The contact stated that several unknown warning lights were illuminated, then turned off. The contact stated that a single flashing red dot remained displayed. The vehicle was driven to an independent mechanic, who diagnosed the vehicle and determined that the starter needed to be replaced. The vehicle was repaired; however, the failure reoccurred. The vehicle was taken back to the same mechanic who determined that the 12-volt battery needed to be replaced. The vehicle was repaired; however, the failure reoccurred. The vehicle was not taken back to the mechanic; however, the vehicle could no longer able to be started. The contact had researched online and related the failure to NHTSA campaign number: 14v373000 (electrical systems, air bags); however, the VIN was not included in the recall. The manufacturer was not informed of the failure. The failure mileage was approximately 140,000.

While the contact's husband was driving 15 mph, the front passenger's side headrest detached from the base and struck the contact in the back of the head extremely hard. The air bag had not deployed. No warning lights were illuminated. The vehicle was taken to an independent mechanic, where it was diagnosed with a detached headrest air bag. The contact was advised to take the vehicle to the dealer for assistance. The contact sustained a neck injury; however, no medical attention was needed. The manufacturer was not made aware of the failure. The failure mileage was approximately 114,000.

Was driving down the highway coming back from orlando when my air bag light went off. Heard cracking in my head test and all of a sudden it expired and deployed and hit me in the back of the head and almost knocked me out. Made me dizzy and I had my kids in the car. This should not have happend if I was not in a car crash and Dodge refuses to fix it. It was faulty without any warning. There are many people this has happend to and the had a extension back in 2024 and to repair them and I never received a letter.

My airbag light goes on intermittently. I have no idea why. I took it to an independent repair shop and they said that they could not help me, I needed to check with the dealer to see if there was a recall. I checked for recalls, and there was no recall. Additionally, the tire pressure sensors also turn on intermittently even when tire pressure is correct.

Greenway cdjr in florence alabama did the t33 airbag recall on my Grand Caravan. I recently found out while investigating another issue that they had taken a shortcut and the work wasn't properly or thoroughly done. The recall was to correct an issue with exposed wiring and the remedy was to install a protective sleeve. Rather than remove the retaining clip to install the whole length of sleeve they cut the sleeve to fit up to the clip leaving wiring unprotected. Greenway is under new ownership as singing river cdjr. I contacted the general manager and sent pics and informed him I couldn't put the air bag assembly back in until the work was done. On October 31st this year he said he would have an advisor contact me to schedule an appointment. I haven't been scheduled nor heard back. I fear this has happened to more vehicles than just my own. In addition to not having a driver side airbag now I also don't have a horn as it is part of the assembly. The sleeve is supposed to be 6mm. It was cut short.

The contact stated while driving approximately 35 mph, when a vehicle pulled out in front of her vehicle she swerved to the left to avoid the vehicle and crashed into a vehicle that was stopped for traffic on the road. The contact's vehicle front driver's bumper, grill, and headlight had crashed into the stopped vehicle's passenger side rear bumper and wheel well. The contact stated the police and ems were on the scene. None of the air bags deployed. The contact stated that she complained about pain in both her collarbones and pain in both her knees. The contact was examined by the ems, but declined to be transported to the er. The contact had her vehicle towed back to her residence. The vehicle was inspected by her insurance provider and was declared a total loss. The manufacturer was informed of the failure. The failure mileage was approximately 150,000.

My headrest, on the passenger side of my minivan, decided that it wanted to act as though the airbags had been deployed and the plastic snapped that holds the metal rod and now my headrest is deployed as though I was in an accident even though I wasn't.

Both front headrests activated without rear collesion which posed a danger to those occupants loacated in either front seat. Took the vehicle to the dealer to have the headrests reset, was not able to do a reset, will have to purchase new headrests and have them installed at a great cost to the customer.

Active front seat headrests deployed without an accident for the second time. Had them replaced and paid for myself the first time years ago. Was compensated by fca as part of special coverage after the fact. When they deployed again I brought it in and had it diagnosed at hopkins Dodge, it was covered under the special policy. They ordered parts. It took forever for the parts to come off back order. They just arrived in August 2024, I verified with their parts department. I am now told that I am out of the special coverage window and they won’t cover unless I pay the dealership over $200 to diagnose the same problem again. It has not changed. They already said it was covered and now they are trying to back out because it took forever for them to supply the required parts. I need this repaired like they said they would two years ago when I got it first diagnosed and the parts ordered.

The front passenger active head restraint deployed without being involved in an accident and without warning. Upon inspection, we found that the plastic used to hold the metal rod under tension (until released by the crash sensor) broke. No one was hurt but could have been as this happened just as we were getting into the vehicle.

Passenger headrest airbag went off /exploded open while I was parking the van in my driveway. No one was sitting in the passenger seat so no one was injured. Yes, you can inspect it. I called Dodge but they said there were no recalls. No one has inspected it. No warning lights.

The vehicle was taken to the local dealer, where it was diagnosed and determined that the passenger's side seat belt sensor needed to be replaced. The vehicle was not repaired. As I was driving down the road, doing about 40-45 mph, the airbag in my driver's seat headrest deployed. I was jolted forward and suffered neck and back pain, as well as a headache. Due to the neck and back pain, I had to visit the er and have a CT scan done. There were no lights on the dash at the time of this happening to indicate it was going to malfunction. This has not been investigated yet, as it happened over the weekend but I will be reporting to the manufacturer as soon as they're open as well.

The contact owned a 2016 Dodge caravan. The contact stated that while driving at an undisclosed speed, the vehicle hit a sign and a guardrail causing the vehicle to flip over. The contact stated the vehicle was in a previous collision where the air bags deployed and had to be replaced. The contact stated during this collision and police report confirmed the steering wheel, driver knee and passenger dash air bags did not deploy. The contact sustained minor injuries which they sought medical care for. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was not made aware of the failure. The vehicle was totaled. The failure mileage was approximately  130,000.

While I was driving, the passenger seat headrest exploded forward. Chrysler calls this the "front seat active head restraint. " this is a safety mechanism supposed to be triggered in a crash, but the plastic holding back the force of the strong springs gave out and the plastic broke. This is a known issue that has caused others concussions, neck injuries, etc. As of yet the person who was sitting in the seat has not shown adverse symptoms, but his safety was put at risk. Chrysler is aware of this issue and had sent out a letter extending the warranty to 10 years, but this vehicle is just past the 10-year mark. No dealer, police, or insurance representatives have inspected this specific incident. There were no warning lamps, messages, or other symptoms of the problem prior to failure.

Airbag light came on with no collision or impact toy the vehicle, took it to dealership and they advised that it is the passenger side active headrest restraint system. Even after admitting it was a faulty safety system, dealer will not replace it or fix it without compensation.

The front airbags malfunctioned and deployed while parking the vehicle. The driver was injured with burn marks and bruised on legs due to the lower area airbag deploying rapidly. The van was towed to the nearest Dodge dealership for further inspection. The incident was documented with police and a case number was provided. The incident was also documented with manufacturer(Dodge) and have a pending case for further inspection. To my knowledge, there were no warning lights on .

Was hit in the side and in my Dodge minivan the side curtain airbags never deployed. The van was a total loss so was the car that hit me and his airbag did deploy. I did injure my lower back and shoulder.
