The car involved is a 2006 Chrysler Pt Cruiser. 2006 chyrsler Pt Cruiser. Bought the vehicle brand new in 2006. In the last two years I noticed intermittently the car wouldn't start which seemed to be the battery. It started after jumping and would be fine for 4 months or more. Since then I have pinpointed it to leaving the key in the ignition for just 2 minutes. Thought this was weird since my battery was oly 3 years old when this first start. Recently, the windshield wipers, turninging signals, and headlights do not work. I checked all fuses, they are good. I read online about the total integrated power module that replaced the 2006 interior fuse boxes. I read many complaints about the same issue. When I found out my turning signals were not working I thought they were the fuses and had no idea my headlights were not working as they worked when I checked them after the turning signals were not working. That night, I started driving home and the headlights went out on a very dark road without notice. My son and I were in serious danger and the only thing we could do was turn on the hazard lights. Think god they worked. The basic safety requirements on a car such as the above should never go bad without notice and without an easy fix. I can not believe that Chrysler has not put out a recall to fix this problem. Also the interior lights would not turn on inside the vehicle except if I opened the door. You could not turn them on if the door was closed. I am sure it has caused accidents as it could have the night ours went bad. I hope that the napa Chrysler can fix mine as the service manager didn't even know with the tipm was. The failure date was 05/24/11.

The car involved is a 2012 Dodge Challenger. First, to happen was I pulled out of driveway when the "brakes' warning light came on. Continued travelling, and soon became nightime driving. Was going approx. 55 mph, when the instrument panel began to flash numerous warning lights. Then, the radio and interior lights began turning off and on. I was not able to manually turn off the radio during this time. I continued to drive to try to make it to a gas station. Then I felt hot air coming thru the vents. It was like I had on the heater, but I didnt. (nor the ac was on. ) the car lost all power. The steering wheel locked. I was stranded. I was not able to put the car in neutral to move out of dangerous roadside nor was I able to put on the hazard lights, they came on but were very dim and slow flashing. All power was lost, no horn, lights. . . Nothing. Did notice a burn smell coming from under the hood, but not sure what the cause was. I had it towed to the dealership. They stated it was an alternator/battery issue. They replaced the alternator/battery, however the airbag warning light was now on and horn no longer working. I didnt understand why the dealership didn't repair the airbag warning light and the horn. They did acknowledge these issues on my invoice yet no repairs were made. I have almost had 2 collisions since this. I was not able to sound horn to warn opposing drivers, and I still have no horn to sound. Both the airbag and horn malfunctions are hazardous and I dont understand why they didn't repair them at the deaership. ? thank you. This problem occurred on Tuesday, October 29, 2013.

The car is a 2007 Ford Focus. Without any indication of a low battery, the car began to lose electrical power while driving. At first, I was on my way to work when I noticed my radio stopped playing. Upon looking at my radio, I realized the lights were completely off, including the clock--which stays on even if I turn the radio off. I noticed I was very cold, and my defrost had stopped working. Thinking it was hopefully just a blown fuse, I continued on my way to work, driving cautiously. My car did sound like it was not shifting gears as much as it should have been. I made it to work fine, and even checked on it during my lunch hour--it started up fine, lights and everything was working, with no indicator lights illuminating on the dash. Thinking it was fine, I drove home that night with the intention of bringing it to the shop to check the battery over the weekend. While driving home (in the dark) that evening, I made it almost to my apartment. I noticed again that my radio suddenly stopped playing. I looked at the radio, at which point I saw the text display "low battery. " I began to notice everything dimming--the dashboard, interior lights, and even my headlights. I was close enough to get to my apartment ok. The next morning I had an appointment to get to the shop. After reading the other complaints of similar drivers of 2007 Ford Focuses having identical issues, I was nervous about driving to the shop. Not knowing if my car would suddenly die on me, and if I would be able to use my hazard lights, or turn signals to indicate to other drivers if my battery started to go again. Luckily my car made it the 3. 1 miles to the shop, before giving out. Same thing--no earlier indications that the battery was low, the power just starts shutting down. This is incredibly dangerous, and I was lucky that I read up on the issue and was only driving locally, and not on a highway. The failure date was Friday, Jan. 17, 2014.

The car involved is a 2011 GMC Sierra. The vehicle's vehicle identification number (VIN) was 1GTN2TEA3B_. The headlights dim then return to normal every time the break is applied. Interior lights dim and return to normal when hazard lights are activated. Dealer found the complaint to be valid. Battery tested and found to be good. Voltage test of alternator checked out normal. Engine and body grounds checked out normal. Dealer replaced brake switch with new part with no change. General motors referenced service bulletin #02-06-03-008d and claim the truck is operating as designed. After several conversations with gm customer service they told me there was nothing they would do about the design of the system and I am stuck with the truck as is. The gm service representative also refused to send me any documentation regarding my customer claim and directed me back to the service bulletin and the dealer. I have invested a lot of money into this vehicle and have no confidence in its safety at night. My gm customer claim #[xxx]. This truck was purchased new and I would not allow my family to operate it at night and I am very concerned about the light system failing or shorting out as the vehicle gets older. Information redacted pursuant to the freedom of information act (foia), 5 u. S. C. 552(b)(6). It was 03/08/11 when this hazard flashing warning light unit issue occurred.

The car involved is a 2001 Honda Accord. Obviously, motor vehicle headlights are essential safety equipment. The headlight lenses in all cars become clouded over time regardless of maintence or brand of vehicle. This is a definite safety hazard as light from the headlights is greatly reduced. Replacement on my car is approx. $400 per lens. This is perfectly avoidable by using better materials. Auto manufacturers should be made to replace them at their cost, and use proper materials in all new cars manufactured. I am stunned that no regulating body has done their job in addressing and correcting this serious issue. This hazard flashing warning light unit issue occurred Saturday, September 21, 2013.

The car was a 2008 Honda Fit. Purchased this car as a certified used car in November 2010 from tarrytown Honda. Numerous incidents of stopping on the road with this car since June 2011 but this is the latest incident. Previous incident also reported on this website. Also filed complaint with american Honda in September 2011 (case#n0122011-09-0701741) and filed for nys lemon law arbitration in 2011 but never received a response. Car dashboard lighting started to go while driving and steering difficult. Car stopped after coming off of brooklyn bridge on 5/10/12 and had to be frantically pushed to the side of the road with help of good samaritan and my passenger because hazard lights wouldn't remain on. Car was towed to Honda dealership by aaa. Car completely died on the road in the middle of highway traffic with steering impossible and hazard lights not operable. Dealership states that alternator and battery need replacement - for the second time in one year! I belong to a Honda Fit online forum where other Honda Fit owners complain of the same problem. The dealership has not been able to resolve the problem since the first incident in June 2011 and I maybe injured or killed if I continue to drive this car. This problem occurred on Thu., May. 10, 2012.

The car is a 2003 Hyundai Tiburon. VIN of the vehicle was KMHHM65D03_. We just bought our son his first car. We found the 2003 Tiburon locally, so we checked it out. Everything worked as expected and drove very nice. After about 3 months, the turn signals and hazard lights stopped working!! no fuse to change, but were told it was a failed bcm module. As I was looking into it further, I found the repair cost to be close to $1,000!! I am outraged that there is no recall on this as it is a safety issue, and a true hazard to the driver, passengers, pedestrians, and all others who share our roadways. The failure date was January 3, 2012.

The vehicle involved was a 2004 Hyundai Sonata. The VIN of this vehicle is KMHWF35H84_. Emergency flashers began flashing for no apparent reason and would not be turned off. Then trying to use the keyless entry door opener, the locks began locking and unlocking. When the car was running this stopped. We took it to the dealer and they replaced the etac module, then the entire fuse box. After returning home, the hazard lights again began flashing while the car was parked. This would stop and begin again intermittently. Returned to the dealer and left the car for several days. Finally a switch was replaced and the problem ceased. This problem occurred on Saturday, August 15, 2009.

The car is a 2006 Jeep Grand Cherokee. While driving in normal dry conditions this past evening unexpectedly all my interior gauges and dash electronics started flickering on and off to include the radio, hazard lights and everything inside. At the same time I realized I was also loosing my exterior headlights and also brake control because my car was essentially running but with no electronics. It started making a weird grinding pulsating noise that would accompany the flickering lights. This forced me to exit the road at a gas station. It then stopped and I began driving again only to have it resume shortly. Thankfully I was close to my destination and was able to shut the vehicle off for the night. This has happened occasionally in the past couple months during the day but this is my first night experience. I thought it was just a glitch but it is definitely getting worse and has become regular. The next morning I was able start and drive my vehicle to work and it repeated this behavior a couple more times. At days end I went to leave and now my Jeep won't start and hasn't for two days. All the electronics come on but when I turn the key it just clicks. I also had this problem in the past but when my mechanic looked at it he was unable to duplicate the problem. This hazard flashing warning light unit issue occurred Apr. 26, 2013.

The car is a 2004 Mazda Mazda6. Vehicle identification number (VIN): 1YVHP84C34_. My battery on my 2004 Mazda 6 went out and I had to jump start it to. Go to work and come from work one day. The next day I took my battery to get checked to see what the issue was and they told me my battery was completely dead and it had only 6 months of usage. I put a new battery in my vehicle and as I installed the new battery I unplugged it to see if my alternator would kick in to make sure it was working correctly and as I put the battery back on a shortage happens and my wipers came on at full speed and all my lights were blinking on my dash and a smell of electrical wires burning started to happen and the entire electrical system has messed up. Turn signal lights don't work, headlights, bose stereo, hazard lights, reverse lights truck don't open windows don't go up or down and while I drive the speedometer does not track the velocity the needle goes crazy and my e-brake light is on and track control light is blinking and the air bag lights come on all the time . I do not know where to start to fix this issue but I'm really disappointed in this because I just have 8 months with my vehicle from a dealer ship and they checked out car just fine. This is going to be a super high cost to repair all the damages. Just from a change of a battery. And to top it all thank god I wasn't in the car because their was a small fire in the wires but it went away quick. This hazard flashing warning light unit problem happened on Tue., Jul. 31, 2012.

The car involved was a 2005 Mercury Grand Marquis. While driving on the massachusetts turnpike at night the headlights shut off with no warning, notice or anything. Never had this happen before. Scared us to death!!! we immediately turned the signal to pull over to the shoulder and then they came back on. About 2 minutes later they went off again. My boyfriend pulled over to the shoulder and turned on the hazard lights and shut the car off. We then turned the car back on and had no headlights, just marker lights. But, if you held the stick for the high beams you could use those. Then the lights miraculously clicked on and stayed on for the rest of the way home. This has been a continuous problem. After much research I have found out it is the lighting control module. Approximately a $650 part and about $200 for labor. I contacted Ford motor company and they do not feel that this is a safety issue and are going to do nothing about it!!! really!!!! I am part of a website and I get numerous emails for people that have the same problem. I am afraid to drive at night because I never know when the lights will work or not. I dont want to blind people driving behind them and it just really sucks that Ford does not think that driving down the road at night with no headlights is not a safety concern. . . . Why doesnt the Ford executive try driving home in the dark with my car and see how he likes it. Then we will see if it is a safety issue or not, huh??. The vehicle involved is a 1996 SAAB 9000. The automatic transmission was replaced three times. The first transmission was replaced due to a worn out oil pump(the fluid was burned), and there was a whining noise. The second incident happened while the consumer was driving and the transmission did not shift properly (had to drive back manually/the mechanic found an internal problem), the vehicle stayed in fourth gear, and had to be replaced with a rebuilt transmission. The rear brake pads had worn out. . The seat heater light was inoperative, the dealer replaced the seat heater switch, the exhaust system was replaced, the drive belt and pulleys were replaced. The mechanic stated something within the vehicle was built incorrectly to cause the transmission to fail. The motor for the rear windshield wipers had to be replaced, which caused the front windshield wipers not to work, and the hazard lights worked intermittently. This hazard flashing warning light unit problem happened on Wed., Aug. 1, 2001.

The vehicle is a 2007 Toyota Rav4. Well-maintained 2007 Rav4 (67k miles, no issues in the past) stalled completely while driving 50 mph. Nearly got rear ended as hazard flasher lights did not work at all. All power cuts off to inside of car. Once fan in engine turns off (about 6 minutes) car can start again. Dealership said "dirty throttle body part" so they cleaned it. Happened 2nd time the next week! then dealership replaced entire throttle body assembly. Happened 3rd time the following week! it's at dealership again now. Each time it happens, car is driving and once about 6 minutes passes, I can drive again. Barely have time to get near the shoulder of road. It was Fri., Mar. 14, 2014 when this hazard flashing warning light unit issue occurred.

The vehicle having this issue was a 2007 Toyota Prius. I was returning home from a trip with a friend and driving on major freeway late at night, very dark with little visibility in a rural, unlit area. . All lights in vehicle shut down while driving 70-75 mph. We could not see anything even the road, was like driving blind and very frightening trying to navigate the Prius off the road safely without getting hit by other vehicles. Once I stopped the vehicle off the side of the road in the far left lane we turned the vehicle off, but left the hazard lights on and let the vehicle sit for a few minutes. We started again, lights came on, and I drove the vehicle another 100 miles without any incident until we entered the city. I had a similar problem driving the 12 remaining miles home, but the front yellow lights worked and I had the hazard and brake lights working. The city lights improved visibility. I took the vehicle in and the only problem they could find was with one of the headlights. I was told I needed to replace both. I can't emphasize how dangerous and frightening this experience was for my friend and I, still unsure how we avoided a tragedy. I would like the NHTSA to investigate and make a recommendation to have some type of warning in the vehicle. The failure date was Apr. 3, 2014.

Last night my 2004 Volvo Xc90 left me on the side of the road interstate 88 near rock falls, IL weather 7:00 pm 28° humidity: 73% wind: nne dew point: 22° pressure: 30. 16 in bone dry while driving 65mph felt a "thump" in the front end, followed by a split second where all dashboard lights, went black. While trying to push the gas pedal the car would not accelerate. Then normal function resumed. A minute or two later, same thing and all lights internally completely went out, I pulled over immediately and turned the car off. Hazard lights could not be turned on. After calling for a tow truck, and waiting about 45 minutes I restarted it. There were 2 messages on the dash, the yellow exclamation point dashboard icon/alert lit up, a service message flashed referring to the anti-lock brake failure, the srs needed urgent attention. I purchased this vehicle in early November 2012 I called the closest Volvo dealership and was told there were no open recalls and they had to have the vehicle for diagnosis because of the different symptoms I reported not a single mention of NHTSA defect investigations for the 2004 Volvo Xc90 that seems specifically related to my symptoms experienced.
