was no give to the brake and the vehicle would not start. Driver was able to start the vehicle 15 minutes later. March 27, 2026: driver was stopped at a light and the vehicle turned off and put itself in park. Driver had to contact police t0 block traffic to prevent vehicle from being hit. Vehicle started after 15 minutes. Driver immediately took the vehicle to the dealership. April 2, 2026: dealership service technician called and said they could not recreate the problem, that all codes were checking out, however they did see a intermittent short in the crank sensor and it was replaced. April 3, owner pick up the vehicle at 5:30, drove five miles away. Put the car in park to get fuel and the vehicle would not start. Again – “press brake and push button to start” was indicated on the dash but the vehicle could not be started. There is no warning to when this happens. No sputtering, engine noise, just unusual turns off and goes into park.
1) while in a gradual turn, with the steering wheel in the 10 or 11 o'clock position, and/or in the 1 or 2 o'clock position, the steering gets stuck and requires to over correct. This happens pretty much all the time and it is very scary at the moment. It feels like you need to yank the wheel so much that you might hit a car next to you. Apparently this is a very known problem with this 2018 Chrysler Pacifica. At the dealership, we have just been brushed off and told they cannot re-create the problem, but yet you can google search and see how many people have this problem. The vehicle is available for inspection. There are no warning lamps or messages when this happens. This has been happening since we bought the car in 2018. 2) loss of power while in a turn. Quite a few times I have been making a left hand turn at an intersection and during the time I am accelerating, there is a couple second period where it feels as though I have lost power and am just coasting, but then the power kicks back in. I'm not sure if the car is not getting fuel to it, or if its power steering. It just feels dead, but there are no warning lights, messages or symptoms prior or during. I have not had this inspected yet, and it has happened a couple of times in the last couple of months.
The back up camera does not work intermittently. The screen is just black. This was supposed to be resolved as part of recall 66b/NHTSA 24v-436. I had that recall work completed in January 2026 but it did not resolve the issue. The dealer said they are unaware of that repair not resolving the issue and said I would be liable for the costs associated with the additional diagnostics if the manufacturer refuses to pay. I went on line and apparently a lot of people are continuing to have the same issues. This is a recurring issues that happens about 25% of the time we back up. The attached photo was the latest time.
The transmission is defective, and according to the warranty, under the lemon law, it has a lifetime warranty, which in my understanding means the entire defective transmission should be replaced. However, I received a call where I was told they would only repair the transmission with a 3-year warranty, and if it breaks down again, I would have to bear the future costs. In my understanding, if I have a warranty on the transmission, they should replace it with a new one. Therefore, I want to make it clear in writing that I do not wish to or authorize any repair of the transmission, but rather the replacement of the transmission as is my right. If there is any problem with the transmission, the company is solely responsible for the decision.
Rear sliding doors have an electrical issue that can cause a warning that the door is open to be displayed on the drivers information screen while the vehicle is in gear, and being driven. After a moment you will hear a relay click, the error message goes away, and after a while the error message shows up again this repeats in cycles. Attempts to disable the sliding door system for safety removing fuses do not work the system will continue to indicate that the door is open even when it has been closed manually. I just had the vehicle serviced at nye Dodge in oneida, new york so they have better information on what caused the issue, and the fix they can be reached at (315) 277-8955 for information on the issue. The issue I would like to bring to light is that in the event of a defect it is impossible to fully disable the sliding door, and it's sensors I have attempted to pull fuses to shut the system down, and can't find anyway to totally isolate the effected door for safety. I have been reassured the door will not open however I will not be using this vehicle to transport my children until this issue is corrected. You should be able to turn the system off in the event of a failure with one of the sliding doors.
I am reporting a safety defect involving my 2021 Chrysler Pacifica hybrid. The vehicle experiences repeated hybrid system failures, including “system charging” errors, rapid battery depletion from full to zero within minutes, and vehicle stalling. These failures have occurred multiple times, even after dealership repair attempts. The vehicle has stalled and been towed more than once, most recently on September 26, 2025. Now, today, January 20, 2026, the vehicle has once again stalled and will have to be towed for the third time since purchasing in August 2024. This has left us stranded once again. This is a serious safety concern, especially given the risk of stalling while driving. I transport elderly family members and depend on this vehicle for medical appointments. I am requesting NHTSA document this issue and review whether this represents a broader safety defect.
2017 Chrysler Pacifica minivan right passenger door fell off and out at the back when my wife was opening the door manually in NJ 20 degree temp - - almost injuring my wife. The back end corner of the door fell to the ground when both the top support and middle support came completely out of their tracks. The door is still attached front bottom. The door is very heavy and could have cause severe injury if rear corner landed on a foot or a child. It also shattered the glass corner and track behind the door throwing shards of broken sharp pieces over the floor as shown in pics.
Red turtle symbol, vehicle going to shut down, indicates issue sensing power from the hybrid drivetrain.
After a curb impact, the vehicle immediately experienced a no?start condition. The failure appears to be collision?related electrical damage in wiring routed through the lowered?floor mobility?conversion area installed by braunability. The vehicle was fully operational before the impact and became completely inoperable immediately afterward, with no warning lights or prior symptoms, creating a safety risk for mobility access. United access (braunability dealer) performed no diagnostic scan, no wiring inspection, and produced no documentation. They stated they could not identify the cause or access the wiring. Houston Dodge Chrysler Jeep ram produced no diagnostic findings. Their only record, a multi?point inspection, showed no codes, no scan results, and no wiring notes, and incorrectly marked the visibly damaged left front tire as “ok. ” the technician who completed the mpi left shortly afterward, and no further diagnostic work was done. The general manager, ben nash, confirmed in writing that no repairs were performed and the diagnostic fee was waived. United access reported that the dealership told them there were “two shorts in the wiring harness,” but this was only a verbal statement with no documentation, scan report, or technician notes, and no facility has been able to confirm it. An independent repair facility replaced the battery, but the vehicle still would not start. They reported that the wiring is extensive and non?standard due to the braunability conversion and reached a wiring?complexity impasse. Braunability stated they conducted a “thorough review,” but provided no diagnostic evidence and relied entirely on undocumented verbal statements. The precise failure point cannot be confirmed because no facility could access or trace the conversion?zone wiring, but the sequence of events clearly indicates collision?related electrical damage. The vehicle remains in a no?start condition.
We bought this vehicle from a local dealer in March (9months ago) in the time we’ve had it… it’s been in the shop for about 6 of them. 1- about 3 weeks after the purchase it shut off while I was driving on the highway. We then took it to the dealership in jasper, GA bc the error code said the pcm needed to be reset. They couldn’t get it to reset so they embarked on a diagnostic that took 3 months to discover the fuel pump was “too small”. The dealership repair guy assured us this was a solve to the issue, they’re tech put 200 miles on it an had no issues. 2- the day we got it back the check engine light came on and while going around a curve to the right, the wheel locked up on me and almost drove me into a mailbox. We turned around and went right back to the dealership. They had it for another couple weeks and told us we needed a new rack and pinion. We couldn’t afford the $6000 they wanted to fix that. So we then called the guy we bought it from and he had a local repair shop take a look and they replaced some parts on the rack and said it was good to go. 3- got it back and the check engine came on again, they code said pcm needed to be reset. We couldn’t get the dealership to call us back so we went to the local shop o’reileys recommended and they deducted some gauges needed to be replaced. 4- within a couple weeks the van was sputtering and cutting off again, we took it back to the local shop and the only thing they could think to do was change the auxiliary battery. 5- we’ve had it for about 2 weeks… 2 days again I went to start it up and it cut off. I had to wait a few minutes to try again and it started. Then yesterday while driving we stopped a red light and something popped and the dash lit up with errors and a grinding noise started. We went to accelerate to turn and started rolling backwards while in drive. We slammed on the brakes, turned the cautions on and put it in park and still rolled backwards. We had to get a tow back to the local shop.
Our Pacifica loses power randomly and doesn't always start when we push the start/stop button. The passenger side window rolls down and power controls all go haywire causing a loss of power. The dash flashes a quick message to service the bcm and the check engine light comes on. It's intermittent and sometimes happens in the middle of the road while driving. It's gotten worse now that it's cold outside, but I don't know if that's related. I checked our VIN against recall 19v-348 but it isn't listed.
I have no damage to my vehicle or no recalls.

This complaint concerns hardship caused by dealer scheduling and parts availability issues during a mandatory recall, not a defect in the recalled part itself. My vehicle was scheduled for a recall repair at an authorized dealership. The dealership confirmed the appointment but did not have the required parts available. I was informed of this only after dropping off the vehicle. As a direct result of this failure in scheduling and coordination: –I was required to rent a vehicle unnecessarily. –no loaner vehicle was available, despite recall instructions advising customers to ask dealers about alternate transportation. –I incurred rental charges, mandatory fees, fuel costs, and extra travel time that would not have occurred absent the dealer’s error. –I also experienced additional personal hardship, including having to transfer a child car seat between vehicles and adjust family obligations, such as waiting at a medical appointment, due to the unexpected need to return and manage the rental vehicle. The manufacturer’s customer care has limited reimbursement strictly to a standard policy cap and has declined to consider dealer-caused hardship or goodwill review, despite conflicting written guidance regarding reimbursement limits. This complaint is about whether recall compliance processes adequately protect consumers from avoidable hardship caused by dealer scheduling failures during mandatory safety recall repairs.
As I began driving errors popped up engine failure, electrical system and hybrid system errors, the gas pedal would not move beyond 10 miles per hour, the brakes made a weird noise as if were trying to start the car. This happened with no warning.
While driving at approximately 65 mph on I-35 in moderate traffic, my vehicle experienced a sudden and complete loss of motive power. I was positioned in the far-left passing lane at the time of the incident. A vague notification message appeared on the dashboard screen; however, there was no accompanying audible warning tone to alert me to the severity of the issue before power was lost. The failure was identified as a malfunction in the hybrid drive motor system, specifically triggering diagnostic trouble code p0a43 (drive motor "a" position sensor circuit intermittent). This sensor circuit failure caused the high-voltage system to shut down unexpectedly. This created a severe safety hazard, as the vehicle was rendered unable to accelerate. I was forced to navigate from the far-left lane across multiple lanes of traffic to reach the exit while the vehicle was effectively crippled, moving at speeds under 10 mph amidst highway-speed traffic. The problem has been confirmed via the retrieval of the p0a43 code. There were no warning lamps, messages, or drivability symptoms prior to the immediate failure; the visual notification appeared simultaneously with the loss of power.
Exhaust (and therefore carbon monoxide) is pumping into my vehicle whenever the cabin air intake is set to pull in out side air (the windshield defogger only works by pulling in outside air). This poses a serious safety concern, as it leaves us with two options: a lose the ability to see out of the windshield by choosing not to run the windshield fan) or b give myself and my children carbon monoxide poisoning. This is a known issue that Chrysler refuses to address because there is no pressure on them to do so, despite the imminent threat to human health and safety.
The headliner ac vent cover fell out while driving striking my minor daughter. We were driving a rental so we did not attempt to correct the issue, leaving the vent cover in the front, but we have found several other incidents reported online of the same issue. We did not take it in for inspection nor were any warning lamps on.
My care had issue and showing message on dashboard for "service shifter" and was diagnosed by the ferman dealership on us 19, new port richey, FL. After dealership fixed the issue, paid around $800, I picked up the car and drove around and took on freeway, after sometimes I got the message of service shifter again on dashboard and steering became hard and other function were stopped working. It felt like car was driving by itself as it would go up in speed even when I did not have my foot on it. Even after hitting the break it would stop but then again it would go up in speed by itself. I parked my car and waited after 15 to 20 min then started driving normally. I had to drop the car back to the dealership and explained the whole situation. They looked at it again and fixed the issue. After picking it up 2nd time I noticed I had issue with my screen. It started acting on its own like it was a ghost touch screen. It would jump to other setting while listening to music or have my navigation on, it would stop. Also, when backing up the care I noticed my back camera went blank, I could not see anything. Also, other time other cameras went blank. Like a black spot where you can't see anything. Called the dealership again and explained them. They told me to do the resent which I could not able to do it so went to dealership and they did reset the screen but after sometimes on the same day it started acting up again. I dropped the car again and told them to look into it. They did reset it again and told me that due to sun exposure screen has bubbles inside. I have this car since 5 years and did not have this problemthis issue I didn't have it before service shifter message popped up. Now they said there will be additional cost to replace the screen which cost around $1700. This is something that dealership needs to cover since issue started after fixing service shifter problem. And this is a safety issue as when I back up I can't see anything and screen acting on its own.
Well driving down the interstate my van slammed on brakes stated safety mode engine will. Shut off soon. Now it will not start. It could have caused us to get in a serious wreck.
The internal bearings on the passenger-side sliding door failed. The failure caused the sliding door to come off its tracks, shatter the rear window, and damage surrounding body paint. The vehicle was parked on level ground when the incident occurred. There was no collision or external damage prior to the event. The owner’s manual contains no scheduled maintenance procedure related to the sliding door bearings, hinges, or tracks. This failure appears to be a design or material defect, as multiple other Chrysler Pacifica owners have reported similar sliding door malfunctions. Estimated repair cost is over $3,000. Chrysler customer service was notified. This failure poses a serious safety risk — the door can detach, potentially injuring passengers or bystanders. Please investigate this issue for potential recall or safety campaign.
After taking my car in for the recall on the battery which required a system update, I’ve had multiple errors that the Chrysler service department cannot resolve. Four times in the last two weeks when I get in the car and crank it, it will not go into drive or the shifter does not work at all. I’m having multiple errors on saying service shifter and service electrical battery that had never occurred before the system recall update. I’ve been stranded multiple times in the last two weeks. It must be related to the recall update that was done in my car. It cannot be a sheer coincidence that I never had issues until the update. The service department is not helpful in anyway and returns my car with no answer. Simple google research shows this is a recurring issue after the update.
While driving, the vehicle suddenly shut down completely. All systems lost power: no lights steering wheel locked/stuck hazard lights were non-functional after approximately 2 minutes, the vehicle was able to restart, but an error message appeared: "ecu needs service" I then drove for about 2 minutes and parked. After stopping the engine and waiting for about 5 minutes, I restarted the vehicle. On restart, there were no error messages and the vehicle operated normally.
