On vehicles equipped the r680 six speed transmission, the oil lines break off at the quick disconnect to the transmission oil cooler due to metal fatigue. When this happens, it causes 14 quarts of automatic transmission fluid to be pumped out onto the roadway from the front of the vehicle and on the tires and roadway. Several things can happen when this happens. 1. Loss of control resulting in a crash. 2. Vehicle stopping on roadway and unable to continue. 3. Roadway gets covered with oil may cause other vehicles to crash. 4. Enviornmental issues. I am reporting this because it is a problem that is common and should be addressed. The problem is well documented on the internet and youtube. (Ford f150 transmission cooler lines breaking. ) in my case the line snaped off when I was in my driveway. Very lucky.

The cause of the accident was due to an extremely hard downshift, then extremely hard upshift. Between the speeds of 25-37 mph the truck will do a hard downshift with a 1-3 second loss of power delay. When it finally grabs gear it takes off like a rocket before grabbing the next gear and back to what feels normal. This is something I never experienced in my 2022 Ford f150 with the same motor and transmission. This particular day, I had pulled off to the side of the road to let the mail carrier go through the narrow section of the road first. This particular section, there is a huge tree right next to the road (approximately 12"-14" off the road). After the mail carrier passed, I started to speed up. Upon hitting 34 mph, the transmission shifted hard, pulled me into the ditch, and by the time the gear grabbed, and I had power to control, I hit the tree. As a habit, since this issue happened numerous times, I looked at the speedometer and just prior to impact I was at 43 mph. The transmission is not the only issue that I have been having with this vehicle. There has been a fuel consumption issue, a hard start issue (does not matter time of day), as well as a grinding issue at the front right wheel. This truck is equipped with a 36-gallon fuel tank. At the city driving mpg and a 34-gallon fill-up, I should have been getting 544 miles/tank. However, that was not the case as I was only getting 440-447 miles/tank (did not matter whether normal, eco, or sport mode driving). Each fill up was a 24-28 gallon fill-up leaving 8-10 gallons unaccounted for. A pcm update was done but it only got me to 510 miles with a 30-gallon fill-up. Each complaint I made with the transmission went ignored with me being told "this is normal for the 10 speeds. You will just need to learn to deal with it. " after the pcm update with no true fix, I was told "drive it to zero mte and calculate from there how far you can drive, then you know. " this is a horrible and extremely unsafe answer to give.

On 7/18/25 I picked up F-150 from being repaired (failed belt drive generator/starter tensioner). About 3 hours later I was driving up a long grade at about 5,000 feet when I stopped to check on my cargo (a bicycle). At this point I noticed the cooling fans were running increasingly fast, much faster then they had ever run before. I checked the digital temperature gauge and it showed a normal temperature. I opened the hood and saw there was no coolant in the overflow bottle. I assumed the truck was just a bit low on coolant and I had some (5 liters) water with me so I added that to the overflow bottle. Very little water was visible in the overflow bottle. I replaced the cap, turned off the a/c and immediately headed back to the dealer. At no point did the check engine light come on. At no point did the temperature gauge move from the "normal" area. At no point did any warning message of any time appear on the driver display - unless it occured when I was refilling the overflow bottle. The engine was not operating normally however, the gas engine stayed on at all times and did not cycle on and off as it normally would do (truck is a hybrid). When I arrived at the dealer they attempted to fill the coolant system and said it was completely empty - no sign of the 5 liters of water I put into the system. The cause of the leak was identified as the eghr system, specifically the plastic coolant inlet port. This failure is discussed in Ford customer satisfaction program 24n04 - attached. This failure should not have occured. But when it did occur the vehcile should have warned the driver that a serious overheating event was occuring. No mil was set. No driver warners were made and the temperature gauge remained in the normal range at all times. The only way I because aware of the problem was by noticing the increasing speed of the cooling fans. This could lead to fire, or unexpected engine failure or burns from overheated radiator.

I was turning left from a side street onto a divided highway and stopped in the middle to wait for an opening in traffic. When one came I hit the gas and nothing happened. Pressed it again, to the floor and nothing happened. I then put the truck into reverse to back out of the way of traffic because the truck rolled forward a bit when I attempted to accelerate. I then waited for a very large opening and attempted to accelerate again. Pushed the pedal and again nothing. I then pressed it to the floor and held it there. The truck very slowly began to accelerate and then after about 20 seconds accelerated like normal. The truck seemed normal the rest of the way home.

When pressing on the break to come to a stop pulling in parking space, my f150 quickly accelerated cause a crash into a building my 2 year old truck has been declared a total loss from insurance.
On [xxx] at [xxx] I dropped my wife and 2 children off at the funland arcade in deep creek, maryland. The parking lot was full from the holiday weekend and I was planning on taking the lightning to a nearby charger while the family gamed so I double parked perpendicular to the arcade entrance, put the hazards on, and let them out. After the 3 of them got out, I placed the lightning in reverse to back up 5-10 feet so that I could exit the parking lot through the same way I came in. Once I had enough space from reversing so I could make the turn to exit I placed the lighting in drive. The moment the lightning switched to drive it accelerated at a very high speed as if I was flooring the gas pedal, which I was not. The lightning was in one-petal mode which I always use unless I’m on a highway. I immediately began pumping the brakes but the truck would not stop, I probably slammed on the brake petal 4-5 times to no avail. I even tried shifting the truck back into park but it would not shift out of drive. I was able to luckily steer the truck into a hillside approximately 150 feet away. The truck ran over numerous small trees and shrubs before crashing to a stop approximately 16 feet up the hill. Shortly after the shock wore off, I slowly reversed the truck down the hill, exited the truck, and informed dozens of onlookers who came out of the arcade to see what the noise in the parking lot was from, that the truck just took off on its own but that I was fine. I called 911 and the police came. Luckily no people or vehicles were between the arcade entrance and the hillside or the outcome would have been significantly worse. This crash was witnessed first hand by wife and 2 children. I have grave safety concerns about this truck now and will not put my family or self back in it until Ford fixes this problem. While driving to sam’s club (approximately 20 miles from home), my 2018 Ford F-150 3. 5l ecoboost began running very rough, and the check engine light came on. I had the vehicle scanned, and diagnostics confirmed that the cam phasers are failing — the exact issue I had replaced under warranty at approximately 59,000 miles. The vehicle now has just 88,000 miles, and this failure occurred in under 30,000 miles since the original repair. I have maintained this truck with extreme care. I only use motorcraft parts and fluids, change the oil every 3,000 miles or sooner, and always use premium fuel. This is not a maintenance issue. This is a design or part failure that Ford has not properly resolved. When the phasers were replaced the first time, I was told there was no warranty on the replacement parts. That is unacceptable. A known failure being replaced with an equally defective part is not a resolution. Ford is aware of this issue — there are multiple tsbs (including TSB 21-2130 and 19-2208) and widespread complaints across owner forums — but they have taken no responsibility. I paid over $44,000 for this truck and should not be facing the same critical engine failure twice before 90k miles. The rough performance and sudden malfunction while driving posed a potential safety risk. I am submitting this complaint so NHTSA can investigate and hold Ford accountable. This seems like a common problem with this engine, and I do not understand why there has not been a recall or why Ford has not been forced to implement a permanent fix.

Vehicle is shuddering as acceleration starts, took to dealership before warranty ended and they dismissed concerns. Issue has intermittently persisted since then with no resolution available from dealership. The acceleration issues causes the vehicle to slow down and halt before jerking into motion again. Additionally the engine eco boost is occasionally shutting the vehicle off in traffic, if the brake is released too quickly with the boost feature on the engine shuts off, you have to put it back in park and then start the engine again. I've almost been rear ended twice when a light turns green and my engine turns off unexpectedly.
Vehicle stalls out every 3-5 minutes after starting. Vehicle stops and will restart after start up but will only once again run for 3-5 minutes. Vehicle is a hazard on the road as well to the passengers. Local Ford dealership and Ford corp. Both admit there is a recall (25s75) on my 2022 F-150 but are unaware of how to repair the vehicle. I am to wait until Ford develops a solution. Unwilling to provide a loaner vehicle while Ford comes up with a solution. They told me 3-6 months.
Received recall notice for vehicle listed above. Contacted the local Ford dealer and talked to the service writer. They stated as the recall notice outlined that currently Ford does not have remedy for the situation. In the recall notice it outlines one of the issues could be the vehicle to stall particularly in hot weather conditions. . I live on the gulf coast (hot humid conditions) and commute 2x daily across elevated bridges that are notorious for accidents from stalled vehicles on the bridge. This is due to a faulty fuel pump system. My concern is that if I stall out on this bridge the results could be tragic for not only me but for other people using the bridge. To me this situation should be remedied in a "now" timeline to prevent the loss of life and equipment. The service writer agreed with me but didn't have a solution. Why doesn't Ford just install a new fuel pump that doesn't have this particular problem. In my opinion if they can't do that they should replace my current vehicle with a new of the same make and model. My life and that of others in this situation is more important than Ford's bottom line. Recall was in July 2025, no remedy is yet available. This is taking an unreasonable amount of time.
While driving, the vehicle has been going into limp mode at varying speeds on the roadway or while pulling onto the road. The problem lies within the components of the electric throttle body. The system malfunctions and locks the throttle body closed, thus putting the truck into limp mode. Shutting the vehicle off, waiting, and restarting temporarily resolves this issue until the pcm detects a fault in the electrical components of the throttle body and sends the truck into limp mode. When this happens, it also triggers a warning message regarding hill start assist. This is an extreme safety issue as pulling onto the roadway or loosing power in traffic and not being able to pull off can lead to collisions especially around blind curves or in heavy traffic. There also isn’t always a location to safely pull off the roadway, potentially leaving the vehicle and its operator in the roadway, increasing the risk of collision and injury.

While parked and shut off at a gas station in lakeside, CA, my 2015 Ford f?150 XLT spontaneously caught fire under the hood. The fire appeared to originate in the fuse box area, specifically where the fuel pump relay is integrated. The incident posed significant risk, occurring near a fuel pump while my child was in the vehicle. A gas station employee assisted with extinguishing the fire. The cause is believed to be an electrical short. There were no modifications or recent repairs.

I just had to replace my fuel pump due to sudden failure. It was repaired this month at crossroads Ford of wake forest nc. I as well the dealer have the receipts for the repair cost. I believe it should be included in the current recall just issued by Ford and be reimbursed.

Transmission has a defective cdf drum assembly causing transmission to loose bypass hydraulic pressure. This is causing transmission to hesitate between gears during acceleration and Ford dealer diagnosed failure and recommends replacing transmission as clutch packs are failing as well. Harsh downshifts as well. Contacted Ford customer service issued case number, then told they would not render and assistance is $9000. Cost of. Transmission replacement.

My 2013 f150 had been acting strangely in showing a wrench on the led screen and shutting off the electrical components on my dash. When this happens, it also sticks the transmission in between 3-5th gear. I am unable to shift down to 1st or 2nd gear, or up to 6th gear. After this happened a few times, it started to shift me in to park and stop my wheels from turning. This happened to me 3 times while traveling down the highway at 80mph. All 3 times, it lasted only a few seconds, but it was enough to screech my brand new tires and leave marks on the ground as if I had slammed on my brakes. It returns to normal and continues down the road.
Had oil change 7 times in one year and each time they indicate at the place I get oil changed that it is a quart or more low before 3,000 miles of usage after oil change. My car is suppose to take unleaded gas or higher and it knocks if I use unloaded. Now my catalytic converter is going out and it is not covered under warranty unless it is consequential to other problems. Well there appears to be engine problems that are not being addressed and I have a warranty and have taken it in. But they do not look any deeper into the problem except for computer scan and the warranty people won’t give me any answers on what they will do if there is engine problems.
I am submitting a formal safety complaint regarding a known but unresolved software defect affecting the oil life monitoring system in the 2024 Ford F-150 (non-lightning models). Faulty or misleading oil diagnostics can result drivers disregarding future oil life warnings, assuming they are incorrect operators running engines with degraded oil beyond safe limits loss of trust in onboard diagnostics, affecting broader vehicle safety behavior avoidable mechanical damage, particularly for owners following the vehicle’s built-in guidance Ford’s TSB instructs dealers to reprogram several modules, including the ipc, apim, bcm, gwm, and tcu, but only if a customer complains. There is no recall, no public campaign, and no direct owner communication. This leaves potentially thousands of vehicles on the road with faulty maintenance guidance—and owners unaware of it. Why this warrants recall action: this is not a cosmetic or comfort issue. It is a misrepresentation of core engine health diagnostics that, if followed blindly (as owners are instructed to), could cause premature engine wear or failure. The behavior directly undermines the purpose of onboard diagnostics and scheduled maintenance. I urge the NHTSA to investigate TSB 25-2331 and consider elevating this matter to a formal safety recall, or at minimum, compel Ford to issue a customer service campaign notifying all potentially affected owners.
The vehicle a 2024 f150 hybrid with under 3,000 miles. First reported engine warning light was on 4/4/2025 at 2:23pm. The warning light indicated that "service engine soon alert is on because an issue has been detected with the fuel tank or lines" after a week while waiting for the appointment date the warning light went off. Then on 4/24/2025 at 3:28pm the same warning light reappeared. Took the truck to the dealership and they said they had to drop the. Fuel tank to repair and issue. After 2 days the vehicle was returned to me. The problem was as noted on the Ford dealership invoice states, "fuel vapor vent valve is leaking". That was replaced. Also the initial diagnostic noted the "ftp would not hold vacuum even when canister and vapor blocking valve are closed".
Driving at highway speeds and my truck began to shake violently and very loud banging sound. Slowed down to pull over on the highway. Found driveshaft broken in half with gas dumping out of the fuel tank. Broken driveshaft damaged heat shield for gas tank, punctured gas tank and dented/broke clips on exhaust. Very lucky back driveshaft didn’t hit pot hole and leaking gas didn’t start a fire. Stranded on side of road for hours. Sent my VIN to the dealership and they said my was not under the recall #21s56 even tho it was built after known driveshaft issue and clearly the insulator caused the issue.

Due to a defective door (cheap plastic pin), the check engine light comes on with associated warnings on the fuel door not being shut. It is shut but not locked due to the defective pin (I believe plastic) that interacts with the warning light and warning message.
In efforts to fix my vehicle for engine engineers have reset the odometer on my vehicle by over 1000 miles breaking federal law. They do not know how or why it happened and are not providing any explanations or follow ups.
I bought a used Ford F-150 from Toyota used care dealership on [xxx]. On [xxx] while vehicle was being driven on [xxx], north of santa clarita, on the way down to san diego. I was in the backseat, my friend who was the driver notified me. That we had to pull over and check because he was noticing smoke, then we pulled over and the truck was leaking a fluid that was on fire for about 20 minutes. We had exited the vehicle, called 911, the fire then stopped after about 20 minutes. The fire department came and then put out a small fire within the truck. We then had to wait on the side of the rode for 5 hours waiting for a tow truck to pull the truck. I got the truck towed to a Ford service center because the truck still had new vehicle manufacturer warranty with Ford. The truck was given full warranty and Ford covered the damages and fixed the vehicle giving it the green light in there service report. I then waited until March 4th 2025 to receive my truck back. On [xxx], the truck was being driven down southband the [xxx], on the [xxx], again due to smoke and also hearing a loud banging, my friends had to pull over and the truck is going to be towed to the service center which it was first fixed. 2016 f150 3. 5l eco boost v-6 was purchased recently from used dealership. Upon taking possession the vehicle would shut down while driving. I would need to get to the shoulder of road, put truck in park, turn off vehicle and then restart. It’s happened 20 times in last 30 days. I’ve taken to dealership 5x and they are unable to resolve, diagnose or offer any help. Online, many other owners have noted same issue. Doing 65 on fwy and truck suddenly dies is big deal! Ford does nothing. Error message on dash says something about stability track. When restarting engine, it turns over many times as if flooded but then starts.
As I was pulling out onto the road it died blocking a 2 lane road at around 5am so was pitch black luckily cars where paying attention & saw me I was able to roll it back off the road. Had a mechanic look at it after being towed back home he noticed fuse 27 was melted that is fuse for the fuel pump. Currently awaiting how much it will be to fix.

First occurrence with my F-150 stx driving home from work. Started my truck and got a block and a half away and the truck stopped accelerating and completely shut off. All the lights and engine was off. I drifted to the side of the road to avoid traffic causing a traffic pile up and confusion. Being overwhelmed I waited for about 20 minutes then started it and drove home. No lights or check engine lamps on. Having a couple days of normal driving and normal function. I drove again but this time I was going into the freeway halfway up the onramp the truck dies again! this time making it completely unsafe and potentially fatal. I drifted as far as I could to the side as I was going up hill onto the freeway. More cars and extremely different speeds made this moment very concerning . Upon delivery to the dealership and having them offer expensive fuel pump replacements and flushes. I could only afford the $600 fuel pump wake up and having no information about any defects or recalls. I feel like a guinea pig to ideas of half ass fixes.

Vehicle has completely unresponsive gas pedal for 3-5 seconds. Gas pedal is completely dead. This occurs randomly but mostly in a stop and start accelerating situation. Once the 3-5 seconds passes gas pedal responds normally. No codes generated during incident.
The cam phasers on my 2018 Ford F-150 raptor are failing now as I hear rattles on a cold start from the engine, which has then been proved out by technicians at the autonation dealership here in frisco, texas. They are suggesting full replacement for all the phasers which causes major safety concerns. I use my car to drop my 21-month son at daycare every day and drive 30 miles each way to work. I know that Ford offered customer satisfaction program for 2018 manufactured F-150s but somehow the recall doesn't apply to my vehicle. The repair costs are $6k quoted by the dealership I mentioned and it is simply too expensive for us to be able to afford. Therefore, I was wondering if you could please add this recall to my VIN which will allow me to get this safety concern resolved at a Ford dealership. We were told, our truck had a loss of fuel pressure and flow from the fuel tank due to fuel pump failure. This can cause a lack of fuel delivery to the engine and result in an engine stall while driving. That’s what happened to us. Prior to fuel pump failure, we experienced poor engine performance (misfiring or running rough), a check engine light, and a reduction in engine power. The engine stalled while driving on the highway and increased the risk of a crash being stuck on the side of the road up a hill like we were. I was reaching out to see if we are able to get a refund.
