Today I had a crash and luckily walked away. I went down an embankment at speed into trees where the vehicle rolled and struck trees head on. All the vehicle airbags deployed and vehicle came to rest on the drivers side. Although there was electrical power and the engine still running none of the right side doors would open and neither would the rear hatch which was clear of obstruction! had there been a fire I wouldn't be making this report! doors need to be operable regardless of situation when none of them are jammed from an accident!.

We recently had a full diagnostic check done on our vehicle (199 point at parkway automotive in little rock, arkansas) that listed some components in need of repair. The only part involving the electrical system needing repair was our battery which was testing as no longer holding enough charge. We took the vehicle to sam's club in north little rock, ar within the last 2-3 weeks and had the battery replaced with a new duracell with 3 year warranty. We drove away with no issues with the vehicle and used it for local driving without issues for those 2-3 weeks. The morning of Thursday, December 29, 2022 we started the car to drive to our church about 2 miles away with our four young children (1, 5, 8, 10). In the short drive, dash lights were flickering and the nav/audio screen system was turning off and restarting. Upon arrival at the church, as we were parking the car, gray smoke began to come from the hood quickly. The children were all still strapped in seatbelts and carseat, so we quickly unloaded them from the car and took them inside the building. While we were unloading them from the vehicle we observed the following: -black smoke pouring from driver's side hood -sparks falling on the ground from driver's side engine compartment, near bumper -spurts of fire coming from the hood and below the bumper on driver's side -charring and melting coming from the vents/lights where each child was seated in the 2nd and 3rd row seating after calling the fire department and locating a fire extinguisher, the fire quickly grew into an uncontrollable blaze so we all went inside the building as quickly as possible to avoid injury. Shortly thereafter, the battery itself seemed to explode (a loud explosion was heard and then a smaller one) and the driver windshield was blown. The fire was large enough to seem to be traveling toward the gas tank and we were afraid of a big disaster. The fire department then arrived and extinguished the fire and made a report.

I would like to make formal complaint in regards to a manufacture defect. I purchased my Toyota Sienna xse brand new in August of 2021 at cerritos Toyota in California. On December 24th at approximately 1:30 p. M. On christmas eve, after filling up gas at sam's club in fountain valley, CA. I pull out on to the street heading northbound on warner approaching intersection as I come to complete stop on red. All of a sudden, my Sienna completely shut itself off and the engine light came on. I put it back on park and restart the car. As I attempt to accelerate forward. The car jerk and the brake didn't work and the engine light came on. Fortunately, I was able to make a u turn and pull into a gas station near by ( shell) and I thank the lord almighty there was no car right behind me. I could of gotten rear ended. It was the most terrifying moment of my life. My wife and I was in the car at the time of the incident. We waited patiently over an hour for triple aaa to have it tow the nearest dealership next to my house in long beach, CA. My 2021 Toyota Sienna currently has 52,023 miles and sitting at the dealership to be repair at long beach west coast Toyota. I have contacted Toyota of this incident and they refused to fully covered repair instead they offered me 700 payment assistant the rest would come out of my pockets. I believed the car should be part of the campaign recall and nothing should come out my expense. I would like you you guys to investigate the incident and look into this matter as soon as possible because there thousands like my mind that could there have the same issue but was not part the campaign recall. A bulletin was published on April 13, 2022 affecting 2021 -2022 Toyota Sienna. Production period March 3rd, 2020 through April 7th 2022. Manufacture's campaign number: Toyota: 22tb03.

The tpms sensors stop working when driving at highway speeds and won't tell you the air pressure. Says see dealer but when I take it there, they can't do anything. Has happened probably 10 times. Usually only on interstate. Can be safety hazard if tire pressure goes low and no idea.

The microphone has an issue and it is tied to the sos call button in the car. I asked my dealership, they wanted to charge $200 to run a diagnostic scan, I contacted Toyota directly, they basically said the dealer can charge it if they want and reverse the charge if they find a problem under warranty. I originally thought the issue was with the blue tooth. I found this video on youtube, his fix did work-my concern is, if I had needed emergency help-the button would not have called anyone, disconnecting your car battery is not something I should I have to do to fix a 2019 vehicle. . But dealership and Toyota refusing to replace it under warranties. . Even they know some owners started class action against Toyota.

For a month prior, there were intermittent incidents of the van shuddering and the rpm needle jumping upon ignition. Sometimes the van would have a hard time accelerating before jerking and driving normally. No issues before this started in late July and no indication when/where would occur. •September drove the van all day running errands. It drove normally. Filled up the gas tank (gulf gas station), then continued errands. About a half hour later, while at a stop sign, a notification appeared that the “traction control system has been turned off” and the check engine light came on. Tried to move through the stop sign, but the van would barely go. Applied the gas, but no results. Able to turn onto a side street and had to have it towed to Toyota of irving. The service advisor stated the issue was due due to bad gas and indicated the van needed to be flushed and the fuel injection system cleaned. Paid $725 for these repairs. •September started van in driveway. Shuddering and rpm needle jumping again. Drove normally. •September same as above. •September drove the van all day with no issues. Filled up with valero plus gas. As we pulled out of the gas station and sitting at the light, the traction control message and check engine light again appeared. The van would barely go to get us through the traffic light and into a nearby parking lot. Once we parked, the screen displayed the following message:"a malfunction in the electronic control of the engine, throttle, or automatic transmission has been detected. " had the vehicle towed to Toyota of irving. The service advisor called 2 days later to say they cannot detect any issues.

Ever since a tire was changed on the vehicle, it has had a recurring error message that states there is a malfunction in the tire pressure monitor system. As such, I rarely know the status of my tire pressure as the system has the malfunction at least 50% of the time. The car has been in service 2x for this issue and they have had it for close to 30 days and driven it all over saying they needed to drive it to make the error go away. They have replaced the sensors on the tire, nothing has worked and the problem still occurs daily.

Driving on dry pavement in mountainous terrain the slip indicator light/sound activates temporarily and the brakes can engage by themselves! usually happens during right turns. I've been to several automotive specialists for mechanical & electronic diagnosis. I've replaced the lower control arm & the clock spring behind the steering wheel but the issue persists. Online searches indicate this is a common problem with Toyota Siennas that is extremely difficult to diagnose & very costly to replace all the possible problematic components! I've had this problem since buying the car used several months ago. This should be a recall issue!!!!!!!!!!!.

The contact owns a 2019 Toyota Sienna. The contact stated that the liftgate failed to lower while using the remote. While reversing, the back-up camera became inoperable. The air bag warning light was illuminated. The vehicle was taken to the local dealer who stated the failure was due to the remote start feature that was installed. The vehicle was not diagnosed or repaired. The contact stated that she noticed the mirrors were fractured. While driving at an undisclosed speed, several unknown safety features became inoperable. There was a burning odor detected inside the vehicle. The manufacturer was contacted and offered to pay a nominal portion of the repair for the mirrors and referred her to the NHTSA hotline for assistance. The approximate failure mileage was 72,000.

We got into an accident today, and none of the sliding doors would not slide open. It would not even allow us to pull the doors opens. The vehicles electrical was damaged, but that should not prevent the other doors from opening. This is a very bad situation if someone is needing to evacuate the vehicle. The rear passengers would be trapped. Please address this issue with Toyota ASAP. People who usually have vans have big families and small children. This would be a very bad situation if vehicle needed to be evacuated ASAP. The rear passengers would be stuck.

The failing component is the vsc/abs system. It puts the occupants of the van and other motorists at risk of either causing a collision or causing the driver of vehicle in question to be unable to control the vehicle . The failure appears when going in a curve in the road at low speed causing the abs to engage lock the brakes and steering wheel rapidly decelerating the car and causing the car to jerk to one side or the other and go into a skid where the driver is unable to control the wheel due to steering wheel locking up when it happens the vsc and trac lights come on and there is a loud and rapid beeping that can be heard inside the vehicle. The issue usually appear when going between 25-40mph in banking curves.

I went on a trip to missouri to visit my son and family. When going through st. Louis on 5/18/2022 I ran through a thunderstorm with a lot of rain. After I stopped, I noticed a puddle of water between the front seats. I could not find out where it came from. Later that same day I went through another storm. Again I found a puddle of water between the seats. This is a vehicle that was modified by vmi into a handicapped accessible van. Water coming in is concerning due to all the electrical components that could be shorted out. I don't want to find out later that there is rust damage nor damage to the electrical systems because of water entering the vehicle.

This van was converted to be wheelchair accesible by vantage mobility International. My parents have repeatedly had an error arise that indicates an issue with the hybrid battery cooling system. This system was modified by vmi, in partnership with Toyota, to allow for the accessibility features to be added to van. Neither the vehicle dealer, superior van and mobility in knoxville, TN, nor the local totota dealer, Toyota of knoxville, have been able to rectify the issue, despite mutliple attempts. We are concerned that this is a safety issue, and have asked for written confirmation of that and for the issue to be resolved. None of the three parties, superior, Toyota, nor vmi, have been willing to do that nor rectify the issue. (I have filled this out for my parents because this has already become so stressful and in the wake of my dad's rapidly progressing disability, I don't want either of them to have to deal with more of the headache with this. ).

I was driving my 2017 Sienna on 5/10/2022 and was approaching a roundabout. As I was going thru the roundabout, my van stalled. I coasted into a gas station. After a few minutes I notice smoke coming from under my hood. I popped the hood and noticed the smoke was coming from the air intake. We called aaa and they towed it to the dealership we bought it new from and had all services done at. The next day we authorized them to do a diognostic on it where they found a burnt airfilter and melted housing. Heiser Toyota in milwaukee WI has not repaired the van yet and we wanted to talk to Toyota customer service and your office to see what we should do. We called insurance and a claim has been started - not sure if they have inspected the van yet. It is available for you to inspect. All warning lights were on when it stalled. There was no power loss or any warning before it stalled.

Wire harness section which controls the parking collision warning and rear cross traffic warning is frayed, causing failure. This failure was determined by the dealer to be caused by mud packing into the rear bumper cavity. Dealer wanted over $1,000 to fix. Denied warranty, due to the cause being "an outside influence". There is no way to effectively wash the inside of the bumper without removing the bumper. If I pay for a new wire harness, the issue will re-occur. This poses an elevated risk to my family and any occupants of the vehicle, as key safety features must be disabled.

While driving vehicle on dry, non slippery roads with slight to moderate curves, vehicle illuminates traction loss light, sounds alarm buzzer, and at times applies brakes and dethrottles engine, even when no loss of traction has occurred. This happens while driving on straight roads with slight unevenness. Had to disconnect wheel speed sensor harness to disable abs/vsc system. When this happens, the vehicle automatically applies brakes to certain wheels and dethrottles the engine, this can pull the vehicle to one side sharply and can cause an accident.

After a big snow storm I cleaned the car off and warmed it up, then tried to open the passenger side power door from the inside side with the button. The passanger side would not open (driver's side did) so I tried to open it manually from the outside and then the inside. It would not open at all. Then as I was driving, the car started beeping and the 'door open' icon lit up on the dashboard. I stopped the car and again tried to manually push the door shut as it appeared to be slightly ajar at this point. It seemed to close but the light and beep stayed on for my 10 minute drive home. When I got home I inspected more and noticed a wire for the power door had snapped at some point, I assume from the cold because it was operable the evening before. At this point I cannot open that door manually either, it is completely stuck shut, which I feel is a safety issue as my kids only have one point of egress. I did some research and this seems to match the description of one of the recalls but I was told my vehicle was not included based on the VIN #.

While driving on dry freeway or other stable road conditions, the electronic stability control kicks in, braked hard, causing the van to swerve, and the lights on the dash started flashing, accompanied by a loud beeping. This has happened multiple times now. There is no reason for this to be occurring. It is involuntary, sudden, and random. If someone were close behind me, they could rear-end the vehicle. It also sometimes pulls the steering so hard that it swerves into the other lane. This is a serious safety issue that Toyota needs to deal with.

On October 15, 2021 at 2 pm I drove into the driveway of a friend. I turned to the right of the driveway on her grass so I could backup and face the road. I’ve done this many times in the past. On this day as I put my foot on the break the van accelerated at a very very high speed. The van was out of my control at this point. I do not know how fast because this all happened in seconds. I was heading straight to their privacy wood fence only feet away. Approximately 20 feet. I don’t recall breaking again but turned the wheel to the right trying to avoid the fence. I hit the fence at a high speed damaged the left bumper and scraped the drivers side of the door. The next second I attempt to break again so I could back out of this when the van continued to accelerate at high speed going straight toward the house only a few feet away. I attempted to avoid the house and hit the right bumper hard at a high speed on the corner of the house. At this point the acceleration stopped and I was able to backup and turn the car around and finally turn the car off. There is major damage to the van and some to the fence and house. I was very upset and in shock and some soreness the next day but thankfully no one was hurt. This is a very small yard area that this took place at. I am still in shock and extremely confused as to why this happened. I do know without question my foot was not in the accelerator. The van is presently in the body shop. I would like to mention I have a clean record at the age of 64 I have never had an accident and/or speeding ticket in my life. This truly was not human error. I’m a very experienced driver. If it was human error the van should have stopped at the first point of impact on the fence, but did not. It continued to accelerate at a fast speed after hitting the fence hard. The van paused but continued to accelerate at this point when I hit the break again and it went toward the right straight for their house.

The contact stated that she was quadriplegic and was unable to exit the vehicle because the door lock switch was disengaged but failed to respond. There were no warning lights illuminated. The contact stated that she was able to jumpstart the vehicle with assistance. The contact then engaged and disengaged the door lock switch and she was able to exit the vehicle. The contact stated that she notified the modifier of the vehicle, but they said they could not assist with the failure. The dealer and the manufacturer were not notified of the failure. The vehicle was not diagnosed nor repaired. The approximate failure mileage was 48,000. The VIN was not available.

I have complained about both rear sliding doors acting erratically for almost 3 years. These doors were under a recall, as they sometimes were overloaded and would cause the fuse to blow and the door(s) would/could swing open while moving. It happened to me numerous times. Once it was so bad, that I couldn’t bring my son yo school because the door wouldn’t stay shut. They beep upon auto open and you have to fight the door open or shut no matter if you switch off the “auto door” button or not. It has been to 3 different dealers in less than 3 years. The first dealer only replaced one junction box, and wiring harness on the passenger door because that it the only door that acted up while they were in possession of Sienna. The door less than a 1000 miles later started to have issues - opening, closing, staying closed. I brought it to another dealer closer to my new house - they did not fix it as it didn’t do it for them with 10 tries while parked using auto and manual settings. That seems irresponsible and irrelevant as the ongoing customer complaint is the door swinging open whilst driving (neither dealer drove it, to see if it would swing open). Nonetheless, as the drivers side door never acted up at any dealer visit - they never replaced the recalled parts even knowing it has issues and saw video of it that I took. My vehicle is now sitting at a dealer with a $1600 repair bill for the same latch, as well as the back latch for the “recalled” parts - and they are refusing to acknowledge any recall wver on the doors (safety recall g-04) as well as Toyota customer care so far is refusing to provide monetary assistance with this ordeal as my extended warrantee expired. One doors “repair” has made the door seemingly worse, and one door has never been repaired in regards to specific recalled parts as dealer states “no issue was seen with power sliding doors”. My niece lost a lot of $70 birkenstock’s when my door flung open so fast at 50mph with no warning (door open light).

Stability light comes on, decelerates independently, vehicle shakes and steering is somewhat compromised. This has happened rounding curves between 40-50 mph. Has happened twice on straight road, 8/24 and 8/25/2021, traveling at 70mph, vehicle decelerated 2-5 miles independently. I put signal on and did smooth lane change to the right to get out of fast lane, and stability light came on, vehicle shook and steering was somewhat compromised. Neither local mechanic or dealership can retrieve any codes with diagnostic equipment. Dealership told me today the light has to stay on for them to retrieve code. This has happened irregularly over the last 6-8 months but always on curves. It has occurred 4 times in 6 days last week, twice on curves, 8/23 & 8/28/2021, and the 2 lane changes above. We were driving down the freeway when we got rear ended hard by a large pickup. Then we lost electrical and engine power. The brakes stopped working. The hazard lights stopped working. Luckily we were able to coast to a complete stop as there was no way to stop the car. The police came and took a report. Then the car was towed to a body shop. We're awaiting the insurance company adjuster for inspection.

When vehicle is park on an angle (with front section lower than the back) power sliding door (behind front passenger) does not fully engages arresting hook and tends to immediately return to close position sometimes hitting passengers trying to enter or exit the vehicle. Vehicle was taken to dealer for check up but dealer claims to have been unable to replicate issue, possibly due to not placing vehicle on an angle. No warning lamps or lights indicate any issue with the vehicle passenger door(s).
