1 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 07/12/2019

A safety critical incident occurred on my new 2019 Mazda 3 hatchback when the vehicle when into "limp" mode while adaptive cruise control was engaged at highway speeds leading to rapid deceleration. The vehicle was towed to the nearest manufacturer service location where a diagnostic was performed. Codes show that communication between the central vehicle processor and multiple sub-processors failed simultaneously. Comments were made by manufacturer service representatives that failure mode is likely different from the recent cylinder deactivation/rocker arm recall. Service representatives were instructed to simply clear the codes and drive the vehicle to attempt to reproduce the failure during which it did not occur. I have attached a sanitized version of the service receipt to this message with the full list of codes. Unfortunately Mazda's representatives were unable to provide any explanation on why this failure occurred or what will be done to prevent it in the future. Engineering analysis will need to be performed to reach an acceptable conclusion. Environmental factors relating to the incident include a very hot outside air temperature, with a reading of 115 deg f from the vehicle's sensor roughly 15 minutes before the incident. The incident also occurred on a very long section of us highway with a consistent, gradual incline. Given that information potential causes are intermittent computer hardware failure in high temperature environments, or a software error in power train control or cruise control causing a communication bus failure. With the age and mileage of the vehicle (336 mi) the potential for this failure occurring is likely present in a large portion of new Mazda vehicles.

2 Adaptive Equipment problem of the 2016 Jeep Cherokee

Failure Date: 12/10/2017

First time transmission would not engage at a red light, I thought it was somehow my fault. On/off allowed me to proceed. Then, a few weeks later, transmission would not engage after full stop at redlight on a multi lane road with a center median. Was able to stop and restart in 4wd to limp off road, before transmission shut down again. On/off restored function after a few minutes and allowed me to drive directly to dealer. Car repaired but hours after return from dealer similar problem attempting to enter a limited access hi-way: transmission disengaged while trying to accelerate. Problem of drive train engaging has also occurred from a cold start, and it took several on/off cycles to engage transmission and get car back to dealer. Dealer has focused on adaptive sensors and wiring harness (because of a low voltage reading), which make no sense to me. Confusion created because with each transmission failure every warning light on the dashboard went off. Reading these NHTSA bulletins, I have seen several similar postings about the power train not engaging, so I know my problem is not isolated. I mentioned to dealer a 2016 grand Cherokee recall for brake transmission shift solenoid, which seems to have caused a similar transmission disengagement my suggestion was dismissed. Car only 1 year old and I have barely had use in last 45 days. We are not crazy, and we have a good basis of comparison that something is fundamentally wrong and still undiagnosed. We own an identical second Jeep, purchased within one month of this one. Second Cherokee is fine. . . So far. . However, on our recommendation my sister in law on other side of country purchased same car withinin a month of our second. Same car and same mileage/year and she is experiencing an engine shutdown problem. Suspect our problems are related.

3 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 08/25/2015

I had a roll away incident back around August 25th 2015 I pulled into my driveway and put the car (2014 Jeep grand Cherokee overland) in park . I turned it off and exited the car. It started rolling forward so I jumped back in the car and put on the brake then moved it backwards a little and exited the car again. Again the car was in park and shut off. As I was walking away from the car it rolled forward again about 8 feet . I know it was in park, when it was not in gear. I got out of the vehicle and it started to roll back on me. I quickly jumped back into the vehicle to stop it without accident, this time. Now I put a block in front of the tire so it wont roll away. I wont park it on a hill. Dealer cant seem to find a problem.

The notification provided instructions to contact u-connect in order to update the software. After the update was installed, the contact mentioned that the radio, navigation, ventilation system, and climate control systems failed immediately. The u-connect technician referred the contact to the dealer who later advised the contact to call the manufacturer. The manufacturer stated that the radio needed to be replaced; however, the parts needed were not readily available. The contact stated the manufacturer exceeded a reasonable amount of time for repair. The vehicle was not repaired. The failure mileage was 24,007.

5 Adaptive Equipment problem of the 2015 Jeep Cherokee

Failure Date: 05/23/2015

Two incidents Jeep Cherokee was on automatic cruise control come to almost stop when front car took a right turn and this caused almost rear collision. I was like 30 feet away from the front car. Another incident: there was nobody around me and car applied brakes by itself and causes almost panic among the family members in the car and reduced the speed to 45mph.

6 Adaptive Equipment problem of the 2015 Jeep Cherokee

Failure Date: 05/19/2015

Vehicle is equipped with the collison avoidance system. On the date of the accident the system failed to work. The vehicle was travelling westbound between 5-7 mph at a high school parking lot, the collison avoidance system was on, the front of the vehicle struck the rear of another vehicle. The system did not deploy. The vehicle suffered 3000. 00 dollars of frontend damage. After vehicle was repaired, owner of vehicle conducted tests of the collison avoidance system at 5, 10, and 15 mph with large objects (boxes) placed at minimum of 100 feet from the vehicle. The system never deployed and the objects were struck. At the time of the test Jeep informed owner all systems, including were fixed and fully functional. On three separate occasions between the following dates June 26-July 05, 2015 the collison avoidance system activated with no objectives in front of the vehicle at distance greater than 200 feet. The vehicle was travelling on two occasions on city streets at approximately 20 mph and once on the highway at 55 mph.

7 Adaptive Equipment problem of the 2015 Jeep Cherokee

Failure Date: 04/02/2015

Interior door handles on this Jeep are faulty and break with only normal use. I called Jeep and they said $400 a door panel plus labor and they can't just fix door handle they said whole panel has to be replaced. The two in front are completely broke and back two have cracks. I went online with this issue and found out there are thousands of others like me in this situation. Because of this cost in repairs I feel like Jeep has put me in a unsafe situation for my family. The Jeep is a sport limited and is flawless in condition, except for these door handles. A recall needs to be made before someone gets stuck in there Jeep during an accident and can't get out because of a broken door handle. Because of all the forums on this issue my family and I are driving are other vehicle as much as possible until I can figure out how to get this problem fixed. I am hoping I do not have to sell this rig because of broken door handles , but with a baby in back I am not feeling like Jeep is looking out for the true safety of there customers at this point in time.

8 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 03/28/2015

While driving the odometer messages started popping up and an alert chimed for each one: acc / fcw unavailable service required; big red battery icon and low voltage message; park assist off message; radio screen went blank; small red battery icon remained lit on the odometer screen. I then selected to view the battery voltage along with my speed on the odometer screen. The voltage moved from 16 to 15 and stopped at 14v (1/2 charge) all in less than 5 seconds. I tried to make a hands-free phone call and as the phone was ringing the phone call was terminated by the vehicle.

9 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 03/27/2015

Picked vehicle up from dealer - they made sure battery was fully charged and suggested it be driven before taking home. On the interstate at 65-75 mph activated the adaptive cruise control. It shut itself off in approximately 10 seconds. Then a message on the odometer: acc / fcw unavailable service required; message low voltage; radio screen went blank then came back on. A small red battery icon remained lit on the odometer screen.

10 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 03/22/2015

The odometer screen popped up several messages: acc/fcw unavailable service required; then a red battery image and low voltage message; and the transmission was locked in d3. After turning it off for 30 minutes it self corrected but a small red battery icon remained lit on the odometer screen.

11 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 02/26/2015

Vehicle's "adaptive cruise control w/ stop-and-go" was activated and set to 40 mph. On approach to an intersection with stopped traffic ahead, the adaptive cruise control feature failed to slow the vehicle and a rear-end collision nearly occurred. The system had a clear field of view and approached stopped traffic from approximately 300 yards. The vehicle's collision alarm and brake booster activated as I waited for the system to slow the vehicle. Using full braking force, the vehicle came to rest approximately 5 feet from the vehicle in front of me.

12 Adaptive Equipment problem of the 2015 Jeep Cherokee

Failure Date: 02/05/2015

2015 Cherokee latitude 4 cyl. Have had vehicle for 2 months, during this time the transmission has had a mind of its own. Accelerating during braking, which in and of itself is extremely dangerous as it has happened while slowing to make a turn, if you need to turn at 10 mph and the car accelerates past 25 you are looking at an accident waiting to happen. Lag in downshift while trying to accelerate, loud un explained clunk while placing into park, numerous engine warning light activations. Called local dealer and was informed that I needed to wait a few days to see if warning light resets itself before bringing in. The transmission never seems to actually shift into 9th gear, which inturn hampers the fwy mileage claims associated with this vehicle. Ive noticed the transmission temperature exceed 240 degrees on numerous times, this cannot be good. I will be taking vehicle to dealer, hopefully in an effort to get this resolved. With all of the reviews and problem, will it take an unfortunate fatality for Chrysler to actually resolve this, as in the case of the Toyota acceleration problems?.

13 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 10/14/2014

As has been reported by others, my Jeep Cherokee has brake for no apparent cause on three different occasions. The "collision imminent" tone has sounded at the same time. There was no accident or loss of control at the time this happened, but it was disconcerting. On two other occasions, the same thing has happened, but these were triggered by a car leaving the road to the right side, to turn at an angle into a street that branched off the route I was on. Although the car was clearly out of the path of the car, apparently the collision avoidance system thought that my car was closing on the other car too quickly, and again sounded the warning and slowed the car down. Again, there were no problems with control of the car, and no accident, although drivers behind me had to slow down by about 15 miles per hour below the speed limit. I have learned to leave some extra distance between me and cars turning off for just this reason.

14 Adaptive Equipment problem of the 2015 Jeep Cherokee

Failure Date: 09/16/2014

I recently leased a new 2015 Jeep Cherokee with safety package and fcw system. When I took vehicle home I relealized that the forward collision warning sytem was not engaiging no audible and no visual alerts so I took it back to the dealer. The vehicle has been back to service several times for re-calibration but they cant get the system to work as designed or advertised. I am waiting for the service tech to get back to me with additional information as he was waiting for a response and overhead support rom Jeep. According to the service tech alot of these vehicles are experiencing similar issues. This is a potenially dangerous issue and real safety concern. If folks are relying on this system they may be at risk.

15 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 09/16/2014

The contact own a 2014 Jeep grand Cherokee. The contact received notification of NHTSA campaign number: 14v641000 (equipment adaptive) however, the part to do the repair was unavailable. The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The contact had not experienced a failure.

16 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 09/03/2014

We purchased this vehicle new, with 4,658 miles on it and within the first day, we noticed the extremely rough shifting between 2nd ¿ 4th gears. After several pcm reprograms and a new transmission, it is still doing the rough shifting and continues to worsen. In heavy traffic, the accelerator is inconsistent, sluggishly moving forward on one occasion and lunging forward as if the gas pedal was mashed to the floor on the next. Same can be said for braking; rough down shifting and often times feeling as though the vehicle is still trying to accelerate as if the gas pedal is still pressed even though the brake is being applied. In another incident, I started the vehicle, with my foot on the brake shifted it to drive, then began to release the pressure from the brake to inch forward, when it suddenly lurched forward nearly striking my parents. Additionally, the vehicle now seems to be driving itself. When entering my neighborhood doing approximately 25mph, upon letting up on the gas completely, the vehicle maintains the rpm¿s and speed on a flat surface as if the cruise control is on. We have filed for the MD lemon law with Chrysler, but after a test drive with one of the their reps where the car was not as violent in the shifting but still quite rough, the rep declared the vehicle as ¿performing as designed and ¿it¿s the adaptive learning of the vehicle and closed the case. This vehicle is completely unpredictable and inconsistent, and quite frankly, unsafe and nerve racking to drive. My wife will not drive nor ride in it any longer for fear of an accident, nor will she allow our son to ride in it. In my 28 years of driving, I have never owned nor driven a vehicle that actually made me feel unsafe in. As I stated to the rep on the test drive: with this vehicle, it¿s not a matter of if this thing gets in an accident, but rather when!.

17 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 08/12/2014

A short time after purchasing the new 2014 Jeep Cherokee, we started having trouble breaking at any speed. As you stepped on the brake, the rpms went way up and it felt like the car was trying to accelerate instead of stopping. It always felt like you had to fight it to stop and it might not stop at all. Took it to the dealership service department and was told it had software updates for the transmission which did not solve anything. At the point that we were calling to take back in, received the "service transmission" warning and was told by service department to drive it in and it would "go into a safe mode". Drove slowly while on a major highway and after slowing down for construction, it would not speed up past about 27 mph. It had to be to towed in. After a new transmission, after waiting about 2 weeks due to 15 local dealerships waiting for transmissions, we were told by the service manager that it was not ready and doing the same thing. He said there was another software update with no estimated release date. There has been an update for the 2015 models that have the same problems since but nothing for the 2014 models still. We have not had our car since September since it is not safe to drive. It is being bought back after much pain and all the decisions being made by the Chrysler area representative, sean werth, who according to the dealership sales manager said that our car was as fixed as it was going to be.

18 Adaptive Equipment problem of the 2014 Jeep Cherokee

Failure Date: 06/28/2014

The first issue arose as the vehicle was being backed out of a driveway. While still in motion, the vehicle produced an audible and visual collision alert with no obstructions present. The vehicle stopped suddenly, partially in the roadway, shifted to neutral and shut the motor off. It was necessary to cycle the vehicle through stop-accessory-run and back to accessory before the vehicle could be placed in park. Once in park, the information center (evic) read "battery voltage low". After returning to the stop position the vehicle was restarted and the malfunction indicator (mil) remained illuminated. Scrolling to the battery status on the evic showed a reading of 13. 9v, within a normal range. The vehicle was placed in drive and operated properly, except for the mil for 24-hours with multiple starts and stops. The following day, upon starting the vehicle, again an audible and visual alarm sounded and the vehicle shut down completely, with the evic reading 4-wheel drive unavailable. It then changed to adaptive cruise unavailable, then forward collision warning unavailable. An indicator light with an exclamation mark stated that service was required. Again, the vehicle was cycled through the stop-accessory-run sequence without the break depressed to re-set the electrical sequencing. The vehicle was restarted and only the mil remained illuminated. The vehicle was then able to be put in gear and returned to service. Both occurrences took in excess of one minute to return the vehicle to operational, and had they happened in an active traffic area, would have left the vehicle a sitting duck for oncoming vehicles. The dealership has been notified and an appointment scheduled to address the issues. No repairs completed to date; owner advised that the vehicle is still able to be driven, but to keep the phone number to roadside assistance handy.

20 Adaptive Equipment problem of the 2015 Jeep Cherokee

Failure Date: 05/12/2005

With the cruse control on and set at 60mpg when going down a hill it picks up speed to over 70mph. Going up hill it varies as much as 4mph. The cruse control does not control the speed as it should, very dangerous!.
