The vacuum brake assist failed with less than slightly less than 50k miles on the car. Not sure exactly when it failed because the brake and traction control lights were on intermittently for awhile before we took the car in for service. We did have a near miss while making a long drive when we were trying to leave the interstate on an off ramp and I was almost unable to stop at the top of the ramp. I had to angle the vehicle to the left to avoid hitting a pickup truck at the top of the ramp.

Tl- the contact owns 2006 Toyota Sienna. The contact stated that while exiting an dependent mechanic lot, while depressing the accelerator pedal, the steering wheel seized, the brake pedal failed, the vehicle started to spin around at least four times to the left, crashing in twice to a park vehicle while spinning. The contact quickly powered off the engine to stop the vehicle from spinning. The contact waited one hour before the vehicle was restarted and the driver was able to drive the vehicle to the contact residence. The contact stated that she was in shock and was not aware of any warning indicator lights illuminated. The contact sustained foot, right hips and back injuries, the front passenger also sustained back injuries and both required medical attention. A police report was filed. The vehicle was not taken to a dealer and was not repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 140,000. Aw.

On 8/2/18, while driving my 2014 Toyota Sienna van, bought new from a local dealer with appr. 89,000 miles and meticulously maintained for all service, I was driving on a city street when the brakes failed after pumping them three times. When braking at speed exiting highway, brakes began vibrating. The siena has 19,300 miles on it. I took it to the dealer and was told the front rotors were warped. The car has not been subjected to extreme driving conditions and a warped rotors may not have been noticed by others. But the rotor after warping would invariably lead to cracked rotor or brake failure at some point and pose a hazard, possibly catastrophic failure and damage or injury. Luckily, I caught it before something serious happened. I think Toyota should look into premature failures.

At random and intermittently the brake pedal feel changes from firm to mushy with the pedal traveling to or near the floor during the stopping. Also at random and intermittently the car can be stopped with the brake pedal applied lightly to prevent car movement when the pedal suddenly slowly begins dropping and will go down to the floor. I have had no accident from either brake failure condition, but several times I felt ready to go into crash avoidance mode when there was the chance that this might also might be on the verge of loosing braking action. I had this checked by two Toyota shops but they said they found nothing wrong. I also called Toyota for assistance and a case number (1510051721) was assigned. The rep talked with both shops and lastly said he was going to talk to someone else and call back. He never called. When I drove from the 2nd Toyota shop, the brakes failed again and I went right back. The shop rep and I took the car out for a test drive and the brakes were ok. We switched driving and 3 blocks from the shop the brakes failed approaching a stop light. The light changed before we had to stop. At the shop, I stopped the car in drive and with my foot lightly on the pedal as in a normal wait for the stop light to change. After a short time, the brake failed and slowly dropped to the floor. This time the rep (who was still in the passenger seat) had the failure brought to his attention and he actually saw the pedal fall to the floor. He immediately took the car into the shop, but again the tech found no problem or failure. I asked the Toyota rep to have a engineer or brake expert decide what could cause this and precautionary replace the primary choice. This was refused. This van was bought new in January, 2015. The problem was first noticed in March. I now have only 6,400 miles on it.

On mon 10/22/2012 at 11:15 am I experienced sudden uncontrolled acceleration driving my van. I was traveling on I-696e in detroit at a constant 70mph, then slowed to 45mph due to construction. As traffic cleared the construction, I pushed the accelerator pedal to pass a semi-truck. I felt the accelerator continue to depress, I completely pulled my foot off but the pedal did not come back nor did the vehicle slow down. Rather, it did just the opposite and continued to accelerate. I attempted to brake hard with no response--the vehicle did not slow down. Then I put the van in neutral to slow down the engine with no results, the van continued increasing 60+mph with no foot on accelerator. This is when I became extremely frightened. The vehicle was pulling from side to side. I looked down to see if there was any interference from the floor mat and none was seen. I tapped gas pedal - no response. Next I put van back in drive and braked --no response again. Then I downshifted to 4th, the engine did not slow, and further downshifted to 3rd gear. Finally, the engine responded and slowed. I used the gas pedal cautiously as I then shifted to 4th so that I could exit the freeway. I parked at my office & immediately called my husband first and explained what happened and I then immediately called the our Toyota suburban dealer. They told me to bring it in for inspection. I was still shaking when I brought the vehicle in and extremely upset. After two inspections, they stated sudden acceleration was caused by the floor mat. Although this seems reasonable, it does not explain why brakes failed to overcome the engine, and shifting to neutral failed to disengage the transmission. The vehicle scares me to death. The vehicle sustained intermittent brake failures. The brakes would function normally, then completely fail, then begin functioning normally again. This incident resulted in a low speed collision. The initial occurrence was after driving at highway speeds for approximately 20 miles and braking for stopped traffic. After braking to approximately 10 mph, the brakes were released, and then the brake pedal was applied again. There was no braking effort. The driver began pumping the brake pedal, and was able to do so three times before impact. The vehicle was then placed in park and an accident report filed. The vehicle was then moved under its own power to a nearby parking lot, during which time the brakes functioned normally. The vehicle was then taken home. The next morning the vehicle was being backed out of the garage and had another complete brake failure. The vehicle was stopped by placing the transmission in park. At this time, the vehicle is scheduled to be towed to the lariche Toyota dealership in findlay, ohio for diagnosis and repair. The telephone number to their service department is 419-423-5656.
We were backing out of a parking place at the hot springs village walmart. The van accelerated on its own, then the brakes failed. No amount of pressure would stop the van. We stopped by hitting a parked car, doing some damage to it. We had the van towed to a body shop to have the exterior repaired. We were afraid to drive it. I bought a brand new 2011 Toyota Sienna xle in may of 2011 from jay wolfe Toyota in kansas city mo. After 1000 miles within less than a month, started having brake problems. When I press the brake, it was gliding, taking the paddle to the floor in order to stop the vehicle even at slow speeds. I was enroute to omaha for the weekend with my family and kids when I started having the brake problem. I took the vehicle back to the dealership and they took 4 days to check and determine it was air in the brake system. I am sure there was more to it but that is all they said and fixed. One person from corporate was there and told us and guaranteed that we should never see this problem again. Now less than a year after at 26000 miles started having same issues with brake. Now the dealership says there may be a leak in the brake system that may not have been look at before. I advised them due to safety of my family and my kids and others on the road, I am not taking this vehicle back and I requested to be replaced with another new vehicle since they are at fault by selling me a vehicle with a brake problem and by not fixing it right in the first place. Noting that Toyota have had problems with many cars with brake failure in the past. Please advise me what I would need to do in order to get my vehicle replaced for the safety of my family and kids since I no longer feel safe being in the current vehicle. Thank you,.

2005 Toyota Sienna brake failures. This seems to be an intermittent problem - step on brake pedal - car does not stop. Brakes have been changed and reinspected. A search on the web shows that this is a known problem and many complaints have been filed regarding this issue.
On July 29, 2010 at 5pm my wife parked her car in front my house on colesville road, facing the road approximately 23 feet from the sidewalk and 30 feet from the street. I had been asked by my wife to drive to the airport. We closed the doors, put on the seat belts, put my foot on the brakes and shifted the gear shaft to drive. Then I released the parking brake, the car start rolling slowly by the time I was moving my foot to the gasoline pedal gently, the car kept rolling , the gas pedal failed to accelerate the engine. I put my right foot on the brake, the brake failed completely. I tried to pump the break to stop the car without success. Suddenly, I found my self entering the busy street and cars rushing toward me. Trying to turn right and push the gasoline pedalto keep up with traffic, the car kept rolling slowly and the car never accelerated. "the gas pedal failed to accelerate as there was no gas pedal". Until I reached the right lane from the second lane, I shifted the transmission to neutral the car slowed and pushed the parking brake to stop. Then I changing transmission to the parking position. The whole traffic stopped, then I tried again to shift the transmission to drive and pushed the gas pedal which resumed its function and the brake too. When I entered colesville road the traffic was rushing, specially on the first lane where first lane car approaching my car, the driver started honking and swerved drastically around my car to the second and third lane to avoid hitting me. The following cars and the whole traffic stopped until I moved to the first lane and resumed moving. It was a horrible and scary experience with my wife and the two years granddaughter screaming and panicking. I called the dealer, the only advise I got was to get an appointment and look at the car. Tonight I took the car to the dealer.
Brake failure in a 2005 Toyota Sienna. I see other reports of this problem in Siennas based on an on-line google search. The problem, in my case, is intermittent. Brake failure has happened two times when attempting to stop at intersections. Once to my wife and once to me. The pedal does not go all the way down to the floor, but pressing hard on the pedal is ineffective to stop the car in a normal distance. The abs system does not engage despite the amount of strong force on the pedal that I exerted. The car eventually came to a stop, but it feels as though there is no vacuum assist. Pumping the brake did not help. The pavement was not slick. The brakes appear to work normally at other times.
I have taken my 2002 Sienna to the dealership (last winter) because the brakes have failed in very cold weather. The failure has happened three times, most recently this past Sunday morning. The first two times the vehicle had been parked outside in below zero temps for a few hours. I backed out of the driveway (no problem braking in reverse). . . All was well. However as I drove toward the stop sign it was as though the power was all gone from the braking system. I pushed them to the floor and held them there and eventually it stopped before I was too far out into the intersection. It has happened twice, under exactly the same conditions over the last two winters. The dealership could find nothing wrong. This past Sunday morning I had my two youngest children in the car and we were headed to church. As I began to brake for the stop sign ahead, again the same thing happened. I had to hold them to the floor and barely escaped entering the intersection. It frightened all of us. This time the car had been inside the garage. The first two times this happened I did research on the internet to see if anyone else was having this problem and found nothing. After this Sunday morning I looked again and now see that different and newer Toyota models have been found to have problems braking in cold weather. I can't help but wonder if I may be dealing with the same issue. I'm really hoping you can help me with this. I am afraid to drive this van, not just for our own safety but that of others. I don't trust it. I would appreciate a reply as soon as possible.

Brakes fail to work properly. When approaching a stop sign and applying the brakes to slow to a stop, the brakes fail to engage properly so it's like you're floating uncontrollably while approaching the intersection and it feels like you will not be able to stop in time then all of a sudden it will stop, causing everyone and everything within the vehicle to lurch (or fly) forward. In cold weather, when applying the brakes to slow to a stop, the brakes shudder and fail to engage causing one to lose control of the vehicle briefly I. E. Can't brake, can't steer, can't stop even though there's no ice. I have had these problems periodically since purchasing the vehicle in 2005. I thought it was something I was doing wrong until recently when I read about other people experiencing the same problems. I am concerned I will be unable to stop in time at some point causing an accident.
Vehicle was a 2005 Toyota Sienna van. Vehicle has always been serviced by Toyota dealers. On 8/29/09, I had rear brakes replaced by local Toyota dealer with 77,410 miles on the vehicle. 10 days later, on 9/8/09, my roommate was picking up children at school. Driving on a 4 lane road in town going 35-40mph. Two h. S. Students abruptly stopped in her lane. My roommate stated she applied brakes but could tell vehicle was not stopping or slowing. Ran into rear end of a Toyota tacoma p/u which in turn rear-ended a Honda crv. No injuries in any involved vehicle. However, due to the extent of damage, my Sienna van was a total loss. Airbags did deploy on impact. My roommate stated from the beginning that she had applied the brake to the floor and the van did not brake or slow down. There were no braking/skid marks on the pavement at area of accident. Either the brake system totally failed or there was faulty work done during brake replacement 10 days earlier.
1999 Toyota Sienna 56000 miles ,, we were driving at 60 mph on I 294 south and the brakes completely failed ,,,my wife kept pumping them and lost control , we hit the median and the car rolled over thrice and ended up stopping on the roof ,, me, wife and two kids were safe by the will of god ,, all inseat belts ,,had gotten oil change ,routine maintenance 13 days ago ,, still have the receipt from jiffy lube that says brake fluid okay ,,,

The contact owns a 2005 Toyota Sienna. While driving between 5-6 mph, another vehicle ran a stop sign and struck the left rear driver's side of the contact's vehicle. Her vehicle fish-tailed and turned to the left. The vehicle accelerated and struck another vehicle. The contact had no control over the vehicle. She attempted to place the vehicle into park, but was unsuccessful. The engine revved and the vehicle accelerated again. The vehicle struck a curb, a boulder, and then a palm tree before coming to a complete stop. The brakes failed to operate during the incident. A police report was filed and there were no injuries. The vehicle was towed to a GMC dealer and they test drove the vehicle, but were unable to duplicate the failure. The vehicle is still at the GMC dealer and will be appraised. When backing 2004 Toyota Sienna le van in driveway the brakes failed. The vehicle has 41,000 miles and has always been serviced by Toyota. The brakes were checked at 34,388. The car has a higher idle (fuel consumption) so in the process of backing, the car was rolling. I put the car in reverse and did not touch the acelerator. The vehicle rolled and continued to pick up speed, since I was unable to stop the car. The brakes were applied around 3 times and then I opted to hit the retaining wall instead of going back into the a busy street with wife and children in the car. It was either pray for no coming traffic or run into the neighbors house and vehicles. We have not experienced any problems in the past. The dealership was unable to identify any root cause. We are concerned about the safety of the vehicle and wanted a complaint as a matter of record. In researching the incident in google this is not an isolated incident and there have been a few records of such cases with the Sienna van around this make and model.

I consumer owns a Sienna ce 2005. Bought it at Toyota of orange on March 4,2005. Van had a oil change service on August 13,05. Although my van has low mileage, about 1, 355 miles after oil change. Went shopping at ralph supermarket in front of toyotya dealer. When were about to go home suddenly brakes did not work. Luckily van was still in parking lot of ralph supermarket. Imagine that if this happened on the street consumer and children could get hurt or killed. Then consumer carefully back to dealer and left van for service. It's about 7:30 pm - 08/13/05. Currently, van was still in service dept for waitting for part. Already reported brake problem to the Toyota manufacturer. Wondered why system didn't show the brake failure? w van had brake problem whlie it was 5 months old and still had low mileage? after fixing the brake problem is it safe to drive that van? wished did not have that serious problem again. Vehicle experienced brake failure when depressed at 15 mph. Consumer's vehicle stopped when it rear ended another vehicle. Dealership performed a diagnostic test in which no failure was detected.
Total brake failure in light, powdery snow conditions. Skidded through a four-way stop when the brakes would not engage. Longer braking distances noted at times.
