After which, upon attempting to apply the breaks and with the brake pedal depressed all the way to the floor board, the vehicle failed to stop, prompting the contact to switch off the ignition. As a result of the unintended acceleration and brake failures, the contact collided with the rear quarter panel of another passenger vehicle. The air bags did not deploy. No warning lights were illuminated. A police report was filed, and no injuries were reported. The vehicle was towed to the dealer, who was unable to diagnose the failure, informing the contact that the event data recorder showed the accelerator pedal had been depressed. 12 Accelerator Pedal problem of the 2005 Toyota Tacoma

Failure Date: 11/01/2011

2005 Toyota Tacoma. Consumer states unintended acceleration the consumer stated approximately 2 1/2 years ago, he experienced his first sudden acceleration problem. The consumer stated as he had his right foot on the brake, the vehicle felt as though it wanted to continue to move forward. When the consumer arrived home, he checked the floor mat, but it was found to be properly secured. The consumer received a safety recall notice regarding sudden acceleration issues. When he contacted the dealer, he was informed the parts were not available and he would receive a call when they arrived. The second occurrence happened while driving 10 mph. The consumer had his foot on the brake pedal, when the engine suddenly begin to revved up with the rear wheels spinning and hopping while slowly moving forward. The consumer turned the ignition in the over drive position and there was some difficulty trying to place the shifter in the park position. The consumer finally took the vehicle to the dealer to have the recall performed. However, two months later,while slowing down, with his foot on the brake, the engine began to rev again. The consumer experienced unwanted acceleration several more times.

Consumer writes in regards to recall notice related to floor mats the consumer stated he received a recall notice that stated an incompatible driver's side floor mat would be replaced if necessary and that the carpets would be cleaned. The consumer stated he signed a work order authorizing the repair. The next day, the consumer complained to the dealership, that no carpet modifications had taken place and the carpet was dirty. The accelerator pedal was too high to the point that he was having decreased control over the accelerator mechanism.

Letter from senator snowe on behalf of constituent re problems with his 2006 Toyota Tacoma the consumer stated on three separate occasions, the accelerator became lodged under the floor mat. The first incident caused severe damaged to the vehicle. The second incident happened to the consumers wife. She was able to get the vehicle to a safe stop without incident. The third time the incident happened the consumer attempted to apply the brake, but the vehicle accelerated out of control and crashed into a stone wall and the vehicle flipped over and landed on its side. The consumer stated Toyota continued to tell him nothing was wrong with the vehicle. Months later the consumer received a recall notice regarding the problem, only for the claim to be denied, because he couldn't remember what side the mats were on.

The contact owns a 2009 Toyota Tacoma. The contact stated that she received a recall notice in reference to NHTSA campaign id number(09v388000-vehicle speed control:accelerator pedal). After getting the recall repairs done she was unsatisfied because the floor mats do not fit comfortable and her foot slips off of the accelerator pedal. She also states that she would like for them to reverse the remedy due to the problem that she experienced at the time. The manufacturer was contacted and they informed her that the government will not allow them to reverse the remedy. The failure mileage was approximately 14,500.

2005 Toyota Tacoma. Consumer is expressing discontent with the service received for the floor mat interference recall. The consumer stated according to the invoice, the accelerator pedal was modified. The consumer stated when he questioned the service tech about replacing the all weather floor mats, she was unaware of that portion of the recall. The consumer showed her a copy of the recall notice. The service tech then asked someone else and the response was; no all weather floor mats have been received for the recall. The consumer asked if he could be placed on a list, he was informed he would have to call in about two months to see if any were available. On two separate occasions, the consumer experienced sudden acceleration.

The contact owns a 2008 Toyota Tacoma. The vehicle was taken to the dealer for recall 09v388000 vehicle speed control: accelerator pedal on September 3, 2010. After the vehicle was repaired the gasoline pedal became stuck twice while driving between 90 and 50 mph. He was able to shift into neutral gear and move onto the emergency lane. The vehicle was parked at the contacts home. The dealer will send a factory representative to the investigate the failure. The failure mileage was 20,973 and the current mileage was 21,173.

The contact owns a 2007 Toyota Tacoma. The contact received a recall notification associated with the NHTSA campaign id number 09v388000 (vehicle speed control:accelerator pedal). The vehicle was taken to an authorized dealer for repairs. After the remedy repair, the contact observed that the accelerator pedal was difficult to engage properly due to the size. The contact measured the length of the accelerator pedal and detected that the pedal was reshaped three inches. The manufacturer was notified and they informed that the accelerator pedal modification was four inches. The contact had no resolution with the manufacturer in explaining the insufficient remedy repair performed by the dealer. The failure mileage was approximately 58,000.

While driving approximately 2 mph into a parking space, the contact engaged the brake pedal and unexpectedly the vehicle accelerated, and crashed into a building. The front seat passenger sustained neck injury. There was no police report filed of the incident. The vehicle body damage repair cost was $5,500. In addition, the metal siding of the building was damaged. The contact received a recall notification after the acceleration failure. I stepped on the gas pedal to pass another car and the pedal got stuck, I tried to pull the pedal with my foot but I couldn't, the rpm went all the way up. It occurs every time I step on the gas pedal to pass or gain speed. I went to Toyota shop and they ignored me stating they never heard that complaint before and that my warranty was over, it happened three times already and now I'm afraid to drive.

The contact owns a 2006 Toyota Tacoma pre-runner. The contact stated the accelerator pedal failure happened on two occasions within one month since he has owned the vehicle. The first incident the contact stated happened on November 8, 2008. The contact was driving approximately 35 mph and came to a rolling stop. The contact stated that when his foot was on the brake the vehicle accelerated then the contact pressed more firmly, but the back tires started to spin in acceleration. The contact was able to put the vehicle in neutral then shutting the vehicle off. The second incident occurred on December 7, 2008. The contact stated while pulling into his garage, he placed his foot on the brakes, and the back tires of the vehicle spun into acceleration onto his concrete driveway. The contact stated that smoke came from the tires. The vehicle was taken to the dealer for diagnostic and testing. The dealer could not duplicate the problem. The dealer stated the contact must of had his foot on the brake and the accelerator pedal at the same time. The contact stated that the floor mat was not against the accelerator, but secured on the floor tabs. The contact stated that the vehicle has not accelerated since 2008. The contact owns a 2008 Toyota Tacoma. When the contact attempted to shift the vehicle in park, it surged forward. On another occasion while approaching a traffic light with her foot on the brake pedal the vehicle surged forward and she continued to press the brake pedal with force until the vehicle came to a complete stop. She took the vehicle to the dealer and they tested the vehicle. Report 1001136136 was filed with the manufacturer who stated that they could not offer her any assistance. 25 Accelerator Pedal problem of the 2008 Toyota Tacoma

Failure Date: 07/28/2008

The contact owns a 2008 Toyota Tacoma. While driving at speeds of approximately 50 mph, the vehicle abnormally accelerated to speeds of over 100 mph. She attempted to depress the brakes, to no avail. She attempted to move the gear shift selector to the park position, reverse and neutral while violently depressing the brakes. The vehicle declined in speed shortly thereafter. The dealer could not duplicate the failure. The failure occurred a second time while driving on the highway at speeds of less than 55 mph, the vehicle accelerated to abnormally fast speeds. The dealer advised the vehicle was under recall pertaining to the floor mats, but not the recall for the sticking accelerator pedal (see NHTSA campaign id number: 09v388000, vehicle speed control:accelerator pedal) which the contact believed to be the cause of the failure. He stated that there were no floor mats in the vehicle. There were no repairs made to the vehicle to date. The failure mileage was 10,227 and the current was 23,363.

26 Accelerator Pedal problem of the 2005 Toyota Tacoma

Failure Date: 06/11/2008

The contact owns a 2005 Toyota Tacoma. While driving 35 mph, the contact attempted to accelerate, but the vehicle would not stop even though the accelerator pedal was not being depressed. The pedal was not stuck in the accelerating position. The contact attempted to use the regular brakes and emergency brake to stop the vehicle, but it continued to accelerate. He struck another vehicle, which caused the contact's vehicle to slow down into the median. The contact was able to jump out of the vehicle, which was still accelerating. When she jumped out, she injured her left knee, and right arm and hand. The driver in the other vehicle was not injured. The contact did not notice any difference in the vehicle prior to the crash. A police report was filed. The vehicle was towed to her insurance company. The vehicle has not been diagnosed by the dealer. The current and failure mileages were 23,800.

The contact owns a 2007 Toyota Tacoma. While preparing to park the vehicle, it suddenly accelerated from 3 mph to 15 mph. The vehicle crashed into the rear of a semi-truck. The contact depressed the brake pedal with both feet, but the rear wheels continued to spin. The vehicle is currently at the dealer. The manufacturer is also performing their investigation on the vehicle. No one was injured in the crash. The current and failure mileages were 22,000.

Brakes and unattended acceleration failure due to design problems with brake and gas pedal. Brake pedal 1 inch closer and one lower to the floor board gas pedal. Shoe is resting slightly on gas pedal as you are braking. The average size shoe will hit the gas pedal as you are braking.

- the contact was having problems with the 2007 Toyota Tacoma, the throttle sticks while driving and trying to stop. The truck will not stop, it accelerated at all times unexpectedly. He took the vehicle to the dealer and they told him that the computer was learning how to drive, that the failure was normal, and that all Toyota vehicles went through that process. It was taken to the dealer for repairs at least three times, and they could not find the cause of the problem. Just last week he was in the parking lot driving at 5 mph and the vehicle accelerated unexpectedly. The current and failure mileage were 2300 miles. After shifting from second gear to third gear, the gas pedal stopped working completely. Seconds later it functioned normally, and as if nothing had happened. This has happened before, and I have identified the problem as a failure of the throttle body motor/sensor. I will be asking the Toyota dealership to fix the part.
