Tl the contact owns a 2007 Chevrolet Trailblazer. While driving 55 mph, the low beam headlights failed to operate and the check engine indicator illuminated. The contact had to activate the high beams and fog lights to see the road. The contact called sauers Buick GMC (1900 e lincolnway, la porte, in 46350, (877) 468-9351), but the vehicle was not diagnosed or repaired. The manufacturer was contacted and did not assist. The contact stated that the failure was similar to NHTSA campaign number: 15v519000 (exterior lighting). The failure mileage was 198,000.

Daylight/headlights sometimes do not turn on upon starting truck and also have shut off seven different times while driving at night forcing me to drive with fog lights and high beams on endangering not only the oncoming traffic, but my family which includes a 5 and 9 year old child. As stated in the recall and which is the exact action occurring not every time but (7 times)1 time alone is enough to be a major safety concern to me and my family, the headlamp driver module (hdm) may overheat and fail, causing the headlamps and daytime running lights to not illuminate. This is the recall on this issue currently which 2006-2007 chevy Trailblazers, yet hendricks chevy says that my vehicle is not included in this recall. This is obviously a huge safety issue especially when my family's safety and lives are seriously in danger every time we travel or drive at night due to the reoccurring issue of the lights not being reliable at all times of turning on due to an overheating issue, that once again is occurring in these models yet my VIN is not a part of the recall. The most recent occurrence of this issue was on July 12, 2019! please address this issue to have other VIN numbers apart of this recall with whom ever necessary so my family's safety as well as other driver's safety are not at risk every time we get into our 2006 chevy Trailblazer! again the main issue at hand is headlamps that do not illuminate reduce the driver's ability to see the roadway and reduce the vehicle's visibility to oncoming vehicles, both of which increase the risk of a vehicle crash. When leaving work at night on 8/21/18, started the car and the low beams came on, and then immediately went off. They would not come back on. Had to drive home using the fog lights and high beams. Have only had this car for a week. Doesn't seem safe. See that lots of Trailblazers have this issue. Looks like recall begins with the 2006 for this same issue.

Driving conditions dark, 11pm, unlit, stars visible paved country road with hills, curves and 2-5 foot ditches. Weather wet but drying. Visibility good. Vehicle in motion near legal speed limit 80 km/hr. Occasional oncoming traffic, so occasionally switching between low and high beams. Noticed low beam headlights flickered occasionally. Effect was only visible after oncoming traffic had passed, before switching back to high beam headlights. I had attributed the effect to my eyes adjusting after the oncoming headlights. On one occasion, after the oncoming vehicle had passed, I switched back to low beam setting, but the low beam headlights were not illuminating the road at all. All I could see was a small amount of yellowish light off to the sides of the vehicle from the other running lights, including yellow lights on the side-view mirrors. There was insufficient light to see any distance down the road, so I ceased to depress the accelerator pedal, and covered the brake applying gentle braking. I still had my fingers on the lever and was able to switch back to high beams after several seconds. Fortunately the high beams still worked. I brought the vehicle to a stop at the next available safe opportunity, to assess what had happened. All other lights, including the fog lights were working. I drove at reduced speed alternately using the fog lights and high beams. The low beams occasionally started working again. By the time I was home, they had ceased to work. I replaced the gm hdm module, and the low beams worked for a year or so until that module failed. There is a recall to replace gm hdm 15016745 with gm hdm 13506836 but the recall apparently does not apply to my 2004 model year. Nhtsa recall no. : 15v-519 manufacturer recall no. : 14291 I consider this to have been a severe life threatening event.

With no warning, both headlights went out and no longer work. Turn signals, fog lights still work.
Oct 15, 2015 headlights turned off while driving at night. Short trip on local streets finished without lights. Dealer, 10-16-15, replaced hcm fuze #53. Nov 3, 2015 headlights again turned off while driving at night. Trip finished with fog lights. Owner replaced hcm fuze #53. Nov 12, 2015 headlights again failed while driving at night. Trip finished with fog lights. Owner felt hcm, it was very hot. Lights turned on again in morning after hcm cooled. Fuze not replaced. Vehicle mileaage is approx 70,000. This vehicle VIN not included in gm recall #14291. This is definitely a serious safety issue!.
While driving on the inter state at 8;30 pm both the low beam headlights failed I moved the light switch from auto to manual with no affect. The fog lights and high beam lights continued to work. However using the high on the interstate could affect on coming drivers so where the highway was lighted I used the fog light. Got home off interstate using high beams. Next morning the daytime lights and low beams working however shortly after driving to store the lights again were not operative stopped at pep boys and asked them about it and they said they could make sure bulbs were okay but normally both bulbs don't fail at the same . Later in the afternoon tried lights and both daytime and low beams worked. A friend told me gm was having trouble with 2006 and 2007 trail blazers with the light just shutting off and a module was believed to be problem. Apparently gm has known about this problem and was suppose to recall the vehicles yet they did not warn anyone one about this issues however people with earlier trail blazers have been having problems also. Even if gm is trying to decide on a recall fix they and the government should mandate that they at least warn the vehicle owners and tell him how to fix the problem. I have have not received any notices about the low beam lights as a point of reference my lights failed on an over pass down ramp and caught me totally by surprise thank god it was not a two lane secondary road with on coming traffic. Its time to force gm to deal with the problem. I am taking car to the local dealer to see how they deal with it. I have also seen on the internet you can get a module at local auto parts store I am looking into this tomorrow since I live in florida and soon it will be dark at 5 o"clock and I will be constantly driving home in dark on the interstate.
I was driving my truck heading home and noticed the road looked dark. I pulled over into a parking lot and realized I had no low beams. My blinkers, fog lights and high beams all worked properly. I had to drive home with my high beams on, blinding other drivers. My headlights and daytime running lights have intermittently went out off and on (approximately 4 times) over the past year. I had the dealership look into it the last time I was there and they couldn't find a problem. When researching what could cause both lights to fail at the same time, I came across the latest recall info. I called chevy and gave them my VIN and was told my vehicle is not affected by this recall. Strange, since I have all the exact same problems listed in the recall and my vehicle has been affected by all of the other recalls on it so far.
I was driving home from a wedding on the interstate at 55 mph, and my headlamps failed. Everything went completely dark. After a panic, I was able to turn on my high beams, and they worked properly. I had to drive the rest of the way home with my high beams on for safety, blinding oncoming drivers. When I got home, I tested my fog lights, and they worked properly. I turned the car off, waited a few minutes, and turned it back on. The low beam headlamps worked then. Since the first incident, I have had approximately 10-15 other incidents of the same occurrence. I was positive that my vehicle was included in the recent recall, but when I called my local dealer, I was advised that my VIN number was not included. I don't understand, since the issue on the recall is exactly the issue that I am dealing with. I have had to limit my use of the vehicle at night for fear that the headlamps will fail. Side note - they have failed at night twice in the last week alone. Generally what I have to do is pull over on the side of the road (scary) and turn the car off, wait, and try them again.
The contact owns a 2004 Chevrolet Trailblazer. The contact stated that the vehicle would not start and the only lights that would function were the fog lights and high beams. Starting June 17, 2008, the low beams on my 2004 Chevrolet trail blazer lt go out intermittently - the high beams, fog lights, hazard lights, and turn signal lights still work - only low beams causing problems. I keep my lights setting in the postion that they come on when I start my car. I consider this a safety factor. The lights will go out suddenly. I can only document one time that they went out during the day time, but have documented seven (7) times that the lights went off at night. The next space on this form asks for the approximate incident date - I put down the first time. As stated above this has happened seven times at night and at least one time during the day. I can only tell during the day time if I am stopped at a traffic signal behind another vehicle and I only have 2 traffic signals between my home and work. I took my car to sandy sansing Chevrolet, pensacola, florida, on June 23, 2008, they said they checked my car for three hours but could not find problem. I would need to bring it in when the lights go out. Unfortunately, the only time I could tell they had gone out during the day time, I was stopped at a traffic signal and before the light turned green, the lights came back on. (my 16 year old grandson was in the car with me and can verify that this happened. In fact he and another grandson was in the car on the night of July 1, 2008, when the low beams went out while I was driving down the road on the way home from the baseball game.
