I own a 2018 Tesla Model X that has multiple breakdowns ( 3 times to date: Feb. 28, 2026, Mar. 7, 2026, Mar. 14, 2026) resulting in the vehicle becoming undriveable and could not be shifted out of neutral. During driving on separate occasions, the vehicle displayed multiple safety warnings and critical systems became disabled. Alerts shown on vehicle included: 1. Automatic emergency braking unavailable 2. Power braking assist reduced 3. Parking brake not applied 4. Safety restraint system issue 5. Abs disabled 6. Park assist unavailable 7. Service required following these alerts, the vehicle became undriveable and could not be shifted properly, remaining stuck in neutral. The vehicle therefore could not be driven safely. The vehicle was previously taken to a Tesla service center after an earlier breakdown. Tesla reported cleaning corrosion during service, but shortly after the vehicle was returned, the problem occurred again with the same or similar safety warnings. This issue is concerning because multiple braking and safety appear to fail simultaneously, including abs, braking assist, and restraint system alerts. The vehicle becomes undriveable and may create a dangerous situation. Given the repeated breakdowns and the disabling of critical safety systems, I am reporting this issue for investigation.

2 Equipment problem of the 2020 Tesla Model X

Failure Date: 03/13/2026

On 03/13/2026, while driving my 2020 Tesla Model X at approximately 65 mph on a freeway, the front hood (frunk lid) suddenly flew open without warning and slammed into the windshield. The hood rotated upward violently and struck the windshield completely blocking the driver’s forward visibility. This created an immediate safety hazard while traveling at highway speed. Prior to the incident there were no warning messages or alerts displayed on the vehicle screen indicating that the hood was open or improperly latched. Tesla vehicles normally provide a dashboard and a sound alert if the hood is not properly latched before driving. No such warning occurred. The vehicle’s front camera recorded the incident. The video does not show any hood movement or flutter prior to the failure, suggesting that the hood was fully closed before the event. After the incident, Tesla service personnel inspected the vehicle. The technician stated that the hood may not have been properly latched prior to driving. However: the vehicle gave no dashboard warning no hood movement or vibration was visible prior to the event the hood opened suddenly and violently rather than gradually if the secondary latch alone had been holding the hood, some hood movement or vibration would normally be expected prior to full release. Safety concern a hood opening at highway speed that: occurs without warning results in complete loss of forward visibility presents a severe safety risk to the driver and surrounding traffic. On February 17 at approximately 9:30 am, while my wife was driving uphill, the vehicle abruptly shut down and became completely disabled. It began rolling backward despite her attempting to accelerate. Multiple warning alerts activated simultaneously, and the vehicle would not respond to forward input. Our [xxx] child was in the vehicle at the time. A total loss of power while in motion, combined with unintended rollback on an incline, represents a catastrophic safety defect. This was not a minor malfunction — it created a direct and immediate risk of collision and serious injury. Compounding this concern, on the morning following the incident at approximately 6:00 am, I discovered one of the vehicle doors open without any user interaction. The vehicle had been secured the night before. This raises serious questions regarding electronic reliability and system integrity. Given the severity of these failures, I have lost confidence in the safety and dependability of this vehicle. My family’s safety cannot be subjected to further uncertainty. If this matter is not resolved promptly and satisfactorily, I am prepared to pursue all available legal and regulatory remedies, including formal complaints to appropriate federal safety authorities. Failure Date: 02/03/2026

The 2018 Model X experienced a sudden and complete loss of power and became totally inaccessible. All doors and access points were inoperable and the vehicle could not be unlocked, disconnected, powered on, or placed into any service or tow mode. Emergency front-trunk release cables had to be used and external 12-volt power applied at the jump terminals to wake the vehicle. The inability to access the vehicle was unexpected and alarming. Children, pets, elderly, and others could have been inside of the vehicle. Thankfully, they were not. Tesla inspected the vehicle and stated the failure was related to the battery coolant heater. The failure resulted in a complete vehicle immobilization and access failure. I had previously experienced restart warnings on the dashboard, but I did not expect a total lockout and loss of access. The inability to access the vehicle was unexpected and alarming. It may be a known safety issue. It should be recalled. This condition presents a serious safety risk because occupants could be trapped inside the vehicle and the vehicle cannot be accessed or moved without external power. In extreme heat or cold, or in a medical or emergency situation involving a child or pet, this failure mode could create a life-threatening situation. Importantly, even after multiple requests, I have not received a copy of the diagnostic report, or anything other than informal emails or calls. Tesla has not provided information regarding this failure other than broad statements, and visuals of the location of the battery pack, in an attempt to deny warranty coverage. The Tesla reports a high voltage system alert currently. The vehicle is subject to Tesla’s battery and drive unit limited warranty. Tesla has stated that the failed component was the battery coolant heater, but has refused to cover the repair under warranty. Tesla confirmed the failure but has not provided the diagnostic report, fault codes, or vehicle logs supporting its determination.

On 02/01/2026 at approximately 2:07 p. M. , my 2018 Tesla Model X (VIN [xxx] , approximately 151,099 miles, software version 2026. 2. 3) experienced sudden unintended acceleration while I was driving near [xxx] . I was [briefly describe what you were doing, for example: “slowing to a stop in traffic,” or “slowly pulling into a parking space,” or “braking to turn into a driveway”]. My foot was on the brake and I was not attempting to accelerate when the vehicle suddenly and unexpectedly surged [forward/backward] with far more power than appropriate for the situation. I had to [firmly press the brake / shift to neutral / steer away] to avoid a collision. At or around the same time, a “sudden unintended acceleration” warning/message appeared on the vehicle’s display. This acceleration did not match my pedal input and created a serious risk of a crash. Water entered the passenger cabin of my vehicle during normal rain exposure while the vehicle was parked overnight. The failure occurred at the roof glass/seal system, allowing rainwater to intrude into the interior behind the second-row seating area and soak the carpet and underlying padding. This resulted in persistent moisture and a strong musty odor inside the cabin, consistent with microbial growth and mold risk. The condition affects interior air quality and poses a potential health risk to vehicle occupants, particularly children or passengers seated in the rear. Prolonged moisture inside the cabin also raises safety concerns related to corrosion and possible impact to electrical components, sensors, and restraint systems routed through the interior floor area. The condition substantially impairs safe and normal vehicle use. The presence of moisture and microbial odor makes the vehicle unsuitable for passenger transport and creates uncertainty regarding the integrity of interior and electrical systems. The issue was inspected and confirmed by the manufacturer’s authorized service center. The service center documented that water intrusion occurred through failed or missing sealant around the roof/door glass assembly and confirmed saturation of interior carpet and padding. The manufacturer recommended replacement of interior padding for safety reasons but declined responsibility due to the vehicle being outside the basic warranty period. There were no warning lamps or messages prior to discovery. The issue was identified after rain exposure when wet carpet and odor were observed. The problem is reproducible under rain conditions. The vehicle is available for inspection upon request.

I am a software engineer and owner of two 2020 Tesla Model X (raven) vehicles. Both exhibit a serious safety defect after installing firmware 2025. 44. 25. 4 with fsd v12. 6. 4. Vehicle configuration: model s/x with mcu2 (intel atom) and hardware 3 (hw3). Defect description: when full self-driving (fsd) is engaged, the vehicle intermittently enters a critical failure state while in motion: • the instrument cluster (driver display) suddenly goes completely black • the center screen issues a red “take control immediately” alert with alarms • the infotainment computer performs a watchdog reset and reboots (Tesla logo appears) while the vehicle is traveling at highway speeds this has occurred multiple times on both vehicles. Suspected cause: the 2025. 44 software update appears to introduce additional dashcam telemetry writes (speed, steering, fsd state) to the usb device while recording video. On mcu2 (intel atom), this creates an I/o and cpu bottleneck that starves display and safety-critical threads, triggering an operating-system watchdog reboot. Disabling dashcam immediately stops the crashes, indicating a resource-exhaustion condition rather than driver error. Safety risk: during the blackout and reboot, the driver loses all visual feedback from the instrument cluster while being instructed to take control. This creates a high risk of collision, especially at highway speeds or in complex traffic environments. Severity: loss of driver display and system reboot while driving constitutes a safety-critical defect. This condition could lead to serious injury or death. I request NHTSA investigation into instrument-cluster blackouts and system reboots on Tesla hw3/mcu2 vehicles when fsd and dashcam are enabled.

It was a windy day I was driving to work I was only driving max 14 mph and there were leaves flying due to the wind and without any warning the phantom break came on and my car just randomly stopped. It was in my neighborhood at 1830. It left me traumatized for few minutes because there was no need for the car to randomly stop like it did. The outcome could have been an injury if I was on the freeway or if a car was behind me. Till this day ever time I drive it I wonder if I will have the same incident again.

I am submitting information regarding repair access and repair estimate concerns involving a Tesla vehicle after a collision. During an insurance claim handled by Tesla insurance company, a repair estimate from a Tesla-preferred repair facility was used to declare the vehicle a total loss. However, a manufacturer-owned repair facility estimate would be significantly lower and could make the vehicle repairable. When I attempted to obtain an inspection and estimate from a Tesla manufacturer repair facility, the appointment was cancelled after communication between the insurance company and the repair facility. This prevented me from obtaining a competing repair estimate. Because Tesla operates the vehicle manufacturing, repair network, and insurance services within the same ecosystem, I am submitting this information for review regarding potential impacts on repair access, repairability, and consumer choice.

1. The horn button in not located in the center of the steering wheel but is located as a separate button off to the side and therefore is difficult to located in times of emergency when it is needed. 2. Where the button is located it is not intuitive in times of panic and emergency to try and located where to press the horn button. If you happen to be turning the wheel, it is almost impossible to find the button. 3. On future Tesla x models. Tesla the manufacture decide to relocate the horn back to the center of the steering wheel. 4. We have had several close calls and missed accidents partly due to the fact we could not located the horn button in times of emergencies. 5. It seems like it would be a simple fix for the manufacturer to install an updated steering wheel for the safety of all occupants and other drivers.

Description of problem: instrument cluster complete failure my 2016 Tesla Model X instrument cluster has completely failed and permanently displays "car off" regardless of the actual vehicle status, including while actively driving at highway speeds. Critical information I cannot see while driving: gear position (p, d, r, n) - I have no visual confirmation of what gear the vehicle is in vehicle speed/speedometer warning lights (brake, abs, tire pressure, etc. ) battery status any critical safety alerts this is an extremely dangerous condition that prevents me from knowing essential driving information required to operate the vehicle safely. Tesla has taken the horn away from the traditional center of the steering wheel and put a ridiculously small button on the side of the steering wheel. Most times when you need to use your horn it’s because of a sudden, unanticipated situation. It’s intuitive to hit the center of the wheel. It’s absolutely impossible to find the tiny button in that situation. One has to take their eyes off the road and look for the button. By the time it is located the situation is over. I understand Tesla has corrected this on their current vehicles and gone back to the traditional center of the wheel. But that does not help owners of the affected autos. They should be required to offer a solution, like a stand alone button that can be attached to the center steering wheel. The current configuration is flat out unsafe.

All trailer hitches installed on Tesla Model X and model y vehicles do not meet the standards for towing in the us. These hitches are from bosal, a european manufacturer, and there is no way to safely tow most trailers in the us including home depot, uhaul, airstream, boat trailers, etc. And meet the manufacturer safety specifications. Model x and model y owners on the road today are not following Tesla's manual and are using a tow ball adapter way outside the safety spec for the Tesla hitch. This poses a serious safety concern which could lead to failure of the hitch and the trailer detaching from the car while in motion. This poses as serious safety concern for the drivers, passengers, and everyone around the vehicle in tow. Please investigate this. I quick look at the Tesla manual specifications and the standard 18" height of trailers in the us makes it clear there is a serious issue here. Please investigate ASAP. I have contacted Tesla at least 10 times and nobody there is taking action on this issue.

On [xxx], around [xxx], I was driving my 2018 Tesla Model X on [xxx] , waiting to turn left into [xxx]. While stopped in traffic on the slight incline, my vehicle unexpectedly shifted into automatic park without input. The car rolled back slightly, and the vehicle behind me bumped into my rear bumper. I shifted back into drive and moved forward, but moments later, the same malfunction happened again—the Tesla went into automatic park, rolled back, and was bumped once more. Both incidents frightened my [xxx] daughter, who began crying and was visibly shaken. The likely cause is a failure in Tesla’s “roll-away prevention” safety feature, known to trigger due to sensor errors. This is a serious hazard. Sudden uncommand shifting into park in live traffic creates risk of rear-end collisions, injuries, or worse, especially in school zones or high-traffic intersections. I have since found numerous reports of similar issues from other Tesla owners, including incidents on highways, showing this is not isolated. See example: reddit thread . At this time, the car has not yet been inspected by Tesla, police, or insurance representatives. There were no warning lamps, error codes, or prior symptoms before or during the failures. This malfunction caused two rear-end collisions in minutes, endangered my child, and could easily have escalated into a major accident. Tesla must urgently investigate and correct this automatic park/roll-away prevention defect before it results in severe injury or fatalities. No repairs for my vehicle as I was able to pop back in the light cover. Other driver is making a claim into which I believe is a preexisting issue but cannot prove. There is an error alert on my car that says front left safety restraint issue service is required. So seems to me the airbags or seat belt may not be working properly and its clearly labeled as a safety issue. We have to drive this vehicle nearly daily and Tesla is refusing to fix for a minimum of 10 days even though this fault could threaten our lives. Make them fix ASAP.

I am reporting a safety concern related to the suspension system in my 2017 Tesla Model X. The front tires are rubbing against the wheel arch liner, especially when turning or driving over uneven terrain. I have not made any aftermarket modifications, and the vehicle has been maintained in accordance with Tesla’s recommendations. This issue appears to match the known problem involving front suspension fore link failures documented in NHTSA investigations and prior complaints for 2016–2017 Model X vehicles. While I have not experienced a complete suspension failure, the current symptoms are consistent with reported cases where the fore links weaken or fail, causing the wheel to shift and contact the fender liner, leading to tire damage, unusual noises, and potential safety risks. I have reported this to Tesla, but they have not acknowledged this as a known issue for my VIN. However, NHTSA previously identified over 400 instances of similar failure and recommended that Tesla expand their service bulletin to include more vehicles. This is a critical safety concern as it can lead to premature tire wear, loss of control, and further suspension failure. I am requesting that NHTSA review whether Tesla should issue a recall or cover this repair under a broader service campaign.

I own a 2025 Tesla Model X. A large crack appeared in the windshield spontaneously, without any impact or road debris. It has since worsened audibly while driving. Tesla is classifying this as an accident without identifying an impact point, and they have refused to cover towing or acknowledge a potential manufacturing defect. I was in full self driving mode. The car wanted to turn left but I thought it was going in the wrong direction so I tried to continue straight by turning the wheel to the right forcefully but the car would not let me take over and I crashed into a guard rail. When I tried to brake it was not able to stop soon enough, I have a dash cam footage. I have taken over the this way numerous times and it usually does not require much force to do so, this time it simply would not allow me to control the wheel. No one was hurt other that a guard rail. The car was totaled by insurance company. The police assisted me but I never received a police report. No pictures to share, just a dashcam video but I dont know if that would be helpful since there isn't a way to upload it that I can see. Hi, several alerts have popped up, regenerative braking unavailable, park assist unavailable, automatic emergency braking is unavailable, traction control disabled, stability control disabled, abs (anti-lock braking system) disabled. The odd thing is that the alerts come and go as they please. I can't figure out what it is. Could be software related. Possibly a software update has messed it up?.

While entering an intersection, a vehicle with a stop sign decided to to not stop. I went to use the horn in my 2023 Model X, and slamming the airbag with my hand did nothing. Every car in the world has the horn as the center via the airbag. Tesla must recall all vehicles without a horn in the center. Lack of use of the horn severely impairs my ability to operate my vehicle.

On [xxx], I visited the Tesla brooklyn showroom to purchase a new Tesla Model X. I tendered a valid, notarized promissory note for $128,487. 79 as full payment for the vehicle. Under ucc § 3-310, a promissory note is a negotiable instrument payable on demand, legally considered a cash equivalent. Despite fulfilling my payment obligation, the vehicle was not delivered. On may 10, 2025, I returned to the showroom to deliver an enforcement packet, which included the promissory note, an affidavit of lien, and a ucc-1 financial statement. When I arrived, I attempted to hand these documents to the general manager, dylan karlok, but he instructed a staff member named marcel to throw away the legal documents and refused to accept them. This act was captured on video. Due to Tesla’s refusal to deliver the vehicle despite valid payment, I have been left without reliable transportation. The manager also falsely claimed that I did not have the funds to purchase the vehicle, which has damaged my credibility and caused significant emotional distress. I am requesting the immediate delivery of the originally ordered Tesla Model X, compensation for damages, and a formal acknowledgment from Tesla regarding the mishandling of the payment. Was stopped at a red light behind the car in front of me. I was stopped at a appropriate distance from the car in front of me. I had my right foot on the brake pedal, when the car accelerated on its own. It launched its self into the car ahead of me. I looked down and my foot was still on the brake pedal. None of the air bags inside the car deployed.

Our insured advised he had his foot firmly on the brake, and without warning the vehicle accelerated forward striking the vehicle in front of him. The occupants of the vehicle that was struck by the Tesla sustained injuries.

One fine morning car would not start and shows some errors. Tried using a aaa service and had the car towed into Tesla SC. After diagnosis they mentioned that a hv x099 connector is corroded. To me this cannot be a vehicle owner fault. This seems to be a failure of an integral part associated with the battery pack. Tesla has charged me a total of $2891. 46 including taxes out of which $85 was for the part replacement and $2626. 80 for labor costs. This seems like a major issue with a connector so deeply protected if corrodes this should be a Tesla design/manufacturing fault. ------------- notes from tech: repair notes: towed vehicle into service center and utilized maintainer to power on. Once vehicle was online attempted to pull logs. While logs uploaded verified hvil alerts present on ui. Began investigation checking hvil circuit from x099 to x098. Removed hv battery pack and found corrosion on connector x099. Inspected battery pack and also found corrosion on pack side connector. After removing ancillary tray I found the corrosion had not gone past the low voltage rapid mate connector. Removed and replaced rapid mate and verified proper operation after reassembly. In multiple cases in California. The small button for the digital horn was impossible to reach or activate an emergency situations. This occurred to me and my wife while I’m parking lots and on roads when other vehicles threatened to hit us. The mechanical horn was removed as a cross cutting measurein the Model X and it is a major safety hazard.

On my 2023 Tesla Model X, the horn button is a capacitive touch button among several other buttons and it is difficult to use on a daily basis and impossible to find in an emergency. Yesterday, another car was drifting into my lane on the highway traveling at 70 mph and I could not locate the horn in time to alert them. I had to drive evasively to avoid a serious accident. This has happened at least 7 separate times already. 2024 Tesla Model Xs now have the button in the center of the steering wheel where it is on every other car I have ever driven. Yesterday, I contacted Tesla service about this critical safety issue and demanded the replace my steering wheel with a center horn button because of the document safety issues. They refused. There needs to be a recall forcing Tesla to replace these defective steering wheels with the new and safe design before people get killed.

I want to bring to your attention a safety issue on the 2024 Tesla Model X. The turn signal buttons are a safety hazard. 1. I have found myself pressing the wrong turn signal button and was nearly in an accident more than once. That’s because the turn signals are placed too close together (see attachment). You have to take your eyes off the road and look down to make sure you are pressing the correct turn signal button. It’s too easy to press the wrong button. 2. At night, it’s hard to read the turn signal buttons as they are not illuminated. This is not only a safety hazard but bad design. Surely Tesla could have put the the left turn signal button on the left side of the steering wheel and the right turn signal button on the right side of the steering wheel. It just makes common sense. 3. It’s even more of a hazard when you are on a roundabout and you want to exit the roundabout. The turn signal buttons are at the bottom of the steering wheel making it very difficult to select the correct turn signal button when you want to safely exit the roundabout (see second attachment). The only real safe solution is for Tesla to revert to the previous turn signals on a stalk, as you are probably aware, with a stalk you select the turn signal with one finger while keeping your eyes on the road. Up for the left turn signal and down for the right turn signal. Very simple, effective and safe. Imho this hazardous situation warrants a recall to install the previous stalk turn signal device. There is a flood of messages on social media about this safety problem. Nobody likes the new system. I have read online that Tesla's response is that with self-driving you won't need to worry about turn signals. Not every Tesla owner wants or likes self-driving, myself included. This is just a lame excuse. I hope you will do something about the unsafe situation.

My model year Model X has a poorly designed horn function on the steering wheel. In several instances, I try to push the middle of the wheel out of habit and reaction, but there’s no mechanical push horn! Tesla installed a small tiny non-tactile horn button off to the side. It’s easily missed and you need to find a small pinky sized area to push. This is unsafe. So much so, that the 2024 and newer Tesla have an actual horn function in the middle of the wheel like every other car. Please make Tesla retrofit these horn function to the newer type which is much safer!.
