I was driving my 2016 Ford f150 in ventura California on August 29th 2025 when my throttle began to accelerate on its own after taking my foot off the gas pedal. My truck sped up out of control, I almost lost control of the vehicle and crashed. Luckily I was able to gain control and get it into neutral and then into park. My trucks rpms raced so high it sounded as if the engine was going to explode. I put it into park and the rpms lowered to a normal level. While this was all happening a wrench icon popped up on my dash. Once the wrench icon came on I noticed my dash wasn’t telling me what gear I was in or if I was in park, neutral or drive. I pulled into the closest parking lot. I put it in park and every time I gave the truck gas the throttle would continue to accelerate on its own and would rev extremely high on its own. I also noticed as I was sitting it the parking lot at idol there is now a thumping sound/ jolt when selecting what gear to go into. I don’t know if this was caused from having to slam the car into neutral and then into park so I wouldn’t crash when the throttle accelerated. I shut my truck off and it wouldn’t turn back on. All of my electronics work but it doesn’t even want to try to start or turn over. I had to tow my truck home and it is now sitting in front of my house unusable. I have videos I can provide.

Fuel pump went bad causing the pump to overdraw electricity on the electrical circuit which melted the fuse in the fuse box. The fuse and fuse position was melted beyond being able to use this again. It could have caused a fire in the engine compartment if the circumstances were different. My mechanic reported that he's seen some of these melt the whole fuse box, creating major damage and a major fire hazard. He reported there is a temporary service bulletin out for this issue, so it seems like the manufacturer knows its and issue, but has not chosen to do anything about it.

My f150 constantly needs oil and seems to lag when the oil tank is low. 163000km on the engine. I've heard other 2019 f150 drivers have the same problem. It also has a hard shift after sitting for the night, it will drop gears while going uphill which seems to be from third gear to fourth but instead drops to 3rd again creating a clunk noise. It hasn't caused an accident or anything yet but from my last truck this thing definitely was not built the way it should have been. I think Ford needs to have a serious look at these trucks before someone gets hurt.

My f150 engine stalls while driving when I am at a1/4 tank or less in hot or cold weather in stop and go traffic, at a stop sign or red traffic signal. I am able to restart the engine after placing the transmission in neutral or park. I have reported this issue to my Ford dealer, but they say there is nothing wrong with the vehicle. There is currently a recall (#25s75) for this issue in later model f150s, but my vehicle is not covered. The Ford hotline instructed me to report this issue to NHTSA.

My truck completely shut down while leaving the shop and I was only going about 6-10mph. The dash alert stated "fuel accessory" or something in orange, sorry, I panicked a little:) I took it to bradley Ford the next day and they couldn't fix it as they didn't know the issue. This happened a second time, but didn't shut off completely, it just felt like I was running out gas (full tank). I drive a 2019 Ford F-150 with the 3. 5l engine and 10 speed transmission. I have received a warning for low fuel pressure, trouble starting, and the truck has cut off once after turning over. It never fails to crank, however, it may take 5 or so seconds of cranking to get it to start. I have had the check engine light come on one time over the last couple of months of the issues starting; the code it showed was a p00c6. I have not brought the vehicle in for service as it just started a couple months ago when it got hot outside and there was a recent recall that did not include this VIN but matches the issue exactly. The warning states to not drive the vehicle and to get it towed if fuel is not low. The recall states there is not a current fix for the issue and I need my vehicle. This is a daily problem, mostly after the vehicle sits for a couple of hours. I am also having an issue with the cam phaser rattle upon startup and rattling while driving. I have noticed some power reduction and it has progressively gotten worse over time. I am at 135,000 miles currently and have experienced this issue for the last 50,000 miles or so.

On vehicles equipped the r680 six speed transmission, the oil lines break off at the quick disconnect to the transmission oil cooler due to metal fatigue. When this happens, it causes 14 quarts of automatic transmission fluid to be pumped out onto the roadway from the front of the vehicle and on the tires and roadway. Several things can happen when this happens. 1. Loss of control resulting in a crash. 2. Vehicle stopping on roadway and unable to continue. 3. Roadway gets covered with oil may cause other vehicles to crash. 4. Enviornmental issues. I am reporting this because it is a problem that is common and should be addressed. The problem is well documented on the internet and youtube. (Ford f150 transmission cooler lines breaking. ) in my case the line snaped off when I was in my driveway. Very lucky.

The cause of the accident was due to an extremely hard downshift, then extremely hard upshift. Between the speeds of 25-37 mph the truck will do a hard downshift with a 1-3 second loss of power delay. When it finally grabs gear it takes off like a rocket before grabbing the next gear and back to what feels normal. This is something I never experienced in my 2022 Ford f150 with the same motor and transmission. This particular day, I had pulled off to the side of the road to let the mail carrier go through the narrow section of the road first. This particular section, there is a huge tree right next to the road (approximately 12"-14" off the road). After the mail carrier passed, I started to speed up. Upon hitting 34 mph, the transmission shifted hard, pulled me into the ditch, and by the time the gear grabbed, and I had power to control, I hit the tree. As a habit, since this issue happened numerous times, I looked at the speedometer and just prior to impact I was at 43 mph. The transmission is not the only issue that I have been having with this vehicle. There has been a fuel consumption issue, a hard start issue (does not matter time of day), as well as a grinding issue at the front right wheel. This truck is equipped with a 36-gallon fuel tank. At the city driving mpg and a 34-gallon fill-up, I should have been getting 544 miles/tank. However, that was not the case as I was only getting 440-447 miles/tank (did not matter whether normal, eco, or sport mode driving). Each fill up was a 24-28 gallon fill-up leaving 8-10 gallons unaccounted for. A pcm update was done but it only got me to 510 miles with a 30-gallon fill-up. Each complaint I made with the transmission went ignored with me being told "this is normal for the 10 speeds. You will just need to learn to deal with it. " after the pcm update with no true fix, I was told "drive it to zero mte and calculate from there how far you can drive, then you know. " this is a horrible and extremely unsafe answer to give.

On 7/18/25 I picked up F-150 from being repaired (failed belt drive generator/starter tensioner). About 3 hours later I was driving up a long grade at about 5,000 feet when I stopped to check on my cargo (a bicycle). At this point I noticed the cooling fans were running increasingly fast, much faster then they had ever run before. I checked the digital temperature gauge and it showed a normal temperature. I opened the hood and saw there was no coolant in the overflow bottle. I assumed the truck was just a bit low on coolant and I had some (5 liters) water with me so I added that to the overflow bottle. Very little water was visible in the overflow bottle. I replaced the cap, turned off the a/c and immediately headed back to the dealer. At no point did the check engine light come on. At no point did the temperature gauge move from the "normal" area. At no point did any warning message of any time appear on the driver display - unless it occured when I was refilling the overflow bottle. The engine was not operating normally however, the gas engine stayed on at all times and did not cycle on and off as it normally would do (truck is a hybrid). When I arrived at the dealer they attempted to fill the coolant system and said it was completely empty - no sign of the 5 liters of water I put into the system. The cause of the leak was identified as the eghr system, specifically the plastic coolant inlet port. This failure is discussed in Ford customer satisfaction program 24n04 - attached. This failure should not have occured. But when it did occur the vehcile should have warned the driver that a serious overheating event was occuring. No mil was set. No driver warners were made and the temperature gauge remained in the normal range at all times. The only way I because aware of the problem was by noticing the increasing speed of the cooling fans. This could lead to fire, or unexpected engine failure or burns from overheated radiator.

I was turning left from a side street onto a divided highway and stopped in the middle to wait for an opening in traffic. When one came I hit the gas and nothing happened. Pressed it again, to the floor and nothing happened. I then put the truck into reverse to back out of the way of traffic because the truck rolled forward a bit when I attempted to accelerate. I then waited for a very large opening and attempted to accelerate again. Pushed the pedal and again nothing. I then pressed it to the floor and held it there. The truck very slowly began to accelerate and then after about 20 seconds accelerated like normal. The truck seemed normal the rest of the way home.

On [xxx] at [xxx] I dropped my wife and 2 children off at the funland arcade in deep creek, maryland. The parking lot was full from the holiday weekend and I was planning on taking the lightning to a nearby charger while the family gamed so I double parked perpendicular to the arcade entrance, put the hazards on, and let them out. After the 3 of them got out, I placed the lightning in reverse to back up 5-10 feet so that I could exit the parking lot through the same way I came in. Once I had enough space from reversing so I could make the turn to exit I placed the lighting in drive. The moment the lightning switched to drive it accelerated at a very high speed as if I was flooring the gas pedal, which I was not. The lightning was in one-petal mode which I always use unless I’m on a highway. I immediately began pumping the brakes but the truck would not stop, I probably slammed on the brake petal 4-5 times to no avail. I even tried shifting the truck back into park but it would not shift out of drive. I was able to luckily steer the truck into a hillside approximately 150 feet away. The truck ran over numerous small trees and shrubs before crashing to a stop approximately 16 feet up the hill. Shortly after the shock wore off, I slowly reversed the truck down the hill, exited the truck, and informed dozens of onlookers who came out of the arcade to see what the noise in the parking lot was from, that the truck just took off on its own but that I was fine. I called 911 and the police came. Luckily no people or vehicles were between the arcade entrance and the hillside or the outcome would have been significantly worse. This crash was witnessed first hand by wife and 2 children. I have grave safety concerns about this truck now and will not put my family or self back in it until Ford fixes this problem. While driving to sam’s club (approximately 20 miles from home), my 2018 Ford F-150 3. 5l ecoboost began running very rough, and the check engine light came on. I had the vehicle scanned, and diagnostics confirmed that the cam phasers are failing — the exact issue I had replaced under warranty at approximately 59,000 miles. The vehicle now has just 88,000 miles, and this failure occurred in under 30,000 miles since the original repair. I have maintained this truck with extreme care. I only use motorcraft parts and fluids, change the oil every 3,000 miles or sooner, and always use premium fuel. This is not a maintenance issue. This is a design or part failure that Ford has not properly resolved. When the phasers were replaced the first time, I was told there was no warranty on the replacement parts. That is unacceptable. A known failure being replaced with an equally defective part is not a resolution. Ford is aware of this issue — there are multiple tsbs (including TSB 21-2130 and 19-2208) and widespread complaints across owner forums — but they have taken no responsibility. I paid over $44,000 for this truck and should not be facing the same critical engine failure twice before 90k miles. The rough performance and sudden malfunction while driving posed a potential safety risk. I am submitting this complaint so NHTSA can investigate and hold Ford accountable. This seems like a common problem with this engine, and I do not understand why there has not been a recall or why Ford has not been forced to implement a permanent fix.

Vehicle is shuddering as acceleration starts, took to dealership before warranty ended and they dismissed concerns. Issue has intermittently persisted since then with no resolution available from dealership. The acceleration issues causes the vehicle to slow down and halt before jerking into motion again. Additionally the engine eco boost is occasionally shutting the vehicle off in traffic, if the brake is released too quickly with the boost feature on the engine shuts off, you have to put it back in park and then start the engine again. I've almost been rear ended twice when a light turns green and my engine turns off unexpectedly.

Vehicle is a hazard on the road as well to the passengers. Local Ford dealership and Ford corp. Both admit there is a recall (25s75) on my 2022 F-150 but are unaware of how to repair the vehicle. I am to wait until Ford develops a solution. Unwilling to provide a loaner vehicle while Ford comes up with a solution. They told me 3-6 months.

Received recall notice for vehicle listed above. Contacted the local Ford dealer and talked to the service writer. They stated as the recall notice outlined that currently Ford does not have remedy for the situation. In the recall notice it outlines one of the issues could be the vehicle to stall particularly in hot weather conditions. . I live on the gulf coast (hot humid conditions) and commute 2x daily across elevated bridges that are notorious for accidents from stalled vehicles on the bridge. This is due to a faulty fuel pump system. My concern is that if I stall out on this bridge the results could be tragic for not only me but for other people using the bridge. To me this situation should be remedied in a "now" timeline to prevent the loss of life and equipment. The service writer agreed with me but didn't have a solution. Why doesn't Ford just install a new fuel pump that doesn't have this particular problem. In my opinion if they can't do that they should replace my current vehicle with a new of the same make and model. My life and that of others in this situation is more important than Ford's bottom line. While driving, the vehicle has been going into limp mode at varying speeds on the roadway or while pulling onto the road. The problem lies within the components of the electric throttle body. The system malfunctions and locks the throttle body closed, thus putting the truck into limp mode. Shutting the vehicle off, waiting, and restarting temporarily resolves this issue until the pcm detects a fault in the electrical components of the throttle body and sends the truck into limp mode. When this happens, it also triggers a warning message regarding hill start assist. This is an extreme safety issue as pulling onto the roadway or loosing power in traffic and not being able to pull off can lead to collisions especially around blind curves or in heavy traffic. There also isn’t always a location to safely pull off the roadway, potentially leaving the vehicle and its operator in the roadway, increasing the risk of collision and injury.

While parked and shut off at a gas station in lakeside, CA, my 2015 Ford f?150 XLT spontaneously caught fire under the hood. The fire appeared to originate in the fuse box area, specifically where the fuel pump relay is integrated. The incident posed significant risk, occurring near a fuel pump while my child was in the vehicle. A gas station employee assisted with extinguishing the fire. The cause is believed to be an electrical short. There were no modifications or recent repairs.

Transmission has a defective cdf drum assembly causing transmission to loose bypass hydraulic pressure. This is causing transmission to hesitate between gears during acceleration and Ford dealer diagnosed failure and recommends replacing transmission as clutch packs are failing as well. Harsh downshifts as well. Contacted Ford customer service issued case number, then told they would not render and assistance is $9000. Cost of. Transmission replacement.

My 2013 f150 had been acting strangely in showing a wrench on the led screen and shutting off the electrical components on my dash. When this happens, it also sticks the transmission in between 3-5th gear. I am unable to shift down to 1st or 2nd gear, or up to 6th gear. After this happened a few times, it started to shift me in to park and stop my wheels from turning. This happened to me 3 times while traveling down the highway at 80mph. All 3 times, it lasted only a few seconds, but it was enough to screech my brand new tires and leave marks on the ground as if I had slammed on my brakes. It returns to normal and continues down the road.

Had oil change 7 times in one year and each time they indicate at the place I get oil changed that it is a quart or more low before 3,000 miles of usage after oil change. My car is suppose to take unleaded gas or higher and it knocks if I use unloaded. Now my catalytic converter is going out and it is not covered under warranty unless it is consequential to other problems. Well there appears to be engine problems that are not being addressed and I have a warranty and have taken it in. But they do not look any deeper into the problem except for computer scan and the warranty people won’t give me any answers on what they will do if there is engine problems.

Summary of the defect: the vehicle's instrument panel cluster (ipc) reduces the reported oil life to 0% in less than 1,000 miles, even under normal driving conditions. Ford attributes this to flawed software logic in the ipc module. The oil life monitor is a critical engine maintenance indicator relied upon by vehicle owners to prevent premature wear or catastrophic engine failure. Faulty or misleading oil diagnostics can result drivers disregarding future oil life warnings, assuming they are incorrect operators running engines with degraded oil beyond safe limits loss of trust in onboard diagnostics, affecting broader vehicle safety behavior avoidable mechanical damage, particularly for owners following the vehicle’s built-in guidance Ford’s TSB instructs dealers to reprogram several modules, including the ipc, apim, bcm, gwm, and tcu, but only if a customer complains. Why this warrants recall action: this is not a cosmetic or comfort issue. It is a misrepresentation of core engine health diagnostics that, if followed blindly (as owners are instructed to), could cause premature engine wear or failure. The behavior directly undermines the purpose of onboard diagnostics and scheduled maintenance. I urge the NHTSA to investigate TSB 25-2331 and consider elevating this matter to a formal safety recall, or at minimum, compel Ford to issue a customer service campaign notifying all potentially affected owners.

The vehicle a 2024 f150 hybrid with under 3,000 miles. First reported engine warning light was on 4/4/2025 at 2:23pm. The warning light indicated that "service engine soon alert is on because an issue has been detected with the fuel tank or lines" after a week while waiting for the appointment date the warning light went off. Then on 4/24/2025 at 3:28pm the same warning light reappeared. Took the truck to the dealership and they said they had to drop the. Fuel tank to repair and issue. After 2 days the vehicle was returned to me. The problem was as noted on the Ford dealership invoice states, "fuel vapor vent valve is leaking". That was replaced. Also the initial diagnostic noted the "ftp would not hold vacuum even when canister and vapor blocking valve are closed".
