The contact stated that while driving at 55 mph, while the brake pedal was depressed, the brakes failed to engage; the contact pulled the vehicle into a home depot parking lot, exited the vehicle, and discovered that the engine compartment was on fire. The home depot employees came out to the vehicle and extinguished the fire with a fire extinguisher. The fire department and police department were contacted; however, the contact was unsure if a report was filed for either department. The orgin of the fire was not confirmed however, the fire department informed that timing was critical because any longer the fire would have caused an explosion. No warning lights were illuminated. The vehicle was towed back to the residence, and the vehicle was not diagnosed. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 360,000. .

My 2025 F-150 tremor (5. 0 v8, 402a package with modular front bumper) has a large open area in the front bumper. Through this opening you can clearly see the front of the engine and related components. This appears to be a design issue rather than a one-off build problem. Other 2025 tremor owners with the same 402a modular bumper are reporting the same concern and asking whether a cover is missing from the factory. My concern is that the exposed opening could allow rocks, road debris, or off-road obstacles to directly impact vital components at highway speed or when driving on rough/uneven surfaces. This may increase the risk of damage, fluid leaks, or sudden loss of power/steering if components are struck. It also appears inconsistent with typical under-engine protection on off-road-oriented trims. I am asking NHTSA to review whether this bumper configuration provides adequate protection for critical components on the 2025 F-150 tremor and whether a design change or shield/plate is needed for safety.

A few weeks ago, while driving the vehicle, it displayed an engine misfire warning had extreme vibrations, and then the vehicle broke down, left me sitting on the side of the road. After getting the vehicle towed to a Ford dealership, we found the intake valve broke off on the motor and causes catastrophic complete engine failure. This is an active recall in 21 and 2022 f1 50s with the 2. 7 eco-boost. Per Ford my VIN is not attached to that recall and they said I had to call or get a hold of the national highway safety administration and get my VIN attached that recall for engine replacement. My vehicle is a 2022 with 100,000 miles on it and now it’s broke down because the engine had catastrophic engine failure due to a problem. Ford knows about this issue and won’t fix it until I get the vehicles then attached to the recall . The current recall number is 24s55 thank you.

Cam phaser rattle, well known issue in 2017-2020 F-150s. Started around November 8, 2025, at cold start. Maybe 78,000 miles. Spoke to repair shop who id'd the issue and suggested a NHTSA case.
I was driving along and the truck shifted out of gear. Throttled to high rpm’s and went back into gear causing me to temporarily loose control and it repeated itself 2 more times before I got it stopped and towed home.

The long block has failed this has caused the vehicle not to allow for accelleration and have significant power loss. Yes this has been confirmed by the dealer. It has not been inspected by the manufacturer at this time. Two days before I took my trunk into the dealer for service, the service engine light came on. I parked my truck and did not drive it til I called the dealership. They told me to drive the truck slowly to their service department. I drove my truck to them after confirming it was ok. They are 6 miles from my home.

-auto stop/start system failed and is available for inspection. I was driving down the road and braked for a vehicle about to pull in front of me, when I let off the brake and pushed the gas I noticed the vehicle had shut off there was an error message on the information center, error message: "auto startstop shift to p, then restart engine" -I could've been in an accident due to immobilized vehicle -vehicle taken to dealer. Was told the failure was unlikely to be duplicated. Was told to unplug(deactivate) auto stop/start system to hopefully avoid recurrence. -no -no messages or symptoms prior to failure.
The vehicle fails to accelerate when coming of a turn, the accelerator pedal does not respond for several seconds, then the vehicle lunges forward. Ford was contacted and no fix for this problem. I think this is very dangerous.
I started smelling gas in the cabin of the truck when I came to a stop. As I was driving on the freeway I noticed my gas gauge going down rapidly. Within minutes I ran out of gas. Got the truck towed to Ford dealer. After diagnosing the issue I was made aware by Ford service department that one of the high pressure fuel injector cracked and was spraying gas out which caused me to run out of gas. With gas spraying in the engine I was at severe risk of a fire. No warning lights or messages appeared when this happened. After investigating I noticed Ford has a recall for this issue for other vehicles but not the F-150.

While driving lost transmission gear selection on the cluster and would no longer go into 1st gear. Wrench light flashed and showed see owners manual. When decelerating for a stop the truck does not downshift into 1st gear, when your expecting a downshift it makes it very hard to stop at intended spot. Dealership mentioned this being a known issue (lead frame replacement needed) but has only been recalled for 2014 and below year models.

Transmission shifted into 1st gear while doing 65 mph on the freeway. The rear tires locked up as a result and the truck spun out in front of on coming traffic putting myself and my 6 month old nephew in a life threatening situation. This is not okay. I was told the most recent recall done on this vehicle was done to not allow this to happen. This issue has been an on going problem with these trucks and Ford has still yet to rectify the problem. This can cause serious bodily harm to myself and others around these vehicles. I feel Ford needs to do something about this issue.

Driving along I-94, 2012 f150 downshifted immediately from 5 gear to 1 gear. There was no previous issues and no warning lights and codes. Caused significant drive issues and had to pull immediately to side of busy highway. Caused immediately backup and believed to have caused back up accident. After taking to dealership was told it was my molded lead frame. Which had been under an extended warranty but since my vehicle is older than 10 years and over 150,000 it is not under warranty. Ford knows about the issued due to the extended warranty issued. However, that warranty still is not effective in taking a major defect off the roads.

The transmission failed prematurely without warning. We were without power to move onto the shoulder of a 2 lane road.
I took my truck in to be repaired for an incompetent tow truck driver wrapping a chain around the pinion yoke to put my truck on a flatbed because the alternator failed. Once the pinion seal repair was completed, the repair shop told me there were codes popping up on how it was running. The codes were related to the timing and were likely the cam phasers needing to be replaced with the timing. It turns out that the engine was needing to be replaced because there were grooves in the cam journals from metal shavings that were caused after the cam phaser lock out pin broke. The engine was on backorder from Ford and after a little over 3 months, I got a new engine for a mere $12,314. This is for a used truck that I purchased and have only owned for just going on 2 years. I called Ford corporate office to see if I could get help paying for the repairs but they told me that I had to take it directly to a Ford dealership. The Ford dealerships nearby are 3 or more months out, just to take a look at your vehicle and diagnose it. I couldn't wait that long, so I went to a local mechanic shop that I trust. It was hard enough having to wait 3 months for my engine to be replaced, let alone waiting 3 months for Ford to diagnose it and then however long for them to actually replace it.

There is a recall for the low pressure fuel pump that has been active since July of 2025 and there is still not resolution. This truck is not usable due to the issue.
Complaint summary (what happened): the cam phasers (variable camshaft timing system) failed on my 2017 Ford F-150 4x4 with the 3. 5l ecoboost engine. This is a widely known issue on these engines. The failure causes a loud rattling/knocking sound during startup and can lead to engine timing problems or internal damage. My truck only has 108,000 miles, has been well maintained, and is just outside of the extended warranty coverage Ford issued for this exact issue. Safety risk: the cam phaser failure causes erratic engine timing, which can lead to power loss or stalling during operation. This creates a safety hazard while merging, towing, or driving at highway speeds. The loud knocking sound and hesitation also distract the driver and reduce confidence in the vehicle’s reliability and acceleration when needed for safety. Problem confirmation: yes, the problem was confirmed by a Ford dealership. They diagnosed cam phaser failure and quoted approximately $6,300 for the repair. Inspection: the vehicle has been inspected by a Ford dealership service department. Ford motor company was contacted but declined to assist financially since the issue occurred just after the extended warranty expired. Warning signs: no warning lights or messages appeared before the failure. The first symptom was a loud rattling/knocking noise on cold start, which worsened over time. The issue started around 85,000 miles. The check engine light just came on at 108,000 miles. Availability for inspection: the failed components remain installed on the vehicle and are available for inspection upon request. However, I am looking to get this fixed as soon as possible.
Car randomly downshifting at freeway speeds. Mechanic is not sure what is wrong or how to fix. When google, it seems to be a common issue with this year and I clearly very unsafe with people in the car on the freeway.

The contact owns a 2022 Ford F-150. The contact stated that while driving at 55 mph, the vehicle lost power and stalled on several occasions, but the vehicle was restarted. The check engine warning light was illuminated. The contact received notification of NHTSA campaign number: 25v455000 (fuel system, gasoline). The vehicle was taken to the local dealer, who confirmed the parts were on back order. The dealer informed the contact about a diagnostic test. The vehicle was not repaired. The manufacturer was not contacted. The failure mileage was approximately 60,000.
Driving on interstate highway. Entered a tunnel at 65 mph. Near the end of the tunnel traffic started to slow down. I applied the brakes and the engine started to accelerate. I had full pressure on the brake pedal but the vehicle kept accelerating. There was a loud fan noise. Fortunately the vehicles ahead started to accelerate up to the speed limit - and beyond. I took my foot off the brake and turned on cruise control. The fan sound stopped and the truck maintained cruise control setting. I had heard the fan speed up twice before but I was parking each time1 and turned off the engine within seconds of hearing the fan motor speed up. I returned to my home and heard the fan motor speed up once more. I took the vehicle in to my local service station. They were able to recreate the problem. "bad thermostat causing engine cooling fan to come, increasing idle speed. Replaced thermostat".
When I was driving on the highway I was going about 65 mph in 6th gear and then without warning my 2012 Ford f150 3. 5 truck shifted all the way down to 1st gear which almost caused me to wreck on the highway and other cars to almost slam into me. Now when I try to drive the truck it keeps throwing a wrench and the wrench blinks and my truck will not shift hardly at all between the gears. After I have researched this issue I see that there has been 4 recalls and a few csp notices for the make and modle of my truck. The recall numbers are #19v075000, 12v190000, 16v248000, 12v198000 and the csp campaign number is #16n02 regarding the lead frame issue that causes the extreme downshift. I contact the Ford dealership to try and get this fixed and they said there is nothing they can do to help me.
Wrench icon came on, truck wouldn’t shift out of first gear, speedometer and mileage stopped working. Lead frame recall according to my mechanic, $2,000 fix out of pocket. Truck only has 140,000 miles but is a 2012 so Ford says I’m outside of the free recall replacement. Had no problems at all until over 135,000 miles, ridiculous that this is paid out of pocket when issues don’t arise until after the approved date.
V6 eco boost 10 speed transmission is failing with 135,000 miles. Need to be replaced.
Transmission has lots of issue, clunky shifting, surges, weird noises. Happens every time you drive. Dealer said it was normal but it's not. I've driven plenty of these 10r80s and they didn't have this issue. Engine makes noises that are not on other examples of this engine, my dad has the same truck and had similar issues and had to spend thousands of dollars to fix an engineering defect in the cam phasers. Dealer also said these noises were normal while under warranty - I see countless posts on forums saying the same thing then for others, after warranty suddenly its a problem because a retail job pays 30-40% more than a warranty job. Such a scam and you just keep allowing it. There is simply no reason that the 10r80 transmission and 3. 5 ecoboost gen 2 engines haven't been recalled yet. Ford has issued updated parts, admitted to engineering issues that caused these issues so why are customers forced to pay to fix engineered issues? they admited fault, they need to make it right for their customers instead of screwing us over.
There was an unknown problem with a fuel injector that resulted in the a vapor lock of the engine. This incident happened in traffic while idling at a stop light. There was zero stress on the engine at the time of the incident. This was a busy intersection, and I could have been pushed into oncoming traffic. There were no warning lights(check engine or otherwise) indicating and engine related failure or that an issue could be present in the fuel or engine systems. Upon trying to restart after what appeared to be a simple stall out, all indicator lights were on and every warning possible was present my local service center reviewed the truck and did confirm the resulting issues. They have also advised they would have expected such issues to trigger a warning light. I believe this was a catastrophic failure on Fords part and as a result has landed me as the consumer with concerns for the safety and a large bill for the work needed just to get it back on the road.
I had just replaced the transmission (r1080; 6 cyl 3. 5l turbo) on September 3, 2025 due to p0734-00 (gear 4 incorrect ratio and p07f7-00 (gear 10 incorrect ratio). The transmission matter can be referenced on NHTSA id # xxx. On September 30, 2025, while driving down the parking deck ramp (at 3-5mph) my check engine light came on with code p0022 (camshaft timing); resulting in replacement of the camshafts. The camshaft had a hole in it (photo supplied). The camshaft issue is another issue many Ford f150 truck owners are experiencing. Again, Ford has continued to give "band-aid" fixes with TSB's and no real fix to a continuing issue with these trucks. On December 3, 2022, this used truck was purchased from a Ford dealership in raleigh, nc (77,100 miles) and was Ford gold/blue certified per my purchase information. The gold/blue paperwork even included that the "customer satisfaction repair" (not a true recall mandated by NHTSA) had been performed by a TSB (along with the unintentional tailgate opening also a customer satisfaction repair). There are no recalls issued on this vehicle. Ford has offered no assistance towards repairs. As evidenced by the number of complaints filed on the NHTSA website for this particular engine, at what point will the NHTSA open an investigation and mandate that Ford issue a recall for the known problems with the r1080 3. 5l and the camshaft/phasers. I am submitting this complaint regarding a serious transmission failure on my 2020 Ford F-150. At approximately 68,000 miles, the vehicle began exhibiting dangerous and unpredictable driving behavior, including hard jerking, sudden hesitation, and unexpected deceleration and acceleration while driving. The vehicle would jerk aggressively during gear changes and decelerate without warning, particularly at lower speeds and during normal traffic flow. This behavior significantly compromised my ability to maintain consistent speed, safely merge, or respond to traffic conditions. On multiple occasions, the sudden loss of power and hesitation created a safety hazard, especially when driving in slower or congested traffic, as surrounding vehicles were not anticipating abrupt deceleration. Due to the severity and persistence of these issues, the transmission ultimately required complete replacement. A brand new transmission was installed to correct the problem at only 5 years old and barely over the Ford mileage warranty expiration criteria. This failure occurred well before what I would consider normal or expected transmission life for a modern full-size pickup truck and presented a clear safety risk to myself, my passengers, and other motorists. Sudden jerking and loss of power at speed creates a high likelihood of rear-end collisions or loss of vehicle control. I believe this issue reflects a potential defect in the transmission system and should be investigated to prevent similar safety risks for other drivers.
My truck is making a ticking and whistling noise once it is cranked and running. Ford issued a recall on f150’s for a manifold exhaust leak with the same noises I am hearing. The recall # is 19s32. My truck has the same problem and issue as described in the recall. My year and model truck needs to be covered under the recall.
Vehicle lost all power and stalled while driving. Vehicle also stalled at idle when waiting at an intersection, and another time while waiting at a stop sign. No longer starts. Appears to not be receiving fuel and is returning code p0627 "fuel pump a control circuit / open" and will crank but not start. Required towing back to my residence. Fordpass sent a "service engine soon warning" stating "the powertrain control system has detected an electrical fault with the fuel pump and its associated wiring". The symptoms match those in 25v455 but this VIN number is not listed in the affected group.
Front engine cover leaking. I have had this issue twice with the same truck two separate engines. I bought my truck used and it was leaking oil. I took it to the dealership and they diagnosed it as the front engine cover leaking. Then before the dealership could get it in to be repaired it started having problems with the timing components. All the problems were supposedly fixed but then about 5 months later the engine failed due to a camshaft failure and I was told this was due to lack of oil. The truck was not even due for an oil change after the dealership had done the repairs. I saved up and bought a brand new engine purchased April 25, 2025. I had the Ford dealership do the first oil change on the new engine and then it started leaking. I took it back and it was determined that it was leaking from the front engine cover, the same as the old engine which I pointed out to the dealership and they informed me that Ford is aware of the problem but there are not any recalls for this. The new engine is under warranty as it isn't even 6 months old and doesn't even have 5000 miles on it.
