1 Automatic Transmission Control Module problem of the 2017 Ford F-150

Failure Date: 05/08/2026

Took my 2017 Ford F-150 in for a diagnostic on may 8 2026 for wrench symbol check engine light and issues down shifting and sliding tires shop manager and technician claimed the vehicle is unsafe to drive and talked to the service manager deemed the vehicle unsafe to drive wanted it grounded do to the safety concerns related to recall 26s28 (pcm/trs) stated the down shifting is cause catastrophic failure to all mechanical components transmission engine and rear end kidding the tires especially in wet weather the truck would cause a accident. . Cause the sliding of the tires. . So my truck is at apple Ford of lynchburg grounded as of may 9 2026 I still owe on the truck and have no ride dealer nor manufacture was willing to pay for a rental that cost $65 a day wanted me to pay then reimburse me once a remedy was available I cant afford that. . 1600 a month we are roughly 2 months away from hopefully a remedy being available stated by recall website. . So if my truck is showing all the signs of the described in the recall(26s28) has stated in it why is Ford not putting a do not drive on the recall for all these trucks if the technician seems them unsafe. . I shouldn't have to pay for a rental I cant afford cause Ford is lacking a remedy and wont even buy it back cause of safety issues codes po732 inccorect 2nd gear ratio.

3 Automatic Transmission Control Module problem of the 2013 Ford F-150

Failure Date: 04/09/2026

Bought that as used at low milage with good service history indicated with pcm twice made me believe to trust as reliability and regular check ups. Carfax is not really honest, as well as I learned that powertrain itself has nothing to mechanism but actually programing meaning actual pcm itself has troubleshoots and not at its stability. It caused no speed on dash board sometimes even though got served but aftermath it gets more worse. I experienced collapsed in middle of high speed traffic to full stop. But the odd that mpg got great better than only during trouble times like first time ever seen over 15 mpg that reached between 16. 4 to 18. 7 for over a week. Now backward to 14 mpg. On the trouble occurances were r080 downshift to first gear notch caused by faulty outpost shift speed loss sensor. Also code of 19s 07 needs reprogram the powertrain control module has been common and I don't believe wiring is the issue thus the sensor is but reprogram itself is factor although it caused various issue like noise installing from park notch to drive notch, cannot increase or stay constant stead on speed, sometimes caused vibration that had no equality the pcm does not make correct specific of the fault. I checked with auto zone to do diagnosis code on mine was much unspecfic and inaccurate fact in which was not fact thus the manufacturer fails to program the correct troubleshoots. Pcm is very bad. I had to stay under 65 mph either stay at 60 mph sometimes and if I increase the truck has been feeling alike unstead and maybe could get collapsed for a weeks far.

4 Automatic Transmission Control Module problem of the 2011 Ford F-150

Failure Date: 04/08/2026

I am writing to formally report a dangerous safety failure on my Ford F-150 that persists after having the recommended recall service (19s07) performed. While traveling at highway speeds, my vehicle experienced an uncommanded downshift from 6th gear to 1st gear. This caused an immediate and violent deceleration, nearly resulting in a high-speed rear-end collision. This exact failure is the core safety issue addressed by Ford's powertrain control module (pcm) recall, yet the "fix" provided has failed to protect the vehicle from this life-threatening defect. I took the truck to bolivar Ford in tennessee for resolution. However, the dealership refused to perform any further repairs under the recall. They stated that because of my current mileage, the recall coverage has "expired. " furthermore, they diagnosed the issue as a faulty lead frame sensor—the very component linked to these downshift events—and quoted me $2,500 just to begin the repair process. A safety recall is intended to permanently rectify a defect that poses a risk to life and limb. The fact that the vehicle is still performing this dangerous maneuver after the recall was "accomplished" suggests the initial remedy was insufficient or the underlying hardware remains defective. I am requesting that Ford motor company intervene to ensure this vehicle is repaired at no cost to me. Given the severity of the safety risk, I do not believe mileage should disqualify a customer from having a documented, lethal defect corrected—especially when the first repair attempt failed. I look forward to your immediate response regarding how Ford intends to resolve this safety matter.

5 Automatic Transmission Control Module problem of the 2016 Ford F-150

Failure Date: 03/26/2026

My vehicle keeps downshifting randomly at highway speeds and will also not shift out of 1st or 2nd. Rear tires keep locking up. 1. Pcm 2. I have a hard time keeping vehicle in control 3. Yes, active recall can't repair until July 4. Yes, it has been inspected 5. Yes, check engine and wrench light.

6 Automatic Transmission Control Module problem of the 2018 Ford F-150

Failure Date: 03/23/2026

While driving in the fast lane on the interstate at highway speeds, my 2018 Ford F-150 with a 10-speed (10r80) transmission experienced a sudden hard jerk followed by erratic shifting. The vehicle began shifting unpredictably, including harsh downshifting behavior that caused rapid deceleration. At the same time, the transmission temperature began increasing rapidly, and the truck started overheating. We were forced to quickly move out of the fast lane to avoid traffic and pull over due to safety concerns. This occurred at night on a sketchy exit, approximately four hours from home. We had to wait for help and ended up calling 911 because the situation felt unsafe. We were stranded and had to stay in a hotel. There was no clear warning before this happened. The condition worsened quickly to the point the vehicle could not be safely driven. This created a serious safety risk due to sudden hard jerking, rapid deceleration, overheating, and the need to immediately exit the fast lane, increasing the risk of collision and loss of control at highway speeds. The vehicle previously received a pcm reprogramming update related to transmission performance based on manufacturer service guidance. Despite this, the failure still occurred suddenly and without warning. There have been reports of updated internal components in newer versions of this transmission, suggesting that improvements have been made over time. This issue appears consistent with other reports of sudden downshifting and unsafe shifting behavior in Ford F-150 transmissions.

7 Automatic Transmission Control Module problem of the 2018 Ford F-150

Failure Date: 03/13/2026

Manufacturer recall #24e12 (pcm software update) triggered a po430:00-ed code (catalyst system efficiency below threshold - bank 2) because the new software more strictly monitors the catalytic converter, causing it to detect previously ignored degradation. This reprogram scamming program makes community bothered by paying parts replacements more than 2k, even vehicle was fine before the recall. This needs resolved with free fixed and free catalytic replacement. This is not right to announce a recall with unfinished repair issues left. This needs action. Thanks, so mote it be!.

8 Automatic Transmission Control Module problem of the 2024 Ford F-150

Failure Date: 03/09/2026

2024 Ford F-150 platinum powerboost hybrid. Diagnostic codes for power steering loss and can bus communication faults documented at 337 miles, well within the 3-year/36,000-mile new vehicle warranty. The vehicle has been at a Ford dealer since March 10, 2026 for symptoms including the truck randomly turning off while driving and loss of power steering communication. As of this report, the truck has been at the dealer 58 days without resolution. Documented diagnostic findings include: lost communication with pscm (u0131), scp/j1850 communications bus fault (u1028), permanent code p1a42 propulsion system status signal performance, hybrid-specific high voltage system interlock u3523-00, steering position sensor p060a, no communication with power steering control module, intermittent pcm communication during self-tests, bcmc fuse 69 with no power, and circuit cbb69 with no voltage. A Ford field service engineer was dispatched April 23, 2026. On April 24, 2026 the dealer located damaged wiring inside a harness behind the bcmc bracket. Prior to that, the dealer escalated through r&I of multiple modules (pscm, pcm, bcmc, gateway module access) over 45 days without locating the actual cause. The vehicle is currently subject to NHTSA recall 26v104 / Ford 26c10 (trailer module loss of communication at startup) issued 3/18/2026, status recall incomplete. The recall describes the same general failure mode (module communication loss at startup) the customer has been experiencing. Power steering loss while driving is a serious safety concern, particularly given the vehicle's hybrid drivetrain and can-bus-dependent safety systems.

9 Automatic Transmission Control Module problem of the 2013 Ford F-150

Failure Date: 03/02/2026

The contact owns a 2013 Ford f150. The contact stated that while driving at 30 mph, the speedometer went to zero unexpectedly. In addition, the transmission failed to properly shift gears. The vehicle was pulled over to the side of the road, turned off, and minutes later, it was restarted. After the vehicle was restarted, the wrench warning light illuminated. The contact moved the shift lever to reverse(r), then to drive(d). The contact test drove the vehicle around the parking lot at the apartment complex, and the vehicle functioned as intended. The contact's son-in-law took the vehicle to the dealer to be diagnosed and the contact was informed that dtc: p0722 (powertrain control module (pcm) was receiving "no signal" from the transmission output speed sensor (oss), p1500 (intermittent vehicle speed sensor (vss) signal detected by the powertrain control module (pcm), and p2195 (powertrain control module (pcm) detected that the bank 1 sensor 1 oxygen (O2) sensor signal was stuck lean). The vehicle was not repaired. The contact stated that on a separate occasion, while the vehicle was turned and when the contact placed the shift lever into reverse(r) the wrench warning light illuminated. The contact parked the vehicle and had not been driven since due to safety concerns. The manufacturer was not notified of the failure. The failure mileage was approximately 166,398.

10 Automatic Transmission Control Module problem of the 2022 Ford F-150

Failure Date: 02/25/2026

10 speed [t]ransmission issue first noticed w/ milage under 10k. [t] would “slam” while downshifting from 5/4/3/2 when coming to stop. Gradually progressing in severity w/ increase of milage. [t] has now started to miss gears when upshifting from 3/4/5/6, causing a brief lag in vehicle movement upon acceleration as the transmission attempts to keep up with applied engine power. Dealer has always stated this as “normal. ” dealer also states they “can’t replicate” the issue, even though it has now become a near-daily occurrence. The vehicle also continues to perform a hard downshift from gears already stated. Vehicle is very well maintained and driven appropriately. The original purchasing dealer did perform a pcm update as well as replace the original pcm module around 40k/mi, but issue still is not resolved and progressively worsening. [t] flush & filter replacement was performed recently with scheduled maintenance and nothing was noted regarding abnormalities. Extended bumper-to-bumper warranty is nearing expiration at 100k/mi. Safety-wise, while entering upon a highway, loss of speed while attempting to enter has caused near misses in the past, even with enough space given to enter with moving traffic behind. Vehicle slamming down while coming to a stop causes faster than anticipated stopping or speed reduction. 11 Automatic Transmission Control Module problem of the 2022 Ford F-150

Failure Date: 02/04/2026

When driving the gas pedal will go dead or not work at all. I will be driving along then try to push the gas at a left hand turn and nothing will happen. I push the pedal to the floor many times and after a period of time it starts to work again. The issue never puts out a code or check engine light. This is a safety issue for not only me and my family but others on the road as well. If the gas pedal was to fully stop working the truck would be stuck and could cause an accident. The dealer told me they could not reproduce the concern, but they did suggest to replace a temp sensor and reprogram pcm which I paid for, the cost was $870. 90.

12 Automatic Transmission Control Module problem of the 2020 Ford F-150

Failure Date: 02/01/2026

I am writing to provide supplemental information regarding my existing safety complaints (ids xxxand xxx). This update concerns a documented failure by Ford’s authorized dealer network to perform mandated customer satisfaction programs (csps) that directly correlate to the catastrophic powertrain failures I have experienced. Timeline and documentation of failure: • August & September 2025: as previously reported, my vehicle suffered sudden "reduced power" malfunctions while in motion (p0734/p07f7), leading to a full transmission replacement. Discovery of non-disclosure: my records now confirm that on November 11, 2022—just weeks before I purchased this vehicle as a Ford blue advantage gold certified unit—crossroads Ford (dunn, nc) documented that 21n03 (cam phasers), 21n08 (pcm reprogram), and 21m06 (tailgate) were outstanding. • breach of duty: despite this knowledge, the dealership sold me the vehicle on December 3, 2022, without completing these safety-related service actions. Furthermore, as the registered owner, I never received the required notification from Ford motor company regarding the 21b10 program or its supplements. Safety implication: the failure to apply the 21b10/21n08 updates directly contributed to the mechanical failure of the cam phasers and transmission. This resulted in the vehicle entering "limp mode" during highway operation, creating a severe risk of rear-end collisions due to the sudden loss of motive power. I request that these facts be added to my existing case files. The lack of manufacturer oversight regarding "certified" vehicle standards has resulted in a vehicle that was represented as safe and fully maintained while actually harboring known, unaddressed mechanical defects. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

13 Automatic Transmission Control Module problem of the 2011 Ford F-150

Failure Date: 01/15/2026

Approximately one month ago, I noticed that the vehicle would not accelerate the way it normally does. A error code came on. I took the vehicle to auto zone for error code check. I was informed that the powertrain control module speed sensor had gone bad. I looked this up and found that Ford had a recall on 2011 Ford F-150s for similar issues. I disconnected the battery, as recommended on an internet site, and the vehicle began to operate normally. About one week later, I noticed that the vehicle would not shift above third gear and was accelerating very sluggishly. On further investigation I fount that the vehicle would not shift out of third gear (up or down). I again disconnected/reconnected the battery, and again it began operating normally. I contacted the local Ford dealership and they said that I should have received a letter in may 2019 about the pcm recall. I didn't recall receiving the letter, but Ford motor CO. Customer service says the pcm was actually reprogrammed on March 29, 2019 to repair the issues related to the recall. My concern is that the pcm speed sensor is malfunctioning again even after the repairs were supposedly performed by Ford. I am concerned that the vehicle may again be potentially unsafe to operate at highway speeds. I wanted to make NHTSA aware of the potential for ongoing safety issues with this vehicle even after Ford's proposed fix.

14 Automatic Transmission Control Module problem of the 2018 Ford F-150

Failure Date: 01/08/2026

In January 2025, covert Ford [xxx] ) performed a federally mandated pcm recall repair via their mobile service unit. Immediately upon completion, my check engine light (cel) cam e on. Prior to this service, the vehicle had zero warning lights, fault codes, or symptoms of any kind. The cel appeared directly and exclusively following the recall work. The timeline is unambiguous. In March 2025, I returned to covert Ford to have the recall-related fault addressed. Rather than reviewing their own recall work, the dealership charged me a diagnostic fee upfront, followed by additional labor charges, resulting in a bill exceeding $1,100. No diagnostic data, scan results, or technical documentation was ever provided to support their findings. The fault was never resolved. An improperly performed recall can suppress additional fault codes within the vehicle's obd system, leaving the driver unaware of developing powertrain or safety-critical issues. I operated the vehicle with an active, unresolved cel for approximately three months — a condition caused entirely by the recall service itself. A federally mandated recall is a manufacturer's commitment to repair a defect at no cost to the consumer. When that repair introduces a new fault and the consumer is billed over $1,100 to diagnose it, the recall process has fundamentally failed. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).

15 Automatic Transmission Control Module problem of the 2013 Ford F-150

Failure Date: 01/05/2026

While driving my 2013 f150 eco boost is started shifting erratically. While at 60 mph the truck would down shift on it's own to 1st gear and my truck would skid. Luckily I did not crash but it's dangerous and it's my only transportation. I took it to Ford and they said I needed a $8k transmission replacement. I ended up taking to a transmission shop for a rebuild but shortly after if did it again. The transmission shop replaced the valve but said Ford needed to reflash the tcm. When I asked for them to follow recall 19s07 they said I would need to bring it in and pay them another $400 for a diagnostic and they couldn't just reflash it. Now I'm in a position where I have to drive what feels like a timebomb and I have no way out of it.

16 Automatic Transmission Control Module problem of the 2017 Ford F-150

Failure Date: 01/04/2026

2017 f150 with only 48,330 original miles. Having issues with transmission stalling, jerking in between gears. After last night I have had enough due to an almost potentially fatal accident. While making a left hand turn the transmission stalled for way too long and an 18 wheeler came over the hill ( it’s a blind spot turn) and came very close to hitting me broad side of my truck. This issue could have costed me my life. The transmission fluid is fine as it was checked very recently at my last service less than 100 miles ago. The dealership wants 200 dollars just to hook it up to a computer just to see if the tcm is showing any codes. I have already had that checked for free somewhere else and it is not. The dealership will not do anything unless I pay for that to be done first. I cannot afford to give away 200 dollars for free. But if I give them 200 dollars then they can give me an estimate for repair that ranges from 1000 to 10,000 dollars which is what other customers have paid or been taken advantage of for that amount.

17 Automatic Transmission Control Module problem of the 2021 Ford F-150

Failure Date: 11/22/2025

My vehicle has been with the Ford dealership for about a month and the mechanic and the engineers still have not found why my vehicle is giving a service wrench warning. When driving the vehicle will flash red warning lights which makes the vehicle unsafe to drive. I have paid for an auxiliary transmission pump and battery. This did not fix my truck. The dealership said they replaced the pcm, but it didn’t fix the truck. I seen that these trucks had recalls.

18 Automatic Transmission Control Module problem of the 2011 Ford F-150

Failure Date: 11/05/2025

The contact owns a 2011 Ford F-150. The contact stated that after the dealer had reprogrammed the pcm under NHTSA campaign number: 19v075000 (power train), the vehicle started to experience failures. The contact stated that while driving at various speeds, the vehicle downshifted to first gear unintendedly. The powertrain warning light was illuminated. The contact was able to pull over to the side of the road. The contact resumed driving; however, the failure persisted. The vehicle was towed to the residence. Additionally, the contact stated that the brake lights, the hazard lights, the turn signal lights, and the infotainment system and the a/c system became inoperable after the recall repair. The dealer was notified of the failure and informed the contact that the failure might be related to the electrical system. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 200,000.

19 Automatic Transmission Control Module problem of the 2013 Ford F-150

Failure Date: 11/03/2025

Took vehicle to dealership to have recall 19v075 completed. Dealership informed me that Ford motor company will only replace the multi- frame part in the transmission that contains the oss sensor if the vehicle is not older than 10yrs and 150,000 miles. The service advisor at the dealership informed me that if they perform the pcm program update without the mechanical repair being done the intermittent downshift problem would get worse. The truck has downshifted while going down the highway at approximately 60mph and the abrupt downshift nearly caused me to loose control of the vehicle.

20 Automatic Transmission Control Module problem of the 2012 Ford F-150

Failure Date: 11/02/2025

While driving 70 mph on the highway my truck suddenly downshifted into first gear causing the truck to go into a skid. The skid was recovered so nobody was injured but if it were a wet day or pulling a trailer, I doubt it could have been saved. The speedometer quit working and a wrench showed on the dash. Upon restarting the truck both of those issues were resolved but the truck continued to downshift into first gear at highway speeds. I called my local Ford dealership, and they said there were no active recalls but that the pcm had already been reprogrammed. If the pcm had been reprogrammed, then this issue should not have happened unless there needs to be another update performed. They alerted me that there was a customer satisfaction program for the molded leadframe in the transmission that would cause this issue, but this is not a satisfaction issue it is a safety issue. If they replace the leadframe but there is never any change made to the way the transmission functions then when it eventually goes bad again the issue could come back, once again posing a major safety risk.

21 Automatic Transmission Control Module problem of the 2014 Ford F-150

Failure Date: 10/28/2025

Took in my vehicle to my local Ford dealership after I experineced sudden downshift of the transmission and getting stuck in 1st gear. I asked for a reantal vehicle or a loaner to which they said, it would depend whether or not there would be any pcm related codes that came back after connecting the car. According to them no codes came back, and explained to me that, that being the case that they would just need to reprogram my pcm. The next day I went and picked up my truck, and after just about roughly 7 miles into driving, I started experiencing sudden downshift again and my truck started slowing down rapidly almost causing an accident and getting rear ended. I was able to pull over and noticed this time my truck had gotten stuck in 4th gear. I was able to drive it back to the dealership and left it parked in the their parking lot as it was after hours at this point. The very next morning by 7:30am I was at their service desk explaining what had happened the night before after picking up my truck from them. They refused to give me any further service as they said that they had already completed the service related to the 24s37 recall, and that if I continued to experience issues, that it was a completly unrelated issue and if I wanted to get it diagnose and repaired that it would be out of pocket. After doing more reserach online, I came to find out that on some of this recall issues related to 24s37, in some cases the dealership would have to replace the entire lead frame which holds the transmission output shaft speed sensor which goes bad and causes the transmission to go crazy and thus the downshifting, and in other cases they don't have to replace this part. In my case, while there were no other codes that showed, they determined that only a reprogramming was necessary. However, given that I continue to experience the same sudden downshift issues, I suspect that it is a bad output shaft speed sensor which should be replaced under the current safety recall.

22 Automatic Transmission Control Module problem of the 2018 Ford F-150

Failure Date: 10/23/2025

Ford f150 2. 7l ecoboost was running fine. Went to dealer for pcm recall and 3 days later check engine light came on. Went back to the dealer and they say I have a po430 code now and that I need a catalytic converter at the tune of $3000. Of course I am 8000 miles over the factory warranty of 80000 miles and the extended warranty I bought does not cover it. Seems not right. Is the any help with this issue? shouldn't Ford be responsible?.

23 Automatic Transmission Control Module problem of the 2020 Ford F-150

Failure Date: 10/10/2025

At around 61,000 miles my transmission started having a shuttering noise and had a harsh jerky shift from 3rd to 5th when vehicle was recently started up for the first time of the day. Multiple TSB's on Ford's website for this issue including the latest TSB 25-2301. It appears that the interior drum has failed which has been a known issue. Dealer states $6219. 87 to repair the transmission. Cdf TSB, replace the valve body, and reprogram the pcm.

24 Automatic Transmission Control Module problem of the 2020 Ford F-150

Failure Date: 09/14/2025

I was driving eastbound on city streets at 30 mph with my three young children when the vehicle exhibited abnormal shifting behavior, searching for a gear. Despite being in 9th gear, traveling the speed limit, and lightly pressing the pedal, the vehicle experienced a rough downshift and lost power while going up a small incline over a freeway overpass. I turned on my hazards and attempted to move over to avoid an accident, which I successfully did. The vehicle regained power after restarting with rough shifts in the lower gears. I put the vehicle in manual mode to better control the shifting, though it remained incredibly rough. The kids were shaken and refuse to drive in the vehicle. Since then, I’ve only driven it to have it serviced at our local Ford dealer. They stated everything was fine after the transmission fluid exchange and battery replacement. However, the vehicle still exhibited rough shifting behavior on the short drive home. A friend pulled codes from the tcm/ecu and found p0734-00, p07f7-00, and p2535-00. He assisted, backed up the ecu/tcm, re-flashed the onboard ecu/tcm with the latest mappings, and reset the adaptive learning. He then drove the vehicle with diagnostics equipment connected and recorded the driving data, finding that the transmission was losing pressure and the shift behavior was slightly better but still rough shifts between 2-4 and 7-8 gear. Once the drive/test was completed, codes p0761 and p2702 appeared on the reader. He explained that the issues pointed to a cdf drum defect, which has several TSB’s published. He advised me to stop driving it and reach out to the dealer or local qualified shop. I was quoted between $7,677-$8,978 to replace trans with reman unit. Vehicle has been maintained with all records on file and is in nearly perfect condition with exception of the transmission due to the cdf defect. I would love to see action taken to prevent injury/death as I could see that occurring at some point.

25 Automatic Transmission Control Module problem of the 2013 Ford F-150

Failure Date: 09/01/2025

Safety recall 19s07 - (19s19) Ford motor covered 19s07 but not 19s19. Software update does not prevent downshifting into 1st on highway. The issue is that 19s19 is truly the root cause of issue, and software was a low cost ineffective countermeasure to safety issue. Covered certain 2011–2013 Ford F-150s with 6r80 transmissions for sudden transmission downshift due to output shaft speed (oss) sensor faults. Included pcm programming and diagnostic checks. Nhtsa this vehicle still exhibits down shifting at high speed (very scary), speed sensor malfunction, air bag disengagement while occurring. After software update transmission shifts produce poor gas mileage which violates epa emissions regulations of mpg of vehicle when purchased.

26 Automatic Transmission Control Module problem of the 2018 Ford F-150

Failure Date: 08/29/2025

After Ford performed a pcm (powertrain control module) reprogramming recall on 16 July 2025, the vehicle began experiencing repeated engine misfires, loss of power, and engine warning lights. The condition progressively worsened, leading to a leaking fuel injector, an egr valve that stuck open, and eventual internal engine damage requiring full engine replacement. These failures involve pcm-controlled systems (ignition timing, fuel injection, and exhaust gas recirculation) and occurred shortly after the recall calibration. The issue has been confirmed and documented by an authorized Ford dealer. The truck has become unsafe to drive due to stalling and shuddering at highway speeds, which could pose a risk of loss of vehicle control. Warning lamps and a wrench icon appeared intermittently before the engine shuttering and loss of power. The vehicle and all repair documentation are available for inspection upon request.

27 Automatic Transmission Control Module problem of the 2017 Ford F-150

Failure Date: 08/20/2025

On 4 occasions, my f150 has went in reverse when transmission was in the drive position. The truck would travel backwards 3 to 5 feet before shutting off. The first time happened at a stop sign. Nearly backed into another vehicle. Pcm was reprogrammed. 2nd time occurred on leaving my driveway. Ford replaced an abs module. 3rd time happened in front of my house. Ford found bent pin on pcm. 4th time occurred at waste facility after unloading trash. Vehicle went in reverse when transmission was in drive. Vehicle went backwards at least 3 feet. Nearly backed into a Ford f250 and pinned driver of vehicle between the 2 trucks. Ford has changed a relay. I have contacted Ford by chat, phone, and email. The case manager refuses to communicate with me about this matter.

28 Automatic Transmission Control Module problem of the 2018 Ford F-150

Failure Date: 08/17/2025

My 2018 Ford F-150 3. 5l ecoboost experienced cam phaser rattle/failure at 72,000 miles. The vehicle never received the 21b10 pcm reflash, and Ford customer care has denied any goodwill assistance. The cam phaser noise is a known widespread defect covered under customer satisfaction program 21n03. I had the repair done out-of-pocket at a Ford dealership because I was fearful that catastrophic engine damage would occur if left un-fixed, leaving me stuck in the roadway. I request that this be logged for investigation and future regulatory review.

29 Automatic Transmission Control Module problem of the 2013 Ford F-150

Failure Date: 08/14/2025

The contact owns a 2013 Ford F-150. The contact stated that while driving at 40 mph, the odometer failed to function as intended. The speedometer went from 40 to zero and failed to provide the correct speed. In addition, the transmission unexpectedly downshifted. The “wrench” warning light was illuminated. The contact pulled over to the side of the road, turned off the vehicle, and pulled the key out of the ignition, opened and closed the door, and restarted the vehicle. The contact stated that the vehicle was reset. The vehicle was taken to an independent mechanic who diagnosed that the output speed sensor circuit had no signal and the pcm and valve body lead frame failed and needed to be replaced. The vehicle was not repaired due to the cost. The contact related the failure to NHTSA campaign number: 19v075000 (power train). The manufacturer was notified of the failure but denied assistance in covering the cost of the repair. The failure mileage was approximately 101,500.

30 Automatic Transmission Control Module problem of the 2014 Ford F-150

Failure Date: 07/25/2025

I was driving about 75mph when my 2014 Ford f150 suddenly downshifted from 6th gear to 2nd. As a result of the unexpected downshift, my body was pushed into the steering wheel, and the rear end of the truck slid sideways as it slowed down to 65mph. The truck had a loss of power and I couldn’t accelerate. When I pulled over on the side of the highway, I used my code reader to read the codes that came up. The codes that came up pointed to powertrain control module. When I looked at the pcm, it said p0720, output speed sensor. I looked up the issue online and found that there is an open recall on the 2014 Ford f150 with the 6r80 transmission, and the recall is the issue I stated above. I went to two different Ford dealerships and they said that my VIN isn’t listed under the recall so they won’t fix it. This is the manufacturers issue and they won’t fix it. The truck is unsafe to drive now.

31 Automatic Transmission Control Module problem of the 2017 Ford F-150

Failure Date: 07/23/2025

Component: 6r80 transmission lead frame safety: driving at 70mph on the interstate on 07/23/2025 and vehicle violently downshifted while in cruise control. Rear tires locked briefly. Driver and passenger were restrained by the seat belts or there would have been a loss of control. I was able to pull to the side of the road. Reproduced: this is the third time in a year that this has happened. Independent shop is scheduled to inspect. Dtc codes p061a and p0722 are logged in the pcm. Warning lamp: after the incident, an amber wrench icon was displayed. Still displayed after restart. Disappeared after 2nd start.

32 Automatic Transmission Control Module problem of the 2018 Ford F-150

Failure Date: 07/22/2025

The contact owns a 2018 Ford F-150. The contact stated that while driving at undisclosed speeds, the vehicle was jerking abnormally. The contact stated that the transmission was shifting hard. The contact stated that while driving with the transmission shifting from 9th gear to 10th gear, the transmission shifted to neutral, and the vehicle started to coast. The contact stated that the wrench symbol was displayed. There were no warning lights illuminated. The vehicle was taken to the local dealer on five occasions, where it was diagnosed and determined that the tcm needed to be reprogrammed, and the transmission needed to be replaced or rebuilt. The vehicle was repaired, but the failure persisted. The manufacturer was contacted and confirmed there was no recall coverage for the failure, but would attempt to assist with the repair. The failure mileage was approximately 38,000.

33 Automatic Transmission Control Module problem of the 2024 Ford F-150

Failure Date: 07/18/2025

The cause of the accident was due to an extremely hard downshift, then extremely hard upshift. Between the speeds of 25-37 mph the truck will do a hard downshift with a 1-3 second loss of power delay. When it finally grabs gear it takes off like a rocket before grabbing the next gear and back to what feels normal. This is something I never experienced in my 2022 Ford f150 with the same motor and transmission. This particular day, I had pulled off to the side of the road to let the mail carrier go through the narrow section of the road first. This particular section, there is a huge tree right next to the road (approximately 12"-14" off the road). After the mail carrier passed, I started to speed up. Upon hitting 34 mph, the transmission shifted hard, pulled me into the ditch, and by the time the gear grabbed, and I had power to control, I hit the tree. As a habit, since this issue happened numerous times, I looked at the speedometer and just prior to impact I was at 43 mph. The transmission is not the only issue that I have been having with this vehicle. There has been a fuel consumption issue, a hard start issue (does not matter time of day), as well as a grinding issue at the front right wheel. This truck is equipped with a 36-gallon fuel tank. At the city driving mpg and a 34-gallon fill-up, I should have been getting 544 miles/tank. However, that was not the case as I was only getting 440-447 miles/tank (did not matter whether normal, eco, or sport mode driving). Each fill up was a 24-28 gallon fill-up leaving 8-10 gallons unaccounted for. A pcm update was done but it only got me to 510 miles with a 30-gallon fill-up. Each complaint I made with the transmission went ignored with me being told "this is normal for the 10 speeds. You will just need to learn to deal with it. " after the pcm update with no true fix, I was told "drive it to zero mte and calculate from there how far you can drive, then you know. " this is a horrible and extremely unsafe answer to give.

34 Automatic Transmission Control Module problem of the 2020 Ford F-150

Failure Date: 07/14/2025

The contact owns a 2020 Ford F-150. The contact stated that while the vehicle was locked and parked on the street with the engine running and unoccupied, the transmission erroneously shifted into gear and the vehicle drove forward. The contact stated that the vehicle crashed into another parked vehicle and landscaping and missed crashing into another vehicle that was driving in the opposite direction. The contact expressed concerns because no one was present inside the vehicle during the failure. The contact stated that the police department was contacted but no officers showed up on scene. No injuries were reported. The cause of the failure was not determined. The dealer was contacted and informed the contact that the pcm needed to be reprogrammed; however, the vehicle had not been diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 22,000.

35 Automatic Transmission Control Module problem of the 2014 Ford F-150

Failure Date: 07/09/2025

After receiving a call from mullinax Ford in mobile, al regarding a recall for our 2014 Ford F-150 raptor (recall related to unexpected downshifting), I brought the vehicle to my workplace on [xxx], where a technician performed the software update (flash) on-site. Before the update, the vehicle was functioning without issue. That same evening while driving home, I experienced extremely hard downshifting, initially in lower gears, and worsening over time. The truck now violently downshifts, primarily into 4th gear, and sounds as if the transmission is slipping. I drove the truck again the next morning and the symptoms persisted. I contacted zachary, the original representative at the dealership, who said it may be part of the “relearning” process. My husband drove it a week later after returning from a business trip and he confirmed the issue. It has progressively worsened to the point of being undrivable. We returned the truck to mullinax Ford for evaluation on 7/29/25, and they claimed the failure was unrelated to the update and that the transmission needed to be replaced, at our expense. The truck unpredictably drops gears at high speeds, creating a sudden jerk that could lead to loss of control. This poses a serious safety risk to both the driver and others on the road. The dealership performed the recall software update and later diagnosed the truck with transmission failure, but is denying responsibility. No other third-party inspections have been done at this time. There were no warning lights or issues prior to the update. The issue started immediately after the recall software was flashed and worsened over the next 24–48 hours. The vehicle is currently at the dealership and available for inspection. We believe the issue stems directly from the pcm/tcm update applied under recall conditions. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6).
