1999 Ford F-350 super duty p/u
clutch slave cylinder failed w/o warning. Truck wouldn't come out of forward gear. Clutch pedal ineffective - went to floor and stayed. Narrowly missed woman and two children crossing intersection. Truck brakes wouldn't stop or hold the truck. Forward motion continued until ignition was shut off. This is the second time this has happened. The first time truck almost went through shopping center windows. Ford charges over $500 to replace the part, which will again fail and probably cause death or serious injury.

The contact owns a 2004 Ford F-350. While driving 5 mph, the clutch pedal broke when depressed. The contact stated that the vehicle would not stop until he shut off the ignition. The vehicle was towed to the dealer and they stated that the bracket that held the fire wall broke. The dealer stated that the bracket was made of plastic and they replaced the bracket and clutch pedal. The failure mileage was 63,000 and current mileage was 65,000. Updated 01/29/08. The consumer stated it's not the bracket that holds the firewall but bolts the firewall. The clutch, brake, and gas pedal all pivot on pins that are assembled to this bracket. Updated 01/29/08.

The contact owns a 2004 Ford f350 superduty. While driving 20 mph, the clutch pedal failed. The contact stated that the plastic bracket separated from the firewall. The vehicle remained in gear even when attempting to stop the vehicle. The vehicle was towed to a dealer and they replaced the clutch pedal assembly. The current mileage was 63,066 and the failure mileage was unknown.

As a last resort, he had to cut the vehicle completely off to prevent it from dramatically jerking forward. The dealer replaced the master slave cylinder assemblies. There had been no further occurrences. The contact filed a complaint with the manufacturer, but they did not assist because there was no recall. Speed was not a relevant factor with his failures. The clutch pin broke completely off. The clutch pedal went all the way to floor, and the consumer lost control of the vehicle. The consumer talked to Ford and they said that the consumer would have to take it to dealer for repairs. There was no way to move the vehicle except towing it. The consumer had similar problems two other times. The first two times the bushing was over the pin. Whom ever welded the pin, made it hard in the back and as a result it fractured and broke completely off. The pin was welded in crooked when it was manufactured. This is what caused the first two bushings to go. These problems started on August 15, 2005. (11/09/05).
2000 Ford f 350. Clutch will not disengage driveline when depressed. 2 nd failure of such nature. Vehicle will keep rolling in current gear and speed when clutch is disengaged. Can do nothing but stall the engine by hard braking or turning off the key both create other dangers. First failure happened at 38k miles second at 52k. Paid for first repair out of pocket- said to be the clutch master cylinder boot ripping while under pressure. Seeking Ford to fix this time. Ford dealerships in norfolk va and knoxville TN deny that this is a safety issue. I do not understand their logic.

My Ford F-350 superduty pickup truck (37,000 mi) was in gear and in motion when I stepped on the clutch pedal, prior to attempting to stop. The clutch pedal remained depressed to the firewall; and the clutch failed to disengage. I was unable to pull the transmission out of gear, and was forced to apply both feet to the brake pedal to get the truck to lurch to a stop. This was at idle speed. If this had happened on the highway at 70mph the results would have been disatrous. Ford has issued a technical service bulletin (01-24-1) on this condition. This should be a safety recall item. The inability to disengage the clutch and safely bring the vehicle safely to a stop will result in serious bodily injury or death. The failure of the manufacturer to label this as a safety recall item is negligent. Despite being specifically asked; the Ford dealer service representative failed to notify me that there was a service bulletin on this specific item, when I took it in for repair at the dealership.
The consumer was pulling out of the driveway when he engaged the clutch, the clutch went to the floor, became stuck and continued to going forward. The consumer could not get the vehicle out of gear and applied the brakes as hard as possible. The consumer then got out of the vehicle and pulled the clutch with his hands and thought the problem was a fluke. The consumer continued down the highway shifting the gear with no problem until he had come to a stop, waiting on the light to change, and the clutch pedal went to the floor. This occurred when the vehicle was at an intersection.
2nd failure of clutch master/slave cylinder assemblies, within 40,ooo miles.
Clutch release bearing (throw out bearing) actuator arm failure resulting in movement of vehicle while clutch pedal is depressed this is the second failure of the subject part in the life of this serial number truck. The first occurred at apx. 37,000 miles; the second at apx. 95,000 miles. Both repairs were completed using oem parts. I am in possession of the second failed part and will make it available for inspection upon request. This failure is a major safety issue as the power train can not be interrupted by normal means. That is, one normally depresses the clutch pedal to interrupt power transmission and applies brake action to stop the vehicle. When the subject arm fails, one depresses the clutch pedal and the vehicle continues movement, causing an inherent reaction delay to asses the abnormality, subsequently followed by application of brake pressure to stop the vehicle, causing a significant bind in the transmission, resulting in the inability to move the transmission shifter out of gear, then one must either turn off the ignition or apply brake pressure of sufficient force to stall the engine; all of which has consumed and exceeded a reasonably planned stopping distance. The resulting delay may result in injury or death of by standers, should they be pinched between the failure vehicle and a stationary object. The operating nature of a truck of this design increases the risk of injury or death, following the described failure. The manufacturer markets this truck to commercial operators and one will routinely find the vehicle employed on work sites and similarly congested areas. This failure leaves the trusting and capable operators of this vehicle and unsuspecting by-standers at risk of injury or death; while at play or on the work place.
While driving on a flat surface at 15mph, the vehicle lost its clutch. Making it hard to stop the vehicle. Dealer notified. The consumer stated that the roll pin that holds the hydraulic line to the slave cylinder on the clutch had come out and the line dropped down. (no clutch).
Clutch pedal will not return when applied. Vehicle taken to dealer, and was informed of a yoke failure. Repairs were made at consumer's cost. Dealer stated that this is not an uncommon problem. Replacement parts were from Ford, consumer is concerned why failed yoke is not a manufacturer defect.
Manual transmission clutch rod cylinder broke while shifting to make a turn. Was going only 25mph on a back road. Was able to restart vehicle and drove it home. Had it towed to dealership, and mechanic could not find cause of breakage, but was able to replace rod and slave cylinder.
Clutch assembly springs separated from the clutch on three occasions. Dealer repaired vehicle three times in three weeks. Manufacturer will be notified.
