1 Automatic Transmission Control Module problem of the 2021 Ford F-250

Failure Date: 07/16/2025

Hi. I took my 2021 f250 6,7 to the Ford dealership as it was having trouble starting, and just wasn’t running great. They changed the batteries, and told me it was a nox sensor. So I went and got my truck to fix the sensors myself; because they wanted to charge me close to 3 grand. The next day I got up, lo and behold it wouldn’t crank. I had to have it towed back to the dealership. I tested codes before I had it towed, and took a video of it not cranking. The following codes showed. P2291 pcm - injector ctrl press too low - engine cranking p2209 pcm - nox sens heater sense circ range/perf bank 1 sens 1 u029d pcm - lost comm with nox sensor a p2535 pcm - ignition switch on/start pos circ high u300a bcm - ignition switch it’s been in the shop a week , they called Friday saying it was starting, and there was nothing wrong with it. So I replied with all the codes, and told them there was definitely something going on. So they checked it again Monday and it wouldn’t start. Now today (Aug 5 tues. ) they say it may have a vacuum leak. Although non of the codes point to vacuum leak. I have no idea what to do now.

2 Automatic Transmission Control Module problem of the 2022 Ford F-250

Failure Date: 02/11/2025

Check engine light comes on with injector codes. Ford clears the code and the problem continues to come back. Truck uses a lot more fuel when this is active. No help from Ford on getting this resolved. Issue: some 2020-2022 f-super duty vehicles equipped with a 6. 7l engine may exhibit an illuminated malfunction indicator lamp (mil) with cylinder injection pulse offset diagnostic trouble codes (dtc) p2b11, p2b13, p2b15, p2b17, p2b19, p2b1b, p2b1d, and/or p2b1f stored in the powertrain control module (pcm) after various fuel system repairs. This may be due to incorrect learned values in the pcm. To correct the condition, follow the service procedure to reprogram the pcm, reprogram the transmission control module (tcm), and perform the reset minimum fuel mass adaptions all cylinders.

3 Automatic Transmission Control Module problem of the 2020 Ford F-250

Failure Date: 10/15/2023

The contact owns a 2020 Ford F-250. The contact stated that while in drive(d), the accelerator pedal was depressed; however, the vehicle failed to accelerate as intended. The contact stated that the accelerator pedal was pumped gradually, and the vehicle accelerated as intended. The check engine warning light was illuminated. The vehicle was taken to an independent mechanic who retrieved dtc code: p0685. The contact was informed that the pcm relay needed to be replaced. The vehicle was repaired; however, the failure persisted. The manufacturer was notified of the failure. The failure mileage was 91,000.

4 Automatic Transmission Control Module problem of the 2020 Ford F-250

Failure Date: 09/13/2021

The contact's husband owns a 2020 Ford F-250. The contact stated while her husband was at a stop sign, the transmission seized in 2nd gear and the vehicle failed to respond while the accelerator pedal was depressed. The contact's husband stated that the check engine warning light illuminated and "wrench" symbol was displayed on the instrument panel. On another occasion, the transmission seized in 7th gear and the vehicle was unable to make it up an overpass. The vehicle inadvertently started to roll back down the overpass. The contact's husband also had alerts on the Ford pass monitoring tool and received a “failure with pcm and 2nd gear bypassing during cycle" alerts on his cell phone. The contact drove the vehicle to a local dealer, where it was diagnosed with needing the transmission replaced. The vehicle was not repaired. The manufacturer had been informed of the failure. The failure mileage was unknown.

5 Automatic Transmission Control Module problem of the 2020 Ford F-250

Failure Date: 09/09/2020

Tl-the contact owns a 2020 Ford F-250. The contact stated that when he starts his truck that when the vehicle switched gears the transition was rough. There are some times when the vehicle will remain stuck in a certain gear and then have a delay in shifting thus putting a strain on the transmission. When the contact used the vehicle to tow the issue was even worse. The dealer gene messer Ford lubbock 6000 west 19th, lubbock, TX 79407 (806) 793-2727 stated that issue may be with the pcm and ptm. They were going to check with the manufacturer and get back with the contact. The contact tried to call the manufacturer but the call disconnected. The vehicle remained unrepaired. The dealer reommended that the vehicle can be driven for now but not to tow anything. The failure mileage was 3,900. Tf.

6 Automatic Transmission Control Module problem of the 2012 Ford F-250

Failure Date: 06/02/2017

Egt sensor fault resulted in a "stop safely now" alert, shutdown of power while approaching the neches river bridge in beaumont, TX, a bridge on a 3-lane interstate (I-10). Engine subsequently shutdown while in heavy afternoon work traffic causing high potential for a major accident. Traffic behind and next to me swerved and braked on the 75 mph interstate while the truck lost power as it just made the crest. Vehicle was towed. Egt was replaced. Bank 1, sensor 4. Second such failure. First was in August, 2015. Replaced with ac3z-5j213-b in Aug, 2015 (the current failed egt). This time replace with ac3z-5j213-c, see photo for differences in sensor design. I have read rq 14-005, vehicle commanded engine shutdown, and cannot believe this topic was revisited with no action resulting to require the 15v-175 remedy reprogramming for non-emergency vehicles. I ask the the NHTSA revisit this topic to consider allowing owners to have the same pcm change to stop a single egt sensor fault to result in an immediate loss of vehicle power and causing high potential for injury to the public. As a mechanical engineer in the petrochemical industry, and having participated in many hazards analyses over 20 years, I struggle to see that the consequence of continued use with a bad sensor for a longer period, but at some reduced power, could ever be greater the chaos, confusion, and high injury potential from prompt removal of 70% vehicle power from the driver in 45 seconds. Towing a 5th wheel trailer and experiencing the "stop safely now" power cutout is a recipe for disaster. 7 Automatic Transmission Control Module problem of the 2013 Ford F-250

Failure Date: 12/19/2016

Stalling while driving at normal speeds. After taking it to gullo dealership while in TX, the dealership charged us $2100 and "fixed" the problem after replacing the pcm and rcm modules. The next day, the truck stalled on the highway! took it back to gullo dealership in conroe TX and after another week and help from Ford service they still can't fix the problem.

8 Automatic Transmission Control Module problem of the 2008 Ford F-250

Failure Date: 08/08/2012

The contact owns a 2008 Ford f250. The contact stated that while driving approximately 10 mph, the check the exhaust filtration system warning message appeared on the instrument panel and suddenly the engine stalled. The vehicle failed to restart was towed to a safer location where the battery was disconnected and reconnected. The vehicle was able to resume in operation. The vehicle was then taken to an independent mechanic who diagnose that the exhaust filtration system sensor was defective and needed to be replaced. The vehicle was repaired. There was a recall under NHTSA campaign id number 07v139000 (power train:automatic transmission:control module (tcm, pcm); however the VIN was ineligible. The manufacturer was notified of the problem. The approximate failure mileage was 53,000.

9 Automatic Transmission Control Module problem of the 2008 Ford F-250

Failure Date: 09/21/2011

The contact owns a 2008 Ford F-250 sd. The contact stated that he received notification of a manufacturer's recall and took the vehicle to the dealer for recalibration. After the recall service, the contact was driving approximately 50-60 mph when the transmission started to slip. The vehicle was taken back to the dealer where they advised the contact that he would have to drive the vehicle around for a while until it functioned normally. The manufacturer was contacted and offered no assistance. The failure mileage was approximately 14,545. The current mileage was approximately 17,344. The consumer stated while driving at an estimated speed of 50-60 mph and attempting a forced down shift, the transmission shifted into 3rd gear instead of 4th gear causing rapid rear wheel deceleration and high engine rpm to occur. It occurred immediately following servicing of the pcm per the manufacturer's recall. The dealer advised the consumer to drive the vehicle up to 500 miles while the transmission re-learn its shift to points. After complying with the dealers request, the consumer continued to experience the problem. The dealer informed the consumer there was nothing wrong with the transmission and it was performing as designed. Updated 12/19/12.

I was coming back home from vaction in las vagas around just pass state line,when my Ford F-250 hd 2006 went into 4x4 low all by it self. My speed was around 75 miles per hour when the truck went into 4x4 low by it self. If thier whould have been a cars behind me this would been a major crash. My speed went from 75 mph to 20 mph in about 10 -20 seconds,I heard something loud from my transmission and or engine. My truck is at the dealership,I got a call about my truck from the dealership the said the transfer case was bad on the 4x4 control and they were going to look at the transmission.

11 Automatic Transmission Control Module problem of the 2004 Ford F-250

Failure Date: 05/01/2004

2004 Ford f250 super duty king ranch. After reflash to repair an idle problem truck developed three very dangerous conditions. 1. Cruise stalled - going down hill at approx 55-58 the truck will stall without warning. No power steering or brakes. Reproducable on demand. 2. Downshift lockup - truck will lockup rear tires in excess of 30 feet if tow/haul mode is used. On first occasion it almost caused a major accident on highway, came within inches of a head on with a large dump truck while towing a boat. Reproducable on demand. 3. Coast stall. Intermittently stalls usually at off ramps after cruise has been released. Extremly dangerous to take offramps in such a large vehilce with no power.

12 Automatic Transmission Control Module problem of the 2002 Ford F-250

Failure Date: 08/01/2002

The vehicle will lose power with the engine light coming on, cause unknown. The Ford dealership has had the vehicle for three days, stating that vehicle was getting some sort of resistance. The vehicle was again failing with the same problem. The consumer states that he had taken the vehicle to another Ford dealer and they seemed to have remedied the problem-linkage accelerator. The windshield was leaking-resealed-still leaking. The vehicle has been okay since the last repair. Now the windshield is leaking water, and is running behind the dashboard. The consumer has also experienced the following: when stopping the vehicle feels like the drivline shifts forward-loud bang,when starting off, the vehicle shifts backwards, the connector is not fully seated(pcm module), and the injector harness intermittenly opens-repair loose left valve cover injector harness connector causing high resistance cleared.

Consumer called complaining about having problems with the transmission. Light on panel came up, and the vehicle has been at the shop a total of four different times, but the problems still remained. Manufacturer was not contacted at the time of this call.
