1 Air Bag problem of the 2011 Mazda MAZDA3

Failure Date: 01/20/2026

The airbag system failed to deploy during a major accident I was in, where my car rolled multiple times, and the collision that totaled my car. This includes the front airbags, impact sensors, and/or the airbag control module not activating. The vehicle is currently in the possession of the insurance company and can be made available for inspection upon request. During the crash, no airbags deployed, which significantly increased the risk of serious injury or death. The lack of airbag deployment left my partner and me completely unprotected at the moment of impact, including the number of times we rolled, seriously putting our safety at severe risk the vehicle is no longer drivable. The police report notes that the airbags did not deploy. The vehicle is in the process of inspection by my insurance adjusters as part of the total?loss evaluation. The manufacturer has not yet inspected the airbag system, but the vehicle can be made available if Mazda requests an inspection. There were no warning lights, messages, or symptoms indicating an airbag or safety system issue before the crash. The airbag system gave no indication of malfunction before the failure occurred.

2 Air Bag problem of the 2019 Mazda MAZDA3

Failure Date: 11/05/2025

1. What component or system failed or malfunctioned, and is it available for inspection upon request? the airbag system, specifically the curtain airbags and related airbag control components, malfunctioned by deploying spontaneously without any collision or impact. The vehicle and deployed airbags are still available and can be inspected upon request. 2. How was your safety or the safety of others put at risk? the spontaneous deployment of all four curtain airbags created an immediate safety hazard. Airbag deployment can cause injury, disorientation, or loss of control, especially if it occurs unexpectedly. Although the vehicle was stationary at the time, deployment without warning posed a serious risk of injury to occupants and could have resulted in a crash if it had occurred while driving. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? the problem has not been reproduced, as the airbags have already deployed. However, the spontaneous deployment itself is clear evidence of a malfunction. The issue has not been resolved or repaired, and responsibility for repair has not been accepted. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? the incident was reported to progressive insurance, which denied coverage because there was no accident or collision. The vehicle has not yet been inspected by the manufacturer for defect analysis. No police inspection occurred because there was no crash or injury. 5. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? yes. The airbag warning light was illuminated prior to the incident, indicating a potential system fault. Additionally, a strong chemical odor was noticed inside the vehicle immediately before deployment, originating from the front passenger side, moments before all four curtain airbags deployed.

4 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 07/19/2025

I recently received a warning on the dashboard that there is an airbag malfunction and needs to be taken in to get serviced. The warning continues to appear. This appears similar to a recent recall of 2024-2025 Mazda 3s and cx-30s, but there does not appear to be a recall issued for 2022 Mazda 3 models. I have scheduled an appointment for July 24, 2025 at the Mazda dealership to get my vehicle inspected, but because there is no active recall, Mazda states that I may have to pay for the diagnosis and any repairs. I disagree that I am required to pay for any diagnosis/inspections/repairs.

5 Air Bag problem of the 2024 Mazda MAZDA3

Failure Date: 07/01/2025

On July 1, 2025, I was involved in a severe rear-end collision while driving my 2024 Mazda3 manual hatchback. The impact was forceful enough to shatter the rear windshield, leave a deep dent in the tailgate, and bend the trailer hitch mount under the vehicle by several inches. Despite this, none of the airbags deployed, and there were no airbag warning lights before or after the crash. After the incident, I checked my vehicle’s VIN on NHTSA’s recall lookup tool and discovered that recall 7525e had been issued on June 6, 2025, for a critical airbag sensing system defect (sas module), which can disable airbag deployment after battery depletion or improper power conditions. According to Mazda and NHTSA documents, this defect can completely disable airbag function, increasing the risk of injury in a crash. As of July 11, 2025, I have not received any recall notice from Mazda by mail, email, phone, or dealer contact—despite being the registered owner. Mazda’s documentation shows that: dealers were notified as early as June 3, 2025 owner letters are not scheduled until July 28, 2025 this means that I was driving a defective vehicle without knowledge of a serious safety issue at the time of the crash. I believe that: Mazda failed in its legal obligation to notify me in a timely and adequate manner. The non-deployment of airbags in my crash is consistent with the failure described in recall 7525e. Other affected Mazda owners may remain unaware and at significant risk. I respectfully request that NHTSA: investigate whether recall 7525e was the cause of airbag failure in this incident. Determine whether Mazda’s recall notification practices met regulatory requirements. Ensure immediate outreach to affected owners to prevent further injury or risk. I am also seeking legal counsel due to the serious safety implications and potential liability arising from this defect. I will be retaining all documentation, vehicle records, photos, and post-.

6 Air Bag problem of the 2014 Mazda MAZDA3

Failure Date: 06/16/2025

The car screen displays, gps, for calls, radio etc, doesn’t works, it’s off, and the airbags are deactivated and they’re off as well and have to press hard on the brake to get the car to stops.

7 Air Bag problem of the 2025 Mazda MAZDA3

Failure Date: 06/05/2025

I found out about this recall today. Mazda doesn’t have a fix to this recall yet. I am scared to drive my car in the event of a crash, due to the fact that my airbags won’t go off due to the recall. Mazda said to just “be aware until we have a fix” I cannot just “be aware” if I get into an accident and my airbags don’t go off.

10 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 12/23/2024

The passenger-side "airbag off" light does not go off, even with an adult in the passenger seat. This is a us car without an ability to disable the airbag, so this should never happen. We assume this means that the airbag will not inflate in an accident. It happens on about 60% of drives with a passenger, and the dealer was unable to reproduce.

11 Air Bag problem of the 2019 Mazda MAZDA3

Failure Date: 12/01/2024

A few weeks ago the following error message popped up on my dashboard: “airbag malfunction. Have vehicle inspected”. This message came and went intermittently whenever I had a passenger in the vehicle. I scheduled and appointment at my local Mazda dealership service center and in the meantime looked up the issue and found that it was extremely common and caused by a design flaw where the wire harness to the seat airbags was too short causing eventual failure. The issue was so prominent that Mazda even issued a service bulletin on how to fix it which included replacing the wire harness and adding extra security clips to help prevent future failure. So I get my car to the service center and after looking at it they confirm this exact issue, say it is not covered under any extended warranty and tell me that it will cost approximately $1,000 to fix (diagnostic, parts plus labor). I asked how a safey issue such as an airbag not deploying isn’t covered and they reaffirm that it is not a safety issue and that there is no recall for it nor would it fail inspection with that error message. They also did confirm that in the event of an accident that airbag would not deploy while that error message was showing. I am at a loss right now and am not sure what to do as I cannot afford a $1,000 fix and am very confused as to how this design flaw for what I believe to be a safety issue is not covered under a recall.

13 Air Bag problem of the 2012 Mazda MAZDA3

Failure Date: 04/05/2024

The dashboard of my 2012 Mazda 3 has separated from the instrument panel causing a gap between them. The dashboard has become very sticky leaving a black sticky film on the surface. This has caused a glare on the windshield making it hard to see at times while driving. There is a letter dated may 2017 from Mazda, special service program (ssp) a9. The letter states that the warranty was extended 7 years (84 months) from the original warranty start date or April 30, 2018, whichever is later, with no milage limitation. As I read it, 7 years of 84 months from 2018 should extend the warranty for repair of replacement through 2025. Mazda refuses to make repairs, or replacement of this equipment that has an impact on the proper function of the airbag system and impairs the driver's vision because of the constant windshield glare. This is an issue. That needs to be addressed as an important safety.

14 Air Bag problem of the 2010 Mazda MAZDA3

Failure Date: 03/31/2024

While driving on the interstate, the driver’s side airbag deployed without any obvious cause. There was no pothole, bump, or clear reason for the airbag to deploy. There was moderate traffic. This put myself and others nearby at risk by causing me to flinch after getting hit by the airbag. I was lucky to not swerve into anything or cause any damage. It caused temporary loss of hearing and a sore neck, but no serious injury. Prior to the failure, there were no dash lights, messages, or other symptoms.

15 Air Bag problem of the 2023 Mazda MAZDA3

Failure Date: 11/30/2023

The contact's daughter had rented a 2023 Mazda Mazda 3. The contact stated that while his daughter was driving at approximately 70 mph, the vehicle veered off to the left side of the highway and side swiped a cable system; the vehicle then crashed into the end cap of a guardrail. The vehicle then hydroplaned and hit a bridge pillar. The vehicle flipped as the contact's daughter was ejected from the vehicle and the vehicle landed on the roof in the grass next to the guardrail. The vehicle caught fire and was incinerated by the time authorities arrived on the scene. The contact's daughter was pronounced dead at the scene. The fire department extinguished the fire and a police report was filed. The guardrail, cable system, and a wooden road post were damaged due to the accident. Due to the extent of the fire, it was unknown if the airbags deployed. The vehicle towed to an independent mechanic. The contact stated that the investigators could determine the exact cause of the crash but believes that it was likely that the contact's daughter fell asleep; the contact contests the investigator's findings. Upon his own investigation, the contact discovered that owners of previous year model vehicles had reported that the air bag and seat belt pretensioners had failed to operate in the event of a crash. The manufacturer had yet to be notified of the failure. The vehicle was destroyed. The failure mileage was unknown.

16 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 11/04/2023

For a few weeks in November 2023, I had my dash alarm/notify me when I would turn on my car and it would state "airbag system malfunction" then disappear. Potential risk with this issue is that the airbag may deploy randomly or not deploy at all if collision happens. This is extremely scary as I have a child and I commute daily for work. I have turned my car into the dealer since 11/29/23 and currently have a loaner. No update so far from the dealer.

17 Air Bag problem of the 2016 Mazda MAZDA3

Failure Date: 09/11/2023

The contact owns a 2016 Mazda Mazda3. The contact stated while driving 40 mph, the front driver's side door air bag and the front driver's seat air bag deployed. The contact sustained some busies due to the failure. The vehicle was taken to the dealer for inspection and diagnostic testing. The dealer inspected the vehicle and determined the failure was the result of an accident. The contact stated that the vehicle was inspected by progressive insurance and noted the failure as mechanical failure by the manufacturer. The vehicle was not repaired. The manufacturer was made aware of the failure but provided no assistance. The failure mileage was approximately 126,000.

18 Air Bag problem of the 2010 Mazda MAZDA3

Failure Date: 02/03/2023

These vehicles have a foam based dash board which is falling off in pieces and sections . This can cause choking hazard in small children adults etc also if air bags where to deploy this section would more then likely explode in many hundreds of pieces causing blinding . As well as it decays or falls off in these pieces children may place objects in there mouths or chew or eat on . The dash board does not seem to be able ro handle a air bag deploy situation . I would have thought this would have been replaced or recalled prior to my intation of this matter. Also dash may contain harmful or chemical agents I'm it that would not be safe to child nor adult.

19 Air Bag problem of the 2016 Mazda MAZDA3

Failure Date: 12/20/2022

Nothing has happened yet but ive done a bunch of research and a lot of the 3rd generation Mazda 3's all have the same airbag code 16 flashing on their dash and it's not on any recall list.

20 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 12/19/2022

I was rear ended while at complete stop by someone going over 40mph on 12/19/22. My airbags did not deploy and my seat belts did not lock out. Directly after being hit my phone flashed a safety fracture malfunction on the dash board and then sent the notification to the Mazda app on my phone.

21 Air Bag problem of the 2007 Mazda MAZDA3

Failure Date: 11/25/2022

My daughter and I were on the way to o'reilly when we turned at a short curve going under 20 miles and the steering wheel got loose and the brakes wouldn't stop us. The car just kept swerving all over the road and we ended up in a ditch the day after my husband purchased the car.

22 Air Bag problem of the 2010 Mazda MAZDA3

Failure Date: 10/25/2022

Said vehicle has 2 recall issues that are listed as expired. 1. It has not had the defective takata air bags replaced campaign sspb3 started 12/1/2017. 2. It has a sticky dashboard campaign ssp99 started 11/30/2015. A third recall for a seat lifer link was completed for campaign 0917b on 9/13/2017. I would like Mazda to take responsibility for the 2 expired recalls for the air bags and the sticky dashboard. Mazda had the vehicle in one of their service centers 9/17/2017 when the seat recall was completed. Mazda’s policy of checking for recalls failed otherwise these recalls would have been completed. Both recalls impact the safe operation of said vehicle.

23 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 10/05/2022

The same or similar issue is described here: https://static. Nhtsa. Gov/odi/tsbs/2022/mc-10209787-0001. Pdf essentially, this issue has to do with the wiring under the seat that is responsible for airbag functioning. Adjusting the seat caused "airbag malfunctioning" alarm/notification to go off on a numerous occasions. Potential risk with this issue is that the airbag may deploy randomly or not deploy at all if collision happens. The vehicle has been inspected by vehicle who is working on the repair. However, the vehicle has been waiting for spare parts for 3 weeks now.

25 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 08/11/2022

On 8/11/2022, my car indicated a message on my dashboard that said “air bag system malfunction. Have the vehicle inspected. ” I also received a safety & security message in my inbox on my app that stated “air bag system and seat belt pre-tensioner system malfunction. Vehicle airbags and seat belt pre-tensioner may not operate correctly in an emergency. Have the vehicle inspected by your dealer. ” this message was received multiple times. I called the dealer and they weren’t sure what it meant so I took it to them on October 6, I dropped off my car at the dealer. They called me a few hours later and told me that my car was not safe to drive because the airbag and seat belts weren’t working. Fortunately they had a rental for me, however, today is 11/10/2022 and I still don’t have my car back. They stated that it may take up to 6 weeks to get the parts in. And this dealer has never seen anything like this before.

26 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 07/14/2022

I purchased my car on February 5, 2022. After two months and 3000 miles later, I started receiving a notification that reads “airbag malfunction. Have the vehicle inspected. ” I took the car to the dealer, and after 6 hours, they determined that the airbag was not receiving enough power, which triggered the warning. They replaced the part, and now two months later, I have received the same notification. This problem is persistent, and I have seen in forums that many are having to deal with this issue. Mazda needs to recall the cars to replace the parts with ones that will not cause this type of inconvenience. After all, airbags are a huge part of safety.

27 Air Bag problem of the 2022 Mazda MAZDA3

28 Air Bag problem of the 2021 Mazda MAZDA3

Failure Date: 06/11/2022

The vehicle was badly rear ended and the airbags didn't deploy. The dashboard indicated an "airbag malfunction" after the collision happened. The failure of the airbags put myself and my passengers at serious risk; if we weren't wearing our seatbelts, which we were, we would have been gravely injured. The vehicle is less than a year old and still under warranty.

29 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 05/12/2022

Air bag warning light went on. Found code b1126:13 driver side airbag deployment. Replaced driver side air bag under warranty.

30 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 04/23/2022

Purchased vehicle new at dealership April 15th 2022 w/ 17 miles on the odometer. After 500 miles on the odometer, began receiving multiple daily warnings through the mymazda app indicating airbag/seat belt pretensioner malfunction. Subsequently noticed the same warning lit up on the dash while driving for approximately 10 seconds and then light went out. Discovered technical service bulletin dated 317/2022 08-001/22 indicating this a known problem for Mazda 3 vehicles between 2019 and 2022. Showing design flaw in path layout for wire harness connecting to side airbag module in seat back. Recommends modifying plastic module cover by removing several inches of plastic, several inches of foam padding, and pinning section of wire harness to steal seat frame with plastic grommet. This is a clear safety hazard concern that should have been addressed before I was allowed to drive the vehicle away from the dealership. Mazda's position is not to address the issue until they receive a customer complaint. If I had been in an accident I may have been severely injured or killed because of this malfunction.

31 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 04/21/2022

When adjusting the driver seat, the vehicle began to display "air bag system malfunction. Have the vehicle inspected. " this was with roughly 20 total miles on the odometer. It is a new car - Mazda3 turbo sedan 2022. After researching online, this is a very common problem with new Mazda3s and there are many customers with the exact same issue. The customers have also made complaints to the NHTSA. Mazda has a technical service bulletin for this problem (bulletin no:08-001/22) and states the issue is due to "driver-side air bag module circuit: open circuit or resistance high". This issue also occurs on the passenger-side air bag. It is a possibility that I could have been in an accident and the side airbag would not function. Or even perhaps the air bag may have had an electrical error and engaged itself while I could have been driving. My local dealership has verified the issue, however, they still have not been able to correct the issue after 8 weeks. So I have had to use a loaner for 7 weeks now. In summary, I'm filing a complaint because 1)this issue is so common 2)it pertains to something crucially important to safety 3)this is occurring almost immediately after purchase 4)Mazda does not seem to be able to readily and quickly fix the issue.

32 Air Bag problem of the 2011 Mazda MAZDA3

Failure Date: 04/12/2022

When I’m driving the car, electricity of the car goes off, including headlights. The steering wheel gets locked. It doesn’t do that regularly. Sometimes, it takes weeks to do that, but sometimes it happens once in every five minutes. The red airbag light on the odometer turns on or off randomly. But, the worse case is that the airbags are not working while my steering wheel gets locked at the same time. They all start happening around the same time. I took my car to the Mazda service and another mechanics. Neither of them found the issue. When I leave the car with them, the car doesn’t show these malfunctions. It literally randomly occurs. Therefore, they told me that I need to bring the car when it start happening in regular basis to understand the exact cause for the issue. I don’t know when it becomes a regular issue. 4-5 days ago, when I was turning right on a small street, the wheel locked again. I was about to crash. It’s a deadly malfunction, so I don’t want to wait until then.

33 Air Bag problem of the 2022 Mazda MAZDA3

Failure Date: 02/15/2022

Hello, I bought my 2022 Mazda 3 sedan in February of 2022. I’ve had my car for a total of 5 months. From the very first month I bought my car I’ve had problems with the “air bag / seat belt pre tensioner. ” I have seen this issue appear on my dashboard as well as my Mazda app to give me warnings numerous times I can’t keep track. I have taken my vehicle to my Mazda dealership a total of 7 times for this same issue “air bag/seatbelt pre tensioner” and yet to have my car fixed. Every time I take my car into the dealership service they claim the issue has been fixed and it has not been till this day. I decided to do my own research on my vehicle and seen so many complaints throughout the internet regarding the same issue on my model 3 sedan. These issues have been happening from previous models of the company vehicles. The service department and corporate are well aware of my issue and say I’m not the first customer with this type of complaint. This issue on the Mazda 3 sedan needs to be investigated as this is a huge safety issue for customers. We shouldn’t have to pay the price as the buyer. Thank you.
