"the vehicle suffered a critical failure of the shift interlock safety system. After parking in a driveway, the ignition key was able to be fully turned to the 'off' position and removed from the ignition cylinder, even though the transmission was not successfully secured in 'park. ' after the key was removed and I and went into the house the van rolled away. It traveled downhill, crashed through a fence, and collided with a residential structure, causing significant property damage to the house and vehicle. This failure could potentially cause serious injury or death and this was the first time that it had occurred and is exhibiting the same failure as described. Immediately following the collision, video evidence was recorded showing the key in hand while the dashboard indicators and shifter remained out of the 'park' position. This indicates the safety mechanism designed to prevent key removal unless the vehicle is in 'park' failed to operate, creating a life-safety rollaway hazard. ".

[xxx] [xxx] / [xxx] [xxx] 2010 Toyota Sienna possible shifter interlock failure: on [xxx] my [xxx] child found his way into the driver’s seat of our 2010 Toyota Sienna while the vehicle was parked in the driveway. According to my wife, there were no keys in the vehicle, and the vehicle was in park. In an instant, my wife (who was outside the van) helplessly observed the van begin to roll backward down the driveway and then across the sidewalk (on our side of the street), then across the street, and then across the sidewalk on the other side of the street and into the neighbor’s yard where it finally came to a rest. She said she heard something like grinding gears. I presume my son was trying to put it in park. Thankfully no property damage or personal injuries occurred during the incident. I [xxx] ) was not home when this happened. When I returned home, I inspected the van to see if my son was able activate the emergency shift lock release. I found the cover was intact and evidence of tampering was not observed. I then checked to see if I was able to pull the shifter out of park. Initially, I was unable to, but after a few tries and a little force, I was able to pull the shifter out of park (no keys in ignition, no brake compression, vehicle off) and the van began to roll. Our driveway is somewhat sloped. I replicated this procedure several times: a little force and a little wiggle and clunk, the shifter released and the van rolled. I discovered there was a recall for this very problem on 2010 Sienna’s (NHTSA recall no. : 17v-657). I entered the Sienna’s VIN number on NHTSA’s website and found no active recall for the VIN number entered. I contacted Toyota of north America (1-800-331-4331) and chose to advocate and spoke to an agent and reported the problem. She was able to determine that the vehicle had been under the recall, but the van was “repaired” in 2020 which is the year we bought the van information redacted pursuant to the freedom of information act (foia), 5 u. Started hearing a whining noise upon acceleration. Took car to Toyota for a diagnostic check. Discovered transmission is failing. Recommended repair is a full transmission replacement, quoted $10,000. After bringing to a local mechanic for a second opinion, he claims this is a widespread issue with Toyota Siennas in similar years. After some research, Toyota quietly claims there was an issue in the manufacturing of these transmissions however won’t take accountability.

I am filing a safety complaint regarding a dealership-provided loaner vehicle that had an open safety recall at the time it was issued to me. On Tuesday 1/20, I received a loaner vehicle from beaver Toyota in saint augustine, florida while my personal vehicle is being held due to a recall. The loaner was a 2025 Toyota Sienna. I was not informed of any safety recalls at the time the vehicle was provided. Late that evening, I discovered that the loaner vehicle had an active, unresolved safety recall listed as open through NHTSA. The recall was active while the vehicle was in my possession. After discovering the recall, I attempted to contact dealership management multiple times regarding this urgent safety issue. On 1/21, I contacted the general manager through the dealership’s website. I also called the dealership on Wednesday at 1:37 pm and left a voicemail for the general manager. I did not receive a response. On 1/22 at 12:24 pm, I called again and left another voicemail. I then followed up with an email requesting a return call due to the safety concern. As of this evening 1/22, I have received no response. Providing a loaner vehicle with an open safety recall, without disclosure, placed me and other occupants at unnecessary risk. I am requesting that NHTSA review this matter for recall compliance and appropriate corrective action.

I am writing regarding my 2025 Toyota Sienna, purchased in June 2025, which is currently subject to safety recall interim notice 25tb12 concerning unsafe second-row seat welds that could result in serious injury or death. I appreciate the professionalism and concern shown by sparks Toyota in acknowledging the seriousness of this recall and providing a loaner vehicle while my Sienna is deemed unsafe to drive. However, the current loaner—a Toyota highlander—does not meet my family’s needs and is not a reasonable substitute for the vehicle I purchased. We intentionally purchased a minivan due to our family size and lifestyle requirements. My daughter is a competitive gymnast, and we frequently travel with athletic equipment and multiple passengers. The highlander is significantly smaller than a Sienna and cannot safely or practically accommodate our needs. Additionally, I am concerned that there is currently no repair solution or estimated completion timeline for this safety issue, leaving us indefinitely without access to a comparable vehicle. At this point, I am respectfully requesting the following: 1. Placement in a replacement vehicle comparable in size and function to a minivan (such as a Toyota Sienna or similarly sized vehicle). • if sparks Toyota does not have an appropriate vehicle available, I request authorization for a comparable vehicle through an outside rental agency at Toyota’s expense. 2. Compensation for the ongoing inconvenience caused by the safety defect, lack of a repair timeline, and the inability to use the vehicle I purchased. 3. A clear estimate or update regarding when my vehicle is expected to be repaired and returned in a condition that is safe for my family. This recall involves a serious safety risk, and I believe it is reasonable to expect transportation that meets the same capacity and purpose as the vehicle I purchased—especially while awaiting a manufacturer-level remedy.

While driving, there was a recurrent sound like doors locking and unlocking, several dozen times over a half hour drive at 50 miles per hour. When stopped, the passenger side sliding door opened by itself. Luckily, my dogs were in a kennel or they might have jumped out of the car. I closed it and it opened by itself again. At home, the passenger side sliding door opens when my neighbor presses the button on his key fob to open his non-Toyota pickup truck. It will not open or close when I press the button on my Sienna key fob.

I am submitting this complaint regarding an unresolved safety recall on my leased Toyota vehicle and the manufacturer’s failure to provide a timely or reasonable remedy, resulting in ongoing child passenger safety risks. Toyota’s recall instructs that no one sit in or use a child restraint in the second-row seats while the vehicle is moving until the remedy is performed. This effectively prohibits use of the safest and recommended seating position for children. The vehicle is used daily for highway driving to transport my family, including three children aged [xxx] , [xxx] , and [xxx] old. Because of Toyota’s seating restriction, my [xxx] toddler must now sit in the third row. This prevents the driver from easily monitoring or comforting the child and has caused driver distraction during highway travel when the child becomes distressed, creating a foreseeable safety risk. Toyota states it is “preparing the remedy” but has provided no repair timeline. After multiple contacts and escalation, I was told it could take up to 60 days with no interim solution. Toyota suggested contacting dealerships for a loaner but advised that an equivalent vehicle was unlikely, availability was uncertain, costs might need to be paid upfront with later reimbursement, and the duration would be open-ended. This is not a reasonable interim safety solution while continuing to pay full lease obligations. I was also informed that returning the vehicle would require paying approximately $15,000 in negative equity, which I believe is unreasonable given that the safety defect and usage restrictions are due to a manufacturer recall. Toyota has not provided a repair timeline, a reasonable interim transportation solution, or a penalty-free way to exit the lease. They could not find the source of the leak. There are other 2025 Sienna owners talking about their sunroofs also leaking. Thank you.

See recall above. We just spent $55,000 on this vehicle in order to safely transport and travel with our two year old and newborn. The fact that Toyota does not have a remedy available for this major issue is completely unacceptable. The recall notice states that our only current "remedy" is to not use the second row seats, which is not feasible since child safety seats only safely install in the second row. This so called "remedy" is absolutely unreasonable, illogical and irrational. It essentially renders our $55,000 vehicle unusable, which constitutes a significant financial loss for my family. I am making a loss of use claim - Toyota's response that it does not have a remedy available now for such a significant safety problem is outrageous.

While driving our vehicle on the highway, the check engine light suddenly came on and traction control turned off. Within moments, the vehicle began to stall and would not accelerate. My wife and children were in the car and we were nearly hit by another vehicle as she struggled to get the car to safety. It was a terrifying experience that put my entire family in danger, along with everyone else on the highway. Upon taking the car to the dealership, we were told the transmission failed and would need to be replaced.

The lift gate strut bent when I tried to close the rear door, then when I tried to open the door to see what the issue was, the strut exploded. The parts went flying and almost hit me in the face, I could feel the air on my cheek as it blew past me. The spring and metal parts inside the strut shot all the way across my cul DE sac, and the noise it created sounded like a gunshot. My neighbors came out of their houses to see what happened because it was so loud. We have an appointment at the dealer tomorrow to get it looked at. If I was standing any closer the spring would have hit me in the face; my children luckily were in the car already, if it happened a minute earlier it could have hit them. I noticed after the fact that the gaps in the back door are uneven when it’s closed, and I wonder if the door is installed unevenly and that put pressure on one side more than the other causing the strut to break. I can’t take a picture of the broken strut because I am too afraid to open the rear door in case the other one explodes.

On June 5 th the vehicle was going approximately 50 miles per hour in the middle lane. I placed on the brakes suddenly and car lost power and stalled out. Unable to restart vehicle for 5-10 minutes on the freeway. Significant safety concern due to vehicles sudden stall out in mid the highway traffic and unable to restart car. Almost hit from behind as well as others that followed from behind. On Jun 7th vehicle was taken to the Toyota dealership and car checked by special mechanic and reported that they were unable to reproduce the stalling or issues with restarting the vehicle. Dealership feels there is nothing wrong with vehicle. There was 2 warning messages on vehicle when stalled suddenly 1-charge 2- fluid.

I bought this vehicle brand new. I quickly realized that both rollers on the sliding doors were causing excessive noise, which I found out that many other owners have complained as well, when closing & opening. This is because of a poor assembly from Toyota as the rollers are not machined prefect painted & are either oversprayed or less sprayed causing gaps on the rollers. These gaps or uneven paints causes excessive noise when rollers are moved on their track. There are several new Sienna owners who are waiting to have this issue fixed as the parts are backordered. I had to make total of 4 trips to my local dealership to have the issue fixed. I can still hear a little bit of over excessive noise from one of the sliding doors. Toyota should immediately recall all the affected vehicle and have this issue fixed.

I bought this van brand new. I quickly realized that the reverse camera view has some issues. I believe images are not clean and sharp, as it is supposed to, and has unreasonable amount of distortions. Cleaning the camera from outside didn't do anything. I also noticed that the bottom part of center blue alignment line is little bit to the right and not align with the top. I checked online and it appears that other owners are also suffering from this same issue. Toyota should recall all affected vehicle and fixed this issue as soon as possible as this is a major safety issue when backing up!.

These dim controls pause a hazard as it takes your attention away from driving. The dealer told me that they could not make them brighter. The issue is at night and day times.

The sunroof spontaneously exploded while in a closed position, not in use in any way, while driving in the rain on the highway. While driving at highway speed, a sudden “shotgun blast sound” occurred. The interior cover was closed. Wind noise became immediately apparent. Upon stopping to inspect, the exterior sunroof glass had shattered with the appearance it blew out from the inside, as the middle section was protruding upward. The sunroof has been used very minimally since purchasing the vehicle right at a year ago. There have been no issues with the sunroof. The vehicle is going to the dealership body shop in the next few days.
